기능적전기자극을 이용한 보행훈련이 편마비 환자의 하퇴근육의 근전위활동에 미치는 영향 (Influence of Functional Electrical Stimulation during Gait Training on Motor Unit Action Potential)

  • This study divided 35 hemiplegia patients into control group having standard physical therapy and gait training and functional electrical stimulation(FES) group using FES during gait training in order to examine the effects of applying FES to ankle joint dorsiflexor on motor unit action potential. Stimulation conditions of FES were pulse rate 35 pps, pulse width $250{\mu}s$, and on-time 0.3 second, treatment hour was 30 min. and treatment period was once a day for five days a week through six weeks. For functional evaluations before and after treatment, root mean square(RMS) were measured and the following conclusions were obtained. : In RMS analysis of motor unit action potential, gastrocnemius was significantly reduced in both weight bearing(p<.001) and bearing condition (p<.05). In conclusion, application of FES to hemiplegia patients in recovery stage during gait training decreased RMS and it was interpreted that it was caused by mitigation of muscular spasticity by reduction of motor unit.

봉침요법(蜂針療法)에 대(對)한 고찰(考察) -월간(月刊) $\ll$양봉계(養蜂界)$\gg$의 '봉침요법(蜂針療法)에 대(對)한 연구(硏究)'를 중심(中心)으로- (A Study on Bee Sting Therapy: Based on'Research on Bee Sting Therapy' of Monthly Yangbonggye)

  • Objectives : To survey concept, meaning, and problems of beekeeper's bee sting therapy by reviewing and summarizing outline, indications, and clinical cases of 'Research on Bee Sting Therapy' described in Monthly Yangbonggye. Methods : As a narrative review, literature researches were carried out based on 'Research on Bee Sting Therapy' in Monthly Yangbonggye in the aspects of outline, indications, and clinical cases. In the concrete, outline was just summarized and described, and indications were categorized by recent version of international statistical classification of diseases and related health problems(ICD) of WHO(World health organization), and clinical cases were collected and summarized from the viewpoint of acupoints and methods of therapy. Results : 1. Bee sting therapy is to inject venom into skin by stimulating affected lesions or acupoints on meridian with sting of bee alive for therapeutic purposes. It can be divided into two classes(direct stimulation(直針法) and indirect stimulation(拔針法)) by the differences of methods, and is different from bee venom acupuncture therapy in the aspects of the way of extraction and injection. 2. In this material, bee sting therapy has 83 indications classified into 17 of 22 chapters of ICD. Among clinical cases, cases on neoplasm, goitre, lump, and haemorrhoids by direct stimulation were especially outstanding. 3. The therapeutic acupoints selected in bee sting therapy are mainly Ashi points(阿是穴), and partly acupoints on meridian(經穴), Extra points(經外奇穴), and New points(新穴) with careful consideration of patients' condition. Conclusions : Although bee sting therapy has more or less controversial points of diagnosis, treatment, and management of side effects, it is thought that the accumulated and inherited experiences from old times can be used as meaningful material by further experimental and clinical researches.

  • 본 연구에서는 근육수축에 의해 발생되는 각 신체부분의 각가속도의 방향이 다음 두가지 요소의 변화에 따라 어떻게 달라지는가를 중점적으로 살펴보기로 한다` (1) 신체매개변수(body-segmental parameter): 길이, 무게 등, (2) 신체 각 부분의 위치. 이러한 상관관계를 더욱 명확히 이해함으로써, 주어진 운동(given task)을 FES 에 의해 수행하고자 할 때 전기적으로 자극되어야 할 근육을 사전에 선택할 수 있을 것이다.

Myocardial Function and Metabolic Energetics in Low Flow Ischemia and with $\beta$-Adrenergic Stimulation in Spontaneously Hypertensive Rat Hearts

  • The effects of cardiac ischemia-reperfusion and $\beta$-adrenergic stimulation on metabolic function and energetics were investigated in Lan gendorff-perfused spontaneously hypertensive rat (SHR) hearts. Sarcoplasmic reticulum {TEX}$Ca^{2+}${/TEX}-dependent ATPase and cardiac lactate dehydrogenase (LDH) are additionally studied. The perfusion medium (1.0 mM {TEX}$Ca^{2+}${/TEX}) contained 5 mM glucose(+5 U/L insulin) and 2 mM pyruvate as substrates. Global ischemia was induced by reducing perfusion pressure of 100 to 40 cm {TEX}$H_{2}${/TEX}O, followed by 20 min reperfusin. Isoproterenol (ISO, 1$\mu$M) was infused for 10 min. Coronary vascular resistance and myocardial oxygen consumption ({TEX}$MVO_{2}${/TEX}) of SHR were increased in parallel with enhanced venous lactate during ischemia and reperfusion compared to those of Sprague Dawley (SD) hearts. Although ischemia-induced increase in venous lactate and combined adenosine plus inosine was abolished, coronary vasodilation produced in SD during reperfusion. In SHR, depressed reactive hyperemia was associated with a fall in cardiac ATP and CrP/Pi ratio and a rise in intracellular lactate/Pyruvate ratio. On the other hand, ISO produced coronary functional hyperemia and an increase in {TEX}$MVO_{2}${/TEX}. However, these responses were less than those in SHR hearts. The ATPase activity of SHR was attenuated in free {TEX}$Ca^{2+}${/TEX} concentrations used under basal condition and with ISO compared to that of SD. Venous lactate output and cardiac LDH activity were augmented in SHR as influenced by ISO. These results demonstrate that coronary reactive and functional hyperemia was dpressed in SHR, which cold be explained by alterations in the cytosolic phosphorylation potential and the cytosolic redox state manipulated by LDH, and by abnormal free calcium handling.

