A Case of a Holstein Cow with Eyelid Carcinomatosis (Holstein 유우의 안검암종 증레)

  • A ten-year-old holstein cow was presented because of prolapse of the third eyelid and apparent hyperplasia of the right lower eyelid. Historical findings included increased appetite as well as polyuria and polydipsia for about two weeks. The most remarkable fadings on physical examination were a large periocular proliferative tissue and bleeding. Surgical incision was used both as a biopsy and therapeutic tool in holstein cow with mass. Histopathological examination of the mass revealed squamous cell carcinoma. Blood and milk tests of patient with squamous cell carcinomatosis were peformed, but normal values.

  • Endoscopic ultrasonography (EUS) has been developed for the diagnosis of various gastrointestinal diseases or extra-gastrointestinal diseases, and extended to interventional approach. In fields of mediastinal diseases, EUS is an effective diagnostic method for the staging of esophageal cancer, lung cancer and other mediastinal diseases. Recently, EUS guided fine needle aspiration or biopsy makes the histologic diagnosis possible in various mediastinal diseases, and would reserve many avoidable surgical cases.

Pleural Mesothelioma [Report of 2 cases] (늑막중피세포종 2례 보고)

  • Pleural mesothelioma which arise from pleura is relatively uncommon tumor. We are reporting 2 cases of pleural mesothelioma which were treated with surgical resection. First case, benign epithelial mesothelioma was confirmed incidentally after decortication due to localized pleural thickening. The second case, malignant mesothelial mesothelioma was diagnosed by examination of chest radiology, diagnostic pneumogram and pleural biopsy as malignancy which was treated with the resection of the tumor mass. In the first case, postoperative recurrence of tumor growth was found within 1.5 months after resection. In the section malignant case, no evidence of recurrence was noted even after 3 months of resection.

  • Between 1981 and 1987, five patients with an interruption of the aortic arch were operated upon. All had a ventricular septal defect and a patent ductus arteriosus as associated anomalies. A two-stage procedure was employed in these cases, the initial procedure being repair of the interrupted arch, ligation of the patent ductus arteriosus, banding of the main pulmonary artery and a lung biopsy which was followed, 5 to 49 months later, by the repair of the ventricular septal defect. Four patients completed the two-stage procedure with one postoperative mortality. The remaining patient is yet to complete the second stage procedure.

  • Oral leukoplakia is the most common premalignant lesion and malignant transformation has been reported from verrucal lekoplakia. Homogenous, benign leukoplakia develops into a line of squamous cell carcinoma such as verrrucous carcinoma, papillary squamous cell carcinoma and invasive squamous cell carcinoma. Early diagnosis and intervention of premalignant leukoplakia is up-most important to prevent transformation into a oral squamous cell carcinoma. Any change in surface, size and color warrants repeated biopsy. If verrucous carcinoma is evidently derived from the previous leukoplakia, wide surgical excision and periodic follow up is needed. Surgically removed lesion of leukoplakia has the tendency to recur. Follow-up is very important to patient and clinician. Although many therapies have been reported to oral leukoplakia and verrucal carcinoma, accepted treatment principle is not exist so far. But surgical removal is recommended as the treatment of choice.

  • Distant metastasis of squamous cell carcinoma from the anal canal is an uncommon event. However, hematogenous spread to the vertebrae may occur in the course of this disease. The route of metastasis from the anal canal seems to be Batson's vertebral venous system. A 52-year-old female patient presented with lower back and right leg pain of one-week history. She has undergone radiotherapy and chemotherapy for squamous cell carcinoma of the anal canal and then was followed by surgical resection. Three months later, magnetic resonance images of the lumbar spine disclosed a well-enhanced mass of L5 vertebral body compressing the thecal sac. Surgical decompression and biopsy were performed. Histopathological study confirmed carcinoma of the squamous cell origin. We report a ra re case of vertebral metastasis from squamous cell carcinoma of the anal canal with a pertinent review of literature.

