The Effects of Korean medicine treatment on Foot drop with common peroneal neuropathy patient: A Case Report (족관절 염좌에 따른 비골신경 신전 손상 환자에 대한 근에너지기법과 한의종합치료 병행 증례보고)

  • Objectives : The purpose of this study was to explore the effectiveness of Korean medicine treatment and the Muscle Energy Technique on foot drop through a case study with a patient with common peroneal neuropathy. Methods : A patient with severe foot drop after an ankle sprain was hospitalized and treated with acupuncture and traditional medicine and the Muscle Energy Technique. The effect of the treatment was evaluated with the Numeric Rate Scale(NRS), dorsiflexion angle evaluation, and the Manual Muscle Test (MMT). Results : The results of the NRS, dorsiflexion angle evaluation and MMT showed greatly improvementwith results changing from 8 to 2, 0 to 15, and Grade 2 to Grade 4, respectively. Conclusions : This study suggests that Korean medicine treatment and the Muscle Energy Technique could be effective treatments for foot drop with common peroneal neuropathy. However further clinical studies are needed to establish a definite conclusion.

A Current Status of Low Back Pain in Rural Peoples (일부 농촌지역 주민들의 요통발병 양상에 관한 연구)

  • Low back pain is generally defined as a complex symptom rather than a specific diseases entity, especially when musculoskeletal lesions of the low back, neurogenic, vasculogenic and discogenic lesions are present. Sixty to eighty percent of all human being have experienced low back pain once or more times during in their lifetime. Low back pain is a one of the commonest cause of physical impairment and induces disability the activities of everyday living or the activities of work. Today, low back pain plays important part for the loss of working capability from social, industrial field and individual daily life. But there are no study about incidence and prevention program of low back pain in rural peoples. This study was conducted to find out current status and characteristics related to low back pain in rural peoples(especially engaged in agriculture), so that could provide basic data to establish prevention program for them. The data were gathered from 301 rural adults(female 62.1%, male 37.9%) with low back pain by the structured interview in the 4 communities(south and north Gyeong-sang Do and Chung-cheong Do), and sampled at local hospital, local clinic, and health clinic center in May, 2000. The results were as followed. About one third of cases(35.5%) were aged from 60 to 69, 34.6% were smoker, and 91.4% not used to regular exercise. Low back pain frequently due to intervertebral articulopathy(42.5%), follewed by herniated intervertebral disk(19.9%), sprain(17.3%), vertebral dislocation(4.0%), and etc(16.2%). Above half of cases(56.5%) were a eight-hour workday, and 61.8% complained of low back pain during work. Mostly, period of agricultural experience was more than 20 years, and cultivated more than 2 crops. Low back pain is common to rural adults, therefore adequate resting time from work and lumbar strengthening exercise program needed to prevent low back pain in rural peoples.

Clinical Study of the Pediatric Patients Who Had Visited Emergency Room(ER) at Oriental Medical Hospital (한방병원 응급실에 내원한 소아 환자에 관한 임상적 고찰)

  • Objectives: By analyzing data of pediatric patients who had visited the emergency room of an oriental medical hospital, we can understand their characteristics and diseases. The purpose of this study was to introduce the excellence of Oriental medicine, to develop various treatments and to revitalize pediatric emergency care at oriental medical hospital. Methods: The study was composed of 371 pediatric patients who had visited the emergency room of the $\bigcirc\bigcirc$ university oriental hospital from January 2008 to December 2009. Results: 1. Average age of the pediatric patients was 4.28 years old, and it has showed that 1 to 3 years old patients (36.7%) were the most common age. 2. The number of pediatric patients was increased in June. According to the weekly distribution data, the number of pediatric patients who had visited on Sunday was the most (29.1%). Also, the number of pediatric patients who had visited ER for 18 to 21 hours(35.6%) were the most common. 3. The major problems of hospitalization were digestive symptoms and nervous symptoms. The nervous symptoms were the most at infancy. The musculoskeletal symptoms were the most common in adolescence. The digestive symptoms were common in other stages of development. 4. The time interval between arrival and onset; within 6 hours were the most(48.0%). Acupuncture and herbal medication treatment(75.2%) were the most common medical treatments. Most of the pediatric patients(97.3%) were discharged after medical treatments. Conclusions: Pediatric patients who had visited emergency room at the oriental medical hospital were mostly not due to acute form of serious diseases. The most common disease states that have preferred to treat with oriental medicine were dyspepsia, crying, febrile fit, and ankle sprain. We have to introduce the excellence of oriental medicine, and we need to try to develop other treatments such as magnetic acupuncture, moxa therapy, aroma therapy and revitalize pediatric emergency care at oriental medical hospital.

