세침흡인 세포검사로 진단된 턱밑 연조직 방선균증 - 1예 보고 - (Submandibular Soft Tissue Actinomycosis Diagnosed by Fine Needle Aspiration Cytology - A Case Report -)

  • A patient with actinomyces infection of the submandibular soft tissue was diagnosed by fine needle aspiration cytology (FNAC). A 38-year-old woman presented with a right submandibular mass which slowly grew in size over one month. Clinically and radiologically, the lesion was considered as tuberculous lymphadenitis or cellulitis. The polymerase chain reaction for tuberculosis was done by aspirated specimen but the result was negative. The smears of aspiration cytology showed characteristic colonies (sulfur granules) of actinomyces in inflammatory background. After antibiotic therapy for eight months, the patient has been well, showing no detectable mass. This patient was simply and rapidly diagnosed by FNAC and can avoid unnecessary surgical biopsy.

도축 젖소에서 Trueperella pyogenes 감염에 의한 턱 밑 화농육아종 증례보고 (A case of submandibular pyogranuloma caused by Trueperella pyogenes in the slaughtered dairy cows)

  • Multiple enlarged submandibular nodules were discovered during a routine antimortem examination at slaughter in 3 dairy cows of 20-month-old. Grossly, submandibular nodules were firm, reddish, and oval in shape. On cut surface, multiple yellowish abscesses in the parenchyma were observed. Histopathologically, the nodules were characterized by severe diffuse pyogranulomatous inflammation and chronic fibrosis with multiple eosinophilic clubs of Splendore-Hoeppli material containing central bacterial colonies. Trueperella pyogenes was isolated from the lesions. This case was diagnosed as submandibular pyogranuloma caused by Trueperella pyogenes in slaughtered dairy cows in Korea.

악하선 공간에 생긴 동정맥 기형의 후이개 내시경 접근법을 이용한 절제술 1례 (A Case of Retroauricular Endoscopic Excision of Arteriovenous Malformation in the Submandibular Space)

  • A 54-year-old female presented with a 4cm sized painless, submandibular gland area mass. Vascular lesion was suspected in computed tomography scan. Surgical excision was performed via retroauricular approach for its excellent cosmetic outcome. The vascular mass with peripheral focal calcification was clearly resected without complication such as bleeding or neural damage. Histopathological examination of the specimen revealed an arteriovenous malformation(AVM) with abnormal communications between arteries and veins without the normal intervening capillary bed. We report a successful excision of AVM in the submandibular space via retroauricular approach.

  • Background: The conventional transcervical resection for submandibular gland disease has some risks and an unsatisfactory cosmetic result. Recently, robot-assisted surgery has been developed as a plausible substitute for conventional surgery which provides an excellent cosmetic outcome. Case presentation: The authors performed robot-assisted sialadenectomy via modified facelift incision using the da Vinci Xi surgical system (Intuitive Surgical Inc., CA, USA) with two endowrist arms (monopolar curved scissors and Maryland bipolar forceps) successfully in a 44-year-old female patient who suffered from sialolith and severe atrophic submandibular gland. Conclusions: If similar studies are done in the future, this robot-assisted sialadenectomy may become established as an alternative to existing disadvantageous surgical methods.

  • IgG4-related chronic sclerosing sialadenitis commonly occurs in the submandibular glands. The gold standard for diagnosis of IgG4-related chronic sclerosing sialadenitis is a tissue biopsy from the affected organ. Herein, we report the probability of occurrence of IgG4-related chronic sclerosing sialadenitis in both submandibular glands that developed at intervals of months. Therefore, long-term follow-up and whole-body evaluation are essential.

Study on the Changes in Distributions and Expressions of Aquaporin5 (AQP5) in Salivary Glands of Mice After Alcohol Ingestion

  • Alcohol intake is known to affect various organs in the human body, causing reduction of salivation in the oral cavity. Hypo-salivation effect of alcohol is a common feature, but the mechanism in salivary glands is still poorly studied. Therefore, in this study, the changes in salivary secretion and water channel protein (aquaporin5, AQP5) in salivary glands of mice were investigated after ethanol administration. Animals were divided in to 4 groups with the control, 4 g/kg ethanol, 8 g/kg ethanol and 16 g/kg ethanol administration groups. One hour after ethanol administration, saliva was collected from the oral cavity, and the animals were killed and parotid and submandibular glands were extracted to analyze the histopathology, AQP5 immunihistochemistry and AQP5 protein level. According to the results, the salivation rate decreased irrespective of the ethanol dose in mice, and viscosities increased with increase in ethanol dose. However, there were no pathological changes in parotid and submandibular glands due to ethanol administration. Expression of AQP5 in parotid and submandibular glands decreased with increase ethanol administration These results indicate that the reduction of salivary secretion due to acute alcohol intake is closely related to decrease of the water channel protein such as AQP5 in parotid glands and submandibular glands, rather than the damage of salivary glands.

