An association of low total cholesterol in blood with psychiatric diseases and suicidal behavior has been suggested. As part of an attempt to further explore this relationship, we examine first, whether serum cholesterol levels in psychiatric patients with suicidal attempt would be lower than in non-suicidal psychiatric inpatients or normal controls, second, whether such significant difference of cholesterol levels would be present when the diagnostic groups are analyzed separately, third, whether low cholesterol level would be associated with a history of serious suicidal attempts, and finally, whether low cholesterol level in suicide attempters is a state or a trait marker. We determined the serum cholesterol levels in 231 patients admitted to an emergency room following an suicidal attempt, in the same numbers of age-, sex- and diagnosis- matched non-suicidal psychiatric controls, and in the same numbers of age-, sex matched normal controls. The seriousness of an attempt was divided into 5 grades according to the degree of the resulting medical injury. Total cholesterol levels in suicide attempters were significantly lower compared with both psychiatric and normal controls, when sex, age, and nutritional status (i.e., body mass index) were controlled for. This significant relationship was observed in major depressive disorders and personality disorders, but not in schizophrenia and bipolar type I disorders. The severity of suicide by a lowering of blood cholesterol was related to the magnitude of the cholesterol reduction. After treatment of their psychiatric ailments, the cholesterol levels in suicide attempters were significantly increased. This result suggests that low cholesterol level in psychiatric patients might be a potential biological marker of suicide risk. It is hypothesized that low cholesterol levels is associated with the suicide by modifying the serotonin metabolism, the production of interleukin-2 and melatonin metabolism in psychiatric patients.

This study aimed to evaluate the relationship of ADHD, depression, anxiety, self-esteem, impulsivity, suicide idea and quality of life as well as burnout of kindergarten teachers for the first time in Korea. Also, it examined the odds ratio with ADHD, depression, anxiety, self-esteem, impulsivity, suicide idea and quality of life between burnout group and the comparison group through logistic regression analysis. A set of questionnaires was provided to a total of 310 participants (80 with higher burnout risk and 230 without burnout risk) from April 2015 to July 2015. All of participants were evaluated using the Korean Adult Attention Deficit Hyperactivity Disorder Scales, Beck's Depression Inventory, Beck's Anxiety Inventory, Baratte' Impulsivity Scale, Rosenberg's Self-esteem Inventory and Qulity of Life Scale. The results of this study indicated that burnout risk group is related to ADHD and anxiety and impulsivity. For the treatment of burnout in kindergarten teachers, effective evaluation and treatment of ADHD and impulsivity, as well as anxiety, seem to be necessary.
Objects:Some studies have suggested that brain-derived neurotrophic factor(BDNF), one of the most important neurotrophins, is involved in pathophysiology of depression and suicide. This study was performed to determine whether there is an abnormality in plasma BDNF concentration in suicidal attempters. Methods:The subjects were 71 suicidal attempters who visited emergency rooms in multi-medical centers. All subjects had been interviewed by using Structured Clinical Interview for DSM-IV(SCID), Hamilton Depression Rating Scale(HDRS), Young Mania Rating Scale(YMRS), and Positive And Negative Syndrome Scale(PANSS). The severity of the suicidal behavior was measured by Lethality of Suicide Attempt Rating Scale(LSARS) and Risk-Rescue Rating(RRR) system. Seventy-one age, sex, and diagnosis matched non-suicidal psychiatric patients who were consecutively admitted to a psychiatric ward during the same period recruited as psychiatric controls. They were drug-naive or drug-free at least more than 2 months. In addition, 80 healthy controls were randomly selected as normal controls. Plasma BDNF level was measured by the enzyme linked immunosorbent assay(ELISA) methods. Results:In overall F-test, differences of the plasma BDNF levels among the groups were statistically significant(F=20.226, p<0.001). In the multiple comparisons(Scheffe), while mean levels of plasma BDNF between normal controls and non-suicidal psychiatric patients were similar(p=0.984), the BDNF levels of suicidal attempters were lower than those of other two groups(p<0.001). LSARS and RRR did not reveal any significant correlations with BDNF levels in suicidal attempters. Conclusion:These results suggest that reduction of plasma BDNF level is related to suicidal behavior and BDNF level may be a biological marker of suicidal behavior.
