지방 중소도시 유형별 1인 가구 특성연구 (A Study on the Characteristics of One-Person Household in Local Small and Medium Cities)

  • 우리나라는 1960년대 이후 국가 경제성장에 힘입어 사회적·경제적으로 주민 생활에 큰 변화가 있었으며 특히 미혼 여성의 경제적 자립, 청년층의 만혼(晩婚), 중년층의 이혼, 고령층의 사별 증가 등으로 전체 가구의 29%가 1인 가구인 시대로 되었다. 하지만 급격히 증가하고 있는 1인 가구에 대한 연구가 인구가 밀집된 대도시에 거주하는 1인 가구에 치중되어 본 연구는 지방 중소도시에 거주하는 1인 가구의 특성을 분석하였다. 연구결과 지방 중소도시는 도시특성에 따라 성장 정체도시, 산업 선도도시, 지역 거점도시, 인구 유출도시 등으로 구분되며 도시유형에 따라 1인 가구특성도 차이가 있는 것으로 나타났다. 성장 정체도시에서의 1인 가구는 산업활동의 근간이 되는 30-40대 연령대가 적으며 자가인 단독주택에서 방 3개 이상에 거주하는 장년층 비중이 높은 것으로 나타나 장년층 위주의 장기임대주택 및 쉐어하우스 등의 주택공급과 함께 사회적 교류를 위한 단지형 연립주택과 다세대 주택공급정책이 필요하다. 산업 선도도시에서의 1인 가구는 모든 연령대에 고루 분포되어 있으며 아파트 거주비율이 타 유형에 비해 가장 높은 것으로 나타나 행복주택 등 청년층 주택공급과 함께 기존주택에 대한 재건축 및 리모델링을 통하여 청·장년층 1인 가구의 주택수요에 대처하여야겠다. 지역 거점도시의 1인 가구는 대졸 학력에 20대 이하의 미혼 가구가 많고 월세주택에서 방 1개에 거주하는 비중이 높은 것으로 나타나 청년층을 위한 소형 주택공급을 확대하는 한편 아파트, 오피스텔 등의 분양주택과 전월세 주택공급을 통해 청년층 주거안정에 초점을 맞추어야 겠다. 인구 유출도시에서의 1인 가구는 초졸 학력에 사별한 가구가 많고 자가인 단독주택에서 방 4개 이상에 거주하는 노령층 비중이 높은 것으로 나타나 고령자 복지주택 등 노령층 위주의 단지형 주택공급을 통해 사회적, 문화적 교류가 가능한 주거환경을 만들고 분산된 주택거주 형태를 집단적 거주형태로 주거공간을 개선할 필요가 있다.

폐암(肺癌)의 동서의결합치료(東西醫結合治療)에 관(關)한 문헌적(文獻的) 고찰(考察) (Bibliographic Study on the Therapy of Lung Cancer by Integrated Oriental and Western Medicine)

  • 폐암(肺癌)의 치료법(治療法)에 대해 서의치료(西醫治療) 동의치료(東醫治療) 동서의(東西醫) 결합치료(結合治療)에 대해 최근문헌(最近文獻)을 중심(中心)으로 고찰(考察)해 본 결과(結果) 다음과 같은 결론(結論)을 얻었다. 1. 폐암(肺癌)의 서의치료법(西醫治療法)은 소세포암(小細胞癌)(SCLS)과 비소세포암(非小細胞癌)(NSCLS)으로 나누어 분기(分期) 및 증상(症狀)에 따라 수술치료(手術治療) 방사선치료(放射線治療) 화학치료(化學治療)를 단독(單獨)으로 또는 두가지 이상 복합(複合)해서 활용(活用)한다. 2. 폐암(肺癌)의 동의치료(東醫治療)는 초기(初期), 중기(中氣), 말기(末期)로 나누어 부정거사(扶正祛邪), 공보겸시(功補兼施), 기혈쌍보(氣血雙補)의 치법(治法)을 쓰거나 증상(症狀)에 따라 폐비기허형(肺脾氣虛型), 폐열음허형(肺熱陰虛型), 습담어조형(濕痰瘀阻型), 기혈어체형(氣血瘀滯型), 기음양허형(氣陰兩虛型)으로 분(分)하여 변증시치(辨證施治)하거나 혹은 단미(單味) 또는 복방(復方)으로 대증치료(對證治療)를 한다. 3. 폐암(肺癌)에 대(對)한 동서의결합치료(東西醫結合治療)를 함으로써 생존율(生存率)이나 생존(生存)의 질(質)에 있어서 현저(顯著)한 향상(向上)이 있는데 수술후(手術後) 동서결합치료(東西結合治療)는 수술후(手術後) 회복력(回復力)을 촉진(促進)시키고 생존율(生存率)을 높였다. 방사선치료(放射線治療)와의 결합치료(結合治療)는 방사선치료(放射線治療)에 의한 독부작용(毒副作用)을 감소(減少)시켜 치료효과(治療效果)를 높이고 생존율(生存率)을 높였다. 화학요법(化學療法)과 동의결합치료(東醫結合治療)는 화학요법(化學療法)의 독부작용(毒副作用)을 경감(輕減)시키고 생존(生存)의 질(質)과 생존율(生存率)을 높였다. 이상(以上)의 결과(結果)로 보아 폐암(肺癌)의 치료(治療)는 진단(診斷)에서부터 치료(治療)의 전과정(全過程)을 통(通)해서 서의치료(西醫治療)와 동의치료(東醫治療)를 결합(結合)하여 종합치료(綜合治療)를 하므로써 폐암(肺癌)의 치료효과(治療效果)를 높일 수 있는 새로운 치료법(治療法)으로 계속적인 연구(硏究)가 필요(必要)할 것으로 사료(思料)된다.

