Similarities and Differences in Patients under Aged 18 with Respiratory Disease on Emergency Departments: Before and after COVID-19 Outbreak (코로나19 전·후 응급실로 내원한 소아청소년 호흡기계 환자의 유사점과 차이점)

  • Background: The purpose of this study was to the impact of the coronavirus disease 2019 (COVID-19) outbreak on emergency departments (EDs) in patients under the age of 18 years with respiratory disease. Also, we analyzed similarities and differences in patients including revisit before and after the COVID-19 outbreak. Methods: This study population was respiratory patients under the age of 18 years who visited all 403 EDs in Korea between January 1st, 2019 and December 31st, 2020, using the National Emergency Department Information System Database. The primary outcome was the number of respiratory patients according to age, sex, the type of EDs, season, Korean Triage and Acuity Scale (KTAS) levels, the result of ED, and length of stay. The secondary outcome was the number of revisit respiratory patients within 72 hours. We calculated the risk-adjusted revisit rates according to the KTAS level using a multiple logistic regression model. Results: The number of ED visits decreased from 274,526 in 2019 to 79,007 in 2020; this number was 71.2% lower than that before COVID-19. In spring 2020, this number was 90.1% lower than during the same period in 2019. For the revisit rate in the study population, the adjusted odds ratio (95% confidence interval) was 1.22 (1.05-1.41) in 2019 and 1.39 (1.07-1.81) in 2020. Conclusion: Implementing appropriate emergency care policies in severe respiratory patients would have contributed to improving the safety of reducing in revisit rate.

Radical surgery for stage IV gallbladder cancers: Treatment strategies in patients with limited metastatic burden

  • Backgrounds/Aims: The present study looked at the role of radical surgery in gallbladder cancers (GBC) with limited metastatic disease. Methods: The retrospective observational study was conducted to screen the database from 1st January 2010 to 31st December 2019. Patients of GBC found to have low-volume metastatic disease upon surgical exploration were included. Results: Of the 1,040 patients operated for GBC, 234 patients had low-volume metastatic disease (microscopic disease in station 16b1 node or N2 disease isolated port-site metastases, or low burden peritoneal disease with deposits less than 1 cm, in adjacent omentum or adjacent diaphragm or Morrison's pouch or a solitary discontinuous liver metastasis in adjacent liver parenchyma) detected intraoperative. Of these, 62 patients underwent radical surgery for R-0 metastatic disease followed by systemic therapy, while the remaining 172 patients did not undergo radical surgery and were given palliative systemic chemotherapy. Patients who underwent radical surgery had significantly superior overall survival (19 months versus 12 months, p < 0.01) and superior progression-free survival (10 months versus 5 months, p < 0.01) when compared to the rest. This difference in survival was more significant amongst patients when operated on after neoadjuvant chemotherapy. Regression analysis showed that a sub-group of patients with incidental GBC with limited metastases showed more favorable outcomes with radical surgery. Conclusions: Authors suggest a possible role for radical treatment of advanced GBC with a limited metastatic burden. Neoadjuvant chemotherapy can be used for preferentially selecting patients of favorable disease biology for curative treatment.

Work Environment and Depressive Symptoms of Webtoon Writers

  • Background: Webtoon, a digital form of comics created in the Republic of Korea, has spread widely with advantages that anyone can become a cartoonist and that autonomy of creation is guaranteed. The purpose of this study is to identify the working conditions of webtoon writers and analyze the relationship between these conditions and depressive symptoms. Methods: A survey was carried out on webtoon writers and a survey data of 312 webtoon writers were analyzed. The questionnaire included basic socio-demographic characteristics, webtoon writers' contractual type, fields of activity (webtoon creator, story writer, illustrator), and working environment (labor discretion etc.). We investigated depressive symptoms and analyzed its relation to the work environment of webtoon writers. Results: Webtoon writers were exposed to long working hours, high labor intensity, limited labor discretion, negative comments from readers, and had a high prevalence of depressive symptoms. Compared to story writers who contracted directly with platforms, story writers and the illustrators who contracted with content providers (CPs) were 9.51 times (OR = 9.51, 95% CI = 1.47 - 61.33) and 6.47 times (OR = 6.47, 95% CI = 1.08 - 38.75) more likely to have depressive symptoms, respectively. Conclusions: This study emphasizes the urgent necessity to improve the overall working environment in the webtoon industry and implement measures to tackle the escalating mental health challenges faced by illustrators and story writers contracted with CPs, especially given the increasing popularity of novel comics.

