계단 오르기 시 정강이뼈 돌림이 하지의 역학과 근 활성도에 미치는 영향 (Tibial Rotation Influences Muscle Activity and Motion of Lower Extremity during The Stair Ascent)

  • This study was performed to investigate the effects of tibial rotation while going up stair on muscle activity of vastus medialis oblique and vastus lateralis, and on patellar displacement. The subjects included 30 people (male: 15; female: 15) who were randomly assigned to the tibial internal-rotation, neutral-rotation, and external-rotation groups. The subjects went up the stair while performing the assigned rotations, and the rotation of the hip and the displacement of the patella were measured using a 3D motion analyzer. In addition, the maximum voluntary isometric contraction (MVIC) of the vastus medialis oblique and vastus lateralis were measured using surface electromyogram. On the tibial internal rotation, the hip rotation significantly appeared in the same direction and so did on the tibial neutral and external rotations(p<.001). Although the MVIC of the vastus medialis oblique and vastus lateralis did not significantly differ by tibial rotation during the stair ascent, the MVIC of the vastus medialis oblique was higher than that of the vastus lateralis during the internal and neutral rotations (p<.05). In addition, during the stair ascent, the displacement of the patella was more significant during the tibial external rotation than during the tibial internal and neutral rotations(p<.001). Thus, patients with patellofemoral pain are required to be considered the effects of tibial rotation for their rehabilitation.

The Effects of tDCS and Montoya Stair Task on Sensorimotor Recovery and GFAP Expression in MCAo induced Stroke Rat Model

  • This study is intended to examine the tDCS and Montoya stair task(MST) on sensorimotor recovery and glial scar expression in MCAo induced stroke model of rat. To achieve this goal, this study selected 80 SD rats of 8 weeks. The experiment groups were divided them into four groups, and assigned 20 rats to each group. Group I was a experimental control group; GroupII was a tDCS application group after MCAo; Group III was a MST application group after MCAo; Group IV was a tDCS and MST application group after MCAo. In each group, neurological function test measurement, motor behavior test, montoya stair task test, immunohistochemistric finding of GFAP expression finding were analyzed. In motor behavior test, the outcome of group I was significantly difference than the other group, especially from 14days. In montoya stair task test, the outcome of group I was significantly lower than the other group especially, group II were significantly different on 14days and group IV was most significantly difference than the other group. In immunohistochemistric finding, group II, III, IV were decrease GFAP expression on depend on time stream. These results throughout the MCAo due to focal ischemic brain injury rat model four weeks tDCS and MST was applied, when the neurobehavioural, upper extremity function and ability, histopathologic data suggest that sensorimotor function recovery and a positive influence on glial scar decrease and confirmed that.

Pelvic, Hip, and Knee Kinematics of Stair Climbing in People with Genu Varum

  • Purpose: This study examined the effects of the lower limb alignment on the pelvis, hip, and knee kinematics in people with genu varum during stair walking. Methods: Forty subjects were enrolled in this study. People who had intercondylar distance ${\geq}4cm$ were classified in the genu varum group, and people who had intercondylar distance <4cm and intermalleolar distance <4cm were placed in the control group. 3D motion analysis was used to collect the pelvis, hip, and knee kinematic data while subjects were walking stairs with three steps. Results: During stair ascent, the genu varum group had decreased pelvic lateral tilt and hip adduction at the early stance phase and decreased pelvic lateral tilt at the swing phase compared to the control group. At the same time, they had decreased minimal hip adduction ROM at the early stance and decreased maximum pelvic lateral tilt ROM and minimum hip rotation ROM at the swing phase. During stair descent, the genu varum group had decreased pelvic lateral tilt at the early stance and decreased pelvic lateral tilt and pelvic rotation at the swing phase. In addition, they had decreased pelvic frontal ROM during single limb support and increased knee sagittal ROM during the whole gait cycle. Conclusion: This study suggests that a genu varum deformity could affect the pelvis, hip and knee kinematics. In addition, the biomechanical risk factors that could result in the articular impairments by the excessive loads from lower limb malalignment were identified.