노인(老人) 저혈압(低血壓)에 대(對)한 문헌적(文獻的) 고찰(考察) (A Literature Review of The Senile Hypotension)

  • This study was perfomed to investigate causes of the senile hypotension, pathogenic mechanism, symptoms, and therapies through medical literatures, recent chinese medical literatures and chinese medical journals. The results are as follows ; 1. The senile hypotension has major symptoms of dizziness, weakness, syncope, palpitation, shortness of breath, and deficiency of Qi. Additionally, it has minor symtoms of letharhy, isomnia, tinnitus, amnesia etc... 2. The prodromal symptoms of Kwul and Kwul are relating to the symptoms of tachycardia, facial pallor, sweating, anxietas, ambiguous consciousness, and fainting. Weakness and dizziness due to deficiency make the symptoms of exhaustion, fatigue, vertigo, lethargy, and brachycardia. 3. The most principal cause of the senile hypotension is deficiency of Shen due to aging, congenital deficiency, and chronic illness. The rest of causes are defciency of Qi and blood, phlegm of retention, stagnation of Qi, blood stasis, blood prostration etc... In the view of the occidental meicine, the causes of the senile hypotension came from the reduction of cardiac output, the decretion of cardiovascular system's extention due to aging, hereditary factor, secondary factor due to exsanguination, diabetes mellitus, C.V.A etc..., and factor of neurogenic system's degeneration. 4. The principal pathogenic mechanisms are the insufficiency of Xing-Yang, the deficiency of Qi in middle jiao, and deficiency of Shen-Qi. The rest of mechanisms are the deficiency of both Qi and blood, stagnation of the Gan-Qi, and the deficiency of Gan and Shen. Zang-Pu Organs have something to do with Xing, Bi, and Shen. 5. As principal therapies, there are warming and recuperation the Xing-Yang, strengthing the middle-jiao and replenishing Qi, replenishing vital essence to tonify the Shen, and warming and recuperation the Shen-Yang. Additionally, the therapies of invigorating the Bi and relieving mental stress, strengthning the Bi and tonifing the Shen, invigorating Qi and nourishing Yin, soothing the Gan and regulating the circulation of Qi, and tonifing the Shen and nourishing the Gan help the cure of the senile hypotension. In prescriptions there are Baohe Yuan Tang, Buzhong Yigi Tang, Zuoguei Yin, Yougui Yin, Guipi Tang, Zhu Fu Tang, Shengmai San, Sini San, and Qi Ju Dihuang Wan. The medical herbs of Astragali Radix, Codonopsitis Pilosulae Radix, Ginseng Radix, Aconiti Tuber, Ephedrae Herba, Cinnamomi Ramulus, Cinnamomi Corfex Spissus, Zingiberis Rhizoma, Polygalae Radix, Liriopis Tuber, Polygonati Sibirici Rhifoma, Lycii Fructus, Schizandrae Fructus, and Glycyrrhizae Radix can be treated. 6. According to the clinical report, the principal causes are the deficiency of Qi, and insufficiency of Yang which symptoms are dizziness, vitality fatigue and acratia, amnesia, body cold and alger of extremity, spontaneous perspiration, and therady and weak pulse. It was improved by taking WenYang YiQi Tang, Zhu Fu Tang about 20-30 days. The improvement was shown on disappearance of subjective symptoms or the ascending of blood pressure to normal figure, and the rate of improvement was over 70%. 7. As regimens, taking warming and recuperating food(a sheep mutton, juglans regia, chiness date, longan aril etc...) and pungent food(chinese green onion, fress ginger, pipers fructns etc...), doing physical training, not being ill in bed at a long time, and preventing descent of blood pressure coming from sudden change of posture are needed. Additionally, the usage of diuretic, abirritant, and depressor needs to be extra cautious.

