가상현실 프로그램이 감각이상이 있는 뇌졸중 환자의 균형능력에 미치는 영향 (Effects of Virtual Reality Program on Standing-Balance in Stroke with Sensory Deficit)

  • Purpose : The purpose of this study was to investigate the effects of virtual reality(VR) program on standing balance in stroke with sensory deficit. Methods : Subjects were assigned randomly to either VR group (n=28) or the control group (n=31) when study began. Both groups were conducted electricity therapy, exercise therapy 5 times per week during six weeks, and a group who have VR group 3 times per week during six weeks. Both groups assessed for standing balance before and after virtual reality program. Results : In standing balance, the virtual reality program which have sensory deficit in stroke group, open eyes from the center area of the body and center line significantly reduced the training period(p<.05). The virtual reality program to instantly move the weight through maximum voluntary safety limits to sensory deficit of virtual reality program(p<.05). Conclusion : Stroke patients with sensory deficit affects the recovery of the standing balance. In addition, virtual reality program was stroke patients with sensory deficit affects the recovery of standing balance.

Evaluation of Standing Balance of the Elderly with Different Balance Abilities by using Kinect and Wii Balance Board

  • Objective: This study aimed to evaluate and identify variables for the standing balance of elderly subjects with different balancing abilities by using Kinect and Wii Balance Board. Method: The Berg Balance Scale (BBS) test was performed in 40 elderly subjects aged ${\geq}65years$ who can perform daily activities. The participants were divided into two groups, the healthy seniors (n = 20, BBS score ${\geq}52$) and the seniors with balancing problems (n = 20, BBS score < 52). Each group performed two standing tests (eyes open and eyes close) with two devices (Kinect and Wii Balance Board). The root mean square (RMS), mean distance (MDIST), range of distance (ROD), mean velocity, and 95% ellipse area were calculated from the measured data. Results: Among the calculated variables, RMS, MDIST, and ROD in the mediolateral direction showed significant differences between the two groups and a negative correlation with BBS scores. Conclusion: The results of the present study show that simple standing balance of the elderly can be measured with Kinect and Wii Balance Board, which are low-cost, easy to carry, and easy to use, by using the selected variables.

PLS (Posterior Leaf Spring) 착용이 편마비 환자의 동적 균형에 미치는 영향 (Effect of PLS (Posterior Leaf Spring) on Standing Balance in Hemiplegic Patients)

  • The Posterior Leaf Spring (PLS) has been used for hemiplegic patients in order to help their walking and to increase their balance function. Past studies have mainly focused on the PLS's influence on patients' walking without taking balance function into consideration. The purpose of this study was to identify the immediate effect of PLS on the standing balance in hemiplegic patients. Fifteen hemiplegic patients participated in this study: 10 men and 5 women, with an average age of 53.8 years. Standing balance was measured using a computerized dynamic posturography device under three conditions namely bare-foot standing, standing in shoes without PLS, and standing in shoes with PLS. The results were as follows: 1) The composite equilibrium scores of patients who stood in shoes only and stood in shoes with PLS were higher than those of patients who stood bare-foot. 2) There were no statistical difference in the composite equilibrium scores between the standing condition of patients who stood bare foot and that of patients who stood in shoes with PLS. In conclusion, it is not clear whether or not the PLS affects the standing balance of hemiplegic patients. Further study is required to determine the precise effect of the PLS on standing balance in hemiplegic patients who are not wearing shoes. This is particularly relevant in Korea culture where custom demands the removal of shoes when entering any house or even many restaurants.

The Influence of Foot Position on Standing Balance on an Unstable Base of Support

