DNA Repair Capacity in Peripheral Blood Lymphocytes Predicts Efficacy of Platinum-based Chemotherapy in Patients with Gastric Cancer

  • Objective: To investigate the correlation between ERCC1 expression levels in tumor tissue and peripheral blood lymphocytes (PBL) from patients with gastric cancer and assess the relationship between PBL DNA repair rate (DRR) and the efficacy of platinum chemotherapy. Methods: A total of 53 patients with gastric cancer receiving surgery and 20 controls were studied. ERCC1 protein expression in tumour tissue and PBL were determined by immunohistochemical staining. The PBL DRRs of 47 advanced patients and 20 controls were estimated by comet assay. Results: The positive expression rates of ERCC1 were 67. 9%, 56. 6% and 10.0% in tumour tissues, PBLs of gastric cancer patients, and PBLs of the control group. PBL ERCC1 expression correlated with that in tissue (${\chi}^2$=15. 463, p=0.000). Pearson contingency coefficient=0.475). DRRs of cancer patients by tail length (TL) (Z=4. 662, p=0.000) and tail moment (TM) (Z=3. 827, p=0.000) were significantly lower than that of control group. When TL was applied as an indicator, the correlation between DRR and chemotherapy efficacy was significant (Spearman rank correlation r=0.327, p=0.032). Patients with low levels of DRR in PBL presented better short-term efficacy of chemotherapy than those with high levels of DRR. Conclusions: The ERCC1 expression in PBLs may indirectly reflect ERCC1 expression in gastric cancer tissues. Compared with non-cancer populations, patients with gastric cancer may have lower DNA repair capacity. DRR in PBL may predict the short-term efficacy of platinum-based chemotherapy for patients with advanced gastric cancer.

  • Objective : We conducted this study to analyze some of the trends of treatments from the recent Chinese medical journals and contribute them to the oriental medical experiments and clinical research for the future. Methods : By referring to the Chinese medical journals that were published in China from 1999 to March, 2002, we analyzed the effective rates on frequency, methods, medical treatment, and diseases. Result : The average effective rate was 90.93%, which was very high. Lately, there have been no big changes in the methods of treatment. The methods of coordinating stomach-Qi, supplying Yin element, replenishing bone marrow, tonifying essence of life, and removing blood stasis are used by means of reinforcing Qi, producing blood, tonilying spleen, and replenishing the kidney. Conclusions : The decreased in bone marrow and leucopenia, and the side effects of radiotherapy and chemotherapy were more effectively treated when we used Korean oriental medical treatments. In addition, more Korean oriental medical experiments and clinical studies are needed in Korea, in the future.

  • We came to the conculsion after considering all of information from many kinds of books on the cause, pathogenesis and treatment of Flaccidity-syndrome. The results were as follows : 1. Flaccidity-syndrome means limb-relaxation due to muscle atony that isn't able to constraction. It's begun as mild degree from extremities, in some cases ended to quadriplegia or expire. 2. Cause factor and pathogenesis of Flaccidity-syndrome is various. After Lung fluid consuption caused by heat-evil was refered in The Yellow Emperor's Canon of Internal Medicine. They were refered as cause factor that Main channel asthenia, excess of sexual intercourse, wetness-evil, heat-evil gets into the interior, asthenia of the spleen and stomach. Since Ming Dynasty, It's classified to wetness-heat evil, wetness-phlegm, deficiency of vital energy, deficiency of blood, deficiency of yin, blood stasis and indygestion, etc. 3. In the view of treatment of Flaccidity-syndrome, Yangming was selected in The Yellow Emperor's Canon of Internal Medicine, and it's been mean to clear away wetness-heat evil located at Yangming. In the method of acupuncture it was same on the base, and many skills have been used that electronic acupuncture, point-injection theraphy, acupuncture point block, catgut implantaion at acupuncture point, cutaneous acupuncture, auriculo-acupuncture and head acupuncture by the through post generation. 4. Flaccidity-syndrome was defined to weak, disuse and non-pain. Beacause it was non-pain, so medicine to expel wind-evil was prevented to use. But through post generation Flaccidity-syndrome has been treated that is able to cause pain or numness as arthralgia-syndrome. Therefore there is tendency that medicine to expel wind-evil is capable within pathological basement of Flaccidity-syndrome in recent. 5. In the view of west-medicine, Flaccidity-syndrome is diplegia or quadriplegia with sensory disorder, muscle atropy in some cases. And there are spinal disease, peripheral nerve disease, muscular disease, nerve-muscle copula disease. The symptoms are able to amyotomia, numness, sensory disorder, pain.