여드름의 동(東)·서의학적(西醫學的) 문헌(文獻) 고찰(考察) (A Literature Study about Comparison of Eastern-Western Medicine on the Acne)

  • Objective : The purpose of this study is to investigate about comparison of Eastern-Western medicine on the acne. Methods : We searched Eastern and Western medicine books for acne. We analyzed these books and examined category, definition, etiology, classification, internal and external methods of treatment of acne. Results : The results were as follows. 1. In Eastern medicine, Acne belongs to the category of the Bunja(粉刺), Jwachang(痤瘡), Pyepungbunja(肺風粉刺). In Western medicine, the other name of Acne is acne vulgaris. 2. In Eastern medicine, the definition of Acne includes manual extraction of comedones and skin appearance. In Western medicine, Acne is a common skin disease during adolescence and a chronic inflammatory disease of pilosebaceous unit of self localization. It is characterized by noninflammatory, open or closed comedones and by inflammatory papules, pustules, and nodules and it affects the areas of skin with the densest population of sebaceous follicles, these areas include the face, neck, back, and the upper part of the chest. 3. In Eastern medicine, the cause and mechanism of Acne arose from the state of internal dampness-heat and spleen-stomach internal qi deficiency due to dietary irregularities and then invaded external pathogen such as wind-dampness-heat-cold-fire in lung meridian lead to qi and blood heat depression stagnation. So it appears in skin. In Western medicine, the etiology and pathogenesis of Acne is clearly not identified, but there are most significant pathogenic factors of blood heat depression stagnation. In Western medicine, the etiology and pathogenesis of Acne is clearly not identified, but there are most significant pathogenic factors of Acne; Androgen-stimulated production of sebum, hyperkeratinization and obstruction of sebaceous follicles, proliferation of Propionibacterium acnes and inflammation, abnormaility of skin barrier function, genetic aspects, environmental factors etc. 4. In Eastern medicine, differentiation of syndromes classifies clinical aspects, and cause and mechanism of disease; the former is papular, pustular, cystic, nodular, atrophic, comprehensive type; the latter is lung blood heat, intestine-stomach dampness-heat, phlegm-stasis depression, thoroughfare-conception disharmony, heat toxin type. In Western medicine, it divides into an etiology and invasion period, and clinical aspects; Acne neonatorum, Acne infantum, Acne in puberty and adulthood, Acne venenata; Acne vulgaris, Acne conglobata, Acne fulminans, Acne keloidalis. 5. In Eastern medicine, Internal methods of treatment of Acne are divided into five treatments; general treatments, the treatments of single-medicine and experiential description, the treatments depending on the cause and mechanism of disease, and clinical differentiation of syndromes, dietary treatments. In Western medicine, it is a basic principles that regulation on production of sebum, correction on hyperkeratinization of sebaceous follicles, decrease of Propionibacterium acnes colony and control of inflammation reaction. Internal methods of treatment of Acne are antibiotics, retinoids, hormone preparations etc. 6. In Eastern medicine, external methods of treatment of Acne are wet compress method, paste preparation method, powder preparation method, pill preparation method, acupuncture and moxibustion therapy, ear acupuncture therapy, prevention and notice, and so on. In Western medicine, external method of treatments of Acne are divided into topical therapy and other surgical therapies. Topical therapy is used such as antibiotics, sebum regulators, topical vitamin A medicines etc and other surgical therapies are used such as surgical treatments, intralesional injection of corticosteroids, skin dermabrasion, phototherapy, photodynamic therapy, and so on. Conclusions : Until now, there is no perfect, effective single treatment. We think that Eastern medicine approach and treatment can be helpful to overcome the limitations of acne cure.

