영적 간호중재가 호스피스환자의 통증과 불안에 미치는 효과

  • The purpose of this study was to elucidate the effects of spiritual nursing intervention on pain and anxiety of the hospice patients. This study was devised with a quasi-experimental design using non-equivalent contrast group non-synchronized design. The data were collected during the period from July 10 to September 25 in 2000 at the general hospital in CheonJu city. The subjects were sixty-seven patients who referred the hospice service. They were assigned to two groups, 36 members of the experimental group and 30 members of the contrast group. They also did not any complication, were alert enough tobe interviewed and agree willingly to participate in this study. The tools used were Yoon's Korean Version of Brief Pain Inventory and Spielberger's State Anxiety Scale. The spiritual care intervention was carried out through Hymn, Scripture, Prayer, the therapeutic use of self over a period of three weeks. Data were analyzed by mean, standard deviation, $x^2$-test, t-test, paired t-test and Pearson's Correlation Coefficients. The results of this study were as follows: 1.The examination of the same quality showed that there were not significant differences in the general characters, disease and therapeutic characters, religional characters between the experimental group and the contrast group. 2.After the spiritual nursing intervention pain scores of the experimental group were remarkably lower than those of the contrast group(right now pain: t=-2.634, p=0.012). 3.Decreasing rate in the pain scores of the experimental group were remarkably lower than those of the contrast group(right now pain: t=5.017, p=0.000). 4. After the spiritual nursing intervention state anxiety of the experimental group were remarkably lower than those of the contrast group(t=-5.987, p=0.000). 5. A positive correlation was found between reported pain and depression following the spiritual nursing intervention. In conclusion, the hospice patients who were offered spiritual care became lower than those who were not offered spiritual care and confirmed to decrease pain. According to these results, spiritual nursing intervention can be regarded as an effective nursing intervention that relieved pain and anxiety of the hospice patients.

간호 대학생의 호스피스 수강 유무에 따른 영적안녕과 죽음에 대한 태도 비교 (Comparison of Spiritual Well-Being and Attitudes toward Death due to Taking a Hospice Care Subject in Nursing Students)

  • Hospice should help a patient maintain spiritual well-being and its attitudes toward death can have great effects on treatment and nursing of patients on terminal patient. The purpose of this study was to examine the differences in spiritual well-being and attitudes toward death due to taking a Hospice Subject in Nursing Students This is a comparative study design in which 31 senior nursing students of taking a hospice care subject and 27 senior nursing students of not taking a hospice care subject in college of nursing K university, that is located B metropolitan city in Korea. The instruments for spiritual well-being measuring tools was Kim's (2006) revision and complement of the translation by Choe (1990) on the basis of the spiritual well-being scale developed by Paloutzian and Ellison (1983). The instruments for attitudes toward death was researches revision and complement of the translation by Kim(1992) on the basis of the attitudes toward death Measuring Tool developed by Thorson and Powell (1988). Data were collected from June 1 through June 30, 2006; the researcher got permission from the subjcts, explained objectives of the research to subjects personally and got their permission, and distributed structured questionnaires to make a response in a self-administered basis. For data analysis, an SPSS WINDOWS 12.0 program was used for frequency, percentage, the mean, standard deviation, the minimum, the maximum, x2-test, ANCOVA, and Pearson's correlation coefficients. The main results of this research are as follows: 1. There were no differecnces the degree of spiritual well-being and the degree of attitudes toward death between nursing students who took a hospice subject and nursing students who did not take a hospice Subject, except several items of attitudes toward death. There was significant positive correlation (r= .508, p= .000) between subjects spiritual well-being and attitudes toward death. That is, the higher spiritual well-being, the more positive attitudes toward death. In conclusion, although there were no differecnces spiritual well-being and attitudes toward death between nursing students who took a hospice care subject and nursing students who did not take a hospice care subject, in nursing students, spiritual well-being was relatively high and their attitudes toward death was relatively positive. To help a nursing students make a positive change in attitudes toward death, a nursing intervention program is necessary to improve spiritual well-being.