Primary Thoracic Neuroblastoma in Children (소아의 원발성 흉부 신경아세포증)

  • Background: Neuroblastoma is the third most common malignancy of chidhood, and is the most common mediastinal mass in children under the age of 2 years. However, the results of surgical treatment have been seldomly reported in Korea. Therefore, we analyzed the results of surgical treatment in children with neuroblastoma and its influencing factors. Material and Method: We studied the clinical characteristics and prognosis of 12 children, 11 makes and 1 female, whose primary thoracic neurobalstomas or ganglioneuroblastomas were diagnosed and operated between 1977 and 1997. Men age at presentation was 29.9 months. Result: Respiratory symptoms were the modes of performed in 9 patients. Complete excision, partial excision, and biopsy only were performed in 9, 2, and 1 patients respectively. Ten patients of thoractic neuroblastomas survived (83.3%) during follow-up period. Conclusion: The postoperatve 5-year survival of thoracic neuroblastoma was 76.4% and the prognosis was related to the stage of neuroblastoma. We suggest that complete resection should be considered as preferential method in the treatment of thoracic neuroblastoma in children, especially with early stage.

A Study on Surgical Treatment of Thymoma -Analysis of 18 Cases- (흉선종의 외과적 치료에 대한 연구 -18례 분석-)

  • Eighteen patients with thymoma have undergone surgical treatment at Pusan National university Hospital from 1983 to 1993. The mean age of the patients was 47.3$\pm$13.3 years old and male to female ratio was 1.6 : 1. Eleven of the patients[61.1%] with thymoma had myasthenia gravis; 12 cases[66.7%] were encapsulated and 6[33.3%] invasive. Operation was complete resection in 14[77.8%] of the patients, partial resection with residual tumor in 3[16.7%], and simple biopsy in one[5.5%]. Adjuvant radiotherapy was administered in 5 of 6 invasive thymoma. Postoperatively one patient died for excessive bleeding and sepsis. A tumor relapse was observed in one patient who had invasive thymoma and underwent partial resection followed by postoperative radiotherapy. The overall 5-year survival rate was 88.2$\pm$ 7. 9%.The presence of myasthenia gravis is not considered as an adverse factor for survival but it contributes to early discovery of associated thymoma.

  • Neurilemmoma is a benign and slowly growing neurogenic tumor. Intrathoracic neurilemmoma often develops in the chest wall and posterior mediastinum, but endobronchial neurilemmoma is extremely rare. The diagnosis of endobronchial neurilemmoma with preoperative imaging findings is challenging and is usually made via postoperative pathological examination. These authors encountered a case of primary endobronchial neurilemmoma in a 52-year-old woman who had no symptoms. A $3.0{\times}2.6$ cm mass in the right lower lobe projecting into the mediobasal segmental bronchus was shown in the results of the contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) of the chest. Benign neurilemmoma was confirmed via bronchoscopic biopsy, and surgical resection (sleeve bronchial excision and end-to-end anastomosis) was performed.

Useful of Magnetic Resonance Imaging in the Diagnosis and Surgical Plan of Intraperineal Lipoma in a Dog (자기공명영상이 개의 회음부 지방종의 진단 및 수술방법에 유용한 1예)

  • A 5-year old female shih-tzu referred to the veterinary Teaching Hospital, College of Veterinary Medicine, Konkuk University. The patient was presented with a mass of ventral to pubic region and intraperineal region. CBC, chemistry, x-ray, fine needle aspiration, biopsy and MRI was performed. The dog was definitively diagnosed as lipoma based on cytologic and histopathologic examination. In order to remove the mass, an approach with two direction was made. First, intraperineal region was removed about 40%, and then ventral to pubic was removed the remainder. And inserted penrose drain for obliterate dead space to prevent the accumulation of serous discharge. The dog is clinically normal and being monitored for development of any abnormal signs for the time being. In this case, diagnostic MRI was very useful procedure. it revealed accurate location of a lipoma and useful manner for decision of surgical approach to remove.