8증례를 통한 사암침법(舍巖鍼法)의 형상의학적(形象醫學的) 운용에 관한 고찰 (A Study of Eight Cases According to Hyeongsang Diagnosis Applying Sa-am Acupuncture Therapy)

  • 1. 형상의학(形象醫學)에서는 사람을 얼굴형태에 따라 정(精) 기(氣) 신(神) 혈(血)과로 이목구비(耳目口鼻)의 기능에 따라 어(魚) 조(鳥) 주(走) 갑류(甲類)로 분류하며, 이러한 분류에 따른 장부(臟腑)의 특성을 사암침(舍巖鍼) 운용에 활용하면 임상에서 활용도가 높다. 2. 어류(魚類)는 수(水)의 기운(氣運)이 많아 수체(水體)라고도 하며 신장(腎臟)이 발달하여 신장(腎臟)과 관련된 병이 오기 쉽다. 그 본치(本治)가 보정보기(補精補氣)이며 주로 신정격(腎正格)을 운용하고 신양허쇠(腎陽虛衰)인 경우 신열격(腎熱格), 신음허(腎陰虛)인 경우 신한격(腎寒格)을 쓰며 정혈(精血)의 휴손(虧損)이 심한 경우 간정격(肝正格)도 운용할 수 있다. 3. 조류(鳥類)는 화(火)의 기운(氣運)을 많이 받아 화체(火體)라고도 하며 심장(心臟)이 발달하여 심장병(心臟病) 신경성 질환이 잘 온다. 그 본치(本治)가 자음강화(滋陰降火)며 심장(心臟)의 음혈(陰血)을 보(補)하고 화(火)를 내리는 심한격(心寒格)을 위주로 하여, 심화(心火)를 사(瀉)하면서 음혈(陰血)을 보해주고 신지(神志)를 안정시키는 심승격(心勝格), 심기(心氣)가 부족한 경우 심정학(心正格), 심담(心膽)이 모두 허(虛)한 경우 담정격(膽正格)을 운용할 수 있다. 4. 주류(走類)의 목(木)의 기운(氣運)이 많아 목체(木體)라고도 하며 간(肝)이 발달(發達)하여 간(肝)과 관련된 병(病)이 잘 온다. 그 본치(本治)가 청열사습(靑熱瀉濕), 자혈양근(滋血養筋)이므로 간정격(肝正格)을 주로 운용하고, 간화(肝火)가 동(動)하거나 간실증(肝實證)이 나타난 경우 간한격(肝寒格)이나 간승격(肝勝格)을, 간기(肝氣)가 항진(亢進)으로 인해 비기(脾氣)가 허(虛)해져 있는 경우 비정격(脾正格)을, 습열이 너무 성(盛)한 경우 대장정격(大場正格)을 운용할 수 있다. 5. 갑류(甲類)는 금(金)의 기운(氣運)이 많아 금체(金體)라고도 하며 폐(肺)가 발달하여 폐(肺)와 관련된 병이 잘 온다. 그 본치(本治)가 해울소담(解鬱消痰)이므로 폐정격(肺正格)을 통해 보폐순기(補肺順氣)하며 울증(鬱症)이 심한 경우 폐승격(肺勝格)으로 통해 소담(消痰)시켜주고 기울방(氣鬱方)으로 해울(解鬱)하기도 한다. 6. 정과(精科)는 그 특성상 정(精)의 누설(漏泄)에 의한 증상 및 정부족(精不足), 양허증상(陽虛證狀)과 정(精)의 과도한 응집(凝集)에 의한 습열(濕熱)이 기본 병리이며 기본처방은 신기(腎氣)를 강화하는 신정격(腎正格)을 중심으로 하여, 신열격(腎熱格) 신한격(腎寒格) 등을 변증에 따라 운용하며 습열(濕熱)이 성한 경우 대장정격(大腸正格) 비승격(脾勝格) 등 습열(濕熱)을 다스리는 처방과 비정격(脾正格), 습담방(濕痰方) 등 습담(濕痰)을 다스리는 처방이 운용될 수 있다. 7. 기과(氣科)의 기본 병리(病理)는 기울(氣鬱), 기체(氣滯)에 의한 구기(九氣), 칠기(七氣), 중기(中氣), 매핵기(梅核氣), 불면증(不眠症) 등 신경성 질환이 많고 특히 여자의 경우 손발과 하복부가 차고 대소변이 안좋으며 징가(癥痂), 현벽(痃癖) 등의 질환이 많다. 또한 지나친 발산(發散)으로 인하여 기허증(氣虛證)이 나타나기도 한다. 따라서 기본처방은 기울방(氣鬱方), 담음방(痰飮方), 담현방(痰眩方), 기수방(氣嗽方), 폐승격(肺勝格), 삼초정격(三焦正格) 등이며, 기허증(氣虛證)이 나타나는 경우 폐정격(肺正格)을 사용할 수 있다. 8. 신과(神科)의 기본병리는 칠정울결(七情鬱結)이나 담화(痰火), 화성음허(火盛陰虛)이며 대표적인 증상은 경계(驚悸), 정충(怔忡), 건망(健忘), 불면(不眠), 전간(癲癎), 전광(癲狂) 등이다. 따라서 기본처방은 심한격(心寒格), 심승격(心勝格), 심정격(心正格), 담정격(膽正格), 비한격(脾寒格)을 중심으로 열담방(熱痰方), 군화방(君火方), 상화방(相火方), 화울방(火鬱方) 등을 사용할 수 있다. 9. 혈과(血科)의 기본병리는 어혈(瘀血) 및 출혈이며 대표적인 증상은 구규출혈(九竅出血)과 어혈(瘀血), 혈허증(血虛證)이다. 따라서 기본처방은 간정격(肝正格), 손혈방(損血方), 심한격(心寒格) 비한격(脾寒格) 심비한격(心脾寒格), 소장정격(小腸正格), 소장한격(小腸寒格), 어혈방(瘀血方), 뉵혈방(衄血方) 등을 사용할 수 있다.