Analysis of Seasonal Injuries in Professional Football Player (프로 축구 팀의 월별 부상실태에 관한 조사)

  • Purpose: To investigate the risk exposure, risk of injury, and injury pattern of football players in seasonal variation. We purposed to expect the injury and its' prevention. Material and Methods: The professional football club(45 players) in Seoul were followed prospectively throughout 2005, January to 2005, December. Injury pattern and incidence were recorded. Results: Total 157 injuries were recorded. Major injuries which could not attend to game and training were 87 cases. Most severe month was June about 16 injuries(18.4%). And, the competitive season injuries were 46(52.9%) in March, April, August and September. Especially, ankle sprain and contusion injuries were common cause of absence in game. And, thigh muscle group injuries had a high risk of reinjury, because of treat insufficiently. Conclusions: We enhance the ability of the soccer team by prevention and education for injuries, especially in season.

Sociomedical Survey on the Occupational Low Back Injuries of the Some Workers in Pusan Area (부산 일부지역 근로자들의 재해성 요부손상에 대한 사회의학적 조사)

  • This survey was performed to obtain the basic information for the prevention and management of occupational low back injury (LBI). The subjects of this survey were 952 workers (male, 892 ; female, 60) who had taken occupational LBI in some industries of Pusan area from January 1 to December 31, 1991. Observation period was 2.6 years from the beginning to the end of medical treatment. The obtained results were summarized as follows ; 1. The proportion of LBI workers was 15.9% of the workers who had taken any occpational injuries and 0.32% of all workers in this surveyed area. 2. 8.0% of the workers had taken LBI on the 1st day of employment and 55.2% of the workers were within one year, 91.4% of the workers were within 10 years according to cumulative frequency distribution. 3. Handling of heavy objects was the most common cause of LBI (32.0%) and fall down (26.9%) and slip down (16.3%) were the next in order. 4. The most common causes of both lumbar sprain and HNP were handling of heavy objects and that of lumbar fracture was fall down. 5. The mean duration for medical treatment of LBI workers was 143 days. 6. The total direct compensation cost for LBI workers was 6,736 million Won and the proportion of medical, resting, disability compensation costs were 25.0%, 37.0%, 38.0 % respectively. 7. The percentages of retreated workers and disabled workers were 11.9% (113 persons) and 22.9% (218 persons) of total LBI workers respectively.

Case Series: Three Patients with Cervical Vertigo Treated with Hwangryunhaedok-Tang Pharmacupuncture (경추부 압통점의 황련해독탕 약침치료를 통한 경추성 어지럼증 환자 호전 치험 3례)

  • Objectives: The purpose of this case report was to evaluate the effectiveness of Hwangryunhaedok-tang pharmacupuncture on three patients diagnosed as cervical vertigo. Methods: Three patients were diagnosed as cervical vertigo with cervical sprain, resulting from an alteration of the neck proprioceptive afferents of the upper cervical spine. Hwangryunhaedok-tang pharmacoacupuncture was injected in the patients once daily for 10 days. The symptom change of dizziness was checked daily before and after treatment by the Visual Analogue Scale (VAS) score. The cervical pain and patient satisfaction were checked by the NRS (Numerical Rating Scale), NDI (Neck Disability Index), and the EQ-5D (EuroQol-Five Dimensions). Results: After the treatment, there was significant decrease in the value of the VAS score, measurement of dizziness, as well as the value of the NRS, NDI, a measure of pain. Also, the life quality measured by the EQ-5D improved. Conclusions: The results indicated that the Hwangryunhaedok-tang pharmacupuncture may have an effect on cervical vertigo with cervical pain.

A Study on the Sports Injuries for Middle and High School Football Players in Pusan (부산 시내 중.고등학교 축구선수의 운동손상에 관한 연구)

  • Recently sports injuries are increasing due to the development of sports leisure industries as well as the increase of exercising population, and the study for sports injuries is needed more than before. Then the author studied the reasons for injuries, incidence rate, sites and sorts of sports injuries for the middle and high school football players during one year period from Jan. to Dec. 1994. The study subjects were 319 middle and high school players in Pusan and the method was questionnaire method. The results were as follows; 1. The average injury rate due to football play was 3.3 times per person for the 319 subjects during the last one year, and it was highest in goalkeeper as 4.6 times by position. 2. 68 % of the subjects did not receive periodic health examination, 74.9 % wore body protectors. The sports injuries due to football play occurred most commonly in soil play-ground(79.6 %), during training(52.3 %), and in winter time(33.7 %). 3. The commonly occurred injuries by motion were coalition(39.2 %), tackle(12.9 %), dribble (11.4 %) in order; those by sorts were contusion(34.4 %), sprain (32.9 %), spasm(9.0 %) ; and those by body portions lower extremities(82 %), trunk(17.6 %), upper extremities(12.5 %). 4. The frequently used treatment area was home care as in 42.7 % ; treatment methods were physical therapy, acupuncture/moxacautery, rest, operation in order; choices of treatment method were recommendation by coach/manager, and that by medical professional was the lowest as in 2.2 %. 5. If sports injury occurred during play, coach or manager let the players take rest in 56.2 %, but emergent management by medical professional was done only in 1.9 %. The return to play was decided mostly by the player himself(52.8 %), but that decided by a doctor was only in 1.0 %. In conclusion, to lessen the sports injuries due to football play, annual physical check-up, strengthening of play rules, improvement of playground, conditioning exercise, regulation of body protectors, and preventive training for expected injuries behaviors should be done and the decision to return to play and emergent management should be done by a medical professional as well. Effect of Independent Suspension Function of Hiking Boots on the Stability and Load of Foot (등산화 아웃솔의 독립적 서스펜션 기능이 발의 안정성 및 부하에 미치는 효과)

