Objective: The timed up and go (TUG) test is method used to determine the functional mobility of persons with stroke. Its reliability, validity, reaction rate, fall prediction, and psychological characteristics concerning ambulation ability have been validated. However, the relationship between TUG performance and community ambulation ability is unclear. The purpose of this study was to investigate whether the TUG performance time could indicate community ambulation levels (CAL) differentially in persons with chronic stroke. Design: Cross-sectional study. Methods: Eighty-seven stroke patients had participated in this study. Based on the self-reporting survey results on the difficulties experienced when walking outdoors, the subjects were divided into the independent community ambulation (ICA) group (n=35) and the dependent community ambulation group (n=52). Based on the area under the curve (AUC), the discrimination validity of the TUG performance time was calculated for classifying CAL. The Binomial Logistic Regression Model was utilized to produce the likelihood ratio of selected TUG cut-off values for the distinguishing of community ambulation ability. Results: The selected TUG cut-off values and the area under the curve were <14.87 seconds (AUC=0.871, 95% confidence interval=0.797-0.945), representing a mid-level accuracy. Concerning the likelihood ratio of the selected TUG cut-off value, it was found that the group with TUG performance times shorter than 14.87 seconds showed a 2.889 times higher probability of ICA than those with a TUG score of 14.87 seconds or longer (p<0.05). Conclusions: The TUG can be viewed as an assessment tool that is capable of classifying CAL.
Many stroke patients undergoing rehabilitation therapy require a quantitative indicator for the evaluation of body function in paretic and non-paretic regions. In this study, the impedance parameters were acquired to assess the physical status in the upper extremity of thirty six stroke patients with hemiplegia caused by cerebral hemorrhage (10 patients) and cerebral infarction (26 patients), using bioelectrical impedance. Prediction marker (PM), phase angle (PA), PM/PA, and resistance (R) versus reactance ($X_c$) were utilized to evaluate the functional status of the paretic and non-paretic regions. In addition, the hand grip strength (HGS) and the pinch strength (lateral, palmer, tip) were measured on the upper extremity of hemiplegic stroke patients. PM was distributed in inversely proportional to HGS, but PA was distributed in proportional to HGS. However, there were a number of patients with HGS of 0, regardless of the impedance parameters (PM, PA, R vs. $X_c$). Paretic and non-paretic status in upper extremity of these patients could not be analyzed using impedance parameters. At the rehabilitation therapist's instructions, they were unable to move the hand and fingers of the paretic upper extremity by cranial nerve damage, motor nerve damage, and severe cognitive decline.
The fatigue behavior of LIPCA was so sensitive to the manufacturing condition, the environmental factors and the change of the test apparatus. Therefore, we could be considering not only the relationship between the stroke range $({\Delta}h)$ and actuating frequency but also the relationship between the stroke range $({\Delta}h)$ and the total effective moment $(M^E)$. Thus, this study proposed the calculation method of the applying $M^E$ when the $({\Delta}h)$ of LIPCA was increased from 1.mm to 20mm. To estimate the relationship between the total effective moment $(M^E)$ and the Bernoulli-Euler bending moment (M) was reviewed. And the residual stress distribution of LIPCA and THUNDER using the CLT was evaluated. In conclusions, converting the $({\Delta}h)$ of LIPCA to the radius of curvature (p) and calculating the $(M^E)$, it was found that the p by the $M^E$ changed similarly as the $({\Delta}h)$. It was found that the $M^E$ was 2.2 times as the M. While CFRP and PZT of LIPCA, which had the superior compressive characteristic, had the compressive residual stress, GFRP was subject to the tensile residual stress. Since this reversed configuration between the compressive residuals stress and the tensile one was made, the requirement of the stroke range $({\Delta}h)$ increase was satisfied.
Objectives : Sagunja-Tang is a famous prescription used in Korean medicine for the purpose of promoting vital energy, and there are few studies using Sagunja-Tang on cerebrovascular diseases yet. As previous studies confirmed that Sagunja-tang is highly likely to be used effectively for stroke, this study was intended to predict the mechanism through which Sagunja-tang would act effectively on stroke. Methods : In this study, a network pharmacology analysis method was used, and oral bioavailability (OB), drug likeness (DL), Caco-2 and BBB permeability were utilized to select compounds with potential activity. For the values of each variable used in this study, OB ≥ 30%, DL ≥ 0.18, Caco-2 ≥ 0, and BBB ≥ 0.3 were applied. Using the above variables, the relations between target genes and diseases that are presumed to be involved in the selected bioavailable compounds were constructed in a network format, and proteins thought to play a major role were identified. Results : Among the compounds included in Sagunja-Tang, 26 bioavailable compounds were selected and it was confirmed that these compounds can be effectively used in cerebrovascular diseases such as Alzheimer's disease and stroke. These compounds are considered to act on proteins related in cell death and growth. The most important mechanism of action was predicted to be apoptosis, and the protein that is thought to play the most key action in this mechanism was caspase-3. Conclusions : In our future study, Sagunja-Tang will be used in an ischemic stroke mouse model, and the mechanism of action will be explored focusing on apoptosis and cell proliferation.
