한국인 총당류 섭취기준 설정 (Establishment of Total Sugar Reference Value for Koreans)

  • Sugars are a ubiquitous component of our food supply and are consumed as a naturally occurring component of many foods and as additions to foods during processing, preparation, or at the table. Most fruits and dairy products are high in sugars and thus naturally occurring sugars are consumed as part of a healthy diet. Some countries developed recommended daily intake figures(daily values : DVs or guideline daily amounts: GDA) for nutrients, and some countries, but not most have developed DV/GDA for total sugars. Dietary Reference Intakes for Koreans established by the Korean Nutrition Society in 2005, did not include the reference values for total sugar or added sugar. The committee on Dietary Reference Intakes for sugar was constituted in 2006 and discussed whether to special added sugars or total sugar. Although added sugars are not chemically or physiologically different from naturally occurring sugars, many foods and beverages that are major sources of added sugars have lower micronutrient densities compared with foods and beverages that are major sources of naturally occuring sugars. But it was so hard to calculate a dietary intake of added sugar for Korean people, because there was insufficient information about contents of added sugar during processing or preparation of Korean food. Currently Korean or US food labels contain information on total sugars per serving but do not distinguish between sugars naturally present in food and added sugars. Therefore the committee decided to set the reference value for total sugar for Koreans. According to the recommended diet pattern for Koreans suggested by the Korean Nutrition Society, estimated sugar intake from the sugar containing food based on 2,000 kilocalories is 67 g or 13% of total energy. Based on the data available on risk of obesity, hypertension, hyperlipidemia, insulin resistance, and metabolic syndrome from the analysis of Korean NHANES, it was insufficient evidence to set a UL for total sugar, but tended to increase serum LDL cholesterol and obesity at over 20-25% of energy from total sugar when consumed with high carbohydrates. Therefore the committee on Dietary Reference Intakes for sugar set the Acceptable Macronutrient Distribution Range for total sugar as 10-20% of total energy intake.

Dietary Carbohydrate, Fiber and Sugar and Risk of Breast Cancer According to Menopausal Status in Malaysia

  • Background: Dietary carbohydrate, fiber and sugar intake has been shown to play a role in the etiology of breast cancer, but the findings have been inconsistent and limited to developed countries with higher cancer incidence. Objective: To examine the association of premenopausal and postmenopausal breast cancer risk with dietary carbohydrate, fiber and sugar intake. Materials and Methods: This population based case-control study was conducted in Malaysia with 382 breast cancer patients and 382 controls. Food intake pattern was assessed via an interviewer-administered food frequency questionnaire. Logistic regression was used to compute odds ratios (OR) with 95% confidence intervals (CI) and a broad range of potential confounders were included in analysis. Results: A significant two fold increased risk of breast cancer among premenopausal (OR $Q_4$ to $Q_1$=1.93, 95%CI: 1.53-2.61, p-trend=0.001) and postmenopausal (OR $Q_4$ to $Q_1$=1.87, 95%CI: 1.03-2.61, p-trend=0.045) women was observed in the highest quartile of sugar. A higher intake of dietary fiber was associated with a significantly lower breast cancer risk among both premenopausal ($ORQ_4$ to $Q_1$=0.31, 95%CI: 0.12-0.79, p-trend=0.009) and postmenopausal ($ORQ_4$ to $Q_1$=0.23, 95%CI: 0.07-0.76, p-trend=0.031) women. Conclusions: Sugar and dietary fiber intake were independently related to pre- and postmenopausal breast cancer risk. However, no association was observed for dietary carbohydrate intake.