Objectives : This study looked at the effects of chronic IPV on suicide ideation among married women in the country. Methods : This study used data from the five-year-old Korea Welfare Panel and analyzed using questions about verbal and physical violence, depression and suicide ideation that were investigated every year. Age, education, and economic levels, drinking, religion, chronic disease, satisfaction with family relationship, satisfaction with social relationship, were included as variables. Results : The five-year cumulative score of verbal violence had a statistically significant effect on the depressive symptom, and the relative risk was 1.180. The relative risk of five-year-old cumulative score of verbal violence in suicidal ideation was 1.276, and it was also significant even if they exclude the effects of depressive symptom. On the other hand, the five-year cumulative score of physical violence was found to have no significant impact on suicide ideation if they excluded the effects of depressive symptom. Conclusions : Unlike previous studies, this study found that chronic verbal IPVs increase suicide ideation, even though they do not contain effects of depression. Such findings suggest that intervention in IPV in married women may help reduce the nation's suicide rate.

Objectives: Deliberate self-poisoning (DSP) is the most common suicide method and can be life-threatening. The purpose of this study was to investigate the factors related to the lethality of DSP and the characteristics of the adolescent group. Methods: A retrospective study was conducted on patients who had visited an academic hospital's regional emergency medical center between 2015 and 2018. The data reviewed through their medical records included sociodemographic factors, clinical variables, and psychiatric treatment. Four groups (Q1-Q4) were categorized by descriptive analysis using the risk-rescue rating scale. Results: A total of 491 patients were enrolled in this study. This study showed that high lethality had statistically significant associations with male sex, older age, admitting suicidal intentions, and the use of herbicides for suicide. Logistic regression analyses showed a significant association between high-lethality and female [odds ratio (OR)=0.50, 95% confidence interval (CI)=0.30-0.81, p=0.01], non-psychiatric drugs (over-the-counter drug: OR=2.49, 95% CI=1.08-5.74, p=0.03; herbicide: OR=8.65, 95% CI=3.91-19.13, p<0.01), and denial of suicide intent (OR=0.28, 95% CI=0.15-0.55, p<0.01). Conclusion: This study showed the clinical factors associated with the high lethality of DSP and suggested that efforts were needed to care for and thoroughly examine patients with DSP.

Purpose : This study examined the factors related to sexual intercourse among adolescents in South Korea. Methods : The study sample was comprised of 65,528 middle and high school students aged 12 to 18 years in 2016. For this study, using the data from the 12th Korea youth risk behavior web-based survey, a chi-squared test, and hierarchical logistic regression analysis we reconducted with the SPSS 22.0 program considering personal, family, school, and regional characteristics. Results : The proportion of adolescents who had sexual intercourse was 6.0 % of boys and 2.7 % of girls, and 6.3 % of high school students and 2.4 % of middle school students. The statistically significant related factors of sexual intercourse were gender, grade, attempted suicide, ever drinking, ever smoking, habitual or purposeful drug use, weekly allowance, and coed school in the final model 4. Adolescents with suicide attempts had 2.49 times more sexual intercourse than adolescents with no experience of suicide attempts. Compared to adolescents without alcohol, smoking, and habitual drug use, those who have experienced alcohol, smoking, habitual or purposeful drug use were 2.22 times, 3.76 times, and 3.39 times more likely to have had sexual intercourse, respectively. Adolescents with a weekly allowance of more than 100,000 won per week and adolescents in coed schools were 2.84 times and 1.40 times more likely to have had sexual intercourse, respectively. Conclusion : Multi-level interventions should be considered in school sexual education programs while considering the substantiality of sex education and the actual adolescents' needs. In addition, sexual health interventions in cyberspace for high-risk groups may be needed to consider the emotional and mental characteristics, given the factors related to sexual intercourse in adolescents.