  • Authors investigated the pathogenesis and treatment of dennatosurgical diseases in the ImJeungJiNamUiAn(臨證指南醫案). 1. The pathogenesis and treatment of dermatosurgery were as follows; 1) When the pathogenesis of BalSa(발사), BalJin(發疹), BalLa(발라) and HangJong(項腫) are wind-warm(風溫), exogenous cold with endogenous heat(外寒內熱), wind-damp(風濕), the treatment of evaporation(解表) with Menthae Herba(薄荷), Arctii Fructus(牛蒡子), Forsythiae Fructus(連翹) Mori Cortex(桑白皮), Fritillariae Cirrhosae Bulbus(貝母), Armeniaoae Amarum Semen(杏仁), Ephedrae Herba(麻黃), Cinnamomi Ramulus(桂枝), Curcumae Longae Rhizoma(薑黃), etc can be applied. 2) When the pathogenesis of BuYang(부양), ChangI(瘡痍) and ChangJilGaeSeon(瘡疾疥癬) are wind-heat(風熱), blood fever with wind transformation(血熱風動), wind-damp(風濕), the treatment of wind-dispelling(疏風) with Arctii Fructus(牛蒡子), Schizonepetae Herba(荊芥), Ledebouriellae Radix(防風), Dictamni Radicis Cortex(白鮮皮), Bombyx Batrytioatus(白??), etc can be applied. 3) When the pathogenesis of SaHuHaeSu(사후해수), SaJin(사진), BalJin(發疹), EunJin(은진) and BuYang(부양) are wind-heat(風熱), exogenous cold with endogenous heat(外寒內熱), exogenous warm pathogen with endogenous damp-heat(溫邪外感 濕熱內蘊), warm pathogen's penetration(溫邪內陷), insidious heat's penetration of pericardium(伏熱入包絡), the treatment of Ki-cooling(淸氣) with TongSeongHwan(通聖丸), Praeparatum(豆?), Phyllostachys Folium(竹葉), Mori Cortex(桑白皮), Tetrapanacis Medulla(通草), etc can be applied. 4) When the pathogenesis of JeokBan(적반), BalLa(발라), GuChang(久瘡), GyeolHaek(結核), DamHaek(痰核), Yeong(?), YuJu(流注), Breast Diseases(乳房疾患) and DoHan(盜汗) are stagnancy's injury of Ki and blood(鬱傷氣血), gallbladder fire with stomach damp(膽火胃濕), deficiency of Yin in stomach with Kwolum's check (胃陰虛 厥陰乘), heat's penetration of blood collaterals with disharmony of liver and stomach(熱入血絡 肝胃不和), insidious pathogen in Kwolum(邪伏厥陰), the treatment of mediation(和解) with Prunellae Spica(夏枯草), Chrysanthemi Flos(菊花), Mori Folium (桑葉), Bupleuri Radix(柴胡), Coptidis Rhizoma(黃連), Scutellariae Radix(黃芩), Gardeniae Fructus(梔子), Cyperi Rhizoma(香附子), Toosendan Fructus(川?子), Curcumae Radix(鬱金), Moutan Cortex(牧丹皮), Paeoniae Radix Rubra(赤芍藥), Unoariae Ramulus Et Uncus(釣鉤藤), Cinnamorni Ramulus(桂枝), Paeoniae Radix Alba(白芍藥), Polygoni Multiflori Radix (何首烏), Cannabis Fructus (胡麻子), Ostreae Concha(牡蠣), Zizyphi Spinosae Semen(酸棗仁), Pinelliae Rhizoma(半夏), Poria(백복령). etc can be applied. 5) When the pathogenesis of BanJin(반진), BalLa(발라), ChangI(瘡痍), NamgChang(膿瘡). ChangJilGaeSeon(瘡疾疥癬), ChangYang(瘡瘍), SeoYang(署瘍), NongYang(膿瘍) and GweYang(潰瘍) are wind-damp(風濕), summer heat-damp(暑濕), damp-warm(濕溫), downward flow of damp-heat(濕熱下垂), damp-heat with phlegm transformation(濕熱化痰), gallbladder fire with stomach damp(膽火胃濕), overdose of cold herbs(寒凉之樂 過服), the treatment of damp-resolving(化濕) with Pinelliae Rhizoma(半夏), armeniacae Amarum Semen(杏仁), Arecae Pericarpium(大腹皮), Poria(백복령), Coicis Semen(薏苡仁), Talcum(滑石), Glauberitum(寒水石), Dioscoreae Tokoro Rhizoma(??), Alismatis Rhizoma(澤瀉), Phellodendri Cortex(黃柏), Phaseoli Radiati Semen(?