The prognosis of splinted restoration of the most-distal implants in the posterior region

  • PURPOSE. The aim of this study was to compare the efficacies of two-implant splinting (2-IS) and single-implant restoration (1-IR) in the first and second molar regions over a mean functional loading period (FLP) of 40 months, and to propose the appropriate clinical considerations for the splinting technique. MATERIALS AND METHODS. The following clinical factors were examined in the 1-IR and 2-IS groups based on the total hospital records of the patients: sex, mean age, implant location, FLP, bone grafting, clinical crown-implant ratio, crown height space, and horizontal distance. The mechanical complications [i.e., screw loosening (SL), screw fracture, crown fracture, and repeated SL] and biological complications [i.e., peri-implant mucositis (PM) and peri-implantitis (PI)] were also evaluated for each patient. In analysis of two groups, the chi-square test and Student's t-test were used to identify the relationship between clinical factors and complication rates. The optimal cutoff value for the FLP based on complications was evaluated using receiver operating characteristics analysis. RESULTS. In total, 234 patients with 408 implants that had been placed during 2005 - 2014 were investigated. The incident rates of SL (P<.001), PM (P=.002), and PI (P=.046) differed significantly between the 1-IR and 2-IS groups. The FLP was the only meaningful clinical factor for mechanical and biological complication rates in 2-IS. CONCLUSION. The mechanical complication rates were lower for 2-IS than for 1-IR, while the biological complication rates were higher for 2-IS. FLP of 39.80 and 46.57 months were the reference follow-up periods for preventing biological and mechanical complications, respectively.

Accuracy of a proposed implant impression technique using abutments and metal framework

  • PURPOSE. This study compared the accuracy of an abutment-framework (A-F) taken with open tray impression technique combining cement-on crown abutments, a metal framework and resin cement to closed tray and resin-splinted open tray impression techniques for the 3-implant definitive casts. The effect of angulation on the accuracy of these 3 techniques was also evaluated. MATERIAL AND METHODS. Three definitive casts, each with 3 linearly positioned implant analogs at relative angulations 0, 30, and 40 degrees, were fabricated with passively fitted corresponding reference frameworks. Ten impressions were made and poured, using each of the 3 techniques on each of the 3 definitive casts. To record the vertical gap between reference frameworks and analogs in duplicate casts, a light microscope with image processing was used. Data were analyzed by two-way analysis of variance and the Tukey test. RESULTS. The open tray techniques showed significantly smaller vertical gaps compare to closed tray technique (P < .05). The closed tray and the resin-splinted open tray technique showed significantly different vertical gaps according to the angulation of implant (P < .05), but the A-F impression technique did not (P > .05). CONCLUSION. The accuracy of the A-F impression technique was superior to that of conventional techniques, and was not affected by the angulation of the implants.

상악 임플란트 피개의치에서 유지장치 종류와 구개 피개 유무에 따른 응력분포에 대한 유한요소분석 (Effect of attachments and palatal coverage of maxillary implant overdenture on stress distribution: a finite element analysis)

  • 목적: 상악 임플란트 피개의치에서 유지장치 종류와 구개 피개 유무에 따른 응력분포를 3차원 유한요소분석법을 통해 알아보고자 한다. 연구 재료 및 방법: CAD를 이용하여 전방부에 4개의 임플란트를 식립한 4종류의 상악 임플란트 피개의치에 대한 3차원 모델을 디자인하였다. 1) Ball-F: 볼 유지장치를 사용하고 구개를 피개한 군, 2) Ball-P: 볼 유지장치를 사용하고 구개를 피개하지 않은 군, 3) Bar-F: 바 유지장치를 사용하고 구개를 피개한 군, 4) Bar-P: 바 유지장치를 사용하고 구개를 피개하지 않은 군. 응력분석은 유한요소 분석 프로그램인 ANSYS Workbench Ver. 14를 사용하였으며, 편측 저작의 상황을 고려하여 우측 제1대구치에 100 N의 정적하중을 적용하여 임플란트, 임플란트 주위 골, 점막에 발생하는 최대 응력값을 기록하였다. 결과: 볼 유지장치를 사용한 군이 바 유지장치를 사용한 군보다 임플란트 및 임플란트 주위골에 발생하는 최대 응력값이 작았다. 하지만 점막에서의 최대 응력값은 그 반대로 나타났다. 구개를 피개하면 유지장치의 종류와 무관하게 임플란트, 임플란트 주위 골, 점막 모두에서 최대응력값이 작아졌다. 결론: 본 연구의 한계 내에서 상악동 전방부에 4개의 임플란트를 식립하여 수복된 피개의치에서 바 유지장치보다는 볼 유지장치를 사용하고 구개를 완전히 피개하는 경우가 임플란트와 임플란트 주위 골의 응력분산에 효과적이다.