본 연구는 일과성허혈발작 및 급성뇌경색환자에서 양쪽 중 간대뇌동맥의 혈류속도 차이가 예후를 예측할 수 있는 지표가 될 수 있고, 이러한 예후예측력이 뇌졸중 아형 간에 차이가 있는 지를 관찰하는 것이다. 모든 대상자는 일과성허혈발작 및 급성뇌경색환자들이며, 기본적인 평가와 경두개도플러검사(TCD)가 실시되었다. 중간대뇌동맥 비대칭 지수(MCA asymmetry index)는 다음의 공식에 의해 구해졌다; MCA asymmetry index=(|RMCA MFV-LMCA MFV|/mean MCA MFV) ${\times}100$. 뇌경색 분류는 TOAST classification에 따라 진행되었다. 나쁜 예후(poor functional outcome)는 mRS score ${\geq}3$ at 3 months after stroke onset로 정의하였다. 총 988명의 급성뇌경색환자가 분석에 포함되었고, 그 중 157명(15.9%)이 나쁜 예후를 가지고 있었다. 뇌경색 하위유형 및 여러 혼란변수를 보정 후 실시한 다변량분석에서 중간대뇌동맥 비대칭 지수만 유일하게 독립적으로 나쁜 예후와 연관성을 보였다. ROC curve 분석에서 중간대뇌동맥 비대칭 지수를 예측모형에 투입하였을 때 투입 전에 비해 나쁜 예후를 예측하는 능력이 상승하는 것을 관찰하였다(from 88.6% [95% CI, 85.2-91.9] to 89.2% [95% CI, 85.9-92.5]). 급성뇌경색환자에서 중대뇌동맥 비대칭 지수는 나쁜 예후를 독립적으로 예측하였다. 따라서 경두개도플러검사는 급성뇌경색환자에서 나쁜 예후를 예측하는데 유용하게 사용될 수 있을 것이다.
핀틀추력기는 추력조절을 위해 노즐목면적을 조절하는 핀틀 스트로크 개념을 사용한다. 충남대학교에서 수행한 공압시험용 핀틀추력기에 대해 MATLAB을 사용하여 1-D 시뮬레이션 성능예측 기법을 연구하였고 전산수치해석과 1-D 시뮬레이션을 수행하여 결과를 비교하였다. 일차원 유동이론에 근거한 성능예측결과는 챔버압력에 대해 경향성 뿐만 아니라 계산값까지 유사한 것을 확인하였지만, 노즐벽면의 박리로 인해 추력에 대해서는 오차가 존재하였다. 수치해석 결과로 모든 핀틀 스트로크 구간에서 노즐의 확장부 부분의 설계 노즐목 부근에서 유동박리가 발생하는 것을 확인하였다. 엠피리컬(Empirical) 추력예측 법은 노즐벽면의 박리를 포함하며, 1-D 엠피리컬 시뮬레이션은 초기 핀틀 스트로크 구간에서 추력을 잘 예측하였다.
Stroke is one of the leading causes of long-term disability worldwide, placing huge burdens on individuals and society. Further, automatic human activity recognition is a challenging task that is vital to the future of healthcare and physical therapy. Using a baseline long short-term memory recurrent neural network, this study provides a novel dataset of stretching, upward stretching, flinging motions, hand-to-mouth movements, swiping gestures, and pouring motions for improved model training and testing of stroke-affected patients. A MATLAB application is used to output textual and audible prediction results. A wearable sensor with a triaxial accelerometer is used to collect preprocessed real-time data. The model is trained with features extracted from the actual patient to recognize new actions, and the recognition accuracy provided by multiple datasets is compared based on the same baseline model. When training and testing using the new dataset, the baseline model shows recognition accuracy that is 11% higher than the Activity Daily Living dataset, 22% higher than the Activity Recognition Single Chest-Mounted Accelerometer dataset, and 10% higher than another real-world dataset.

Development of an engine with good fuel economy is very important for successful implementation of long endurance miniature UAVs (unmanned aerial vehicles). In the study, a 4-stroke glow-plug engine was modified to a gasoline-fueled spark-ignition engine. Engine tests measuring performance and friction losses were conducted to tune a simulation program for performance prediction. It has been found that excessive friction losses are caused by insufficient lubrication at high speeds. The simulation program predicts that engine power and fuel economy get worse with high altitude due to increasing portion of friction losses. The simulation results suggest quantitative guidelines for further development of a practical engine.
In this study, the computer program was developed to predict the engine performances and exhaust emissions of a 4-cylinder 4-stroke cycle ignition engine including intake and exhaust system. The simulation models applied to each process were as follows. For the combustion process, two zone model which requires only one empirical constant was applied, and for the gas exchange process, the method of characteristics that allows the calculations of the time variation and spatial variation of properties along the pipes was used. Constant pressure perfect mixing model was applied to take into account of the interaction at manifold branches. To predict exhaust emissions, twelve chemical species were considered to be present in combustion products. These species were calculated through equilibrium thermodynamics and kinetic theory. The empirical constants reduced to least number as possible were determined through the comparison with the experimental indicator diagram of one particular operating condition and these constants were applied to other operating conditions. The predicted performances and emissions were compared with the experimental results over the wide range of operating conditions.
