현종(懸鍾)($GB_{39}$).양릉천(陽陵泉)($GB_{34}$)의 전침(電鍼) 및 레이저침이 백서(白鼠)의 신경병리성(神經病理性) 동통(疼痛)에 미치는 영향 (Effects of Electro and Laser Acupuncture Treatment with $GB_{39}$ and $GB_{34}$ on Neuropathic Pain in Rats Induced by Tibial and Sural Nerve Ligation)

  • Objectives : We studied the effects of electro and laser acupuncture treatment with $GB_{39}$ and $GB_{34}$ on neuropathic pain in rats induced by tibial and sural nerve ligation. Methods : To produce the model of neuropathic pain, the tibial and sural nerves of rats were ligated by a 6-0 silk thread. Three days after the neuropathic surgery, only electro acupuncture(EA), electro acupuncture and 830 nm laser acupuncture(EA-LA-1), and electro acupuncture and 904 nm laser acupuncture(EA-LA-2) were treated with $GB_{39}$ and $GB_{34}$ twice a week for 8 weeks. We observed the withdrawal response of neuropathic rats' legs by von Frey filament and acetone stimulation. We also observed c-fos and nocieptin on the central gray area in the midbrain of neuropathic rats. Results : As we observed the effect of mechanical allodynia, the EA and EA-LA-1 groups in 5 and 6 weeks and the EA-LA-2 group in 6 weeks increased significantly compared with the control group. As for the effect of c-fos activity in the central gray region, the EA, EA-LA-1, and EA-LA-2 groups decreased significantly compared with the control group. The EA-LA-2 group increased significantly compared with the control group as regards the effect of nociceptin activity in the central gray region. Conclusions : We noticed the synergic effect of electro and laser acupuncture treatment because the EA-LA-1 and EA-LA-2 groups had more controllable effect compared with the control group. This study can be used in clinical therapy for neuropathic pain.

상완골 골절에 동반된 요골 신경 마비 환자에서 초음파 검사의 유용성 - 술기 보고 - (Usefuless of Ultrasonography Examination in Radial Nerve Palsy Associated with Humerus Fracture - Technical Report -)

  • 상완골 골절에 동반된 요골 신경 마비는 전체 상완골 골절의 약 10%에서 발생하는 것으로 보고되고 있으며, 상완골 골절 수술과 관련하여 발생하는 의인성 요골 신경 마비는 10~20%로 알려져 있어 임상에서 흔히 경험하게 된다. 상완골 골절에 동반된 원발성 요골 신경 마비나 의인성 요골 신경 마비가 발생한 경우 요골 신경이 단열된 것인지 아니면 주위 구조물에 압박된 것인지 등의 요골 신경의 해부학적 상태의 평가는 자연회복을 기다릴 것인지 아니면 수술적 탐색술을 실시할 것인지 여부를 결정하는데 중요하다. 저자들은 상완골 골절에 동반된 원발성 요골 신경 마비 1예와 수술과 관련한 의인성 요골 신경 마비 1예를 초음파 검사로 신경의 해부학적 상태를 확인한 후 신경의 단열이 있었던 1예는 비복신경 이식술을 시행하였고, 신경유착이 있었던 1예는 신경박리술을 시행하여 만족할 만한 임상결과를 얻었기에 초음파를 이용한 요골 신경 검사를 중심으로 문헌 고찰과 함께 보고하고자 한다.

정상성인의 신경전도속도에 관한 연구 (A Study of Nerve Conduction Velocity of Normal Adults)

  • 건강한 성인 83명을 무작위로 선정하여 실시한 신경전도속도 검사에서 다음과 같은 결과를 얻었다. 1) 상지의 정중신경에서는 운동신경의 TL이 3.0-4.2msec 이고, MNCV는 각각 52.1-70.3m/sec(W-E), 44.6-71.0m/sec(E-Ax), 56.6-70.8m/sec(W-E), 50.5-75.1m/sec(E-Ax)이며, CNAP의 진폭은 $6.5-46.1{\mu}V$였다. 2) 척골신경에서는 운동신경의 TL이 2.4-3.4msec이고, MNCV는 각각 54.6-72.8m/sec(W-E), 41.1-64.9m/sec(E-Ax)이며, 진폭은 3.1-12.0mV였다. 척골신경의 MNCV는 각각 31.1-44.7m/sec(F-W)m, 55.9-70.9m/sec(W-E), 46.9-67.1m/sec(E-Ax)이며, CNAP의 진폭은 4.8-42.9${\mu}V$범위였다. 3) 요골신경에서는 운동신경의 TL이 1.9-2.7msec이고, MNCV는 53.1-77.5m/sec(W-E)이며, CMAP의 진폭은 1.1-6.6mV범위였다. 요골신경의 SNCV는 각각 38.5-52.1m/sec(F-V), 53.2-75.2m/sec(W-E) 이며, CNAP의 진폭은 $2.5-9.2{\mu}V$범위였다. 4) 하지의 비골신경에서는 운동신경의 TL이 3.5-5.7msec이며, MNCV는 각각 44.4-58.6m/sec(A-FH), 42.8-65.8m/sec(FH-PF)이며, CMAP의 진폭은 0.6-12.7mV 범위였다. 5) 후경골신경에서는 TL이 4.0-6.2m/sec이며, MNCV는 40.6-60.6m/sec이며, CMAP의 진폭은 3.9-29.2mV범위였다. 6) 비골신경의 SNCV는 37.5-49.5m/sec이며, CNAP의 진폭은 $0.7-17.1{\mu}V$범위였다. 7) H-반사의 평균 잠복기는 28.4msec였다.