한국 성인의 총당류 섭취와 대사증후군과의 관계 -2001년과 2002년도 국민건강영양조사자료를 이용하여- (Association of Total Sugar Intakes and Metabolic Syndrome from Korean National Health and Nutrition Examination Survey 2001-2002)

  • The purpose of this study was to establish an association between the percent of energy from total sugar and disease prevalence of obesity, hypertension, dyslipidemia, insulin resistance, and metabolic syndrome with the context of the current population dietary practice in Korea. The Korean National Health and Nutrition Survey, 2001 and 2002 dataset were used as the source of data for this research. Usual nutritional intakes for over 20 years old people were calculated from the two non-consecutive dietary intake data from KNHANES 2001 and 2002 dataset. SAS and SUDAAN were used for statistical analyses. Sample weighted means, standard errors, and population percentages were calculated, and multiple logistic regression model with adjustment for covariates were used to determine the odds ratios(ORs) and 95% confidence intervals. Subjects were categorized as 3 ways and compared the LS means and ORs for heath factors. First, subjects excluding pregnant women, were categorized according to percent of energy from the usual total sugar intakes as ${\leq}10%$, 11-15%, 16-20%, 21-25%, >25%. Risk of LDL cholesterol showed a tendency to increase in the '>25%' group compared to the '<10%' group. The risks of the other health effects did not show any significant differences. Second, the subjects were categorized considering both Acceptable Macronutrient Distribution Range(AMDR) from carbohydrate and %Energy from total sugar as 'CHO<55% & Total sugar ${\leq}10%$', 'CHO 55-70% & Total sugar 11-25%', and '$CHO{\geq}70%$ & Total sugar ${\geq}25%$'. The risk of obesity tended to increase in the '$CHO{\geq}70%$ & Total sugar ${\geq}25%$' group compared to the 'CHO<55% & Total sugar ${\leq}10%$'. Third, the subjects were categorized as 'CHO<55% & Total sugar ${\leq}10%$', 'CHO 55-70% & Total sugar 11-20%', and '$CHO{\geq}70%$ & Total sugar ${\geq}20%$'. The risk of obesity also tended to increase in the '$CHO{\geq}70%$ & Total sugar ${\geq}20%$' group compared to the 'CHO<55% & Total sugar ${\geq}20%$' group. In conclusion, risk of LDL cholesterol showed a tendency to increase in the over 25% total sugar intake group, and the risk of obesity tended to increase in the 20-25% total sugar intake and high carbohydrate intake group. The risks of hypertension, hyperlipidemia, insulin resistance, and metabolic syndrome were not associated with total sugar intakes. More research to elucidate the association for Korean between the intakes of total sugar, added sugar, glucose, fructose, and sweeteners and diseases prevalences shoud be excuted in the future.

유아의 당 저감화를 위한 프로그램의 효과평가 (Evaluation of a Nutrition Education Program Designed to Reduce Sugar Intake in Preschool Children)

  • The purpose of this study was to evaluate a nutrition education program designed to reduce sugar intake in preschool children. The nutritional knowledge, dietary attitude, dietary behavior, and food intake with a sweet taste in preschool children were tested before and after 4 weeks of nutrition education from March 8 to April 1, 2016. The preschool children were recruited in Dobong-gu, Seoul. The subjects were divided into two groups; educated (N=59) and non-educated group (N=40). Statistical analyseis were performed using SPSS 22. The nutritional knowledge about sweet tasting food was significantly increased (P<0.01) in the educated group and significantly decreased (P<0.01) in the non-educated group. The dietary attitude toward sweet tasting food was significantly decreased (P<0.05) in the educated group but not significantly changed in the non-educated group. The dietary behavior toward sweet tasting food was significantly decreased (P<0.05) in the educated group and significantly decreased (P<0.01) in the non-educated group. The major food sources of sugar intake were not significantly changed in either groups. The major food sources of sugar intake in the 5 years old children were yogurt products>sweetened breads>cookies/biscuits>chocolate>cereals>beverages>sweetened milk>sweet spicy chicken>Tteokbokki. Although there are some limitations in this study, due to the short length of the nutrition education and lack of active participation by the parents, the results suggest that effective nutrition education on sweet tasting food can bring about changes in nutritional knowledge as well as dietary attitude.