내부연결 임플란트 상부구조물에서 나사조임술식이 풀림토크값에 미치는 영향 (The effect of screw tightening techniques on the detorque value in internal connection implant superstructure)

  • 연구 목적: 본 연구는 잘 맞는 임플란트 상부구조물에서 서로 다른 나사조임순서와 조임방법이 나사의 풀림토크값에 미치는 영향을 평가하였다. 연구 재료 및 방법: 서로 수동적 적합 관계를 갖는 완전 무치악 하악 주모형과 네 개의 평행한 임플란트 (Astra Tech)에 직접 연결되는 금속구조물을 제작하였다. 금속구조물과 잘 맞는 실험모형을 위해 주모형에서 연결인상법을 이용하여 여섯 개의 경석고 모형을 얻었다. 20 Ncm로 조인 후 나사의 풀림토크값을 두 번 측정하였다. 세 가지 나사조임순서 (1-2-3-4, 2-4-3-1, 그리고 2-3-1-4)와 두 가지 나사조임방법 (two-step과 one-step)에 대한 나사의 풀림토크값과 최소풀림토크값을 유의수준 .05에서 각각 다원분산분석법 (multi-way ANOVA)과 이원분산분석법 (two-way ANOVA)을 이용하여 통계분석 하였다. 결과: 나사조임순서에 대한 나사의 평균 풀림토크값은 12.8 Ncm (2-4-3-1)에서 13.1 Ncm (2-3-1-4)의 값을 보였고, 나사조임방법에 대한 값은 13.1 Ncm (two-step)와 11.8 Ncm (one-step)였다. 나사조임순서에 대한 나사의 평균 최소풀림토크값은 11.1 Ncm (1-2-3-4)와 11.2 Ncm (2-4-3-1과 2-3-1-4)였고, 나사조임방법에 대한 값은 11.2 Ncm (two-step)와 9.9 Ncm (one-step)였다. 나사의 풀림토크값과 최소풀림토크값은 세 가지 나사조임순서 간에 통계학적으로 유의성 있는 차이가 없었다. 그러나 두 가지 나사조임방법 간에는 두 값 모두 통계학적으로 유의성 있는 차이가 있었다. 나사를 두 단계로 나누어 조이는 방법이 한 번에 조이는 방법보다 더 큰 풀림토크값 (P = .0003)과 더 큰 최소풀림토크 값 (P = .0035)을 보였다. 결론: 본 연구와 같은 조건하에서, 연결인상법으로 얻은 잘 맞는 임플란트 상부구조물에 있어서 나사조임순서는 나사의 풀림토크값에 영향을 미치는 중요한 요소는 아니었다. 그러나 나사를 두 단계로 나누어 조이는 방법은 한 번에 조이는 방법보다 더 큰 풀림토크값을 보였다.

임플란트 피개의치에서 지대주와 골의 변형률에 관한 연구 (Strains of abutment and bones on implant overdentures)

  • 연구목적: 임플란트 피개의치에서 연결고정형과 비연결고정형 보철물 지대주와 골의 변형률을 비교하고 임플란트 식립 경사에 따른 변형률을 비교 분석하고자 하였다. 연구 재료 및 방법: 2개의 아크릴릭 레진 모형의 좌측과 우측 견치 부위에 2개의 외부연결형 임플란트를 각각의 모형에 식립하였다. 한 모형에는 두 개의 임플란트를 평행하게 식립하였고, 다른 모형에는 순측과 설측으로 10도의 이개도(divergence)를 가지게 임플란트를 식립하였다. 모형에서 바를 이용한 연결고정형 지대주와 볼을 이용한 비연결고정형 지대주를 고정체에 번갈아 연결하고, 각각의 경우 대응하는 유지장치가 부착된 피개의치를 제작하여 지대주 상에 안착시켰다. 전기저항 스트레인 게이지(strain gauge)를 이용하여 피개의치에 100 N에서 200 N의 부하를 가하였을 때 지대주와 가상골 표면의 변형률(strain)을 측정하였다. 결과 및 결론: 측정된 값들을 통계분석하여 얻은 결과는 다음과 같다. 바를 이용한 연결고정형 보철물은 비연결형 보철물 보다 높은 변형률 값을 보였다. 그러나 방향은 주로 압축력이었으며 부하는 분산되었다(P<.05). 볼을 이용한 비연결고정형 보철물은 연결고정형보다 낮은 변형률 값을 보였다(P<.05). 순측경사된 임플란트에서는 비연결고정형 보철물이 연결고정형 보철물보다 더 높은 변형률 값을 보였다. 설측경사된 임플란트는 순측경사된 임플란트보다 낮은 변형률을 보였다(P<.05). 임플란트가 평행하게 식립된 경우보다 평행하지 않게 식립된 경우 지대주와 가상골의 변형률 값이 더 높았다(P<.05).

