Diagnostic Evaluation of the BioFire® Meningitis/Encephalitis Panel: A Pilot Study Including Febrile Infants Younger than 90 Days (BioFire® Meningitis/Encephalitis Panel의 진단적 유용성 평가: 90일 미만 발열영아에서의 예비 연구)

  • Purpose: Rapid detection of etiologic organisms is crucial for initiating appropriate therapy in patients with central nervous system (CNS) infection. This study aimed to evaluate the diagnostic value of the BioFire® Meningitis/Encephalitis (ME) panel in detecting etiologic organisms in cerebrospinal fluid (CSF) samples from febrile infants. Methods: CSF samples from infants aged <90 days who were evaluated for fever were collected between January 2016 and July 2019 at the Seoul National University Children's Hospital. We performed BioFire® ME panel testing of CSF samples that had been used for CSF analysis and conventional tests (bacterial culture, Xpert® enterovirus assay, and herpes simplex virus-1 and -2 polymerase chain reaction) and stored at -70℃ until further use. Results: In total, 72 (24 pathogen-identified and 48 pathogen-unidentified) CSF samples were included. Using BioFire® ME panel testing, 41 (85.4%) of the 48 pathogen-unidentified CSF samples yielded negative results and 22 (91.7%) of the 24 pathogen-identified CSF samples yielded the same results (enterovirus in 19, Streptococcus agalactiae in 2, and Streptococcus pneumoniae in 1) as those obtained using the conventional tests, thereby resulting in an overall agreement of 87.5% (63/72). Six of the 7 pathogen-unidentified samples were positive for human parechovirus (HPeV) via BioFire® ME panel testing. Conclusions: Compared with the currently available etiologic tests for CNS infection, BioFire® ME panel testing demonstrated a high agreement score for pathogen-identified samples and enabled HPeV detection in young infants. The clinical utility and cost-effectiveness of BioFire® ME panel testing in children must be evaluated for its wider application.

  • A lab-on-a-chip (LoC) was designed for simultaneous monitoring of microorganisms, antibiotic residues, somatic cells, and pH in raw milk. The LoC was fabricated from polydimethylsiloxane (PDMS) using microelectromechanical system (MEMS) technology, which consisted of two parts; a protein array and microchannel. The protein array was fabricated by immobilizing five types of antibodies corresponding to two microorganisms, two antibiotic residues, and somatic cells. A sol-gel film was deposited on a glass substrate to immobilize the antibodies. The target analytes in raw milk could be bound with the corresponding antibody by an immunoreaction, and the antigen-antibody complex was detected using fluorescence microscopy. SNARF-dextran was used as a pH indicator, and the SNARF-entrapped hydrogel was attached to the microchannel in the chip. After injecting the milk sample into the channel, the pH was measured by monitoring the change in fluorescence intensity by fluorescence microscopy. The on-chip simultaneous assay of two microorganisms (E. coli O157:H7 and Streptococcus agalactiae), two antibiotic residues (penicillin G and dihydrostreptomycin), and neutrophils was successfully accomplished using the proposed LoC system.

  • Scrotal swelling may be acute or chronic, painful or painless. Common causes of scrotal swelling in newborns are hydrocele, inguinal hernia, testicular torsion, testicular tumor, scrotal hematoma, meconium peritonitis and epididymitis. Abrupt onset of a painful scrotal swelling necessitates prompt evaluation. Testicular torsion and incarcerated inguinal hernia require urgent surgical management. We report a case of scrotal swelling caused by a tunica vaginalis abscess in a 20-days-old boy. He was admitted to the hospital due to fever, irritability and left scrotal swelling with local heat, tenderness and redness. Exploratory laparotomy was performed to rule out testicular torsion. On the operative field, congestive erythematous inflammation on the left tunica vaginalis was noted and it was filled with a pus like discharge. The cultured organism was Streptococcus agalactiae(group B). He recovered quickly after debridement and administration of empirical antibiotics.

Efficacy of the AHI Mastitis Detector for the Diagnosis of Bovine Mastitis (전도율측정법(傳導率測定法)(AHI Mastitis Detector)을 이용(利用)한 유방염진단(乳房炎診斷)의 야외적용시험(野外適用試驗))

  • A total of 136 dairy cows were subjected to test for bovine mastitis by AHI mastitis detector, microbiological test (MT) and California mastitis test (CMT). The results obtained were summarlized as follows: MT indicated that the most important causative microorganisms isolated from mastitic milk were Staphylococcus aureus (59.3%), Staphylococcus epidermidis (16.4%), Streptococcus agalactiae (12.4%) and Streoticoccus uberis (4.4%). In Holstein breed, the critical threshold of electrical conductivity values of normal and mastitic milk were found to be less than $6,900{\mu}S$ and above, $7,700{\mu}S$, respectively. Although there was goad agreement (92.0%) between AHI mastitis detector test and CMT for the diagnosis of bovine mastitis, the diagnostic efficiency of AHI mastitis detector (80.0%) was higher than that of CMT (74.0%) when compared with microbiological findings. In addition, handiness and objectivity of AHI mastitis detector for the detection of mastitic milk suggested that this could effectively be used for the diagnosis of both clinical and subclinical bovine mastitis in field.

