Complex Korean Medicine Therapy for Hypoestrogenic Side Effects of Gonadotropin Releasing Hormone Agonist Administration to Postoperative Endometriosis Patient: A Case Report (자궁내막증 수술 후 GnRH-agonist 투여 중인 환자의 저에스트로겐 부작용에 대한 복합 한의 치료: 증례보고)

  • Objectives: This study is to report the clinical effectiveness of the complex Korean medicine therapy on a postoperative endometriosis patient's hypoestrogenic side effects who is treated with GnRH-agonist injection. Methods: The patient in this case was diagnosed with endometriosis and has been treated with GnRH-a injection after laparoscopic operation. The patient complained hot flash and sweating mainly after GnRH-a treatment. The patient received complex Korean medicine therapy during 10 days admission period. The clinical effects were evaluated through KI (Kupperman's Index) and SF-36 (36 item Short Form Health Survey). Results: After the complex Korean medicine therapy, the various clinical symptoms including hot flash and sweating were improved. Also, the quality of life was enhanced. Conclusions: This case report shows that the complex Korean medicine therapy was effective for treating hypoestrogenic side effects occurred after GnRH-a treatment in postoperative endometriosis patient.

  • Idiopathic hyperhidrosis is a physically and emotionally distressing symptom, which lacks a precise definition. Although operative and nonoperative methods have been shown to be effective in the treatment of idiopathic hyperhidrosis, limitations, cautions and complications are inherent in their extended use. The purpose of this report is to describe the theraputetic effects of magnetic resonance for managing idiopathic hyperhidrosis. A 28 years old woman had been suffering from profuse sweating on both planter and palmar sides for 15 years. We successfully treated this symptom with the 6 magnetic resonance treatments without any complications. Now she lives well in a condition of normal sweating after the applications. It suggest that short daily periods of exposure to appropriate magnetic resonance can beneficially modulate the balance of autonomic nervous system that are responsible for sympathetic overflow, and that there is an effective window of induced electrical magnetism in which sympathetic function can be controlled in the absence of side effects.

Seasonal acclimation in sudomotor function evaluated by QSART in healthy humans

  • The quantitative sudomotor axon reflex testing (QSART) is a classic test of routine postganglionic sudomotor function. We investigated sudomotor function by QSART after summer (July 2012) and winter (January 2013) seasonal acclimation (SA) in the Republic of Korea. QSART with acetylcholine (ACh) iontophoresis were performed to determine directly activated (DIR) and axon reflex-mediated (AXR1, 2) sweating rate. Onset time of axon reflex, activated sweat gland density (ASGD), activated sweat gland output (ASGO), tympanic and skin temperatures ($T_{ty}$, $T_{sk}$), basal metabolic rate (BMR), and evaporative loss volume changes were measured. Tympanic and mean body temperature (${\bar{T}}_b$; calculated from $T_{ty}$, $T_{sk}$) were significantly lower after summer-SA than that of winter-SA. Sweat onset time was delayed during winter-SA compared to that after summer-SA. BMR, AXR(1), AXR(2), and DIR sweat rates, ASGD and ASGO, and evaporative loss volume were significantly diminished after winter-SA relative to after summer-SA. In conclusion, changes in sweating activity measured by QSART confirmed the involvement of the peripheral nervous system in variation of sudomotor activity in seasonal acclimation.

Clinical Trial to Evaluate the Hot Flush Relief Efficacy and Safety of Yiseontang-gami in Climacteric Women with Hot Flushes (갱년기 여성의 안면홍조에 대한 이선탕가미방(二仙湯加味方)의 증상완화 효과 및 안전성 평가를 위한 임상연구)