Effects of Micro-Electrical Stimulation on Regulation of Behavior of Electro-Active Stem Cells

  • Purpose: Stem cells provide new opportunities in the regenerative medicine for human or animal tissue regeneration. In this study, we report an efficient method for the modulating behaviors of electro-active stem cells by micro-electric current stimulation (mES) without using chemical agents, such as serum or induction chemicals. Methods: Dental pulp stem cells (DPSCs) were cultured on the tissue culture dish in the mES system. To find a suitable mES condition to promote the DPSC functions, the response surface analysis was used. Results: We found that a working micro-current of 38 ${\mu}A$ showed higher DPSC proliferation compared with other working conditions. The mES altered the expressions of intracellular and extracellular proteins compared to those in unstimulated cells. The mES with 38 ${\mu}A$ significantly increased osteogenesis of DPSCs compared with ones without mES. Conclusions: Our findings indicate that mES may induce DPSC proliferation and differentiation, resulting in applying to DPSCs-based human or animal tissue regeneration.

전기자극이 체중 부하를 제거한 신생 흰쥐 척수조직의 NT-3 및 MAP2 발현에 미치는 영향 (Influence of Electrical Stimulation on NT-3 and MAP2 Expression of Neonatal Rat Spinal Cord During Suspension Unloading)

  • The development of neonatal neuromuscular system is accomplished by the functional interaction between the spinal neurons and its target cells, skeletal muscle cells, and the intrinsic and extrinsic factors affecting this process. The aim of this study was to identify the effect of suspension unloading (SU) and neuromuscular electrical stimulation (NMES) upon the development of the neonatal spinal cord. For this study, the neonatal rats were randomly divided into three groups: a control group, an experimental group I, and an experimental group II. The SU for experimental group I and II was applied from postnatal day (PD) 5 to PD 30, and the NMES for experimental group II was applied from PD 16 to PD 30 using NMES that gave isometric contraction with 10 Hz for 30 minutes twice a day. In order to observe the effect of SU and NMES, this study observed neutrophin-3 (NT-3) and microtubule associated protein 2 (MAP2) immunoreactivity in the lumbar spinal cord (L4-5) at the PD 15 and PD 30. The results are as follows. At PD 15, lumbar spinal cord of experimental group I and II had significantly lower NT-3 and MAP2 immunoreactivity than control group. It proved that a microgravity condition restricted the spinal development. At PD 30, lumbar spinal cord of control group and experimental group II had significantly higher NT-3 and MAP2 immunoreactivity than experimental group I. It proved that the NMES facilitated the spinal development by spinal cord-skeletal muscle interaction. These results suggest that weight bearing during the neonatal developmental period is essential for the development of neuromuscular development. Also, the NMES on its target skeletal muscle can encourage the development of the spinal cord system with a full supplementation of the effect of weight bearing, which is an essential factor in neonatal developmental process.

뇌파 관련 국내 한의학 연구에 대한 고찰 (The Review on the Domestic Korean Medicine Studies of Electroencephalogram)

  • Objectives : To research the changes of electroencephalogram(EEG) signals for acupuncture stimulation and to establish the hereafter direction for the study on EEG. Methods : We reviewed the domestic papers searched by search engine of Korean Acupuncture & Moxibustion Society and Korea Institute of Oriental Medicine. Results : We have searched 31 articles in 10 journals. The 13 articles were concerned with acupuncture. 1. All articles were published after 2001. In 2007 there were 10 articles. 2. The studies dealing with the changes of EEG signals were 24, the studies dealing with correlation of EEG signals were 5, and the studies analyzing EEG with Korean medicine were 2. 3. In the studies dealing with the changes of EEG signals, the case-control studies were 9, the non case-control studies were 14, and the case study was 1. 10 studies used electro-acupuncture, 1 study used herbal acupuncture, and 2 studies used manual acupuncture. Conclusions : We need more various kinds of studies. 1. Excited condition by acupuncture stimulation may reduce $\alpha$ wave. 2. There may be the acupuncture point-specific variation of EEG signal patterns. 3. The number of responding channels for acupuncture stimulation may correlate with the quantity or variety of acupuncture effect.

