  • Purpose: Antivenin is a standard therapy in snakebite victims. While the required antivenin dose can be easily estimated, based on the initial symptoms, this strategy may be unsuccessful if the initial symptoms progressively worsen. The purpose of this study was to identify the progression rate of the initial symptoms following snakebite and its associated factors. Methods: The medical records of 44 patients treated for snakebite from give the actual dates of the study period were retrospectively examined. Thirty-two of these patients were enrolled. Demographic data, local wound grade and local effect score at initial presentation (G-0 and LES-0, respectively) and 12 hours after admission (G-12 and LES-12, respectively) were reviewed, along with laboratory data. Results: The 32 patients had an average age of $54.0{\pm}14.5$ years and were predominantly male (n=26) and presented mainly during summer. Compared to G-0 and LES-0, re-evaluated G-12 and LES-12 were significantly increased despite initial administration of proper antivenin dosage (p=0.001 and p=0.000, respectively). Total amounts of antivenin correlated with LES-12 (correlation co-efficiency 0.558, p<0.05). However, factors associated with symptom progression were not revealed. Conclusion: Initial snakebite symptoms might progressively worsen within hours despite acceptable initial antivenin therapy. Therefore, re-evaluation within several hours must be considered if when the initial snakebite symptoms are minimal or mild.

  • Parkinson's disease (PD) is a neurodegenerative disease caused by the death of dopaminergic neurons in the nigrostriatal pathway, leading to motor and non-motor dysfunctions, such as depression, olfactory dysfunction, and memory impairment. Although levodopa (L-dopa) has been the gold standard PD treatment for decades, it only relieves motor symptoms and has no effect on non-motor symptoms or disease progression. Prior studies have reported that 6-shogaol, the active ingredient in ginger, exerts a protective effect on dopaminergic neurons by suppressing neuroinflammation in PD mice. This study investigated whether cotreatment with 6-shogaol and L-dopa could attenuate both motor and non-motor symptoms and dopaminergic neuronal damage. Both 6-shogaol (20 mg/kg) and L-dopa (80 mg/kg) were orally administered to 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine/probenecid-induced PD model mice for 26 days. The experimental results showed that L-dopa alleviated motor symptoms, but had no significant effect on non-motor symptoms, loss of dopaminergic neuron, or neuroinflammation. However, when mice were treated with 6-shogaol alone or in combination with L-dopa, an amelioration in both motor and non-motor symptoms such as depression-like behavior, olfactory dysfunction and memory impairment was observed. Moreover, 6-shogaol-only or co-treatment of 6-shogaol with L-dopa protected dopaminergic neurons in the striatum and reduced neuroinflammation in the striatum and substantia nigra. Overall, these results suggest that 6-shogaol can effectively complement L-dopa by improving non-motor dysfunction and restoring dopaminergic neurons via suppressing neuroinflammation.

  • With the increased use of computers in medical fields, we can consider introducing computer mediated communication into the area of patient care such as medical or health counseling and education. But little was known about what problems can be counseled, what is the main direction of counseling, and what difficulties are met during the computer mediated counseling. So I conducted this study. This is a fundamental study concerning the health counseling by the inter-net. In this study, medical counseling using a method of questioning -answering through a computer mediated communication system was carried out from June 1998 to May 1999. I analyzed the contents of questions and answers by the ICPC classification. The ICPC classification emerged as a standard for information categorization in Primary Health Care within a few years. One of the most prominent features of this classification is the inclusion of the complaints of the patients (symptoms & complaints) and the social problems (Chapter Z). Thus, and for the first time, the demand of patients may be assessed such as it is expressed in the reality. The ICPC list is now an international standard whose validity is not questioned any more. It is translated in more than twenty different languages. The data collected were analyzed by descriptive statistical method, c2 test by cross tabulations. Followings are the results of this study. 1. The 3,054 users composed of 64% of males and 36% of females. The highest number of users was showed in the age group of $25{\sim}29$ (39.4%). 2. During the one-year period, the average number of counseling per user was 2.5 cases and per day was 21.1 cases. 3. In classifying symptoms and complaints by 17 chapter, digestive(17.6%), general and unspecified (12.7%), skin(12.7%) and musculoskeletal system (9.6%) were most common questions. 4. The distribution of diagnosis by 17 chapter, general and unspecified (21.4%), digestive (15.3%), skin (11.9%) and mus- culoskeletal system (8.9%) were most common answers. 5. Many users wanted to know symptoms and complaints(3,609 cases), treatment principles (2,928 cases), prognosis and preventive methods for health problems which were previously diagnosed (284 cases).