  • Purpose: To evaluate the hot flush relief efficacy of Yiseontang-gami in climacteric women with hot flushes, a vasomotor symptom. Methods: The 20 subjects who signing on the clinical trial written consent by self-will is registered this clinical trial after decided suitable by selection and exception standard, after take a medical experiment and checkup according to clinical trial plan. Registered subject should valuated by settled schedule after take the testing medicine(Yiseontang-gami)during thirty-day. The evaluating indexes of this trial are hot flush VAS, hot flush frequency, hot flush consistence time, sweating VAS, palpitation VAS, sleep disturbance VAS, MRS, MENQOL, PGA. Results: The results were as follows 1. 4 subjects dropped out of the clinical trial and 16 subjects completed it. 2. After Yiseontang-gami treatment, hot flush VAS, hot flush frequent, sweating VAS. Palpitation VAS, sleep disturbance VAS improved significantly. 3. After Yiseontang-gami treatment, hot flush consistence time was not improved significantly. 4. After Yiseontang-gami treatment, blood test value were not different significantly. Conclusion: In this clinical trial we consider that Yiseontang-gami is suitable treatment for the hot flushes and related symptoms.

Thermal Insulation of Protective Clothing Materials in Extreme Cold Conditions

  • Background: Thermophysiological comfort in a cold environment is mainly ensured by clothing. However, the thermal performance and protective abilities of textile fabrics may be sensitive to extreme environmental conditions. This article evaluated the thermal insulation properties of three technical textile assemblies and determined the influence of environmental parameters (temperature, humidity, and wind speed) on their insulation capacity. Methods: Thermal insulation capacity and air permeability of the assemblies were determined experimentally. A sweating-guarded hotplate apparatus, commonly called the "skin model," based on International Organization for Standardization (ISO) 11092 standard and simulating the heat transfer from the body surface to the environment through clothing material, was adopted for the thermal resistance measurements. Results: It was found that the assemblies lost about 85% of their thermal insulation with increasing wind speed from 0 to 16 km/h. Under certain conditions, values approaching 1 clo have been measured. On the other hand, the results showed that temperature variation in the range (-40℃, 30℃), as well as humidity ratio changes (5 g/kg, 20 g/kg), had a limited influence on the thermal insulation of the studied assemblies. Conclusion: The present study showed that the most important variable impacting the thermal performance and protective abilities of textile fabrics is the wind speed, a parameter not taken into account by ISO 11092.

Long Term Outcome of Endoscopically Clipping the Upper Part of R4 Sympathetic Block and R4 Sympathetic Block for the Treatment of Palmar Hyperhidrosis (수장부 다한증에서 Clipping에 의한 흉부4번 교감신경절 상부 차단술과 흉부4번 교감신경절 완전 차단술에 대한 장기성적 비교)

  • Background: Thoracic sympathetic block surgery is a safe and effective procedure for palmar hyperhydrosis, and this maintains sufficient moisture and prevents compensatory hyperhidrosis. To avoid compensatory hyperhidrosis, the authors performed sympathetic block surgery just above the R4 level to maintain sympathetic tone affecting the caudal area. Material and Method: A total of 71 subjects (45 males and 26 females) were categorized into two groups. Group 1 (31 patients, mean age: 25.5 years) had clips placed both on the upper and lower part of R4 sympathetic ganglion, and group 2 (40 patients, mean age: 25.9 years) underwent clipping of the upper part of R4. Telephone surveys were done to collect data on 8 categories, and the average follow up interval was 24.9 months (group 1) and 18.9 months (group 2). Result: For group 1, 41.9% experienced no sweating and 48.4% re-plied they experienced some sweating depending on the surrounding conditions. Group 2 showed that 60% experienced no sweating and 35% replied they experienced some sweating depending on the surrounding conditions, 58.1% in group 1 experienced sweating right after the surgery, and 40.0% in group 2 experienced the same. Group 1 (38.1%) and group 2 (37.5%) replied they experienced no hand dryness and more patients in group 2 than in group 1 had hand dryness, but without uncomfortable symptoms. 71.0% (group 1) and 62.5% (group 2) replied they had no compensatory hyperhidrosis or related symptoms. One patient in group 1 and two in group 2 reported they regretted undergoing the procedure. The regions of compensatory hyperhidrosis were the back, thigh and chest in group 1 and the group 2 reported the back, chest, and abdomen in the order of frequency. Fewer incidences of the gustatory hyperhidrosis were noted in group 2. Most of the patients were satisfied with their treatment. Conclusion: Clipping the upper part of the R4 ganglion or R4 sympathetic block are both effective for treating palmar hyperhidrosis and these treatments decrease the occurrence or symptoms of compensatory hyperhidrosis. The upper R4 sympathetic block procedure is easier and safer with fewer incidences of gustatory hyperhidrosis and a higher percentage of patient satisfaction.