신경병리성 통증과 포르말린 테스트 통증 모델을 이용한 오가피(五加皮)의 효과 (The effect of Acanthopanax sessiliflorus using the model of neuropathic pain and formalin-induced pain.)

  • Objectives : This study was conducted to determine the analgesic effect of Acanthopanax sessiliflorus using the model of neuropathic pain and formalin-induced pain. Methods : A model of neuropathic pain was made by injuring the tibial nerve and sural nerve while the common peroneal nerve was maintained. After 2 weeks, the Acanthopanax sessiliflorus was orally administered to rats. The author performed behavioral teststo try out mechanical allodynia using von frey filament and cold allodynia using acetone, which are calculated by counting withdrawal response on foot. Thirty minutes after the Acanthopanax sessiliflorus injection in the abdominal cavity, the formalin test was performed. 2% formalin in a volume of $20{\mu}l$was injected subcutaneously into the plantar surface of the hindpaw with 26-G needle. To access formalin-induced pain behavior, paw licking time was measured every 5 min. Results : The Acanthopanax sessiliflorus 400mg/10ml/kg group showed significant decrease the withdrawal response of mechanical allodynia using von frey filament in the 10min, 30min, 60min and 120min increments compared with the control group. There were no significant differences in each group in the withdrawal response of cold allodynia using acetone. The Acanthopanax sessiliflorus group showed significant decrease in the formalin-induced pain behavior in the 15min, 20min and 25min increments compared with the control group. Conclusions : The Acanthopanax sessiliflorus may have a significant analgesic effect on the general pain as well as nerve injury pain.

A Case of Cauda Equina Syndrome in Early-Onset Chronic Inflammatory Demyelinating Polyneuropathy Clinically Similar to Charcot-Marie-Tooth Disease Type 1

  • To present a case of cauda equina syndrome (CES) caused by chronic inflammatory demyelinating polyneuropathy (CIDP) which seemed clinically similar to Charcot-Marie-Tooth disease type1 (CMT1). CIDP is an immune-mediated polyneuropathy, either progressive or relapsing-remitting. It is a non-hereditary disorder characterized by symmetrical motor and sensory deficits. Rarely, spinal nerve roots can be involved, leading to CES by hypertrophic cauda equina. A 34-year-old man presented with low back pain, radicular pain, bilateral lower-extremity weakness, urinary incontinence, and constipation. He had had musculoskeletal deformities, such as hammertoes and pes cavus, since age 10. Lumbar spine magnetic resonance imaging showed diffuse thickening of the cauda equina. Electrophysiological testing showed increased distal latency, conduction blocks, temporal dispersion, and severe nerve conduction velocity slowing (3 m/s). We were not able to find genetic mutations at the PMP 22, MPZ, PRX, and EGR2 genes. The pathologic findings of the sural nerve biopsy revealed thinly myelinated nerve fibers with Schwann cells proliferation. We performed a decompressive laminectomy, intravenous IgG (IV-IgG) and oral steroid. At 1 week after surgery, most of his symptoms showed marked improvements except foot deformities. There was no relapse or aggravation of disease for 3 years. We diagnosed the case as an early-onset CIDP with cauda equine syndrome, whose initial clinical findings were similar to those of CMT1, and successfully managed with decompressive laminectomy, IV-IgG and oral steroid.