豆皮), Bombycis Excrementum(?沙), Bombyx Batryticatus(白??), Stephaniae Tetrandrae Radix(防己), etc can be applied. 6) When the pathogenesis of ChangPo(瘡泡), hepatic abscess(肝癰) and appendicitis(腸癰) are food poisoning(食物中毒), Ki obstruction & blood stasis in the interior(기비혈어재과), damp-heat stagnation with six Bu organs suspension(濕熱結聚 六腑不通), the treatment of purgation(通下) with DaeHwangMokDanPiTang(大黃牧丹皮湯), Manitis Squama(穿山甲), Curcumae Radix(鬱金), Curcumae Longae Rhizoma(薑黃), Tetrapanacis Medulla(通草), etc can be applied. 7) When the pathogenesis of JeokBan(적반), BanJin(반진), EunJin(은진). BuYang(부양), ChangI(瘡痍), ChangPo(瘡泡), GuChang(久瘡), NongYang(膿瘍), GweYang(潰瘍), Jeong(정), Jeol(癤), YeokRyeo(疫?) and YeokRyeolpDan(疫?入?) are wind-heat stagnation(風熱久未解), blood fever in Yangmyong(陽明血熱), blood fever with transformation(血熱風動), heat's penetration of blood collaterals(熱入血絡). fever in blood(血分有熱), insidious heat in triple energizer(三焦伏熱), pathogen's penetration of pericardium(心包受邪), deficiency of Yong(營虛), epidemic pathogen(感受穢濁), the treatment of Yong & blood-cooling(淸營凉血) with SeoGakJiHwangTang(犀角地黃湯), Scrophulariae Radix(玄參), Salviae Miltiorrhizae Radix(丹參), Angelicae Gigantis Radix(當歸), Polygoni Multiflori Radix(何首烏), Cannabis Fructus(胡麻子), Biotae Semen(柏子仁), Liriopis Tuber(麥門冬), Phaseoli Semen(赤豆皮), Forsythiae Fructus(連翹), SaJin(사진), YangDok(瘍毒) and YeokRyeoIpDan(역려입단) are insidious heat's penetration of pericardium(伏熱入包絡), damp-warm's penetration of blood collaterals(濕溫入血絡), epidemic pathogen's penetration of pericardium(심포감수역려), the treatment of resuscitation(開竅) with JiBoDan(至寶丹), UHwangHwan(牛黃丸), Forsythiae Fructus(連翹), Curcumae Radix(鬱金), Tetrapanacis Medulla(通草), Acori Graminei Rhizoma(石菖蒲), etc can be applied. 9) When the pathogenesis of SaHuSinTong(사후신통), SaHuYeolBuJi(사후열부지), ChangI(瘡痍), YangSon(瘍損) and DoHan(盜汗) are deficiency of Yin in Yangmyong stomach(陽明胃陰虛), deficiency of Yin(陰虛), the treatment of Yin-replenishing(滋陰) with MaekMunDongTang(麥門冬湯), GyeongOkGo(瓊玉膏), Schizandrae Fructus(五味子), Adenophorae Radix(沙參), Lycii Radicis Cortex (地骨皮), Polygonati Odorati Rhizoma(玉竹), Dindrobii Herba(石斛), Paeoniae Radix Alba(白芍藥), Ligustri Lucidi Fructus (女貞子), etc can be applied. 10) When the pathogenesis of RuYang(漏瘍) is endogenous wind in Yang collaterals(陽絡內風), the treatment of endogenous wind-calming(息風) with Mume Fructus(烏梅), Paeoniae Radix Alba (白芍藥), etc be applied. 11) When the pathogenesis of GuChang(久瘡), GweYang(潰瘍), RuYang(漏瘍), ChiChang(痔瘡), JaHan(自汗) and OSimHan(五心汗) are consumption of stomach(胃損), consumption of Ki & blood(氣血耗盡), overexertion of heart vitality(勞傷心神), deficiency of Yong(營虛), deficiency of Wi(衛虛), deficiency of Yang(陽虛), the treatment of Yang-restoring & exhaustion-arresting(回陽固脫) with RijungTang(理中湯), jinMuTang(眞武湯), SaengMaekSaGunjaTang(生脈四君子湯), Astragali Radix (황기), Ledebouriellae Radix(防風), Cinnamomi Ramulus(桂枝), Angelicae Gigantis Radix(當歸), Ostreae Concha(牡蠣), Zanthoxyli Fructus(川椒), Cuscutae Semen(兎絲子), etc can be applied.