Balance trainer training with transcutaneous electrical nerve stimulation improves spasticity and balance in persons with chronic stroke

  • Objective: The purpose of this study was determine the effect of Balance Trainer training with Transcutaneous Electrical Nerve Stimulation (TENS) on spasticity and balance in persons with chronic stroke. Design: Randomized controlled trial. Methods: A total of 30 subjects with hemiparetic stroke were recruited and randomly divided into the Balance Trainer training with TENS group (n=15) and Balance Trainer training with placebo TENS group (n=15). The Balance Trainer training with TENS group practiced additional Balance Trainer training with TENS for 30 minutes a day, 5 days per a week during 4 weeks and the Balance Trainer training with placebo TENS group practiced additional Balance Trainer training with placebo TENS for the same period. Spasticity and balance were assessed by ability (static balance, dynamic balance) and were measured before and after the 4-week programs. Results: The result of spasticity and dynamic balance were improved significantly in both groups (p<0.05). The Balance Trainer training with TENS group showed significantly greater improvement in spasticity of the gastrocnemius & dynamic balance, compared to the Balance Trainer training with placebo TENS group (p<0.05). The Balance trainer training with TENS group showed a significant improvement in static balance, especially during the eye-closed condition (p<0.05). Conclusions: The Balance Trainer training with TENS was effective in improving spasticity and balance in subjects with chronic stroke. Based on these results, it is suggested that Balance Trainer training with TENS could clinically be used more actively in conjunction with conventional physical therapy.

가압훈련의 혈류 압박 정도에 따른 복합근 활동전위의 변화 (Changes in Compound Muscle Action Potential Depending on Pressure Level of Blood Flow During KAATSU Training)

  • Purpose: In recent years, there has been increasing interest in using blood flow-restricted exercise (BFRE) or KAATSU training. The KAATSU training method, which partially restricts arterial inflow and fully restricts venous outflow in the working musculature during exercise at reduced exercise intensities, has been proven to result in substantial increases in both muscle hypertrophy and strength. The purpose of this study was to investigate the proper level of pressure for KAATSU training using compound muscle action potential (CMAP) analysis. Methods: Twenty-two healthy adults voluntarily participated in this study. CMAP was conducted by measuring the terminal latency and amplitude using a motor nerve conduction velocity test. For reference-line, supramaximal electrical stimulation was applied to the median nerves of the participants to obtain CMAP for the abductor pollicis brevis. For baseline, the intensity of the electrical stimulation was decreased to a level at which the CMAP amplitude was about a third of the CMAP amplitude obtained by the supramaximal electrical stimulation. The pressure levels for the KAATSU were set as a systolic blood pressure (strong pressure), the median values of systolic and diastolic blood pressure (intermediate pressure), and diastolic blood pressure (weak pressure). In the KAATSU condition, CMAP was performed under the same conditions as baseline after low-intensity thumb abduction exercises were performed at the subjects' own pace for one minute. Results: As the pressure increased, the CMAP amplitude was significantly increased, signifying that more muscle fibers were recruited. Conclusion: This study found that KAATSU training recruited more muscle fibers than low-intensity exercise without the restriction of blood flow.

기사혈(氣舍穴, ST11)에 적용한 혈위 초음파요법이 교감신경계에 미치는 영향: 전향적 무작위 대조군 연구 (Effect of Ultrasound Therapy at the ST11 on Sympathetic Nervous System Change: A Prospective Randomized Controlled Study)

  • Objectives This study was designed to assess the effectiveness of ultrasound therapy at the ST11 for regulation of sympathetic hyperactivity. Methods Forty healthy adult subjects were assigned to experimental group and control group. After taking mental stress, ultrasound therapy was applied at the ST11 in experimental group and sham-ultrasound therapy was applied in control group. The evaluation of sympathetic activity was measured by blood pressure, pulse rate, and heart rate variability at 3 times (Time 1: before the stress stimulation, Time 2: after the stress stimulation, Time 3: after the intervention). The primary end point was consisted of normalized (norm) low frequency (LF)/high frequency (HF) ratio, LF (norm), HF (norm). The secondary end point was consisted of systolic blood pressure, diastolic blood pressure, pulse rate, mean heart rate, standard deviation of NN intervals, root mean square of the successive differences, total power (log). Results After the stress stimulation, all subjects showed sympathetic hyperactivity. After the intervention, the experimental group showed lower sympathetic activity than the control group. Comparing the Time 3 and Time 1, the experimental group showed no significantly differences in sympathetic activity while the control group showed higher sympathetic activity in Time 3 than Time 1. Comparing the Time 3 and Time 2, the experimental group showed lower sympathetic activity in Time 3 than Time 2 while the control group showed higher sympathetic activity in Time 3 than Time 2. Conclusions We suggest that the ultrasound therapy at ST11 can decrease sympathetic activity in sympathetic hyperactivity condition.