국소적 전도차단을 보인 n-Hexane induced neuropathy 1례 (A Case of n-Hexane Induced Neuropathy with Focal Conduction Block)

  • A 32-year-old man who had worked at aluminum processing plant for 4 months visited us. He complained of numbness and paresthesia of both foot and hands and weakness of all extremities. Electrophysiologic study showed motorsensory polyneuropathy of diffuse axonal type with focal conduction block. And we discovered higher concentration of n-Hexane in his workplace. On sural nerve biopsy, bubbly enlarged nerve fibers in light microscope and thick myelin sheath and axonal degeneration on electron microscope were found. We diagnosed it as n-Hexane induced neuropathy.

이상감각과 통증을 호소하는 당뇨병 환자의 전기생리학적 특징 분석 (Electrophysiological Analysis in Diabetic Patients with Abnormal Sensation and Pain)

  • 당뇨병 환자들은 말초신경 손상의 합병증으로서 이상감각과 통증을 느낄 수 있는데 이는 날카로운 느낌, 차갑고 시린 느낌, 무딘 느낌, 살갗이 벗겨진 듯한 예민한 느낌, 화끈거리는 느낌, 따끔하고 가려운 느낌 등 다양하게 나타날 수 있다. 이를 진단하기 위해서 여러 선별검사와 전기생리학적 검사를 활용할 수 있다. 본 연구에서는 신경병통증척도와 미시간 신경병증 선별도구 및 신경전도검사를 통해 이상감감/통증을 호소하는 당뇨병 환자 11명과 호소하지 않는 당뇨병 환자 10명 간에 차이점 및 특징을 분석하였고, 세부 감각양상과 신경전도검사 지표간 상관성을 분석하였다. 그 결과 이상감각/통증을 호소하는 환자군에서 신경병통증척도와 미시간 신경병증 선별검사의 점수가 통계적으로 유의하게 높았고 신경전도검사에서는 주로 정중신경 운동신경과 비골신경이 의미 있는 기능 변화를 보였다. 세부 감각양상에 따른 신경전도검사 지표는 주로 정중신경 운동신경, 후경골신경, 장딴지신경에서 유의한 변화를 보였다. 이러한 결과들은 감각이상과 통증이 실제 말초신경 이상과 관련이 있음을 시사한다. 향후 많은 수의 대상자를 포함한 추가적인 연구가 필요하다.

종골의 부정 유합 (Calcaneal Malunion)

  • Calcaneal fractures account for 2% of all fractures and approximately 60 to 70% of tarsal fractures. These fractures typically are the result of high-energy injury, such as a motor vehicle accident or a fall from a height. The potential for disabling malunion following intrarticular displaced calcaneal fracture is high, regardless of treatment. Fracture displacement typically results in loss of hindfoot height, varus and widening of the hindfoot, with possible subfibular impingement and irritation of the peroneal tendon and/or sural nerve. Frequently, subtalar joint develops posttraumatic arthritis. In symptomatic patients with calcaneal malunion, systemic evaluation is required to determine the source of pain. Nonsurgical treatment, such as activity and shoe modification, bracing, orthoses, and injection, is effective in many patients. Surgical treatment may involve simple ostectomy, subtalar arthrodesis with or without distraction, or corrective calcaneal osteotomy. A high rate of successful arthrodesis and of patient satisfaction has been reported with surgical manamgent.

고유감각소실을 주증상으로 보인 나병성 말초신경병증 (Leprous Neuropathy Associated with Predominant Proprioceptive Sensory Loss)

  • Some patients with leprosy may present with atypical features, such as isolated peripheral neuropathy without skin lesions, or marked proprioceptive dysfunction. We report a 56-year-old female who presented with predominant proprioceptive loss without skin lesion, but was finally confirmed as leprous neuropathy by sural nerve biopsy. It is postulated that large myelinated fibers were affected by chronic immunological reactions triggered by inactive bacterial particles, producing a peripheral neuropathy presenting as predominant proprioceptive sensory loss without typical skin lesions.

사르코이드증에 동반된 다발신경병증 (Sensorimotor Polyneuropathy Associated with Sarcoidosis)

  • Sarcoidosis is a multisystemic granulomatous disease of unknown etiology ogenesis, and most frequently presented with bilateral hilar lymphadenopathy, pulmonary infiltration, skin and eye lesion. However, neurological involvement including peripheral neuropathy is relatively rare. We report a patient who had sensorimotor polyneuropathy without other systemic symptoms or organ involvements frequently reported in sarcoidosis. Laboratory investigation suggestive of sarcoidosis lead to sural nerve biopsy for confirmation, which demonstrated noncaseating granulomatous changes. Sarcoidosis shoud be included in the differential diagnosis in subacute polyneuropathy even if there is no usual symptoms or signs suggestive of the systemic disease.