암 환자를 돌보는 간호사의 영적안녕과 영적간호수행 (The Spiritual Well-Being and the Spiritual Nursing Care of Nurses for Cancer Patients)

  • 목적: 본 연구는 암환자를 돌보는 간호사의 영적안녕과 영적간호수행간의 관계를 규명함으로써 영적간호교육과 영적간호중재 개발에 기초자료를 제공하고자 시도되었다. 방법: 전주시에 소재한 1개 기독교종합병원의 암환자를 돌보는 간호사 209명을 대상으로 2008년 9월 17일부터 30일까지 구조화 된 설문지를 통해 자료를 수집하였다. 자료 분석은 SPSS PC 14.0 프로그램을 이용하여 실수와 백분율, 평균, 표준편차, 특성별 변수간의 관계는 t-test와 ANOVA, Duncan test, 변수간의 상관관계분석은 Pearson Correlation Coefficient 등으로 분석하였다. 결과: 대상자의 영적안녕 정도는 평균 63.41 (도구범위: $20{\sim}80$점), 평균평점 3.17로 중상정도로 나타났고, 영적간호수행 정도는 평균 26.96 (도구의 범위: $15{\sim}60$점), 4점 만점에서 평균평점 1.80으로 중하 정도로 나타났다. 영적안녕의 하위영역 중 종교적 안녕은 평균 32.07 (평균평점 3.21), 실존적 안녕이 평균 31.34 (평균평점 3.13)로 종교적 안녕이 더 높은 점수를 나타냈다. 영적안녕과 하위영역인 종교적 안녕, 실존적 안녕 간에 모두가 상관관계가 있었으며(r=.959, P=.000; r=.940, P=.000) 그 중에서 종교적 안녕과 더 높은 관계가 있었다. 대상자의 일반적 특성에 따른 영적안녕에서는 학력, 신앙생활기간, 예배참여정도, 종교가 삶에 미치는 정도, 믿음정도에서 통계적으로 유의한 차이가 있었다. 대상자의 일반적 특성에 따른 영적간호수행에서는 연령, 결혼, 학력, 간호사역할, 임상경력, 신앙생활기간, 예배참여정도, 종교가 삶에 미치는 정도, 믿음정도에서 통계적으로 유의한 차이가 있었다. 대상자의 영적안녕과 영적 간호수행 간에는 유의한 순 상관관계를 나타냈고 (r=.353, P=.000), 하위영역인 종교적 안녕(r=.331, P=.000), 실존적 안녕(r=.342, P=.000) 각각에서도 영적 간호수행과 유의한 상관관계를 나타냈다. 결론: 암환자를 돌보는 간호사의 영적안녕은 중상수준으로 비교적 높았으나 영적간호수행은 중하수준을 보여주었으며, 영적안녕과 영적간호수행은 서로 밀접한 관계에 있었다. 또한 영적안녕과 영적간호수행은 교육, 신앙생활기간, 예배참여정도, 종교의 중요성, 믿음정도에서 공통적으로 유의한 차이가 있는 것으로 보아 간호사들의 교육수준을 높이며, 신앙적인 삶을 권장하고, 예배참여와 개인의 믿음성장을 위한 노력이 요구된다.

말기 환자간호 실습교육이 간호대학생의 죽음에 대한 태도, 임종간호 태도, 영적간호역량에 미치는 효과 (Effect of palliative care practical training on nursing students' attitudes toward death, end-of-life care nursing attitude, and spiritual nursing competency)

  • Purpose: This study aimed to assess the effect of palliative care practical training for nursing students. Methods: This quasi-experimental study included 38 third-grade nursing students form one university. Practical training, develooed by experts, was provided for 2 weeks (90 h) in a palliative care hospital. Participants received education on palliative care but no clinical practical experience. Collected data were analyzed using independent t-test, χ2 test and paired t-test using the WIN SPSS 23.0 program. Results: Students showed significant pretest-posttest differences in attitude toward death (t=-2.43, p=.021), end-of-life nursing attitude (t=3.90, p=<.001) and spiritual nursing competency (t=3.82, p=.001). Conclusion: The study results revealed that palliative care practical training was an effective learning method to improve nursing attitude, toward death, end-of-life nursing attitude and spiritual nursing competency. Further studied are needed to assess the effects of various education programs of palliative care.

