  • Objectives: There are few studies which reflects of each generation's own features, of the New Middle Age and the Elderly's hearing status and their quality of life even though the population of old age has been weighted as baby boom generation's entering into old age. This study is to identify the effects of the new middle age and the elderly's hearing status on their quality of life and to prepare the customized health policies for their health promotion and quality of life improvement as well as the base line data for hearing enhancement programs. Methods: This study was analyzed using the data of the 7th term of the National Health and Nutrition Survey of the Disease Management headquarters. Out of 8,150 adults over the age of 50 who participated in the hearing test, 3,306 were selected and analyzed. Results: At the result of identifying the correlation of hearing status and the quality of life between the New Middle Age and the Elderly, it was confirmed that hearing loss affects low quality of life. The average of quality of life between the New Middle Age and the Elderly was 0.95 and 0.85 each, resulting in higher quality of life in New Middle Age than in the Elderly. The factors affecting the quality of life of the New Middle Age were the level of the education, household income, the type of health insurance, subjective health status. The factors affecting the quality of life of the Elderly were gender, the type of health insurance, subjective health status. Conclusion: Hearing loss of the New Middle Age and the Elderly affects their low quality of life. Therefore the development and the provision of policy program is needed, so as to maintain and manage hearing through age-specific health education. It is expected that the second half of the New Middle Age's life will be much happier, if not taking the treatment-oriented approach of hearing loss only but strengthening the education needed for the maintenance and the management of healthy hearing at their work place, where 50 and 60 generations' workforce has been increased.

  • Crassostrea gigas were fermented using L. brevis BJ20 to prepare fermented oyster extract (FO). The participants of this study were randomly assigned to FO and placebo (CON) groups. The FO group was given 1.0 g of FO supplementation and the CON group was given sucrose each day for eight weeks. The effects of FO supplementation on body composition, muscula r strength, and blood factors associated with muscle growth were assessed. The FO supplement was enriched with arginine (6,183.3 mg), phenylalanine (217.9 mg), leucine (122.6 mg), isoleucine (59.8 mg), valine (16.4 mg), and γ-amino butyric acid (GABA, 1,053.7 mg). The total fat was significantly decreased in the FO group compared with the CON group (p < 0.05). 60D/S Ext.T/Wo rk and 60D/S Flex.T/Work concomitantly with 60D/S Flex.PeakTQ/BW were significantly increase d by FO treatment compared to CON group (p < 0.05). However, posture stability was not significa ntly different between the groups. The levels of angiotensin-converting enzyme were significantly decreased within the FO group (p < 0.05). The FO group showed significantly decreased levels of tumor necrosis factor-α and increased levels of human growth hormone compared with the CON group (p < 0.01). The levels of insulin-like growth factor-1 increased (p < 0.01) in the FO group while that of creatine kinase and triglyceride decreased significantly compared with the CON group (p < 0.05). These results demonstrated that FO supplementation is effective in preventing sarcopenic obesity and maintaining and strengthening muscular function in elderly wom en. Hence, FO supplements can be used as functional ingredients for these benefits.

  • In chronic stroke patients, respiratory muscle weakness leads to decreased pulmonary function and balance ability. Therefore, this study was conducted to investigate the effects of respiratory training using a device and training methods that apply resistance to respiratory muscles on pulmonary function, thoracic cage expansion, and balance. A total of 20 participants were included in the study, divided into two groups: Respiratory Training Group 1 (n:10) using a resistive inspiratory device and Respiratory Training Group 2 (n:10) undergoing chest expansion resistance training. Lung function, the Korean Version of the Trunk Impairment Scale (K-TIS), Chest Expansion Measurement (CEM), and the Functional Reach Test (FRT) were assessed before and after the respiratory training. The training was conducted five times a week for a total of six weeks. Both Respiratory Training Group 1 and Group 2 showed significant improvements in lung function, K-TIS, CEM, and FRT scores, with no significant differences between the groups. The results of this study showed that breathing training using a device and breathing training that applied resistance to respiratory muscles were effective for pulmonary function, thoracic cage expansion, and balance, and that breathing training that applied direct resistance to respiratory muscles had a positive effect on strengthening respiratory muscles and improving balance ability, and that it was judged important to perform it in conjunction with general rehabilitation treatment for chronic stroke patients for functional recovery.