The Origin, Change of the Time and Constructive Principle of Insamjinpi-tang(人蔘陳皮湯) (인삼진피탕(人蔘陳皮湯)의 기원(基源), 변천과정(變遷過程) 및 구성원리(構成原理))

  • 1. Objectives: This paper was written to understand the origin, changes and the constructive principles of Insamjinpi-tang(人蔘陳皮湯) 2. Methods: Insamjinpi-tang and other related prescriptions were analyzed in terms of pathology, based on "Dongyisusebowon Chobongwon(東醫壽世保元 草本卷)", "Dongyisusebowon Gabobon(東醫壽世保元 甲午本)", "Dongyisusebowon Sinchukbon(東醫壽世保元 辛丑本)", "Donguisasangsinpyeon(東醫四象新編)" 3. Results and Conclusions: (1) The origin of Insamjinpi-tang is Doksamyangbung-tang(獨蔘良崩湯), which is combination of Yijung-tang(理中湯), and Insamkeji-tang(人蔘桂枝湯) (2) The origin of Insamjinpi-tang was derived from Doksamyangbung-tang of "Dongyisusebowon Gabobon". Insamjinpitang greatly increases Ginseng Radix Alba(人參) when Insamjinpitang was compared with Yijung-tang, Insamkeji-tang. Insamjinpitang put to use Amomi Fructus(砂仁) newly first. (3) The Insamjinpitang composed of 5 herbs, Ginseng Radix Alba(人參), Zingiberis Rhizoma Crudus(生薑), Citrus unshiu (陳皮), Zizyphus jujuba(大棗) and Amomi Fructus(砂仁); Ginseng Radix Alba(人參) and Zingiberis Rhizoma Crudus (生薑) treat Taeumjeung(太陰證) of Soeumin(少陰人) through warming the Stomach(溫胃), Citrus unshiu(陳皮) makes the Yin(陰) downbearing, Zizyphus jujuba(大棗) making harmony between innerside and outerside of body and Amomi Fructus(砂仁) making stable one's mind.

  • Objectives : Chim-Gu-Kyung-Heom-Bang(CGKHB; 鍼灸經驗方) is the needle and moxa specialized document written by the doctor specialized in needle and moxa treatment of Joseon(朝鮮), Heo Im(1570-1647). The document was published in April 1644(22nd year of King Injo). CGKHB contains the needle and moxa treatment techniques accumulated by the Joseon Dynasty as well as the personal experience of Heo Im. The aim of this study is to restore the past treatment method as a method of Needle Insertion Method at CV12(NIM-CV12, needle to penetrate blood vessel technique) in CGKHB.. Methods : Through Dong-Yi-Bao-Gam(DYBG; 東醫寶鑑), the implication of Korean medicine study of the Jungwan(CV12) has been studied. Next is the contemplation of the NIM-CV12 of CGKHB with the Acupuncture Treatment Using Jungwan(中脘鍼法) of Park Tae-won and Acupuncture Treatment for Byeokjeok(癖積鍼法) of "Geup Yubang". Results and Conclusions : 1. CV12 is one of the acupuncture points representing stomach(脾胃), middle energizer(中焦), phlegm-fluid retention(痰飮) and greater yin(太陰). 2. NIM-CV12 of CGKHB is the technique to penetrate the needle into the CV12 strictly relying on tactile sense of a person who give the penetration. This CV12 administration was carried out at intervals of every 7 or 8 day. During the administration period, the patient was not allowed to intake excessive amount of food. 3. The Acupuncture Treatment Using Jungwan(中脘鍼法) of Park Tae-won and Acupuncture Treatment for Byeokjeok(癖積鍼法) of "GeupYubang", existed in the same era of the NIM-CV12 of CGKHB, have similar linkage to the NIM-CV12 of Heo Im.