상악 구치부 임플랜트 보철수복시 치관/치근비에 따른 응력 분포에 대한 유한 요소 분석 (FINITE ELEMENT ANALYSIS ON MAXILLARY MOLAR IMPLANT UNDER DIFFERENT C/R RATIO)

  • Statement of the problem: In cases of low bone level in maxilla followed by extraction due to severe periodontitis or enlarged maxillary sinus, crown-root ratio of implant prosthesis will increase. The prognosis of these cases is not good as expected. Purpose : The purpose is to compare stress distribution due to crown-root ratio and effect of splinting between two implants in maxillary molar area under different loads Material and methods: Using ITI($4.1{\times}10$ mm) implant. two finite element models were made(model S: two parallel implants, model A: one of two is 20 degree inclined). Each model was designed in different crown-root ratio(0.7:1, 1:1, 1.25:1) and set cement type gold crown to make it splinted or non-splinted clinical situations. After that, 300 N force was loaded to each model in four ways.(load 1 : middle of occlusal table, load 2 : middle of buccal cusp, load 3 : middle of lingual cusp, load 4 : horizontal load to middle of buccal cusp), and stress distribution was analyzed. Results: On all occasions, stress was concentrated on neck of implant near cortical bone. In the case of inclined implant, stress was increased compared with parallel implants. Under load 1, 2, 3, stress was not increased even when crown-root ratio increases, but under load 4, when crown-root ratio increases, stress also increased. And more stress was concentrated under load 1 than load 2, 3. When crown-root ratio was same, stress under load 1, 2, 3 decreased when splinting, but under load 4, stress did not really decrease. Conclusion: Under vertical load, stress distribution related to crown-root ratio did not change. But under horizontal load, stress increased as crown-root ratio increases. Under vertical load, splinting decreased stress but under horizontal load, effect of splinting was decreased as condition of implant changes for the worse such as increase of crown-root ratio, inclined implant.

하악 구치부에서 임플랜트 배열방식에 따른 임플랜트지지 고정성 국소의치의 광탄성 응력 분석 (PHOTOELASTIC STRESS ANALYSIS OF IMPLANT SUPPORTED FIXED PROSTHESES WITH DIFFERENT PLACEMENT CONFIGURATIONS IN MANDIBULAR POSTERIOR REGION)

  • Statement of problem. More than 70% of patients who need the implant supported restoration are parially edentulous. The principles of design for implant supported fixed partial denture in mandibular posterior region are many and varied. Jurisdiction for their use is usually based on clinical evaluation. There are several areas or interest regarding the design of implant supported fixed partial denture in mandibular posterior region. 1) Straight and tripod configuration in implant placement, 2) Two restoration types such as individualized and splinted restorations. Purpose. The purpose of this study was to compare the amount and distribution of stress around the implant fixtures placed in the mandibular posterior region with two different arrangements and to evaluate the effects of splinting using the photoelastic stress analysis. Material & methods. 1) Production of study model: Mandibular partially edentulous model was waxed-up and duplicated with silicone and two models were poured in stone. 2) Fixture installation and photoelastic model construction: Using surveyor(Ney, USh), 3 fixtures(two 4.0 $\times$13 mm, one 5.0$\times$10 mm, Lifecore, USA) were insta)led in straight & tripod configurations. Silicone molds were made and poured in photoelastic resin (PL-2. Measurements group, USA). 3) Prostheses construction: Four 3-unit bridges (Type III gold alloy, Dongmyung co., Korea) were produced with nonhexed and hexed UCLA abutments and fitted with conventional methods. The abutments were tightened with 30 Ncm torque and the static loads were applied at 12 points of the occlusal surface. 4) Photoelastic stress analysis : The polarizer analyzer system with digital camera(S-2 Pro, Fujifilm, Japan) was used to take the photoelastic fringes and analysed using computer analysis program. Results. Solitary hexed UCLA restoration developed different stress patterns between two implant arrangement configurations, but there were no stress transfer to adjacent implants from the loaded implant in both configurations. However splinted restorations showed lesser amount of stresses in the loaded implants and showed stress transfer to adjacent implants in both configurations. Solitary hexed UCLA restoration with tripod configuration developed higher stresses in anterior and middle implants under loading than implants with straight configurations. Splintied 3 unit fixed partial dentures with tripod configuration showed higher stress development in posterior implant under loading but there were no obvious differences between two configurations. Conclusions. The tripod configuration of implant arrangement didn't show any advantages over the straight configuration. Splinting of 3 unit bridges with nonhexed UCLA abutments showed less stress development around the fixtures. Solitary hexed UCLA restoration developed tilting of implant fixture under offset loads.