Objectives: Economic hardship has a serious impact on adolescents' mental health. The financial impact of the coronavirus disease 2019 (COVID-19) pandemic was more severe for low-income families, and this also impacted adolescents. This study aimed to examine the associations of economic deterioration (ED) caused by the COVID-19 pandemic and low socioeconomic status (SES) with adolescents' suicidal behaviors. Methods: This study analyzed data from the 2020 Korea Youth Risk Behavior Web-based Survey, which included 54 948 middle and high school students. Odds ratios (ORs) of suicidal ideation, suicidal planning, and suicide attempts related to ED and SES were calculated using multivariable logistic regression. We calculated relative excess risks due to interaction to assess additive interactions. Results: The ORs for suicidal ideation, suicidal planning, and suicide attempts related to combined severe ED and low SES were 3.64 (95% confidence interval [CI], 3.13 to 4.23), 3.88 (95% CI, 3.09 to 4.88), and 4.27 (95% CI, 3.21 to 5.69), respectively. Conclusions: ED and low SES were significantly associated with suicidal behaviors in adolescents. Although no significant additive interaction was found, the ORs related to suicidal ideation, suicidal planning, and suicide attempts were highest among adolescents from low-income families with severe ED. Special attention is needed for this group, considering the increased impact of economic inequality due to the COVID-19 pandemic.

Purpose : This study was implemented to identify the risk conditions influencing suicidal ideation in a nationally representative sample of elderly South Koreans. Methods : Data from 1,152 men and 1,581 women aged 65 years or older were gathered from the 2013 and 2014 Korea National Health and Nutrition Examination Survey VI. All analyses were performed using SPSS. To determine significant correlations between risk condition and suicidal ideation, a t-test was used. Results : There were differences in suicidal ideation according to the following individual factors: age, educational background, marital status, economic activity, recognition of stress, experience of depression, and economic status. There were differences in suicidal ideation according to the following health-related factors: subjective health status, EQ-5D (EuroQoL-5 Dimensions), hours of sleep, and BMI. There were also differences in suicidal ideation according to the following disease-related factors: HTN, COPD, asthma, stroke, depression and osteoarthritis. Conclusion : The findings indicate that broad intervention programs should be distributed to prevent suicidal ideation. It is also recommended that programs be developed in a way that can help manage the variables identified in this study. Furthermore, follow-up studies should be conducted to verify the program.
Purpose: The purposes of the study were to understand the levels of older adults' perceived stresses in terms of family relationship, physical health, financial difficulty, death of family members, and household environment, and to identify the influences of the stresses on their suicidal ideation. Methods: The subjects were 302 older adults residing in Daegu, Korea. The data were collected by questionnaires between March and April 2007. Results: Older adults perceived the highest level of stress from the death of family members and had high levels of stresses from physical health, family relationships, financial difficulty, and household environment in order. The variables influencing suicidal ideation were stresses from family relationships, physical health, and financial difficulty. Conclusion: Based on the findings, it is recommended that nurses provide in-depth screening of suicide risk for older adults who visit general health care settings through the identification of the levels of stresses in terms of family relationships, physical health, and financial difficulty. It is further suggested to provide collaborative interventions between primary health care providers and psychiatric professionals for suicide prevention and treatment in older adults.
Background : Patients with command hallucinations are commonly assumed to be at high risk for dangerous behavior. However the issue of whether command hallucinations hold any clinical relevance in schizophrenic patients has not been established. Method : The author analyzed the clinical and research records of schizophrenic patients with auditory hallucinations who participated in outpatient research follow-up for 9 months after discharge. Patients with auditory hallucinations were classified as experiencing or not experiencing command hallucinations based on clinical psychiatric assessment. Results : Of 63 patients with auditory hallucinations, 29(46%) reported the command hallucinations and these hallucinations often were violent in content(44.8%). Patients with command hallucinations were not significantly different from patients without command hallucinations on sociodemographic and clinical characteristics, and clinical or prognostic course variables, but patients with command hallucinations had significantly more short hospitalizations(less than one month) than patients without command hallucinations. 3 of the patients with command hallucinations who committed suicide during the follow up periods were died. Conclusion : Command hallucinations may be frequent, and in most cases they have minimal influence on the outcome of schizophrenia, but if the patients with command hallucinations have a history of suicide attempts before admission, the possibility of suicide attempts by command hallucinations should be considered.
