식도암 수술 후 흉곽 내 위장에 발생한 원발성 위암에 대한 광역동치료 -1예 보고 - (Photodynamic Therapy for Neoplasms in Intrathoracic Constructed Stomach)

  • 광역동치료는 흉부외과 영역에서 조기 식도암, 기관 및 기관지 종양, 위장관 점막 내 종양 및 바레트식도 등에서 광감작물질의 광파장 및 광선의 암세포에 대한 선택적이고 직접적인 독성 효과를 이용한 비수술적 치료로 안전하고 효과가 있으며, 기타 진행된 폐암이나 식도암에서 보존적 요법으로도 이용될 수 있다고 여겨지고 있다. 그러나 광역동치료 후 광독성에 의한 피부 독성이 흔하여, 직사 광선의 차단 및 약물 요법이 병행되어야 한다. 저자들은 식도암 수술 후 흘곽 내에 거치된 위장에 발생한 원발성 위암에 대한 광역동치료(Photodynamic tharapy)로 좋은 결과를 얻었기에 문헌 고찰과 함께보고하고자 한다.

  • 식도, 위, 폐 그리고 구강에 동시 발생한 다발성 원발암에 대해 보고하고자 한다. 65세 남자 환자가 구강 내 종괴에 대해 상피세포암을 진단받고 광범위 절제술을 시행 받았다. 수술 후 정확한 병기 결정을 위해 시행한 검사에서 식도암, 위암 그리고 폐암이 추가로 확인되었다. 저자들은 상기 세가지암에 대하여 한 차례 수술로 근치적 절제술을 시행하여, 만족할 만한 결과를 얻었기에 이에 대해 보고하는 바이다.

  • 위장관 간질 종양은 위장관 전역에서 발생하며 그 중 위와 소장 등에 많이 생기며 식도에서도 발생한다. 44세 남성이 연하곤란과 체중감소를 주소로 내원하여 시행한 상부 위장관 내시경 검사 및 흉부 전산화 단층 촬영에서 식도 하부의 완전폐쇄, 위의 전방 편위 소견과 중앙부 괴사를 동반하는 최대 직경 15cm 정도의 저음영의 거대 종괴를 보여 흉복부 절개를 이용하여 하부 식도 및 위전적출술을 시행하였다 종양은 CDl17 (c-kit)와 CD34에서 양성을 보여 악성 위장관 간질 종양으로 확진되었으며 현재 외래 추적관찰 중이다.

  • Plexiform angiomyxoid myofibroblastic tumor (PAMT) of the stomach is a very rare mesenchymal tumor of the gastrointestinal tract. We report a case of asymptomatic gastric PAMT that was pathologically confirmed after surgical resection. The tumor had a multinodular plexiform growth pattern, bland-looking spindle cells, and an Alcian bluepositive myxoid stromal matrix rich in small blood vessels. Immunohistochemistry analysis revealed that the tumor cells of the PAMT were positive for smooth muscle actin (SMA) and negative for c-kit, CD34, S-100 protein, epithelial membrane antigen (EMA), and desmin. PAMT should be differentiated from other submucosal tumors of the stomach by immunohistochemical findings. Considering the benign features of this tumor, observation without resection may be an option for the treatment of PAMT if the tumor is asymptomatic.

An Effect of Letrozole on Gastric Cancer?

  • Letrozole is a drug used in the treatment of postmenopausal women with breast and ovarian tumours. There is no evidence in the literature indicating its use in treating gastric cancer. We present a 68 year old lady admitted from the emergency department with weight loss, malaise and anaemia. Investigations confirmed the presence of two different primary tumours in the left breast and the stomach. Following that this patient with oestrogen receptor positive breast cancer and oestrogen receptor negative gastric cancer was treated with letrozole for her breast cancer followed by a gastric resection. Independent histology by two pathologists pre-operatively diagnosed gastric adenocarcinoma. Post-operatively, independent analysis of the resected stomach, omentum and lymph nodes revealed no evidence of gastric cancer. Therefore we conclude that there is a possibility of letrozole having an effect on gastric cancer. Further studies are needed.

Mixed Exocrine and Endocrine Carcinoma in the Stomach: A Case Report

  • We report a rare case of the coexistence of a gastric small cell neuroendocrine carcinoma with a gastric adenocarcinoma. A 62-year-old man presented with epigastric soreness for 1 month. Esophagogastroduodenoscopy revealed a Borrmann type I tumor at the lesser curvature of the lower body of the stomach. The patient underwent a distal gastrectomy with D2 lymph node dissection and the resected specimen exhibited a $3.5{\times}3.5$ cm sized, fungating lesion. Two separated, not intermingling, lesions with non-adenocarcinoma components encircled by well differentiated adenocarcinoma components were identified microscopically. The non-adenocarcinoma component showed neuroendocrine features, such as a solid and trabecular pattern, and the tumor cells showed a high nuclear grade with minimal cytoplasm, indistinct nucleoli, and positive response for synaptophysin, CD56. The final pathological diagnosis was a gastric mixed exocrine-endocrine carcinoma (MEEC) composed of an adenocarcinoma and small cell neuroendocrine carcinoma of the collision type.

Early Gastric Cancer with Signet Ring Cell Histology Remained Unresected for 53 Months

  • The natural course of untreated patients with signet ring cell carcinoma of the stomach remains poorly understood while assumptions have been made to distinguish it from other types of gastric cancer. A 74-year-old Korean woman was diagnosed with early gastric cancer with signet ring cell histology and refused surgery. A satellite lesion was identified 46 months after the initial diagnosis. The patient finally agreed to undergo distal subtotal gastrectomy 53 months following the initial diagnosis. Postoperative histological examination of both lesions confirmed signet ring cell carcinoma associated with submucosal invasion. There was no evidence of lymph node metastasis.

  • Gastric complications following unintentional foreign body ingestion are extremely rare. Here, we report the case of a 59-year-old healthy woman who presented with nonspecific abdominal pain and an apparent gastric submucosal tumor that was incidentally detected by gastrofiberscopy. The patient underwent laparoscopic surgery, which revealed an intact gastric wall with no tumor invasion, deformity, or evidence of a gastric submucosal lesion. However, an impacted fish bone was found.

Totally Robotic Esophagectomy

  • Totally robotic esophagectomy is performed using a robotic technique without additional thoracoscopy or laparoscopy. However, most robotic esophagectomies are currently performed in a hybrid form combining robotic and other endoscopic techniques. Laparoscopic stomach mobilization and thoracoscopic esophagogastric anastomosis are commonly used methods in robotic esophagectomy. In this paper, totally robotic esophagectomy without thoracoscopic or laparoscopic assistance is presented.

  • 식도절제술 후 재건술에서 위장은 가장 흔한 식도 대체물로 사용된다 그러나, 인두 이상의 위치까지 위장을 끌어 올려 연결하는 경우 위장만으로는 길이가 모자라는 경우가 많이 있다. 후두암과 식도암을 동반한 환자에서 후두전절제술과 식도전절제술을 시행하였고 유리공장이식편을 이용하여 경부에서 인두공장위장문합을 시행하였다. 유리공장을 이용하여 문합에 충분한 길이를 확보함으로써 문합부 긴장을 줄이고, 혈액공급을 확보할 수 있었다.