Effect of Korean Red Ginseng on Psychological Functions Patients with Severe Climacteric Syndromes : A Comprehensive Study from the Viewpoint of Traditional KAMPO-medicine and Western Medicine

  • Objective; Antistress effect of Korean red ginseng (RG) on postmenopausal women with severe climacteric syndrome (CS) were evaluated from the viewpoint of traditional KAMPO-medicine and Western medicine. Methods; All patients with CS were treated with daily oral administration of 6g RG for 30 days. Nine patients with CS were evaluated with the use of diagnostic scores for KI-deficiency (deficiency of vital energy) and OKETSU (blood stagnation) syndrome from the viewpoint of KAMPO-medicine. In the same patients with CS, peripheral blood levels of ${\beta}$-endorphin and total plasminogen activator inhibitor-1 (t-PAI-1) were measured before and after treatment with RG. In another group, 12 patients with CS, psychological test using CMI, STAI and SDS were performed from the viewpoint of Western medicine. Stress related hormones, such as ACTH, cortisol and DHEA-S in those 12 patients with CS were also measured before and after treatment with RG. Results; KI-deficiency score and OKETSU score in patients with CS were significantly (p<0.001) higher than those in patients without CS. After treatment with RG, both scores were markedly (p<0.001) decreased compared to before treatment with RG. ${\beta}$-endorphin levels in patients with CS were significantly (p<0.05) higher than those in patients without CS. Total PAI-levels in patients with CS were increased before treatment with RG. No significant difference, however, were observed between patients with and without CS. After treatment with RG, both levels of ${\beta}$-endorphin and total PAI-1 in patients with CS were significantly (p<0.001 and p<0.05, respectively) decreased compared to before treatment with RG. CMI and STAI scores in patients with CS were significantly (p<0.05) higher than those in patients without CS. SDS scores in patients with CS were also markedly (p<0.00l) higher than in those without CS. After treatment with RG, all scores decreased within normal range. DHEA-S levels in patients with CS were about a half of those without CS. Consequently, cortisol/DHEA-S (C/D) ratio was significantly(p<0.001) higher in patients with CS than in those without CS. Although the decreased DHEA-S levels were not restored to the levels in patients without CS, the C/D ratio decreased significantly (p<0.05) after treatment with RG. Conclusion; Reinforcement of vital energy and improvement of stagnant blood circulations by oral administration of RG were elucidated from the viewpoint of traditional KAMPO-medicine. From the viewpoint of Western medicine, effect of RG on postmenopsusal women with CS seemed to be brought about in part by not only an improvement of psychoneuroendocrine dysfunctions but also an amelioration of blood coagulation systems.

DSOM 변수와 일반혈액검사 및 일반화학검사와의 상관 관계 (Study on Correlation with DSOM Fluents and CBC, Biochemical Examination)

  • This study was investigated to know the correlation of complete blood count(CBC), biochemical examination and DSOM fluents(Diagnosis System of Oriental Medicine, (C)2005-01-122-004154). There are 5 fluents in DSOM such as DSOM score(病機點數), mean of the index for pathogenic factor(病機指標 平均), 5-division of DSOM score(病機點數 五點尺度), 5-division of the index for pathogenic factor(病機點數 五點尺度), weighted pathogenic factor(病機加重値).We have carried out clinical trials who volunteered for Sasang constitutional medicine and Oriental OB & GY, Oriental Medical hospital of Dong-Eui University, from May 2005 to June 2006. Volunteers were 245 persons. Because 7 persons didn't checkup DSOM, so we analyzed the results statistically for 238 persons. In the relation of CBC and DSOM, the scores(病機點數, zp) and mean of the index for Pathogenic factor(病機指標 平均, zps) showed correlation more frequently, and correlation with results of RBC, hemoglobin, hematocrit was more significant. Correlation with fluents of pathogenic factor(病機) were more significant and high in deficiency of blood(血虛), insufficiency of Yang(陽虛), coldness(寒), damp(濕), dryness(燥), kidney(腎), phlegm(痰), heat syndrom(熱), lung(肺), and was very low in spleen(脾). There was no correlation with deficiency of Yin(陰虛). If volunteers have DSOM fluents, results of RBC and RBC index was decreased(- derection), and results of RDW, ESR was increased(+ direction). But increase and decrease direction in heat syndrom(熱), lung(肺) was contrary to the others. Correlation with fluents of Pathogenic factor(病機) of WBC, platlet, PDW, MPV was not many. In the relation of biochemical examination and DSOM fluents, correlation with results of albumin, uric acid, triglyceride was more significant. If volunteers have fluents of pathogenic factor(病機), result of examination was usually decreased. Especially result of examination was decreased(- derection) highly in deficiency of blood(血虛), stagnation of (氣滯) coldness(寒), dryness(燥), and was increased(+ direction) highly in heat syndrom(熱), lung(肺). But there was a tendency to show high correlation with specific pathogenic factor (病機) and specific examination in biochemical examination.