Evaluation of Aquatic Ecological Characteristics in Sinpyongcheon Constructed Wetlands for Treating Non-point Source Pollution (비점오염원 저감을 위한 신평천 인공습지의 수생태학적 특성 평가)

  • To evaluate the aquatic ecological characteristics in Sinpyongcheon constructed wetlands for treating nonpoint source pollution, the removal rates of nutrients in water, the total amounts of T-N and T-P uptakes by water plants, and chemical characteristics of T-N and T-P in sediment were investigated. The concentrations of BOD, COD, SS, T-N and T-P in inflow were 0.07~1.47, 0.60~2.65, 0.50~4.60, 1.38~6.26 and $0.08{\sim}0.32mg\;L^{-1}$, respectively. The removal rates of BOD, COD, SS, T-N, and T-P were 14%, 6%, 18%, 24%, and 10%, respectively. The maximum amount of T-N uptake by water plants in August was $813mg\;plant^{-1}$ for Phragmites communis TRIV in $2^{nd}$ bed, $1,172mg\;plant^{-1}$ for Typha orientalis PRESL in $3^{rd}$ bed, respectively. The maximum amount of T-P uptake by water plants in August was $247mg\;plant^{-1}$ for Phragmites communis TRIV in $2^{nd}$ bed, $359mg\;plant^{-1}$ for Typha orientalis PRESL in $3^{rd}$ bed, respectively. Organic matter, T-N, and T-P contents in sediments were high in the order of $1^{st}$ bed > $2^{nd}$ bed > $3^{rd}$ bed. Microbial biomass C/N/P ratios in sediments in $1^{st}$, $2^{nd}$, and $3^{rd}$ were 78~110/3~6/1, 73~204/1~6/1, and 106~169/1~6/1, respectively.

Angiotensin Converting Enzyme(ACE) Gene Polymorphism in Korean Stroke Patients (한국인(韓國人) 중풍(中風) 환자(患者)의 Angiotensin Converting Enzyme 유전자(遺傳子) 다형성(多形成)에 관(關)한 연구(硏究))

  • Objective : This study was designed to investigate the relation between the angiotensin converting enzyme(ACE) gene polymorphism and stroke in the Korean population. Methods : This study was carried out on 58 stroke patients who were hospitalized in the department of acupuncture & moxibustion, college of Oriental Medicine, Kyung-Hee University and 61 healthy control subjects. Blood samples from all subjects were obtaind for DNA extration. The extracted DNA was amplified by polymerase chain reaction(PCR). PCR products were visualized by 2% agarose gel electrophoresis. Results : The sub-genotypes of ACE gene were II homozygotes, ID heterozygotes, DD homozygotes. While the distribution of ACE polymorphism in control subjects was 31%, 51%, 18%, the distribution of it in stoke patients was 33%, 52%, 16%(II, ID, DD). Thus, there was no significant different between the control and stroke groups. Conclusions : we conclude that there is no significant association between ACE gene polymorphism and storke in Korean papulation. However, the findings of this study need to be confirmed in large patients and further studies. Additional epidemiologicallly based studies of the effects and relationship between ACE or other genes and lifesyles with regard to stroke required.

Genetic Diversity and Population Structure of Kaloula borealis (Anura, Microhylidae) in Korea

  • To assess the genetic diversity and population structure of Korean K. borealis, allozyme analysis was performed. The average genetic variability of Korean K. borealis populations was %P=13.2, Ho=0.048, and He=0.045. This value was the lowest in comparison with other Korean amphibian species studied. Also, the value was much lower than that of a reference population from Chinese K. borealis (%P=50, Ho=0.125, He=0.172). Wright's F-statistics showed that Korean K. borealis has distinctly low level of gene flow among regional populations (F$_{ST}$=0.339, Nm=0.487) in comparison with other Korean amphibian species studied. However, the average level of genetic divergence among Korean K. borealis populations was moderate (Nei's D=0.020). Therefore, it appeared that low levels of genetic diversity (He=0.045) and gene flow (Nm=0.487) among regional populations ave probably due to the results of decreasing population size and patchy distribution of this species in Korea.