The Effects of Stair Climbing Training with Functional Electrical Stimulation on Muscle Strength, Balance, and Gait in Patients with Chronic Stroke

  • Objective: The weakness of muscle strength due to stroke affects the posture control and gait in the patients with stroke. Stroke This study examined the effects of the stair climbing training with functional electrical stimulation on muscle strength, dynamic balance, and gait in individuals with chronic stroke. Design: Randomized controlled trial. Methods: Total forty-eight patients were randomly assigned to the 3 groups. Participants randomly divided to stair climbing training with functional electrical stimulation group (SCT+FES group, n=16), stair climbing training group (SCT group, n=16) and control group (n=16). Subjects in the SCT+FES group and SCT group performed stair walking training with and without functional electrical stimulation for 30 minutes, 3 sessions per week for 4 weeks and all subjects received conventional physical therapy for 30 minutes with 5 sessions per week for 4 weeks. Outcome measurements were assessed using the sit-to stand Test for strength, timed up and go test and modified-timed up and go test for dynamic balance, and 10m walk test and GaitRite system for gait. Results: In the SCT+FES group, subjects have been shown the significant increase in lower extremity strength (p<0.05), significantly improve in dynamic balance (p<0.05), and significantly improve in their temporal gait parameter (p<0.05). The SCT+FES group was significantly better than other groups in all parameters (p<0.05). Conclusions: This result suggested that the SCT+FES may be effective strategy to improve muscle strength, dynamic balance, and gait for individuals with chronic stroke.

The Immediate Effect of Medial Arch Support on Dynamic Knee Valgus During Stair Descent and Its Relationship With the Severity of Pronated Feet

  • Background: Pronated foot posture (PFP) contributes to excessive dynamic knee valgus (DKV). Although foot orthoses such as medial arch support (MAS) are widely and easily used in clinical practice and sports, few studies have investigated the effect of MAS on the improvement of DKV during stair descent in individuals with a PFP. Moreover, no studies reported the degree of improvement in DKV according to the severity of PFP when MAS was applied. Objects: This study aimed to examine the immediate effect of MAS on DKV during stair descent and determine the correlation between navicular drop distance and changes in DKV when MAS is applied. Methods: Twenty individuals with a PFP (15 males and five females) participated in this study. The navicular drop test was used to measure PFP severity. The frontal plane projection angle (FPPA) was calculated under two conditions, with and without MAS application, using 2-dimensional video analysis. Results: During stair descent, the FPPA with MAS (173.1° ± 4.7°) was significantly greater than that without MAS (164.8° ± 5.8°) (p < 0.05). There was also a significant correlation between the navicular drop distance and improvement in the FPPA when MAS was applied (r = 0.453, p = 0.045). Conclusion: MAS application can affect the decrease in DKV during stair descent. In addition, MAS application should be considered to improve the knee alignment for individuals with greater navicular drop distance.

  • 본 연구에서는 의사사인(Sine) 방식을 사용한 간단한 계단파 PWN을 소개하였다. 제어기의 구성은 기본파 성분인 Sine값을 계단 레벨로 분할하고 각각의 값을 디지탈 합성하여 3상 PWN 인버터를 구성하였다. 3상 출력 펄스는 캐리어 주파수를 일정하게하고 기본파만 변화시켜 낮은 주파수에서 고조파 감소 효과를 얻어냈다. 실험으로는 스위칭 주파수가 20[KHz]일때 0.5마력 유도 전동기 구동을 실행하였다.

여러 가지 쾌속조형 방식의 경사면 거칠기 특성 (Characteristics of Roughness of Inclined Surface Fabricated by Various Rapid Prototyping Processes)

  • Surface of rapid prototype has inevitably stair-stepping error, which is attributed to the continuous building process of 2 dimensional area. In this study, rounded edge model was established to estimate the roughness of inclined surface which has stair-stepping error. To investigate the roughness of rapid prototypes, specimens that have various surface inclinations were manufactured by various types of RP machines. As the surface inclination increased, the roughness of the specimens manufactured by SL, FDM, or LOM process decreased, which coincides with the simulation results. However, surface roughness of 3DP specimen was almost independent of the inclination. Furthermore, as the angle of surface increased, roughness of poly-jet specimen also increased, which is attributed to the frictional behavior between writing head and scanned area.