한국판 영적간호역량 측정도구의 타당도와 신뢰도 (Validity and Reliability of Korean Version of the Spiritual Care Competence Scale)

  • Purpose: The aim of this study was to examine the validity and reliability of the Korean Version of the Spiritual Care Competence Scale (K-SCCS). Methods: A cross-sectional study design was used. The K-SCCS consisted of 26 questions to measure spiritual care competence of nurses. Participants, 228 nurses who had more than 3 years'experience as a nurse, completed the survey. Confirmatory factor analysis was used to examine the construct validity and correlations of K-SCCS and spiritual well-being (SWB) were used to examine the criterion validity of K-SCCS. Cronbach's alpha was used to test internal consistency. Results: The construct and the criterion-related validity of K-SCCS were supported as measures of spiritual care competence. Cronbach's alpha was .95. Factor loadings of the 26 questions ranged from .60 to .96. Construct validity of K-SCCS was verified by confirmatory factor analysis (RMSEA=.08, CFI=.90, NFI=.85). Criterion validity compared to the SWB showed significant correlation (r=.44, p<.001). Conclusion: The findings suggest that K-SCCS serves as an appropriate measure of spiritual care competence with validity and reliability. However, further study is needed to retest the verification of the factor analysis related to factor 2 (professionalisation and improving the quality of spiritual care) and factor 3 (personal support and patient counseling). Therefore, we recommend using the total score without distinguishing subscales.

호스피스간호사의 영적갈등 경험: 현상학적 연구 (Experience of Spiritual Conflict in Hospice Nurses: A Phenomenological Study)

  • Purpose: This aim of this phenomenological study was to describe and understand the experience of spiritual conflict in hospice nurses by identifying the meanings and structures of the experience. Methods: Participants were 12 nurses working for one year or more at hospice units of general hospitals in a metropolitan city and experiencing of spiritual conflict as hospice nurses. Over six months data were collected using individual in-depth interviews and analyzed with the method suggested by Colaizzi. Results: The experience of spiritual conflict in participants was organized into three categories, six theme-clusters, and 13 themes. The participants felt existential anxiety on death and a fear of death which is out of human control and skepticism for real facts of human beings facing death. They also experienced agitation of fundamental beliefs about life with agitation of the philosophy of life guiding themselves and mental distress due to fundamental questions that are difficult to answer. Also they had distress about poor spiritual care with guilty feelings from neglecting patients' spiritual needs and difficulties in spiritual care due to lack of practical competencies. Conclusion: Findings indicate the experience of spiritual conflict in hospice nurses is mainly associated with frequent experience of death in hospice patients. The experience of spiritual conflict consisted of existential anxiety, agitation of fundamental beliefs and distress over poor spiritual care. So, programs to help relieve anxiety, agitation and distress are necessary to prevent spiritual conflict and then spiritual burnout in hospice nurses.

영적간호 교육이 간호학생들의 죽음에 대한 태도변화에 미치는 영향 (The Influences of Spiritual Care Nursing Education Towards Death and Dying)

  • In order to care the persons who are dying a nurse should first solve her / his own conflicts about death. and be aware of their own concepts of death and dying. In order to find out patient's spiritual needs and to give better spiritual nursing care. a nurse should know her / his own spiritual needs and be aware of their own concepts of spiritual nursing problems. To improve nurse's understanding towards death and dying and nurse's knowledge towards spiritual needs and spiritual nursing care. 14 weeks(two hours a week) spiritual nursing care education was given to 3th grade baccalaureate nursing college student. Before and after spiritual nursing care education. 30 items of prepared questionare focused on the attitudes toward death and dying was asked. Pre and post results are as follow ; 1. The dying patient's emotional and physical needs. There was no significant difference between pre and post educated groups. Both of the situations. they agreed upon$(69.64\%)$ that the dying patients have high emotional and physical needs to solve. 2. Telling the truth of dying process. There was no significant difference between pre educated group$(53.33\%)$ and post educated group$(55.95\%)$. 3. Attitudes of medical personnels. There was no significant difference between pre$(51.49\%)$ and post educated groups $(53.87\%)$. These responses indicate that nursing college student didn't have enough experiences on dying patients care. 4. General attitudes on death and dying. Number of nursing students who were thinking positively toward death and dying were Increased (pre $39.68\%$. post $45.44\%$) and who were thinking negatively toward death and dying were also decreased (pre $37.30\%$. post $33.93\%$). 5. Attitudes toward mechanical assistance for life-expanding of helpless patient. There was a significant difference between pre and post educated groups. About $34.13\%$ of them approved upon mechanical assistance for life and about $33.14\%$ of them disapproved. 6. Attitudes of family members of dying patient. There was no significant difference between pre and post educated groups. About $45.24\%$ of both groups, agreed upon that the family members feel annoyed with dying patients and about $22.42\%$ of both groups disagreed. Whether they received the spiritual nursing education or not, they were aware of that the family members feel annoyed with dying patients. 7. Special facility and educational preparation for dying patient. There was a significant difference between pre$(82.14\%)$ and post$(90.87\%)$ educated groups. These responses indicated that after they received the education, they felt more about the necessity of special facility and educational preparation for the death and dying patients. 8. Special facility and welfare system for the old. There was a significant difference between pre$(58.33\%)$ and post$70.64\%$ educated groups. There responses indicated that after they received the education, they felt more about the necessity of special facility and welfare systems for the old.