  • To investigate the effects of independent suspension technology(IST) of hiking boot on the stability and load of foot, eight participants performed medial and lateral drop landing from 33.4cm height and 85cm distance to uneven surface while wearing normal & IST hiking boots. For the stability of foot during the drop landing, the balance angle & suspension angle and rearfoot angle was analyzed using high-speed video analysis. Also kinetic analysis using the force plate and insole pressure measurement was conducted to analyze vertical & breaking ground reaction force and pressure distribution. Not only the balance angle & suspension angle but also rearfoot angle was improved with IST boots for lateral drop landing. These results indicate the IST boots may have the suspension function which keeps the foot to be stable during landing. However the IST boots did not show any effect for medial landing. This might be related to the hardness of medial part of outsole. Therefore the softer outsole of medial part could be recommended. Furthermore the impact force & breaking force and insole pressure were reduced with IST boot. These results means that IST boot has not only cushioning effect but also good grip effect. Therefore the hiking boots applied the independent suspension function may help to reduce fatigue and prevent injury such as ankle sprain in hiking on uneven surface.

  • When we see normal gait, gait cycle is seperated as stance phase and swing phase. It needs 6 determinant of gait of pelvic rotation, pelvic tilt, knee joint of stance phase, ankle and foot motion, ankle and knee motion, and pelvic movement to be accomplished. In addition, a joint and muscle action is accomplished biomechanically at the same time with its gait cycle. In oriental medicine, the relationships between chang-fu physiology and meridian physiology are summaried as follows ; ${\bullet}$ chang-fu physiology : Spleen manages the extremities. Liver manages soft tissues. Liver stores blood. Kidney stores essences. Kidney manages bones. ${\bullet}$ meridian physiology : The Leg Greater Yang Meridian and meridian soft tissues The Leg Yang-Myeong Meridian and meridian soft tissues The Leg Lesser Yang Meridian and meridian soft tissues The Leg Greater Yin Meridian and meridian soft tissues The Leg Lesser Yin Meridian and meridian soft tissues The Leg Absolute Yin Meridian and meridian soft tissues Especially, we can find out relations between in a "blood supplied feet can walk well" that explains "blood regulations and by liver nourishing effects"that is the closest concept of muscle. Abnormal gaits are due to three causes as following; first, physical defect secoud, pain third, nervous system or instability of muscle. In oriental medicine, we can know relationship in "atrophy, numbness, stroke, convulsion, muscular dystrophy of knee, rheumatoid arthritis, five causes of infantile growing defects, five causes of softening, sprain". Especially, atrophy is the most important symptom. Gait evaluation should be emphasized where a point can walk 8 feet to 10 feet considering stride width, stride length, the body weight center, stride number, flexion, extension, rotation of a joint as a standard factor. The point is we should find out something strange in a patient's side, front and back view. After that we should find out its cause as an index that we can observe abnormal findings in a joint and muscle.

Study on Bee venom and Pain (통증 관련 봉독(蜂毒) 연구에 대한 고찰(考察))

  • In order to study Bee venom and Pain, We searched Journals and Internet. The results were as follows: 1. The domestic papers were total 13. 4 papers were published at The journal of korean acupuncture & moxibustion society, 3 papers were published at The journal of korean oriental medical society, Each The journal of KyoungHee University Oriental Medicine and The journal of korean sports oriental medical society published 1 papers and Unpublished desertations were 3. The clinical studies were 4 and the experimental studies were 9. 2. The domestic clinical studies reported that Bee venom Herbal Acupuncture therapy was effective on HIVD, Subacute arthritis of Knee Joint and Sequale of sprain. In the domestic experimental studies, 5 were related to analgesic effect of Bee vnom and 4 were related to mechanism of analgesia. 3. The journals searched by PubMed were total 18. 5 papers were published at Pain, Each 2 papers were published at Neurosci Lett. and Br J Pharmacol, and Each Eur J Pain, J Rheumatol, Brain Res, Neuroscience, Nature and Toxicon et al published 1 paper. 4. In the journals searched by PubMed, Only the experimental studies were existed. 8 papers used Bee Venom as pain induction substance and 1 paper was related to analgesic effects of Bee venom. 5. 15 webpage were searched by internet related to Bee Venom and pain. 11 were the introduction related to arthritis, 1 was the advertisement, 1 was the patient's experience, 1 was the case report on RA, 1 was review article.