  • The "ShikLyoChanYo", written in 1460 by JunSoonYi (全循義), master court doctor in JoSeon (朝鮮) Dynasty, is the very first specialty publication of Korean dietary treatment existing today. Both Chinese and Korean scholars have assumed that this book had been lost long time ago. In November 2003, however, a Korean philologist found a version of the book, Yangyang (襄陽, a district name in Korea), and this book has attracted a lot of interest of Korean traditional medical science and agricultural science since then. This paper is to dissert the document of food therapy from the book with profound document study and statistical analysis in the fields of traditional Chinese medicine and traditional Korean medicine on dietetics. It completes the study of the application of all the dietetic treatments according to symptoms of diseases and all the plants and medication applied to cure chronic conditions that are clinically examined for the purpose of food therapy. A general survey on sundry records related to this food therapy of the "ShikLyoChanYo" has been done to make this dissertation and it carried out a statistic analysis of all the dietetic mixing technique of all plants and medication. Among other symptoms of illnesses from the book, there are 7 frequently addressed ailments chosen from the aspect of food therapy - a stroke, a disease diagnosed by thirst, a serious cough, an ache resulting from numbness, a disease relating to stomach, blurry vision and weak hearing, and a drinking related disease. This part is to discuss these illnesses and how to cure them with food based on its characteristics and rules of application.

  • The xin(心) has various meanings in ${\ll}$Huangdineijing(黃帝內經)${\gg}$ but they sometimes contradict each other. This thesis divided the xin into the meaning and the Zang-xiang(藏象), and then analyzed the xin's notion in detail. The concept of the xin in ${\ll}$Huangdineijing(黃帝內經)${\gg}$ is sorted out into : the notion of space, yin-yang five elements(陰陽五行) and shen(神) The xin is the upper part of body and it possesses the character of yang(陽). So the concept of the breast has originated from this character and it rightly belonged to the top. The xin is assigned to fire among five elements, 'chang(長)', which has the energy of moving forward, noon at a day when yang-qi(陽氣) is properous and shows 'gu(鉤)' & 'keo(矩)' in pulse condition. The xin possesses the character, 'Taiyang of the yang(陽中之太陽)' along with the notion of space combined with five elements. That is, the notion of upper space means 'of the yang(陽中)', and, fire in five elements means 'yang'. This is similar to '=(Taiyang)' of Sasang(四象) at ${\ll}$the Book of Changes(周易)${\gg}$ Since the xin puts shen(神) in order, actions of spirit have effect on the xin. And it depends whether the sense of vitality is broad or narrow. The xin related with broad sense of spirit is 'monarch of the organs(君主之官)'. Therefore it has control over the human body. As it also directly effects the life or death, Pericardium(心句) substitutes the xin and protects the external invasion. In Shi-er-won(十二原) and Bonsu(本輸), instead of the Xin Channel the Pericardium Channel was used in healing patients. The xin can be interpretable as the mind, because the xin includes spirit. The mind can be distinguished into 'desire' and 'state of profound reason'. In ${\ll}$Huangdineijing(黃帝內徑)${\gg}$, the disease of the xin caused by emotion was mentioned many times. This emotion is 'desire' which resorted to the sentiment. The reason one mind has both character is; man preserves given principle (reason) and emotion reveals via the reason exercised. The above is about the xin related with the broad sense of vitality. Concerning the narrow sense of vitality, one of the five vitalities is stored with the others away in the five solid organs. Then it takes part in the operation of five body constituents and it is linked with the personified description of five solid organs. The xin, spleen, stomach and kidney are 'the ground of life'. Spleen and stomach are the origin of making qi and blood, which 'means the ground after birth'. Kidney keeps the essence of life, and manages the growing and generative function of human body. The xin keeps 'Shin-myung(神明)', in other words, it has control over and supervise whole activity of body. Therefore xin's role is needed for the appropriate working of spleen, stomach and kidney. And 'Shin-myung' is its motive power. In ${\ll}$Huangdineijing(黃帝內經)${\gg}$, the reason why xin was assigned to September and October is that yang-qi of the human body goes to the inner part, with xin at the same time. This explains that yang-qi of the human body is adapted to change of season and goes into xin-fire(心火) in order to get away from the cold. In this case, heart means more inner part than liver, spleen and lung. Mengzi(孟子), philosopher of the China's turbulent ages emphasized the thinking function of xin. Sunzi(荀子) asserted that xin is 'heaven monarch(天君)' and the other organs are 'heaven rninisters(天官)'. This conception is similar to 'monarch of the organs' of ${\ll}$Huangdineijing(黃帝內經)${\gg}$. After the Ming Dynasty, commentators of Huangdineijing(黃帝內經) explained the heart, as 'monarch of the organs', or 'the master of body(一身之主)'. This was due to the influence of Sung Confucianism.