  • This study is to develop a standard tool for pattern identifications in Korean Medicine for Lung Cancer. The advisor committee for this study was organized by 12 Korean Medicine professors from the Korean Association of Traditional Oncology. The item and structure of instrument were based on review of published literature. We took the consultation twice from the advisor committee and additional advices by e-mail correspondences. We divided the symptoms and signs of lung cancer into 6 pattern identifications. - lung spleen both deficiency (肺脾兩虛), lung kidney both deficiency (肺腎兩虛), dampness phlegm obstructing the lung (濕痰蘊肺), qi stagnation blood stasis (氣滯血瘀), yin deficiency toxin heat (陰虛毒熱), and heat toxin accumulation (熱毒蘊結). We obtained the mean weights which reflected the standard deviations from each symptoms of the 6 pattern identifications which were scored on a 5-point scale by 12 experts. We designed the Korean medicine pattern identification tool for lung cancer. It was composed of 57 questions in the question-and-answer format. Though there are some limits that this study is not proved about validity and reliability, the instrument is meaningful and expected to be applied to the subsequent research.

  • This study was performed to investigate using of herbal medicine based on pattern identification of symptoms, such as Qi deficiency pattern, Yin deficiency pattern, Phlegm -dampness pattern, Static blood pattern, Fire-heat pattern. These patterns settled by Korean Standard Differentiation of the Symptoms and Signs for Stroke in 2005. This study was done with 177 patients in Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Among the five types of pattern identification, Phlegm -dampness pattern showed significantly high frequency especially in 3 weeks after stroke. The sort of herbal medicine was 43 and Dodamhwalhyoel-tang(26.61%), Sunhwanki 1 hobang(14.52%), Banhabaekchulcheonma-tang(4.84%), Ansinchongnoi-tang(4.84%), Chongryuldodam-tang(4.03%) were most frequently used. Especially Dodamhwalhyoel-tang showed significantly high frequency in 2 weeks after stroke. Based on these results, it is suggested that more practical Korean Standard Differentiation of the Symptoms and Signs for Stroke would be established through continuous clinical studies by giving weight on relationship between herbal medicine and pattern identification.

  • Hwamunsok(mat with flower design) is a special product of Kanghwa, which is made up of cyperus exaltatus. According to Allergy, cyperus exaltatus is one of the inhalant allergens and provokes respiratory symptoms and signs. There have been few reports about respiratory allergic disease or contact dermatitis occurring in farmers who cultivate the cyperus exaltatus or manufacture Hwamunsok with it. We studied the relationship between allergic symptoms and exposure to cyperus exaltatus. First, we questioned 141 voluntaries living in Kangwha on the symptoms of contact dermatitis, allergic rhinitis and bronchial asthma. Secondly, open tests with cyperus exaltatus were done to permitted 73 voluntaries. Patch tests with the European standard series(23 antigens) were only done to exposure 49 subjects to cyperus exaltatus. The results were as follows : 1. As the manifestation rate of allergic symptoms by age, its of 60 years old over in male showed 333 per 1,000 persons. Its of 50 years old in female showed 412 per 1,000 persons. Those age groups in both genders exhibited the highest manifestation of allergic symptoms than all other age groups. Also, if age and sex were adjusted with the rural population of Kanghwagun('93 base population), the manifestation rate of allergic symptoms in male showed 283 per 1,000 persons. Its in female and totals of study populations showed which is each 206 and 234 per 1,000 persons. Specially, it showed that statistics significantly(p=0.002), exposured subjects by cultivation of cyperus exaltatus & manufacture of Hwamunsok have experienced symptoms of contact dermatitis more frequently than unexposured subjects. Secondly, it tested statistics significantly that the risk factor as manifestation of allergic symptoms was the exposure of cyperus exaltatus's allergen(p=0.024). The manifestation risk of allergic symptoms in exposure groups was 3.73 times higher than unexposured subjects. Thirdly, positive reactions in open tests with cyperus exaltatus were higher in exposured subjects than non-exposured subjects. After 72 and 96 hours, positive reactions in open tests were all 10 subjects(20.4%). Also, after 1 week, positive reactions were 8 subjects(16.4%). In conclusion there is a close relationship between contact dermatitis and exposure to cyperus exaltatus while cultivating it and manufacturing Hwamunsok. Therefore, various studies should be continued and a health education program undertaken on contact dermatitis with those who were be engaging in Hwamunsok's manufacture.