한국 청소년의 가당음료 섭취 관련 건강 행동 요인 분석 (Health Behavior Factors Associated with Sugar-sweetened Beverage Intake among Adolescents)

  • Objectives: This study was conducted to measure the intake rate of SSBs (sugar sweetened beverages) and examine the relationship between health behavior factors and SSBs intake by adolescents. Methods: This study used data from the 2016 Korea Youth Risk Behavior Web-based Survey, which included 65,528 study participants. SSBs intake frequency was measured by asking respondents if they consumed soda, high-caffeinated beverages, and sugary drinks during the previous week. Type of intake was categorized into three groups according to the number of consumed drinks [SSBs (0): None; SSBs (1-2): 1 or 2 consumed; SSBs (3): 3 consumed]. Multinomial logistic regression analysis was used to examine health behaviors that affected SSBs consumption. Results: Increased SSBs intake was significantly correlated with current smoking (OR=2.4, 95% CI=1.82-3.17), current drinking (OR=2.13, 95% CI=1.82-2.51), sedentary time increase (OR=1.31, 95% CI=1.15-1.49), three days or more physical activity per week (OR=1.12, 95% CI=1.02-1.24), <8 hours sleep (OR=1.6, 95% CI=1.43-1.78), increased internet usage time (OR=1.44, 95% CI=1.25-1.65). Conclusions: Sugar-sweetened beverages intake by Korean adolescents was associated with health behaviors such as smoking, drinking, sedentary time increase, more physical activity, poor sleeping time, and increased internet use time. Based on these results, it is necessary to recognize the influence of SSBs intake and to intervene to reduce consumption of SSBs.

Short-term impact of sugar consumption on hunger and ad libitum food intake in young women

  • The hypothesis of this study was that greater sugar consumption at breakfast promotes a stronger sensation of hunger and a later increase in energy consumption. The objective was to assess the relation between sugar consumption in a meal and the subsequent sensations of hunger and ad libitum food consumption. Sixteen women consumed a breakfast accompanied by 2 drinks sweetened ad libitum with sugar. After 3 h, a lunch was offered to evaluate ad libitum food consumption. During the period from breakfast to lunch, hunger sensations were evaluated at 30 min intervals. Women were divided according to the median amount of sugar used to sweeten the breakfast drinks (20 g). The group who consumed sugar above the median showed a greater hunger sensation in the preprandial period, and a greater ad libitum intake at lunch ($390{\pm}130g{\times}256{\pm}67g$, P = 0.002), compared to the group who had a lower sugar consumption. The amount of sugar consumed at breakfast was correlated positively with the sensation of preprandial hunger and food intake at lunch. We concluded that foods with a high glycemic index can modulate the appetite within a short period of time.

당류 저감화에 대한 소비자 인식 및 태도 (Consumer Perceptions and Attitudes towards Reducing Sugar Intake)