Non-toxic and Anti-oxydative effect of Dioscoreae Rhizoma on PC12 Cell (안태(安胎)에 활용되는 산약(山藥)의 신경세포주에 대한 안전성 및 항산화효과에 대한 연구)

  • Purpose : This study examined the non-toxic and the anti-oxidative effect of Dioscoreae Rhizoma on PC12 cells. Sanyak(Dioscoreae Rhizoma; chinese yam, shan yao) is well-known for its curing power for kidney, lung, spleen. Tonifies and augments the spleen and stomach. Tonifies the lung gi and augments the kidney yin. Tonifies the kidneys and also stabilizes and binds. it also binds the essence and treats spermatorrhea, frequent urination, and vaginal discharge. We are therefore interested in whether Dioscoreae Rhizoma is capable of causing abnormal apoptosis processes, and whether this condition can be rectified through Dioscoreae Rhizoma herb treatment. Methods : We used aqueous extract to treat PC12 cells with different concentrations treated with a water or a MeOH extract of Dioscoreae Rhizoma (0, x10, x20, x40, x80). The MTT (3, (4, 5-dimethyl-thiazol) 2, 5-diphenyl-tetraxolium bromide) reduction assay was employed to quantify the differences in cell activity and viability. The Bax expression level was monitored using western-blotting techniques. The patterns of the changes in expression were scanned and analyzed. Results : Bax and GSK-3${\beta}$ promotes cell death and down-regulated during the development of the PC12 cells. This is indicated that Dioscoreae Rhizoma is capable of inducing apoptosis in PC12 cells. The induced cell death and significantly inhibited by Dioscoreae Rhizoma, which can be explained by the increase in the inhibition of Bax and GSK-3${\beta}$ expression. It was also shown that Dioscoreae Rhizoma inhibits the release of $H_2O_2$ and prevents lipid peroxidation. Furthermore, the accumulation of wild type Bax protein significantly downregulated in a dose-dependent manner upon treatment with Dioscoreae Rhizoma. Conclusion : In conclusion, Dioscoreae Rhizoma can induce apoptosis via a Bax-dependent pathway or GSK-3${\beta}$ dependent pathway in PC12 cells into anti-oxidant and protective effect.

An Outlook of the Oriental and Western Medical Diagnosis and Treatment on Large Bowel Cancer (대장암(大腸癌)의 동서의(東西醫) 결합(結合) 진치근황(診治近況))

  • Large bowel cancer shows the 4-5th frequency in cancers that occurs in Korea. The western medicine cures the Large bowel cancer by radiation, surgery and chemotherapy. While, Oriental medicine cures the Large bowel cancer by Herb-drugs, acupuncture, moxa and et al. With just one way of treating Large bowel cancer can't be effective remedy. Because each medicine has a strength and weakness, it is effective treatment when two medicine combines and supplement each other. We got the following result about a trend of oriental and western combination treatment for Large bowel cancer through studding records. 1. In Large bowel cancer, colon cancer is referred hematochezia(腸風下血), rectal cancer is refereed enterotoxin(腸毒), and anal cancer is accumulation of pathogens in yin(結陰). 2. The western medicine treats Large bowel cancer patient with surgery first. They need on assembly treatment such as chemical, radiation and immune treatment. In oriental medicine, they treats Large bowel cancer patients with differentiation of symptom and signs and treatment(辨證施治) for example, insufficiency of spleen and stomach(脾胃虛弱), collapse of the spleen-ql(脾氣下陷), stagnation of blood stasis and toxic agent(瘀毒內結), reinforcing both qi and blood(脾血下陷), stagnation of damp-phlegm(痰濕凝結) and cure for them by acupuncture and moxa too. 3. In combination with oriental and western medical treatment princple of Large bowel cancer by each stage is as follows. First stage is cured with radical surgery and herb-drugs without chemotherapy. The intermediate and terminal stage patients is used radiation before surgery, or after palliative surgery cour with chemotherapy, radiation and Herb-drugs. In terminal stage patients, unable for surgery, is used combination between chemotherapy, palliative radiation and Herb-drugs. 4. After radiation surgery, the terminal stage patients who have extensively lymph node metastasis or local contraindication is able to undergo combination of Herb-durgs and chemotherapy. 5. The cure-effect with oriental and western medicine combination treatment was better than that just with oriental or western medical treatment. 6. The merits of oriental and western medicine combination treatment lengthen one's life and diminish the bad effect of chemotherapy and complete radiation treatment, prevent from relapsing, maintain the balance in their environment of body and improve immunity.