  • The purpose of this study was to compare the balance ability at different foot positions using K A T 2000 (Berg, Inc, vista, CA.1994). Thirty (male 15, female 15) normal subjects participated in this study. All subjects were tested at a one leg position or a two leg position that were toe-in $25^{\circ}$, toe-out $25^{\circ}$, and $45^{\circ}$. The starting position was where the subject crossed their arms across the chest and flexed knees slightily. The results of each test were displayed on a screen in a score format, which indicated balance index. These collected data were analyzed by using one way ANOVA, and t-test. The results of this study were as follows: When changing the angle of the foot in the one-foot and two-foot standing positions, there was no statistically significant difference, but the balance performance with the foot rotated by $25^{\circ}$ was better than that with the foot rotated by $45^{\circ}$. When changing the direction of the foot in the one-foot and two-foot standing positions, there was no statistically significant difference, but the balance performance with the foot rotated laterally was better - except for the case when the foot was medially rotated by $25^{\circ}$ in the right-foot static standing position. When the feet were medially rotated by $25^{\circ}$ in the two-foot static standing position, and were medially rotated by $25^{\circ}$ and $45^{\circ}$ in the one-foot static standing position, the balance performance of females was better than that of males. In this study, it was found that the balance performance of the subjects changed when the position of the foot was shifted on an unstable base of support. However, there was little correlation between balance performance and the height, weight and foot length of the subjects. It is necessary to conduct a follow-up study targeting various age groups and those with various diseases using an unstable platform or applying different physical or visual conditions, such as the length of the legs. Physical therapists need to consider the position of the foot in clinical settings for a better balance training or assessment.

편마비환자에게 플라스틱 단하지 보조기착용 전${\cdot}$후 선자세 균형과 동적 움직임에 미치는 효과 (The immediate effect standing balance and dynamic activity on barefoot, wearing SPAFO and wearing HPAFO in hemiplegic patients)

  • Objective: The purpose of this study were to investigate the standing balance, dynamic activity in hemiplegic patients according to the types of ankle-foot orthosis(AFO) and to determine the most effective type of AFO for gait training. Method: A prospective study was performed for 16 patients with hemiplegia who was able to walk independently. Static balance and dynamic activity were compared in two condition : 1) barefoot and SPAFO, 2) barefoot and HPAFO. Static balance and dynamic activity characteristics were evaluated by Active Balance while they were standing with in two condition AFO and barefoot. Results: There were significant difference in standing balance between barefoot and wearing SPAFO and HPAFO(p<0.05). There were significant difference in dynamic activity balance between barefoot and wearing SPAFO and HPAFO(p<0.05). There were significant difference in gait speed between barefoot and wearing SPAFO and HPAFO(p<0.05). Conclusion: This study showed that wearing SPAFO and HPAFO gave fair amount of improvement to balance and gait ability of hemiplegic patients.

슬관절 근육 피로가 한 발 정적기립 균형능력에 미치는 영향 (The Effects of Knee Muscular Fatigue on One-Leg Static Standing Balance)

  • The purposes of this study were to determine wether knee muscular fatigue affects on one-leg static standing balance. Sixty four healthy subjects were used for this study : 44 men and 20 women, with an average age of 19.52. One leg static standing balance was measured at pre-fatigue and post-fatigue by an instrumented balance assessment system(kinesthetic ability training balance platform) which is commercially available for testing or training balance. Isokinetic exercises were used to evoke muscle fatigue at 180 degree/see by Cybex 1200. One leg static standing balance ability was significantly decreased after knee muscular fatigue. Although these phenomenons were not clearly understood, these results have important implications for rehabilitation in fatigable patients. These results suggest that the excessive fatiguing during rehabilitation in patients with fatigable disease may increase risk of reinjury and falling injury due to balance disturbance. Further studies are required to determine the physiological mechanisms of muscle fatigue that can play in decreasing one-leg static standing balance ability.

Transient Effects of Calf Muscle Fatigue and Visual Control on Postural Balance During Single Leg Standing

  • PURPOSE: Muscle fatigue is a cause to change proprioception. The purpose of this study was to investigate the effects of calf muscle fatigue and visual control on postural balance during single-legged standing in healthy adults. METHODS: Nineteen healthy adults (male) were participated in this study (mean age: 24.36 years; mean height: 171.32 cm; mean weight: 64.58 kg). The postural balance (sway length, sway area, sway velocity of COG displacement) was measured by Balance Trainer System (BT4) in before and after calf muscle fatigue feeling in single legged stance. In this study, repetitive single-legged heel rise test was used to induce fatigue of the calf muscle. Paired t- test was used to compare the postural balance between before and after calf muscle fatigue. Data of subjects were analyzed using SPSS 22.0 (SPSS Inc., Chicago, IL, USA). Level of significance was set to .05. RESULTS: The sway length, sway area, sway velocity of COG (center of gravity) displacement after calf muscle fatigue feeling was significantly increased compared to before calf muscle fatigue feeling during single leg standing both eye open and close conditions (p<.05). CONCLUSION: This study suggested that calf muscle fatigue feeling has affected on postural balance when standing one leg both eye open and close conditions and postural control was disturbed by muscle fatigue and visual feedback in single leg standing.