부분 무치악 환자에서 고기능성 폴리머(PEKK) 지대주를 이용한 임플란트 고정성 보철물 수복 증례 (Implant-supported fixed prostheses with high-performance polymer (PEKK) abutments in partial edentulous patients: A case report)

  • 완전무치악 환자한테 처음으로 시도되었던 임플란트 보철치료는 현재는 단일치아 상실된 경우까지 그 활용범위가 넓어졌으며, 임플란트 보철치료는 현대 치과에서는 거의 필수적인 치료방법으로 자리매김 하였다. 임플란트 보철물을 제작하는데 사용되는 재료도 초기에 비해 다양해졌는데, 그 중 하나가 고기능성 폴리머의 적용이다. 예전부터 의과에서 많이 사용되어 왔던 고기능성 폴리머를 치과에서도 시도하는 빈도가 점점 늘어나고 있다. 이에 고기능성 폴리머를 구치부 임플란트 지대주로 사용한 3가지 증례를 보고하고자 한다. 첫번째 증례는 하악 좌측 제2소구치가 상실된 부위에 PEKK 지대주 및 PFG 크라운(시멘트-나사-유지형)을 제작, 장착하였으며, 두번째 증례는 상실된 상악 좌측 제1대구치 부위에 PEKK 지대주 및 monolithic zirconia 크라운(시멘트-나사-유지형)을 제작, 장착하였으며, 마지막 증례는 상악 우측 제1,2 대구치 상실부위에 PEKK 지대주 및 splinted PFM 크라운(시멘트-나사-유지형)을 제작하여 장착하였다. 4년의 추적 관찰 결과 상기 환자 모두 심미적 및 기능적으로 만족스러운 결과를 얻었기에 증례를 보고하고자 한다.

국부의치 설계에 관한 임상적 연구 (A CLINICAL EVALUATION ON THE DESIGN OF REMOVABLE PARTIAL DENTURE)

  • The purpose of this investigation was to evaluate the mouth preparation and design of removable partial dentures. A total of 187cases for the prefabricated partial denture frameworks in both maxillary and mandibular semi-dentulous situations (66 cases and 203 cases) was selected from this study. The evaluations of mouth preparation and design observed here involved the classification of edentulous spaces, status of abutment splinting with location, design of direct retainer and structure of maxillary major connector according to the incidence of both dental arches, ages, sexes and segment of semidentulousness. The analyzed results were as follows: 1) The order of frequency rate in removable partial denture construction was Class II (50.27%), Class I (36. 90%), Class III (10.69%), and Class IV (2.14 %). 2) The distribution on design of maxillary removable partial denture prosthesis was 33.22% and 64.11% in mandibular removable partial denture prosthesis. 3) The age distribution of removable partial denture prosthesis was prominent after40 years (41.71%). 4) The design pattern of maxillary major connectors was in order of anteroposterior bar, single palatal bar, palatal strap, U-shape connector. 5) The design pattern of direct retainer was in order of Aker's clasp, I-bar clasp, backaction clasp, cuspid universal clasp. 6) The abutment for partial denture clasp splinted between premolar and premolar and its frequency rate revealed 53.44%. 7) It seemed that the location and design of the indirect retainer showed accepatble limit.

Stress-strain distribution at bone-implant interface of two splinted overdenture systems using 3D finite element analysis

  • PURPOSE. This study was accomplished to assess the biomechanical state of different retaining methods of bar implant-overdenture. MATERIALS AND METHODS. Two 3D finite element models were designed. The first model included implant overdenture retained by Hader-clip attachment, while the second model included two extracoronal resilient attachment (ERA) studs added distally to Hader splint bar. A non-linear frictional contact type was assumed between overdentures and mucosa to represent sliding and rotational movements among different attachment components. A 200 N was applied at the molar region unilaterally and perpendicular to the occlusal plane. Additionally, the mandible was restrained at their ramus ends. The maximum equivalent stress and strain (von Mises) were recorded and analyzed at the bone-implant interface level. RESULTS. The values of von Mises stress and strain of the first model at bone-implant interface were higher than their counterparts of the second model. Stress concentration and high value of strain were recognized surrounding implant of the unloaded side in both models. CONCLUSION. There were different patterns of stress-strain distribution at bone-implant interface between the studied attachment designs. Hader bar-clip attachment showed better biomechanical behavior than adding ERA studs distal to hader bar.