  • Bovine mastitis is an important disease causing serious economic loss in dairy production and food poison in public health. The major causative agents of bovine mastitis include Escherichia coli (E. coli), Streptococcus agalactiae (S. agalactiae), Staphylococcus aureus (S. aureus). These bacteria were found in milk and environmental condition such as feces, water, soil and so on. Recently, many cases of mastitis are derived from environmental contamination of micro-organisms, which important factors for the spread of this disease in farm. Ultraviolet irradiation (UV) has been used as disinfection for waste and water in clinical and industrial facilities. Moreover the UV irradiation has been used as useful bactericidal agents to remove bacterial biofilms in environmental condition. In this study, we determined the bacterial replication in different percentage of water content (PWC) in sterilized saw dust and feces complexes from farm, and results showed that slightly decreased growth pattern of E. coli and S. agalactiae but increased growth pattern of S. aureus in various PWC (200, 400 and 600%) until 144 h incubation. In the bacteriocidal effect of UV irradiation to bacteria in saw dust and feces complex, the results showed that bacteriocidal effect was depended on the UV irradiation time, irradiation distance and PWC. Especially the antibacterial activity of UV irratiation is stronger in low PWC (50%), long time irradiation (50 sec), and short distance (5 cm) than other condition of this study. Furthermore UV irradiation with stirring showed increased the bactericidal effect compared without stirring. These results suggested that bovine mastitis causing agents may survive long time in environmental condition especially saw dust and feces complexes in farm and can cause a various disease including mastitis. Moreover, these data can be used as basis for application and development of UV disinfection to control of bovine mastitis from environmental contaminated bacteria in dairy farm.

Isolation and antimicrobial susceptibility of microorganisms from milk samples of dairy goat (유산양 유즙으로부터 분리된 세균의 분포 및 항균제 감수성 검사)

  • The aim of this study was to isolate microorganisms from half milk samples of dairy goats by California mastitis test (CMT) during the lactation period and to further investigate the susceptibility of isolated organisms to antimicrobial drugs. From a total of 235 half milk samples with CMT scores of 2 or above from 366 dairy goats distributed throughout Jeonnam province, microorganisms were isolated from 198 (83.5%) samples either singly (99.0%) or in combination (1.0%). The most prevalent microorganism was the coagulase-negative Staphylococcus spp., (44.4%, n=88) followed by Staphylococcus aureus (24.2%, n=48), Escherichia coli (11.1%, n=22) and Streptococcus spp. (7.6%, n=15). Isolated bacteria also included Bacillus spp. (2.5%, n=5), Pseudomonas spp. (2.5%, n=5), Micrococcus spp. (1.5%, n=3), Corynebacterium spp. (1.5%, n=3), Enterococcus facium (1.0%, n=2), Morganella morganii (0.5%, n=1) and Streptococcus agalactiae (0.5%, n=1). During the summer season, a high prevalence of all microorganisms were observed in which Staphylococcus spp. (30.8%), Escherichia coli (8.6%), and Streptococcus spp. (5.6%) were among the most prevalent bacteria isolated. Staphylococcus spp. was also shown to be high in the winter (21.7%). In most samples, the presence of bacterial pathogens in goat milk led to the increase in the total somatic cell count (SCC). Most of the half milk samples of dairy goats with bacterial contamination showed SCC of ${\geq}1{\times}10^6cells/ml$ (90.4%). Minor pathogens (11.4%) were more detected from milk samples with SCC of < $1{\times}10^6cells/ml$ than major pathogens (4.1%), while the major pathogens tended to be higher from samples with SCC of ${\geq}3{\times}10^6cells/ml$. Susceptibility of these bacteria to 12 antimicrobial agents was tested by the Kirby-Bauer disc diffusion method. Results indicated that more than 90% of bacteria isolated from CMT 2+ dairy goat half milk samples were susceptible to trimethoprim/sulfamethoxazole, amoxicillin/clavulanic, enrofloxacin and cephalothin while they were resistant to tetracycline (44.7%).