  • Purpose : The purpose of this study was to examine the differences in sit-to-stand movement and static standing balance between young adults and older adults Methods : Thirty young adults and thirty older adults participated in this study. The Good Balance System was used to assess participants' sit-to-stand movement and static standing balance. The sit-to-stand movement was measured as mediolateral and anterioposterior displacement of the centre of pressure (mm/s) while sit-to-stand on a force platform, and time required to complete a sit-to-stand movement on a force platform. The static standing balance was measured as mediolateral and anterioposterior displacement of the center of pressure (mm/s) and velocity moment (mm2/s) while standing on a force platform with opened eyes and with closed eyes. Result : A significant difference was found in the mediolateral and anterioposterior center of pressure displacement and the time required to complete a sit-to-stand movement of the two groups (p<.05). A significant difference between the groups was found as to mediolateral centre of pressure displacement and the velocity moment when standing with opened eyes and with closed eyes (p<.05). Conclusion : In conclusion, older adults showed decreased sit-to-stand movement and static standing balance ability compared to the young adult. Clinicians should consider sit-to-stand and balance training for older adult.

정상아동의 정적 균형과 감각통합의 발달 (Developmental Changes of Static Standing Balance and Sensory Organization in Children)

  • Multi-sensory systems, including the visual, somatosensory, and vestibular ones, are involved in maintaining standing balance. The organization of these sensory systems is as important as the efficiency of each individual system in maintaining optimal balance. The purpose of the present experiment was to investigate the developmental changes in static standing balance and sensory organization under altered sensory conditions. This study involved 64 children (from 4 to 15 years of age) and 17 young adults. The children were divided into four age groups: 4~6, 7~9, 10~12, and 13~15 years. Static standing balance was assessed with the one-leg standing test under four different sensory conditions: the children stood on a firm surface with (1) eyes open or (2) closed, and they stood on a foam surface with eyes (3) open or (4) closed. In balancing ability, the age groups exhibited significant differences. The function of sensory organization for balance control was poorer for the children than for the young adults. The functional efficiency of the somatosensory system of the children aged 7~9 years was at the young adults' level, and the visual function of the children aged 10~12 years had also reached the young adults' level. However, the functional efficiency of the vestibular system of children was significantly lower than that of the young adults, even at the age of 15 years. This may indicate that sensory organization and standing balance are still developing after the age of 15 years.

Correlation between Uni-standing Balance Ability, Jumping Ability, and Anaerobic Lower Extremity Muscle Power of High School Taekwondo Players

  • Objective: This study analyzed the correlation between uni-standing balance ability, jumping ability, and anaerobic lower extremity power of high school Taekwondo players. Design: Cross-sectional study Methods: Two high schools recruited 36 taekwondo players, but 5 were dropped due to personal circumstances and preparations for the tournament, so the number was counted as 31. In order to test uni-standing balance ability, the Romberg test was performed using Biorescue to measure the sway area. In order to test jumping ability, standing long jump and standing highjump were measured. For anaerobic lower extremity muscle power test, the peak power and average power were measured through 30-second pedaling using a Wingate system bicycle. Results: Participants did not show any correlation with other measures in the uni-standing balance ability test (p>0.05). Looking at the correlation in the standing long jump, standing high jump (r=0.892, p<0.05), anaerobic peak power (r=0.698, p<0.05), anaerobic average leg power (r=0.662, p<0.05) showed a positive correlation. Looking at the correlations in the standing high jump, anaerobic peak power (r=0.663, p<0.05), anaerobic average power (r=0.697, p<0.05) showed a positive correlation. Looking at the correlation in the anaerobic peak power, average power (r=0.785, p<0.05) showed a positive correlation. Conclusions: This study shows that balance ability and anaerobic leg strength are independent of each other. The results of this study can be used as basic data for the training plan of high school taekwondo players in the future.