Three Cases of Menopausal Hot Flush and Sweating Treated by Ascending Kidney Water and Descending Heart Fire (AKDH) Pharmacopuncture Treatment (약침을 이용한 수화조절법으로 호전된 갱년기 상열감 및 발한과다 환자 치험 3례)

  • Objectives : The purpose of this study is to evaluate the clinical effects of ascending kidney water and descending heart fire pharmacopuncture treatment for hot flush and sweating. Methods : Ascending kidney water and descending heart fire pharmacopuncture is achieved by injecting pharmacopuncture on a specific acupoint to change from the state of water-fire disharmony to harmonious state. Hwangryunhaedok-tang (黃連解毒湯) pharmacopuncture is injected on both side Gyeonjeong (GB 21 ), Pungji (GB 20 ). Total 0.4 cc was injected by 0.1 cc each point. BUM (Bear’s gall bladder, ox bezoar and musk) pharmacopuncture is injected on both side Jeonjung (CV 17 ), Jungwan (CV 12 ), Gihae (CV 6 ). Total 0.15 cc was injected by 0.05 cc each point. Treatment was done daily. The method of evaluation are Hot Flush Score (HFS), VAS and sweating areas. Results : In case 1, after treatment hot flush score was reduced from 24 to 4 points. VAS scale was reduced from 7 to 2. It took about two weeks for the symptoms to decrease by half. Associated symptoms almost did not stay at discharge. In case 2, after treatment hot flush score was reduced from 28 to 2 points. VAS scale was reduced from 10 to 3. And emotional symptoms were reduced about 70%. In case 3, after treatment hot flush score was reduced from 8 to 1 points. VAS scale was reduced from 6 to 1. And headache and chest discomfort symptoms have disappeared. Conclusions : Ascending kidney water and descending heart fire pharmacopuncture treatment is effective for improve ascending kidney water and descending heart fire energy. Therefore, it will be used to alleviate hot flush and sweating.

Clinical Results According to the Level and Extent of Sympathicotomy in Essential Hyperhidrosis (본태성다한증에서 흥부교감신경의 차단 범위와 부위에 따른 임상결과)

  • Video-assisted thoracic sympathicotomy is a safe and effective therapy for the treatment of essential hyperhidrosis with immediate symptomatic improvement. However, this is offset by the occurrence of a high rate of side effects, such as embarrassing compensatory hyperhidrosis. Therefore, by comparing and assessing the degree of symptomatic improvement or compensatory sweating following sympathicotomy at various levels and the extent of block, we are to determine the optimal level of sympathicotomy and which method will result in minimal side effects and maximal benefits. Material and Method: From January 1998 to June 2001, the thoracoscopic sympathicotomy was performed in 150 patients suffering from essential hyperhidrosis in the Dept. of Thoracic and Cardiovascular Surgery, Wonkwang University Hospital. The patients were divided into three groups. GroupI(n=50): patients having undergone 72,3,4 sympathicotomy, GroupII (n=50): patients having undergone 72 sympathicotomy which consist of blocking the interganglionic neural fiber on the second rib, and group 111(n=50): patients having undergone 73 sympathicotomy which consist of blocking the interganglionic neural fiber on the third rib. The parameters were composed of the satisfaction rate of treatment, the degree of compensatory sweating, postoperative complications, and changes of plantar sweating. Results: There was no difference in age and sex among the groups. All of the treated patients obtained satisfactory alleviation of essential hyperhidrosis in immediate postoperative period. However the rate of long-term satisfaction were 80%, 92%, and 96% in groupsI,II, and III respectively(p<0.05). More than embarrassing compensatory hyperhidrosis was present in 50%, 28%, and 18% in groups I,II ,and III respectively(p<0.05). Slight but comfortable amounts of palmar humidness was expressed in decreasing order, group III(34%), groupII(6%), and group I(4%) respectively(p<0.05). In regard to plantar sweating, decrease in sweating was expressed in each of the three groups, but was not significant between the groups.