  • Through a simple study of the medical classics in the '$\bar{A}yurveda$', we have summarized them as follows. 1) Traditional Indian medicine started in the Ganges river area at about 1500 B. C. E. and traces of medical science can be found in the "Rigveda" and "Atharvaveda". 2) The "Charaka" and "$Su\acute{s}hruta$(妙聞集)", ancient texts from India, are not the work of one person, but the result of the work and errors of different doctors and philosophers. Due to the lack of historical records, the time of Charaka or $Su\acute{s}hruta$(妙聞)s' lives are not exactly known. So the completion of the "Charaka" is estimated at 1st${\sim}$2nd century C. E. in northwestern India, and the "$Su\acute{s}hruta$" is estimated to have been completed in 3rd${\sim}$4th century C. E. in central India. Also, the "Charaka" contains details on internal medicine, while the "$Su\acute{s}hruta$" contains more details on surgery by comparison. 3) '$V\bar{a}gbhata$', one of the revered Vriddha Trayi(triad of the ancients, 三醫聖) of the '$\bar{A}yurveda$', lived and worked in about the 7th century and wrote the "$A\d{s}\d{t}\bar{a}nga$ $A\d{s}\d{t}\bar{a}nga$ $h\d{r}daya$ $sa\d{m}hit\bar{a}$ $samhit\bar{a}$(八支集)" and "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$(八心集)", where he tried to compromise and unify the "Charaka" and "$Su\acute{s}hruta$". The "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$" was translated into Tibetan and Arabic at about the 8th${\sim}$9th century, and if we generalize the medicinal plants recorded in each the "Charaka", "$Su\acute{s}hruta$" and the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", there are 240, 370, 240 types each. 4) The 'Madhava' focused on one of the subjects of Indian medicine, '$Nid\bar{a}na$' ie meaning "the cause of diseases(病因論)", and in one of the copies found by Bower in 4th century C. E. we can see that it uses prescriptions from the "BuHaLaJi(布哈拉集)", "Charaka", "$Su\acute{s}hruta$". 5) According to the "Charaka", there were 8 branches of ancient medicine in India : treatment of the body(kayacikitsa), special surgery(salakya), removal of alien substances(salyapahartka), treatment of poison or mis-combined medicines(visagaravairodhikaprasamana), the study of ghosts(bhutavidya), pediatrics(kaumarabhrtya), perennial youth and long life(rasayana), and the strengthening of the essence of the body(vajikarana). 6) The '$\bar{A}yurveda$', which originated from ancient experience, was recorded in Sanskrit, which was a theorization of knowledge, and also was written in verses to make memorizing easy, and made medicine the exclusive possession of the Brahmin. The first annotations were 1060 for the "Charaka", 1200 for the "$Su\acute{s}hruta$", 1150 for the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", and 1100 for the "$Nid\bar{a}na$", The use of various mineral medicines in the "Charaka" or the use of mercury as internal medicine in the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", and the palpation of the pulse for diagnosing in the '$\bar{A}yurveda$' and 'XiZhang(西藏)' medicine are similar to TCM's pulse diagnostics. The coexistence with Arabian 'Unani' medicine, compromise with western medicine and the reactionism trend restored the '$\bar{A}yurveda$' today. 7) The "Charaka" is a book inclined to internal medicine that investigates the origin of human disease which used the dualism of the 'Samkhya', the natural philosophy of the 'Vaisesika' and the logic of the 'Nyaya' in medical theories, and its structure has 16 syllables per line, 2 lines per poem and is recorded in poetry and prose. Also, the "Charaka" can be summarized into the introduction, cause, judgement, body, sensory organs, treatment, pharmaceuticals, and end, and can be seen as a work that strongly reflects the moral code of Brahmin and Aryans. 8) In extracting bloody pus, the "Charaka" introduces a 'sharp tool' bloodletting treatment, while the "$Su\scute{s}hruta$" introduces many surgical methods such as the use of gourd dippers, horns, sucking the blood with leeches. Also the "$Su\acute{s}hruta$" has 19 chapters specializing in ophthalmology, and shows 76 types of eye diseases and their treatments. 