A study on Nutrition Knowledge, Meal Management and Dietary Intake of Self-boarding Highschool Students (자취하는 남녀 고등학생의 영양 지식과 식생활 관리 및 식사 실태)

  • The purpose of this study was to investigate the knowledge on nutrition, meal management and dietary intake of self-boarding highschool students. Total of 354 self-boarding students living in Ku-mi and Kim-chon, 182 males and 172 females, who cooks foods for oneself participated in this study. The data were collected through questionnaires and the results were summarized as follows: 1. Most subjects had adapted the lifestyle of self-boarding from the 1st grade of highschool, and half of them lived alone. About two-thirds of subjects visited their home once a week or more, and most of their boarding rooms were located at near the highschool they attended. 2. Their nutrition knowledge score was rather high with average 13.95 point out of possible 20. Female students got more points than male students. 3. In meal management, foods purchasing and sanitary control were managed quite well but meal planning and cooking were not managed well enough. In general, meal management score of female students were higher than those of male students. 4. About the dietary intake, female students took more various foods than male students as judged by foods intake frequency score. Two-thirds of subjects often skipped meals and only half of them always took boxed lunch. Also they depended too much upon processed and instant foods. 5. Meal management score showed significantly positive correlations with nutrition knowledge score and with foods intake frequency score. But the correlation between nutrition knowledge score and foods intake frequency score was not significant.

Equivalence of Traditional and Individual Preparation of Ssanghwa-tang in terms of Polyphenol Contents and Radical Scavenging Activity (폴리페놀 및 항산화 관점에서의 쌍화탕 및 단미혼합쌍화탕의 동등성)

  • Traditional medicines usually use combinational formula that is prepared by mixing different varieties of medicinal herbs and boiling them in water to yield a decoction. In recent years a modified method has been proposed and practiced wherein the individual herbs are boiled with water separately and later these extracts are mixed together for use. This practice has lead to a constant controversy concerning the chemical and biological equivalence between the preparations arriving out of these two methodologies. In this research, we attempted to study the effects of traditional and individual preparation of Ssanghwa-tang (ST) to evaluate their equivalence in terms of its antioxidant activity and polyphenol content. The study results indicate that the polyphenol contents as well as the extraction yields of these two preparations were very similar to each other.

Synthesis and Electrochemical Properties of Li[Ni1/3Co1/3Mn1/3]O2 Nanowire by the Electrospinning Method (전기방사법을 이용한 Li[Ni1/3Co1/3Mn1/3]O2 나노 섬유의 합성 및 전기화학적 특성)

  • Nano-fibers of the $Li[Ni_{1/3}Co_{1/3}Mn_{1/3}]O_2$ electrode were synthesized from a metal oxide precursor using the electrospun method. The XRD patterns of all prepared powders showed a hexagonal ${\alpha}$ - $NaFeO_2$ structure (space group: R-3 m, 166). Scanning electron microscopy showed that all the synthesized samples were comprised of nanofibers with a size of 100~800 nm. Among the samples tested, the calcined $Li[Ni_{1/3}Co_{1/3}Mn_{1/3}]O_2$ nanowires in oxygen heating atmosphere showed a high charge and discharge capacity of 239.22 and 172.81 $mAhg^{-1}$ at the $1^{st}$ cycle, respectively. In addition, the charge transfer resistance was also improved significantly compared to the other samples.

The Influence of Academic Self-efficacy, Empathy and Communication Skills on Problem Solving Ability in Nursing Students (간호대학생의 학업적 자기효능감, 공감능력과 의사소통능력이 문제해결능력에 미치는 영향)

  • Purpose: This study aimed to investigate factors affecting the problem solving ability of nursing students. Methods: The data in this study was collected through structured questionnaires from September 1st to October 30th, 2021. Study participants were 172 fourth grade nursing students at four departments of nursing located in B and K city. IBM SPSS WIN v 21.0. program with t-test, ANOVA, Scheffe' test, Pearson's correlation coefficients, and hierarchical regression was used for data analysis. Results: The mean scores of academic self-efficacy was 3.88±051, empathy was 3.76±0.33, communication skills were 3.94±0.48, and problem solving ability was 3.70±0.40. There were significantly positive correlations between problem solving ability and academic self-efficacy (r=.45, p=<.001), empathy (r=.51, p=<.001), communication skills (r=.52, p=<.001), Factors affecting participants' problem solving ability were empathy (β=.31, p=<.001), and communication skills (β=.23, p=.006), which explained about 38.4% of the problem solving ability. Conclusion: In this study, it is necessary to develop curricular and non curricular nursing program that can strengthen empathy and good communication skills together when developing programs that increase problem solving ability.