  • 1. Objectives This study is purposed to classify deficiency syndrome(虛證) and Excess syndrome(實證) of chief lung-disorder diseases. 2. Methods It was researched on the comparative and literal study about the relation to Yeong-Chu Gyeong-maek Chapter (${\ulcorner}$靈樞 經脈編${\Ircorner}$) and Dongui-Bogam (${\ulcorner}$東醫寶鑑${\Ircorner}$) and Dongui-Susebowon (${\ulcorner}$東醫壽世保元${\Ircorner}$) in chief lung-disorder symptoms or diseases. 3. Results and Conclusions (1) The chief lung-disorder diseases of Yeong-Chu Gyeong-maek Chapter (${\ulcorner}$靈樞 經脈編${\Ircorner}$), Dongui-Bogam (${\ulcorner}$東醫寶鑑${\Ircorner}$) and Dongui-Susebowon (${\ulcorner}$東醫壽世保元${\Ircorner}$) are asthma(喘), coughing(咳), fullness in the chest(胸滿), sweating symptoms(汗出), cold-heat symptoms(寒熱), dysphoria(煩躁) and sneezing. (2) Asthma symptom(喘) was shown to Soeumin's 4 exterior deficiency diseases, Taeumin's 1 exterior deficiency disease and 1 interior excess disease, and Soyangin's 1 interior excess disease. (3) Coughing symptom(咳) was shown to Soeumin's 1 extrerior deficiency disease and Taeumin's 1 interior excess disease. (4) Fullness in the chest(胸滿) was shown co Soyangin's 4 exterior excess diseases. (5) Sweating symptoms(汗出) was shown to Soeumin's 10 exterior deficiency diseases, Taeumins's 1 exterior deficiency disease, and Soyangin's 1 exterior defecieny disease. (6) Cold-heat symptoms(寒熱) was shown to Soyangin's 4 exterior excess diseases. (7) Dysphoria(煩躁) was shown co Soeumin's 1 exterior deficiency disease and 8 interior deficiency diseases, Taeumin's 1 interior excess disease, and Soyangin's 4 exterior excess and interior excess diseases. (8) Sneezing symptom was shown to Taeumin's 1 exterior excess disease.

Comparison Between T2 and T2.3 Thoracic Sympathetic Block in Palmar Hyperhidrosis (수장부 다한증에서 제 2번 및 제 2,3번 흉부 교감신경절 차단술의 비교)

  • Background: Thoracoscopic sympathetic block in palmar hyperhidrosis has merits in its immediate responsiveness and recovery. In palmar hyperhidrosis, the level of sympathetic chain to be blocked has been somewhat obscure. Materials and methods: To compare the results of T2 with T2,3 sympathetic block, we retrospectively studied 192 patients (T2 group: 84, T23 group: 108) operated on at SNUH with palmar hyperhidrosis between April 1994 and July 1997. We reviewed medical records and recently interviewed the patients by telephone call. Sex and age distribution between two groups showed no significant differences. We performed sympathectomy at the early phase of the syudy until April 1997, and after then, we adopted sympathicotomy rather than sympathectomy. Results: All patients showed symptomatic improvement after the operation. Mean operation times of T2, T23 groups were 61.3$\pm$22.5min, 82.7$\pm$24.8min, respectively(p<0.01). Early postoperative complications, such as Horner's syndrome or chest tube insertion, were not different in two groups. There were no statistical differences of late complications such as compensatory truncal hyperhidrosis, gustatory sweating, and phantom sweating. No patient experienced recurrence of palmar hyperhidrosis during the study period. The only difference was the extent of compensatory truncal hyperhidrosis. The compensatory sweating occurred from axilla to suprapatella in T2 group whereas its extent was from nipple to suprapatella in T23 group. Conclusions: We concluded that T2 thoracic sympathetic block is mandatory for the treatment of primary palmar hyperhidrosis.