9) Since anatomy did not develop in Indian medicine, the inner structure of the human body was not well known. The only exception is 'GuXiangXue(骨相學)' which developed from 'Atharvaveda' times and the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$". In the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$"'s 'ShenTiLun(身體論)' there is a thorough listing of the development of a child from pregnancy to birth. The '$\bar{A}yurveda$' is not just an ancient traditional medical system but is being called alternative medicine in the west because of its ability to supplement western medicine and, as its effects are being proved scientifically it is gaining attention worldwide. We would like to say that what we have researched is just a small fragment and a limited view, and would like to correct and supplement any insufficient parts through more research of new records.

  • The objectives of this study was to analyze the major causes of decreasing utilization rate of health care institutios in pilot-project area of regional medical insurance, Kwang-hwa and Kun-wi country. After the implementation of medical insurance, utilization rate of health institutions turned out' to be lower than it was estimated, when the pilot-project of regional medical insurance was planned. It might be due to changes in inhabitant's behavioral attitude toward medical insurance. So this study was made to find measures for financial stability by increasing utilization rate of health care institutions and to be available for basic demand-supply program of medical care. The hypothesis of this study was as follows; First. there is difference in understanding health care institutions between Kun-wi and Kwang-hwa. Second. respondesnts of inquiry survey have exact knowledge of their past experience of treatment taken prior to enforcement of medical insurance, Questionaire survey was made as to each 700 household among total 11, 884 households in Kun-wi and 20,919 households in Kwang-hwa. In case of Kun-wi, 70% of inquired households (491) gave their answers. In Kwang-hwa, the number was 560 households (80% of inquireds). Dollected data was processed and analyzed by way of using SPSS batch system. To evaluate facto rs distribution aspects of data and to make comparison between two area, percentage and $X^2$ distribution were applied. The results were as follows; L The utilization rate of health care institutions in Kun-wi and Kwang-hwa was lower than it was estimated. when pilot-project of medical insurance was planned. 2. Prior to the implementation of medical insurance. inhabitants in two area chose the medical institutions considering such factors. First was medical care fee cheap. second in habitant's residence, Third was the institutions conveniently easy of access. 3. After the implementation of medical insurance. 26.1% of inqurieds in Kun-wi and 41.6% in Kwang-hwa, changed medical institutions. In case of Kwn-wi, from health care institution (p 0.05), and in case of Kwang-hwa, vice versa, from general medical institutions to health care institutions. 4. Evaluation by factors were made such as follows. Inquired gave high marks to following facts: In case of Kun-wi, general medical institutions were difficult of access and relation between patients: was not friendly, but burden of medical expenditure was light. Effects of treatment and facilities was good. In case of Kwang-hwa, inquired gave high evaluation marks to the follow ing facts; facilities of medical institutions was not good, but the burden of medical expenditure was light. 5. After the implementation of medical insurance, the services was evaluated as good, but inquired hopec for lessening the burden of medical expenditure. 6. In case of exact understanding of cost-sharing, the evaluation rate in Kwang-hwa was higher than that of Kun-wi (p < 0.005). And positive attitude toward necessity of medical insurance was also good in Kwang-hwa (p < 0.05). 7. In case of inquired's attitude toward medical institutions, Kwang-hwa showed positive response (p < 0.05) 8.
