Cerebral Hemorrhage in Patients on Maintenance Hemodialysis (혈액투석을 받고있는 환자에서 자발성 뇌출혈)

  • Objective : The cause and clinical outcome of cerebral hemorrhage in patients on maintenance hemodialysis have been poorly studied in korea. The purpose of this paper is to clarify the clincal features and the outcome of cerebral hemorrhage in patients on maintenance hemodialysis. Method : We analyzed clincal features and the outcome of cerebral hemorrhage in 14 patients on maintenance hemodialysis. Hematomas were reviewed and evaluated for location, size, and intraventricular extension by the one of the authors without any prior informations. The axial slice of CT film that the hematoma was appeared in maximal dimension was chosen for evaluations. Result : Hypertension was found in 71.4%(10 cases) and motality rate was 78.5%(11 cases). Basal ganglia hemorrhage was found in 50%(7 cases), subcortex in 28.5%(4 cases), pons in 14.2%(2 cases). Size of hematoma in patients on maintenance hemodialysis was significantly larger than that of hypertensive cerebral hemorrhage patients(p=0.0061). The 4 cases of basal ganglia hemorrhage without intraventricular hemorrhage and subarachnoid hemorrhage were good mental state at the onset of stroke because of small mass effect relative to the size of hematoma. The duration of hemodialysis treatment prior to strokes ranged from 1 to 107 months. Strokes developed within 6 hours of the previous hemodialysis are 5 cases. Average serum albumin concentration was 3.4g/dl. The use of heparin is less responsible for the development of cerebral hemorrhage in patients on maintenance hemodialysis Conclusion : Cerebral hemorrhage in patients on maintenance hemodialysis is more severe in terms of hematoma size and clinical outcome. Therefore, the prevention and treatment of cerebral hemorrhage in patients on maintenance hemodialysis should be more aggressive.

The Effects of Balloon-Guide Catheters on Outcomes after Mechanical Thrombectomy in Acute Ischemic Strokes : A Meta-Analysis

  • Objective : Mechanical thrombectomies with balloon-guide catheters (BGC) are thought to improve successful recanalization rates and to decrease the incidence of distal emboli compared to thrombectomies without BGC. We aimed to assess the effects of BGC on the outcomes of mechanical thrombectomy in acute ischemic strokes. Methods : Studies from PubMed, EMBASE, and the Cochrane library database from January 2010 to February 2018 were reviewed. Random effect model for meta-analysis was used. Analyses such as meta-regression and the "trim-and-fill" method were additionally carried out. Results : A total of seven articles involving 2223 patients were analyzed. Mechanical thrombectomy with BGC was associated with higher rates of successful recanalization (odds ratio [OR], 1.632; 95% confidence interval [CI], 1.293-2.059). BGC did not significantly decrease distal emboli, both before (OR, 0.404; 95% CI, 0.108-1.505) and after correcting for bias (adjusted OR, 1.165; 95% CI, 0.310-4.382). Good outcomes were observed more frequently in the BGC group (OR, 1.886; 95% CI, 1.564-2.273). Symptomatic intracranial hemorrhage and mortality did not differ significantly with BGC use. Conclusion : Our meta-analysis demonstrates that BGC enhance recanalization rates. However, BGC use did not decrease distal emboli after mechanical thrombectomies. This should be interpreted with caution due to possible publication bias and heterogeneity. Additional meta-analyses based on individual patient data are needed to clarify the role of BGC in mechanical thrombectomies.

Variance Recovery in Text Detection using Color Variance Feature (색 분산 특징을 이용한 텍스트 추출에서의 손실된 분산 복원)

  • This paper proposes a variance recovery method for character strokes that can be missed in applying the previously proposed color variance approach in text detection of natural scene images. The previous method has a shortcoming of missing the color variance due to the fixed length of horizontal and vertical windows of variance detection when the character strokes are thick or long. Thus, this paper proposes a variance recovery method by using geometric information of bounding boxes of connected components and heuristic knowledge. We have tested the proposed method using various kinds of document-style and natural scene images such as billboards, signboards, etc captured by digital cameras and mobile-phone cameras. And we showed the improved text detection accuracy even in the images of containing large characters.

Experimental brush wear pattern and cariostatic effect of Biscover (Biscover의 잇솔질에 따른 마모양상과 항우식 효과)

  • Objective: The aim of this study was to investigate the experimental brush wear pattern of a light cured surface sealant, Biscover (Bisco, Schaumburg, IL), and to evaluate its cariostatic effect. Methods: Caries-free human premolars were used for the Biscover coating group (n = 90) and the control group (n = 10). The Biscover coating group was randomly assigned to nine subgroups of 10 each and the control group was assigned to two subgroups of 5 each according to the number of brushing strokes. An experimental 3-body wear test was conducted under different strokes of wear test. Tooth-brushing was accomplished with movement of each brush head set at a frequency of 100 rpm under a force of 1.5N, Surface roughness was tested before, and after Biscover coating, and after brushing. Then, each of the 10 teeth of both groups were placed in artificial caries inducing solution for 7 days. All tooth surfaces were assessed using scanning electron microscopy. Results: Biscover coated surfaces showed a smoother texture than enamel surfaces. The roughness was increased after experimental brushing and after 10,800 brushing strokes, the whole layer of Biscover wore out. However, teeth in the Biscover coating group had a cariostatic effect in cariogenic conditions. Conclusions: We suggest that white lesions in orthodontic patients can be suppressed by topical applications of Biscover.