  • 본 연구에서는 당류 저감화에 대한 소비자의 인식 및 태도분석을 통해 국가 및 관련 산업체들이 우리 국민의 당류 섭취를 줄이고 당류 적정 섭취 수준을 유지시키기 위한 가이드라인을 제시할 수 있도록 하는 데 기초자료를 제공하고자 하였다. 당류 저감화 식품의 이용 경험에 대해서는 절반 이상의 소비자가 이용 경험이 있었고, 당 함량을 줄인 식품에 대한 인지가 비교적 높은 것으로 나타났다. 당류 저감화 식품 자체 내 정보표시가 대중매체나 주변인의 추천보다 소비자 인식 경로에 더 큰 영향을 미치고 당류 저감화 식품 구매에 있어 단맛과 같은 식품의 내적 요인보다는 건강과 같은 외적 요인이 더 큰 것으로 드러났다. 또한 당류 저감화 식품의 이용 경험이 없는 소비자가 당류 저감화 식품을 구매하지 않는 이유는 판매 경로의 부재와 같은 환경적 요인보다는 단맛과 같은 내적 요인이 큰 영향을 미치는 것으로 나타났다. 대표적 당류식품인 탄산음료류와 초코쿠키류의 단맛에 대한 선호도와 당 함량 감소 시 단맛에 대한 수용도를 분석한 결과 초코쿠키류는 50% 이하 저감화한 단맛을 선호하는 소비자의 비율이 높은 반면, 탄산음료는 50% 이상을 저감화한 단맛을 더 선호한 소비자의 비율이 높았다. 한편 당 함량감소 시 단맛 수용도에 있어서는 두 당류 제품 모두 단맛을 50% 이상 저감화할 경우 수용 가능한 소비자의 비율이 높은 것으로 나타났다. 또한 두 제품군에 대한 당류 저감화 방향으로써 단맛과 당 성분에 대한 인식분석 결과 대부분의 소비자가 단맛을 감소시킬 때 설탕을 감소시키는 대신 천연 또는 인공감미료로 대체하는 방향을 선호하였다. 그러나 가정 내 음식 섭취 시 당류 저감화 방향에 대한 인식에서는 천연감미료나 인공감미료로 대체하는 방향보다는 설탕을 감소하는 방향을 선호하였다. 또한 연령별로 50~60대의 경우 당류 제품에 대해서는 설탕 감소를 선호하였지만 가정 내 식품 섭취 시에는 천연 또는 인공감미료 대체를 선호하여 당류 저감화 대상에 따른 인식의 차이점을 파악할 수 있었다. 당류 저감화 식품의 가격수용성 분석 결과 기존의 시판 제품보다 가격에 대한 부담이 더 클 경우에도 절반 이상의 소비자가 수용 가능한 것으로 나타났다. 이때 수용가격의 상승 범위는 10~14% 수준이 적절한 것으로 나타났다. 당류 저감화 식품의 필요성과 향후 구매의사에 대해서는 전반적으로 긍정적이었으나, 현재 외식 시 당류 섭취 저감화를 위해 노력하고 있는 것보다는 향후 당류 섭취 조절 의사에 대해 더 긍정적인 태도를 보이는 것으로 나타났다. 50~60대가 20대와 30~40대보다 더 적극적이며 20대 소비자의 태도가 가장 소극적인 것으로 나타났다. 향후 연구에서는 본 연구에 제시 된 당류 제품 이외 다양한 종류의 당류 제품을 대상으로 당류 제품 섭취 실태에 대한 조사를 병행하여 연구 결과의 일반화에 대한 타당도를 높일 수 있을 것으로 사료된다.

노인 입원환자들의 영양섭취 실태에 관한 조사연구 (A Studies on Nutririon of Aged patients in hospital)

  • This survey was conducted to investigated the status of nutritional consumption and relationship of nutrition of patients in hospitals from 1998.9.1 to 1998.9.10. Calorie intake of male patients was lower than RDA but that of female patients were higher than RDA except female younger than 64 years old. Protein intake of male patients was 130% higher than RDA, and that of female patients was 150% more than RDA. Calcium intake was lower than RDA in male patients but higher than RDA in female patients except older than 75 years. Iron intake was higher than RDA in all patients except in male patients older than 75 years. while vitamin B$_1$and vitamin C intake was higher than RDA in all patients but vitamin A and niacin intake was lower than RDA. The rich patients intake enough iron through animal foods and stabilized patients intake vitamin B$_2$ more than RDA. All patients were recognized the importance of protein and vegetable for health and they were recognized also careful choosing of foods for health. Positive relationships were showed between intake of nutritional tablet and nutritional status, between less vegetable intake and less intake of calorie, fat, sugar and fiber and between appetite and vitamin B$_1$ and vitamin C. The result showed that the shorter period staying in hospital, the more intake of sugar, vitamin A and vitamin C.

대구지역 성인의 가당음료 섭취와 영양지수를 활용한 식사의 질과의 관련성 (Association between Sugar-Sweetened Beverage Intake and Dietary Quality using Nutritional Quotient among Adults in Daegu, Korea)