  • The purpose of this study is to evaluate the effects of the lower extremities patterns in proprioceptive neuromuscular facilitation to the gait and stair up of the hemiplegic stroke patient. the patient was taken lower extremities patterns in porprioceptive neuromuscular facilitation with 5 times per week for 8 weeks. The results were as followings ; 1. The walking times was decreased from 78 seconds to 39 seconds at 20m, 2. The stride length was increased from 48.3cm to 93.3cm. 3. The step length was increassed from 25.2cm to 47.2cm. 4. The stair up was increased from 3.2cm to 15.5cm.

대학교육시설(大學敎育施設)의 복도(複道).계단(階段) 폭(幅)과 보행자(步行者) 밀도(密度)가 피난시간(避難時間)에 미치는 영향(影響)에 관한 연구 (A Study on the Evacuation Time According to the Width of Corridor, Stair and Density of Evacuee in University Education Facilities)

  • The aims of this study are to verify the relationships of evacuee speed and affections to the evacuation time according to the width of corridor and stairs in university education facilities. To achieve these purposes, the present conditions of university education facilities were analyzed, and the evacuation experiments were executed in relation to the width of corridor and stairs. The results of this study are as follows ; (1) The deviations of numbers of residents per stair is very high, and the number and width of stairs, corridors are only depend on the minimum guidelines of building code, not considering the residents' evacuation. (2) The width of corridor and stairs are affects on the evacuation speed and time. The rate of decreasing speed is large in stairs, and also the possibility of danger is increasing depend on the speed and density.

지속적인 계단 보행에서 부하가 하지 근육의 생체역학적 변인과 근사 엔트로피에 미치는 영향 (Effects of Loading on Biomechanical Analysis of Lower Extremity Muscle and Approximate Entropy during Continuous Stair Walking)

  • Objective : The purpose of this study was to investigate the changes of gait patterns and muscle activations with increased loads during stair walking. Also, it can be used as descriptive data about continuous stair walking in a real life setting. Method : Twelve sedentary young male adults(Age: $27.0{\pm}1.8yrs$, Weight: $65.8{\pm}9.9kg$) without any lower extremity injuries participated in this study. Participants performed stair walking up 7 floors and their ascending and descending motion on each floor was analyzed. A wireless electromyography(EMG) were attached on the Rectus Femoris(RF), Biceps Femoris(BF), Gastrocnemius(GN), Tibialis Anterior(TA) muscle to calculate integrated EMG(iEMG), median frequency(MDF) and co-contraction index(CI). Chest and left heel accelerometer signal were recorded by wireless accelerometer and those were used to calculate approximate entropy(ApEn) for analyzing gait pattern. All analyses were performed with SPSS 21.0 and for repeated measured ANOVA and Post-hoc was LSD. Results : During ascending stairs, there were a statistically significant difference in Walking time between 1-2nd and other floors(p=.000), GN iEMG between 2-3th and 6-7th(p=.043) floor, TA MDF between 1-2nd and 5-6th(p=.030), 6-7th(p=.015) floor and TA/GN CI between 2-3th and 6-7th(p=.038) floor and ApEn between 1-2nd and 6-7th(x: p=.003, y: p=.005, z: p=.006) floor. During descending stairs, there were a statistically significant difference in TA iEMG between the 6-5th and 3-2nd(p=.026) floor, and for the ApEn between the 1-2nd and 6-7th(x: p=.037, y: p=.000, z: p=.000) floor. Conclusion : Subjects showed more regular pattern and muscle activation response caused by regularity during ascending stairs. Regularity during the first part of stair-descending could be a sign of adaptation; however, complexity during the second part could be a strategy to decrease the impact.