방사선 조사선량에 따른 백서 악하선의 caspase-3 발현양상 (Effect of radiation dose variation on expression of caspase-3 in rat submandibular glands)

  • Purpose : To investigate the caspase-3 expression in the acinar and ductal cells of rat submandibular glands after the irradiation of various doses. Materials and Methods : The male Sprague-Dawley rats weighing approximately 250 gm were used for this study. The experimental group was irradiated with a single absorbed dose of 2, 5, 10, and 15 Gy on the head and neck region. The rats were sacrificed on the 1st, 3rd, 7th, 14th, 21 st, and 28th day after irradiation. The specimens including the submandibular gland were sectioned and observed using histopathological and immunohistochemical methods. Results : The local destruction of the acinar and ductal cells and the karyopyknotic nuclei of the acinar cells were observed in the 2 Gy and 5 Gy irradiation groups later than in the 10 Gy and 15 Gy irradiation groups. And the expression of caspase-3 was prominent only in the ductal cells in the 2 Gy and 5 Gy irradiation groups. Conclusion : This experiment suggests that radiation-induced apoptosis in the ductal cells of rat submandibular glands was induced by a low dose radiation associated with the activation of caspase-3 and radiation-induced necrosis was induced by a high dose radiation.

Distribution of Taste Receptors in Submandibular and von Ebner Salivary Glands

  • Taste is a critically important sense for the survival of an organism. However, structure and distribution of taste receptors were only recently investigated. Although expression of the ion channels responsible for the sense of salty taste and acidity was observed in the non-taste cells, receptors for sweet and bitter taste were only identified in taste cells. Salivary glands are involved in the sensing of taste and plays important roles in the transduction of taste. The purpose of this study is to examine whether taste receptors are present in the salivary glands and to provide clues for the investigation of the taste-salivary glands interaction. Using microarray and RT-PCR analyses, the presence of taste receptor mRNAs in the rat von Ebner gland and submandibular gland was confirmed. Type I taste receptors were preferentially expressed in von Ebner gland, whereas type II taste receptors were expressed in both von Ebner gland and submandibular gland. The tastespecific signal tranducing proteins, $G_{\alpha}gustducin$ and phospholipase C ${\beta}2$, were also detected in both salivary glands by immunohistochemistry. Finally, the activation of the calcium signal in response to bitter taste in the acinar cells was also observed. Taken together, these results suggest that taste receptors are present in the von Ebner gland and submandibular gland and that type II taste receptors are functionally active in both salivary glands.

Levamisole이 백서 악하선에서의 화학적 발병현상에 미치는 영향 (An Effect of Levamisole on the Chemical Carcinogenesis in the Submandibular Salivary Gland of Rats)

  • The purpose of this study is to evaluate an effect of levamisole on the chemical crcinogenesis in the submandibular salivary gland of rats through histopathologic observation. 60 male Sprague Dawley rats were employed in this study, divided into one control and two experimental groups. An pellet of 5 mg of 9, 10-dimethyl-1,2-benzathracene(DMBA) powder was implanted into submandibular salivary gland of each animal among 20 in control. And each animal among 20 in experimental group 1 received 0.7 mg of levamisole hydrochloride orally every day starting at the beginning of the fifth week after DMBA implantation under the same methods as in control. And each animal among 20 in experimental group 2 received the same treatment as in control at the beginning of the fifth week after oral administration of levamisole hydrochloride under the same method as experimental group 1. Each 5 animals in control at the end of 2nd, 4th, 6th 8th, week after experiments, and each 10 animals in experimental group 1,2 at the end of 6th, 8th week after experiments were sacrificed at random. Also the specimens from experimental sites of submandibular salivary glands were routinely processed for histopathologic observation under Hematoxilin-eosin(H-E) staining. The obtained results were as follows : 1. In control, generally, the glandular ductal cell showed the tendency of dysplasia or malignancy with progression of experiment. 2. In experimental group 1, generally, the dysplasia or malignancy of the glandular ductal cell was less prominent than in control, while the lymphocyte infiltration and fibrosis were prominent. 3. In experimental group 2, generally, the dysplasia of the glandular ductal cell was significantly less prominent than in control, while the fibrosis was prominent. 4. Under above results levamisole was thought to delay or prevent the chemical carcinogenesis in the submandibular salivary gland.

방사선조사와 당뇨병이 백서 악하선의 caspase-3 발현에 미치는 영향 (Effect of irradiation on the expression of caspase-3 in the submandibular gland of streptozotocin-induced diabetic rats)

  • Purpose : To observe the histopathological changes and caspase-3 expression in the submandibular gland in streptozotocin-induced diabetic rats after irradiation. Materials and Methods : The male Sprague-Dawley rats weighing approximately 250 gm were divided into four groups: control, diabetes, irradiation, and diabetes-irradiation groups. Diabetes mellitus was induced in the rats by injecting streptozotocin. Rats in the control and irradiation groups were injected with citrate buffer only. After 5days, rats in irradiation and diabetes-irradiation groups were irradiated with a single absorbed dose of 10 Gy to the head and neck region. All the rats were sacrificed at 3, 7, 14, 21, and 28 days after irradiation. The specimen including the submandibular gland were sectioned and observed using histopathological and immunohistochemical methods. Results : In the irradiation group, the condensed nucleus, karyolysis, and degeneration of the acinar cells and atrophy of the duct cells were observed in the early experimental phase. However, the acinar cells were found to be normal at 28 days after irradiation. In the diabetes group, the condensed nucleus, karyolysis, atrophy, and degeneration of the acinar cells were observed in the early experimental phase. However, the acinar cells were found to be normal at 21 days after diabetic state induction. In the diabetes-irradiation group, the ductal epithelial cells were predominant in their glandular tissues at 28 days after irradiation. In all of the experimental groups, the most prominent change of the acinar cells and ductal cells were observed at 14 days after diabetic state induction and irradiation. Conclusion The expression of caspase-3 in the acinar cells and ductal cells of the submandibular gland was weak after irradiation, but that in the acinar cells, ductal cells, and fibrous cells of the submandibular gland was prominent after diabetic state induction.