The Origin, Change, and Composition of Soeumin's Bojungikgi-tang (소음인(少陰人) 보중익기탕(補中益氣湯)의 기원, 변천과정 및 구성 원리)

  • 1. Objective : The purpose of this study is to understand the origin, changes and composition of Soeumin's Bojungikgi-tang. 2. Methods : 1) Analyzed the context of the progressive transformation of soeumin's Bojungikgi-tang in related passages from "Donguibogam (東醫寶鑑)", "Donguisusebowon Chobongwon (東醫壽世保元 草本卷)", "Donguisusebowon Gabobon (東醫壽世保元 甲午 本)", "Donguisusebowon Sinchukbon (東醫壽世保元 辛丑本)", "Dongmuyugo (東武遺稿)" 2) Examined the differences between the utilization of Soeumin's Bojungikgi-tang in pre-existing manuscripts according to pathological pattern and more specifically at a pathological stage. 3) Analyzed the total primary effect of the herbs as a prescriptive category, as well as the herbal properties of each component. 3. Results and Conclusions : 1) Originally Dongwon's Bojungikgi-tang was prescribed as a stomach & spleen organ tonifying medicine so it was easily applied and established as a prescription appropriate for Soeumin constitutional types with a weaker spleen organ. 2) From Gabobon(甲午本), Soeumin's Bojungikgi-tang was modified, excluding Bupleurum falcatum(柴胡), Cimicifuga heracleifolia (升麻) and substituting Teucrium chamaedrys(藿香), Ophiopogon japonicus(蘇葉) which are herbs appropriate for the Soeumin constitutional type. 3) In the Gabobon(甲午本), the Gegi-tang which has yang raising properties is included in the Soeumin's Bojungikgi-tang. In Sinchukbon(辛丑本), the Gegi-tang of the Soeumin's Bojungikgi-tang in the Gabobon was excluded and the Gegi-tang mutated into Hwangigyeji-tang. 4) In the Gabobon(甲午本), as the Soeumin's Bojungikgi-tang was prescribed for Soeumin disease regardless of favorable-unfavorable patterns and mild-severe patterns, the stage of the disease when to prescribe Soeumin's Bojungikgi-tang was not clarified. 5) As of the Sinchukbon(辛丑本) edition, as Soeumin's prescriptions have been obvious, the soeumin's Bojungikgi-tang was applied to Mang-yang disease with Hwangigyeji-tang and Seungyangikki-tang 6) As of the Sinchukbon(辛丑本) edition, the Soeumin's Bojungikgi-tang has been applied to parients having both exterior-heat Mang-yang disease and Greater-yin disease, at the same time. 7) As of the Sinchukbon(辛丑本) edition, finally, the Soeumin's Bojungikgi-tang formula has been established as gathering Qi and middle tonification with Panax ginseng(人蔘), Astragalus membranaceus(黃芪), keeping the spleen healthy with Artractylodes japonica(白朮), keeping the spleen strong with Angelica acutiloba(當歸), rooting the spleen with Citrus reticulata(陳皮), raising Yang and middle jiao with Teucrium chamaedrys(藿香), Ophiopogon japonicus(蘇葉), middle tonification with Glycyrrhiza glabra(甘草灸), Zingiber officinale(生薑), Zizyphus jujuba(大棗).