해외사례 벤치마킹에 기반한 국내 CM 대가체계 개선 시사점 도출 (A Benchmarking Study on CM Fee Estimation)

  • 국내 CM은 2001년 건설산업기본법에 의해 개념과 범위가 정의된 후 지속적인 성장을 계속해 오고 있다. 하지만, 이러한 CM 제도에 기반한 성장에도 불구하고 국내에서의 CM 적용성과는 기대치에 비해 상대적으로 낮은 것으로 나타났다. 그 주요원인의 하나로 국내 CM제도가 책임감리에 비하여 업무범위가 넓지만, 대가기준은 책임감리에 비해 낮기 때문인 것으로 분석되었다. 따라서, 국내 CM 대가체계의 합리적인 기준을 정립하는 것이 시급할 것으로 판단된다. 본 연구에서는 CM 대가체계의 합리적인 기준을 마련하기 위하여 CMAA, ASCE, DOE, DOL 등의 다양한 해외사례 벤치마킹 연구를 통하여 국내 CM 대가체계를 비교하여 대가 상승요인을 추정하고 대가산정 개선방향을 제시하였다. 분석결과 대가수준은 해외에 비하여 현저하게 낮은 수준인 것으로 판단되었으며, 업무범위는 해외와 비교하여 그 폭이 좁으며 획일적인 구조를 가진 것으로 분석되었다. 따라서, 본 연구에서는 글로벌스탠다드에 따르는 국내 CM 대가체계를 개선하기 위하여 실비정액보수가산(Cost Plus Fixed Fee) 방식을 제시하고, 입 낙찰 전반에 걸친 CM 대가체계에 대한 기준(Framework)을 제시하였다.

농지연못습지의 수질 및 토양환경 분석 (The Analysis of Water and Soil Environment at Farm Pond Depression)

  • This study was conducted to understand the water and soil properties to propose the promotion of vegetation environment at farm pond depression. We selected 8 palustrine wetlands from agricultural area after consideration of human interference, surround land use, and size of area. Water quality analysis showed that the average SS, T-N, T-P were over the limit of agricultural water quality standard level at some sites. The cause for deterioration of water quality is supposed by the long-term stagnation of water in palustrine wetland. The recommended measures to improve water quality are as follows; improving water circulation by connecting with nearby natural water, preventing oxygen depletion by dredging deposit, lowering down T-N and T-P by removing autumn plants, preventing inflow of phosphorus in fertilizer ingredients which is the main cause for high T-P. The soil contamination of the surveyed area was about the same level of average heavy metal contents in soils from 2,010 paddy fields in Korea, which was much lower than soil contamination standards. As for soil texture, sand content was 40~90% and clay content was less than 20%. The content of silt and clay in soil from community of floating-leaved:submerged hydrophytes and community of emergent hydrophytes was higher that of soil from community of hygrophytes, and the content of sand in soil from community of hygrophytes was 10% higher than underwater soil. In terms of bulk density, the average was 0.24~0.96g/$cm^3$, which was quite low, because of high content of peat and organic matter in soil of the surveyed area. As for the average content of organic matter, community of floating-leaved:submerged hydrophytes was 18.25g/kg, community of emergent hydrophytes was 16.88g/kg, and community of hydrophytes was 25.63g/kg. The range of content of T-N in soil of community of floating-leaved;submerged hydrophytes was 0.022~0.307%, and that of community of emergent hydrophytes was 0.029~0.681% and that of community of hydrophytes was 0.088~0.325%. Apart from three sites in the surveyed area, most parts were over the standards or below the standard. After this study, we will conduct and discuss the relationship between vegetation characteristics and environments, which will be used of the best practical management and restoration of wetland.