  • We isolated microorganism secreting antimicrobial substance from tomato and identified as Enterococcus faecium. This substance was completely inactivated by pretense treatment and retained activity after catalase treatment. This result indicated that the antimicrobial activity of this substance was due to proteinaceous substance known as bacteriocin. The bacteriocin inhibited growth of Gram positive bacteria, such as Listeria monocytogenes, Leuconostoc mesenteroides, Lactobacillus plantarum, Streptococcus agalactiae, Streptococcus pyrogenes, and Gram negative bacteria, such as Pseudomonas aeruginosa. Purification of the bacteriocin was achieved by ethanol precipitation, ion exchange chromatography on CM Sepharose CL-6B, and gel filtration on Sephacryl S-100 HR. After these purification steps, the specific activity of the bacteriocin was increased 35.8 fold compared with culture broth. Purified bacteriocin was shown single band on SDS-PAGE and molecular weight was estimated 51 kDa. The residual activity of this bacteriocin was 3.3% at 10$0^{\circ}C$ for 60 min, and this bacteriocin was stable at pH 2~7.

  • Recurrent bacterial meningitis is the reappearance of two or more episodes of meningitis caused by a different or same organism after an intervening period of full convalescence. Predisposing factors for recurrent bacterial meningitis include immunodeficiency and developmental and traumatic anatomical defects. Therefore, careful evaluation of children with recurrent meningitis is essential. We report the case of a patient with a skull base abnormality who had suffered from four events of bacterial meningitis between 8 and 17 years of age. He experienced another event of bacterial meningitis at the age of 21, without undergoing correction for the anatomical defect. Streptococcus agalactiae was identified in the patient's blood and cerebrospinal fluid culture. Afterwards, he underwent surgical treatment for a meningioma; this meningioma was strongly suspected as the cause of the recurrence.

Antioxidant and Antimicrobial Effects of Extract with Water and Ethanol of Oriental Melon (Cucumis melo L. var makuwa Makino) (참외 (Cucumis melo L. var makuwa Makino)의 물과 에탄올 추출물의 항산화 및 항균효과)

  • The biological activities of water and ethanol extracts from different fruit parts, such as peel, flesh, and placenta of oriental melon were investigated. The total phenolic concentration of water extract was the highest such as 151.64 ${\mu}g/g$ in the peel, also that of ethanol extract was 224.77 ${\mu}g/g$ in the peel, respectively. The total flavonoid content in the water and ethanol extracts were high such as 45.53 ${\mu}g/g$ and 67.16 ${\mu}g/g$ of peel, respectively. In the physiological activities, DPPH in the water and ethanol extracts were high such as 25.0% and 83.3% of peel in 1% concentration. Extract of peel was higher than those of flesh and placenta. ABTS in the water extracts was 79.2% of peel, 57.6% of flesh and 74.0% of placenta in 1% concentration. Ethanol extracts was 99.9% of peel, 52.1% of flesh and 41.2% of placenta in 1% concentration. In addition, xanthine oxidase inhibitory activity and ${\alpha}$-Glucosidase inhibition activity of the peel of water and ethanol extracts appeared to be higher than those of placenta and flesh. This study showed that the antioxidant and ${\alpha}$-Glucosidase inhibition activity of peel extracts were higher than those of placenta and flesh. Also, the antimicrobial effect of ethanol extract from different fruit parts was shown only on Streptococcus agalactiae.

Trends in Pathogen Occurrence and Antimicrobial Resistance of Urinary Isolates in a Tertiary Medical Center over Ten Years: 2004~2013

  • To provide guidelines for the empirical treatment of urinary tract infections, we observed annual changes in the occurrence frequency and antimicrobial susceptibility of urinary isolates in a university hospital in the Chungbuk province, South Korea, over a period of 10 years (2004~2013). Escherichia coli (38.2%), Enterococcus faecalis (11.7%), Klebsiella pneumoniae (7.3%), Pseudomonas aeruginosa (4.3%), E. faecium (4.3%), and Staphylococcus aureus (4.1%) were commonly isolated urinary pathogens. The prevalence of E. coli, E. faecium and Streptococcus agalactiae were significantly higher in females (P < 0.001), whereas E. faecalis, P. aeruginosa and S. aureus were significantly more common in male patients (P < 0.001). E. coli mostly frequently showed resistance to ampicillin (67.94%), followed by trimethoprim/sulfamethoxazole (36.06%) and ciprofloxacin (26.84%). Over the studied time period, resistance rates of E. coli to ciprofloxacin significantly increased (20.44% to 33.55%). Moreover, extended-spectrum $\beta$-lactamase (ESBL) producing isolates also significantly increased in E. coli (4.2% to 18.3%) and K. pneumoniae (9.6% to 26.9%). In addition, the proportion of vancomycin-resistant Enterococcus facium (VRE) also increased (15.7% to 25.0%). In conclusion, over the last 10 years, the proportions of ciprofloxacin resistant E. coli and multidrug-resistant bacteria, such as ESBL and VRE have significantly increased. This trend must be strictly controlled and demonstrates the need for more updated guidelines for the treatment of urinary tract infections.