Numerical Study of Flow Characteristics due to Interaction Between a Pair of Vortices in a Turbulent Boundary Layer

  • This paper represents a numerical study of the flow field due to the interactions between a pair of vortices produced by vortex generators in a rectangular channel flow. In order to analyze longitudinal vortices induced by the vortex generators, the pseudo-compressibility method is introduced into the Reynolds-averaged Navier-Strokes equations of a 3-dimensional unsteady, incompressible viscous flow. A two-layer $k-{\epsilon}$ turbulence model is applied to a flat plate 3-dimensional turbulence boundary to predict the flow structure and turbulence characteristics of the vortices. The computational results predict accurately the vortex characteristics related to the flow field, the Reynolds shear stresses and turbulent kinetic energy. Also, in the prediction of skin friction characteristics the computational results are reasonably close to those of the experiment obtained from other researchers.

The Relationship Between Asymmetrical Weight Bearing and Bone Mineral Density in Chronic Hemiplegic Limbs

  • Hemiplegia-induced immobilizatoin and reduction of mechanical loading in chronic stroke limbs are common cause of disuse osteoporosis. The purpose of this study was to investigate the effects of asymmetrical weight bearing on the loss of bone mineral in the individual with chronic stroke. Sixteen hemiplegic patients with strokes were evaluated. The measurements of bone mineral density (BMD) were evaluated with the quantitative ultrasound system on the calcaneus region of the paretic and non-paretic side. Plantar pressure was measured using the Mat-Scan system. The paretic side showed significantly smaller values in the T-score of BMD, and peak value of plantar pressure, which included forefoot, midfoot, and hindfoot, than the non-paretic side (p<.05). Results from the pearson correlation analysis showed statistically significant correlation between the BMD difference and the peak-pressure difference of midfoot pressure (p<.05). This finding indicated that BMD loss depended on decrease of body weight born on the paretic leg.

  • The effects of stroke change on turbulent kinetic energy for the in-cylinder flow of a four-valve SI engine were studied. For this study, the same intake manifold, head, cylinder, and the piston were used to examine turbulence characteristics in two different strokes. In-cylinder flow measurements were conducted using three dimensional LDV system. The measurement method, which simultaneously collects 3-D velocity data, allowed a evaluation of turbulent kinetic energy inside a cylinder. High levels of turbulent kinetic energy were found in regions of high shear flow, attributed to the collisions of intake flows. These specific results support the more general conclusion that the inlet conditions play the dominant role in the generation of the turbulence fields during the intake stroke. However, in the absence of two counter rotating vortices, this intake generated turbulent kinetic energy continues to decrease but at a much faster rate.

  • Research for Manchu character digitalization is at an early stage. This paper proposes a preprocessing algorithm for Manchu character recognition. This algorithm improves the existing Hilditch thinning algorithm so that it corrects thinning error for Manchu characters. The existing algorithm separates the characters into the left-hand side and right-hand side, while our alogorithm uses the central point between the points that strokes exist when it classifies each of characters. The experimentation results show that this method is valid for thinning and classification of Manchu characters.

  • For the efficient teaching of Korean characters(Hangul) handwriting method, it needs to the collection of Hangul handwriting samples and the study of analysis methods. For the collection of Hangul handwriting samples, the students of teacher's university take part in this survey. The analysis is performed according to the characters and graphemes in Hangul. The analysis features include the number of strokes, the direction of handwriting, the relational position of graphemes, the height and width of characters, and so on. The results of this analysis will be used for the reference data for the teaching of Hangul handwritings, the study of Hangul fonts, and the research of Hangul information engineering.

  • 자연영상이나 스팸메일 영상으로부터 텍스트 영역을 추출하고 추출한 텍스트 영역에 이진화를 수행하고 나면 가로 방향이나 세로획 방향으로 놓여 있는 "1" 그리고 "ㅡ" 에 해당하는 한글의 종성부분이 이미지 내의 잡영을 지울 때 종종 지워지는 결과를 볼 수 있다. 이렇게 지워진 획 부분을 되살리기 위한 방법으로 텍스트 Hinting 알고리즘을 제안한다. 텍스트 Hinting 알고리즘은 이진화된 이미지의 텍스트 픽셀 위치와 동일한 좌표에 해당하는 원본 이미지의 RGB 값을 추출하여 추출된 텍스트 후보 영역의 색상을 알아낸다. 추출된 텍스트 색상 레이어 이미지와 이진화된 이미지에 OR연산을 수행하게 되면 지워진 획 부분을 복원할 수 있다. 제안한 방법을 스팸 이미지에 적용한 결과 텍스트 추출결과를 획기적으로 개선할 수 있음을 보였다.