  • Objectives: This study investigated an association between sugar-sweetened beverage (SSB) intake and the dietary quality of adults in Deagu, Korea. Methods: A questionnaire survey was conducted in 1,022 adults aged 19 ~ 49 years (502 men and 520 women) in the Deagu area of Korea. Daily intake of SSB was obtained by the food frequency questionnaire, and the dietary quality was assessed using the nutrition quotient (NQ) for Korean adults. Multiple logistic regression analysis was used to estimate the association between dietary quality and daily intake of SSB in adults. Results: Daily intake of SSB was 463.6 mL/d for total subjects, and the highest intakes were sweetened coffees (192.7 mL/d), followed by carbonated drinks (77.1 mL/d). Higher intake of SSB was associated with higher intake frequency of fast food or sweet and greasy bread, processed beverage, ramyon, eating out or delivery food and night snack, and also associated with lower frequency of water, breakfast intake and nutrition label checking in men or women. Men and women who had a higher intake SSB had significantly greater odds for being in the low grade of NQ (P for trend = 0.0006 for men, P for trend = 0.0007 for women), especially in the moderation factor (P for trend < 0.0001 for men and women). Conclusions: This study showed that high SSB intake was significantly associated with low dietary quality among adults. These study results suggest that nutrition education programs and guidelines should be provided to adults for improving their consumption of SSB and related diets.

인천, 춘천 지역 학교급식 메뉴의 당 함량 및 당 섭취량 실태조사 (Monitoring the Sugar Content and Intake in School Meals from Incheon and Chuncheon)

  • 본 연구에서는 인천과 춘천지역에서 각각 초등학교 4개 학교와 중학교 4개 학교 등 총 16개 학교를 선정하여 주식, 부식 및 후식 등 1334건의 메뉴에서 당 함량 및 당 섭취실태를 조사하였다. 학교급식을 통한 섭취량은 전체 평균 $410.9{\pm}85.8g$/인이었다. 전체 급식메뉴의 평균 당 함량은 $1.93{\pm}3.35g/100g$이었으며, 급식메뉴를 주식, 부식 및 후식으로 나누었을 때 평균 당 함량은 후식류($7.21{\pm}6.32g/100g$)>부식류($1.69{\pm}2.44g/100g$)>주식류($0.32{\pm}0.77g/100g$) 순으로 나타났다. 평균 당 함량은 주식류에서 면류>죽류>일품식밥류 순으로 높았고, 부식류에서는 소스류>튀김류>절임류>무침 나물류>볶음류 등의 순으로 높게 나타났다. 당 섭취량 또한 후식류($4.63{\pm}5.11g$)>부식류($0.64{\pm}1.01g$)>주식류($0.55{\pm}1.48g$) 순으로 높게 나타났다. 급식 중 당 섭취량은 주식류에서는 일품식밥류의 당 섭취량이 높았고, 부식류에서는 튀김류>볶음류>소스류>조림류의 순으로 당 섭취량이 높았다. 후식류에서는 음료와 유제품류의 당 함량과 섭취량이 다른 음식군에 비해 높았다. 급식일별로 점심 급식 한 끼에 섭취하는 평균 당 섭취량은 초등학생 $4.03{\pm}3.67g$/인, 중학생 $6.97{\pm}6.59g$/인이었으며, 전체 평균 당섭취량은 $5.50{\pm}5.53g$/인으로 한국인 총당류 섭취기준치와 비교했을 때 높지 않은 수준으로 나타났다. 그러나 Kang과 Yoon(8)의 연구결과 초등학생의 91.5% 가 주로 하루에 1-2회 간식을 섭취하며, 선호하는 간식이 과자, 과일 또는 과일주스, 빵 또는 케이크류 순으로 나타나 학생들의 간식을 통한 당 섭취는 높을 것으로 사료된다. 따라서 학교급식을 통해 단 맛에 길들여지지 않으며 올바른 식습관을 형성하도록 영양교육할 필요가 있다. 또한 학교별로 조리사와 조리법에 따른 당 함량에 차이를 보임에 따라 당 함량을 줄일 수 있는 조리법의 개발, 통일된 조리법의 보급 및 조리사의 교육과 급식 식단 구성 시 당 섭취량이 높았던 음식군을 중복하지 않고, 후식류로 과일류를 제공하는 등 메뉴의 조합에 주의함으로써 학생들의 당 섭취량을 줄일 수 있을 것으로 사료된다.