Comparison of the Spiritual Needs of Terminal Cancer Patients and Their Primary Family Caregivers

  • Purpose: This study was conducted to examine differences in Spiritual Interests Related to Illness Tool (SpIRIT) scores and the degree of spiritual needs (SNs) between patients with terminal cancer and their primary family caregivers and to compare spiritual needs between them. Methods: The study participants were inpatients with terminal cancer and their primary family caregivers at 40 national hospice centers. The final analysis included 120 SpIRIT surveys from patients and 115 from family members, and 99 SNs questionnaires from patients and 111 from family members. Data analysis was conducted using descriptive statistics, the t-test, one-way analysis of variance, and Pearson correlation coefficients. Results: There were no significant between-group differences in SpIRIT scores or SNs. The SpIRIT sub-dimensions that ranked high for both patients and primary family caregivers were "maintaining positive perspective", "loving others", and "finding meaning". The SNs sub-dimensions were ranked identically in both groups, in the order of "love and connection", "hope and peace", "meaning and purpose", respectively. In both groups, the recognition of the importance of spiritual matters and religion were major factors influencing SpIRIT scores and SNs. Conclusion: The SpIRIT scores and degree of SNs of patients with terminal cancer and their primary family caregivers were found to be very closely related, and the needs for coherence and meaning were greater than religious needs. When providing spiritual care for patients with terminal illness, family members should also be considered, and their prioritization of spiritual needs and the importance of spiritual matters and religion shall be taken into account.

임상간호사의 임종간호태도와 영적건강이 임종간호스트레스에 미치는 영향 (The Impact of Clinical Nurses' Terminal Care Attitude and Spiritual Health on Their Terminal Care Stress)

  • 목적: 본 연구는 임상간호사의 임종간호태도와 영적건강이 임종간호 스트레스에 영향을 미치는 요인을 파악하기 위한 조사연구이다. 방법: G 광역시 일 종합병원에 종사하는 근무하는 238명의 간호사를 대상으로 자가 보고식 설문지를 이용하여 자료를 수집하였다. 결과: 임상간호사의 임종간호스트레스에 영향을 미치는 요인은 임종간호태도, 영적건강, 결혼유무, 총 임상경력 순이었으며, 임종간호태도와 영적건강이 좋을수록, 기혼일수록, 총 임상경력이 높을수록 임종간호스트레스가 낮게 나타났다. 이들 변수들은 임종간호스트레스에 대하여 52.3%의 설명력을 나타냈다. 결론: 이상의 결과로 임상간호사의 임종간호스트레스의 가장 중요한 요인으로는 임종간호태도로 나타났다. 따라서 간호사의 임종간호스트레스를 효과적으로 대처하고 완화시키기 위해 임종간호태도와 영적건강을 증진시킬 수 있는 교육적 중재프로그램 개발이 필요하다.

고통(suffering)과 영적 간호 (Suffering and Spiritual Care)

  • Although the general concept of suffering care includes palliative care for the terminally ill that is designed to alleviate pain, it is much more holistic and encompasses emotional, spiritual and other life dimensions. Human suffering is multi-dimensional including spiritual and religious aspects, which diverge from the concept of pain understood in the context of materialistic medical approach. In this caring perspective, the body, mind and spirit are integrated so that objectivity and subjectivity can merge. The extended awareness with inner source or energy, and positive thinking about the personally-relevant God can be meaningful the dying person, family members and the caring team. Despite the importance of an inclusive understanding of human suffering, actual nursing practice still does not fully embrace the full understanding of human suffering. A more fundamental meaning of human suffering from the nursing perspective may fruitfully adopt a more inclusive view of human suffering.
