Health characteristics and symptom of workers in reactive dye industries (염료공장 근로자의 직업성 천식에 관한 역학적 연구)

  • This study investigated the symptoms, medical and occupational history of 424 workers of 5 reactive dye Industries in the Inchon area in Korea. The study was performed on March 6 and July 19, 1989. The tests applied to the subjects were : serum total IgE, specific IgE, skin prick test with 7 inhalatory antigens, pulmonary function test, chest X-ray, methacholine test, and bronchoprovocation test. The workers were classified according to these tests into 4 groups (healthy, realtively healthy, need careful medical observation, and occupational asthma), and were compared in terms of the group characteristics and the symptom prevalence. The prevalece of occupational asthma of workers in reactive dye was 5.9% Significant differences were observed among the 4 groups. The groups were significantly different in the variables of sex and duration of smoking among their general characteristics ; and dyspnea, wheezing, chest pain, cough, nasal symptoms and sore throat among symptoms ; asthma, bronchitis, and other respiratory diseases with respect to their past medical history. This study suggests that we should pay special attention to the workers exposed to the risk of occupational asthma.

여성들의 찜질방 이용 현상에 관한 일상 생활 기술적 연구

  • The ZZimzilbang is a room where many women eagerly go due to the special meaning women give to it related to their health. It is a different type of sauna room which maintains low degree of temperature, consisting of an Ondol room(Korean under-floor heating system, hypocaust) built with mineral stone radiating ultrared rays. Even though many women mention that they utilize it for health, there is no precise evidence of the effects on their health. This ethnographic study sought to define the phenomenon from the perspective of the women who experienced the ZZimzilbang. A convenience sample of 27 women was interviewed during a 15 month period from December 1995 to July 1997 in 12 ZZimzilbangs located in Seoul, Korea. The mean ages of the women 57.3 years ; seventeen women were housekeeper and only eight women had job : twenty women were married and three women among them were widows. The main reasons women patronize the place are : for mitigation and healing of physical signs and symptoms : composure ; safe lodging and boarding : control of outward appearance : control of health ; meeting and fellowship with friends : and custom. The outcomes of the utilization of the place were : mitigation of physical signs and symptoms : psychological tranquility : cosmetic and diet ; good use of spare time : and utilization of services provided there. Most women who visited ZZimzilbang for relief of physical signs and symptoms strongly mentioned a correlation to inadequate Sanhujori, the traditional postpartal and postabortal care for woman. Some of specific kinds of services provided in that place were alternative therapy such as acupuncture, negative cupping, finger-pressure, mugwort steam and various kinds of massage including massage of blood vessels that are influenced by Oriental medicine ; health education of breathing such as abdominal breathing or Danjeon(단전호흡) : and selling of many things including health foods, drugs for osteoporosis, and eutrophics. This study suggests that professional caregivers should further study this phenomenon for the development of adequate care of women with a resulting important in their quality of life.

An Anxiety, Depressed Mood, and Insomnia in Newly Diagnosed Women Breast Cancer Patients and Thyroid Cancer Patients (처음 진단받은 여성 유방암 환자와 갑상선암 환자에서 나타나는 불안, 우울 및 불면 증상)

  • Objectives In this study, we identified the symptoms of insomnia, anxiety, and depressed mood in newly diagnosed women breast and thyroid cancer patients. Methods The subjects of this study were 1794 women patients who visited the Ewha Womans University Cancer Center for Women. They included 1119 newly diagnosed primary breast cancer patients and 675 newly diagnosed primary thyroid cancer patients. The patients completed the National Cancer Center Psychological Symptom Inventory (NCC-PSI) during their first follow-up visit after surgery, before starting chemotherapy or radiotherapy. The NCC-PSI is composed of the modified distress thermometer (MDT) and the modified impact thermometer (MIT) for insomnia, anxiety, and depressed mood. Results Anxiety severity was found to be greater in breast cancer patients than in thyroid cancer patients. Significant levels of anxiety, depressed mood and insomnia were present in 28, 24.5, and 20.7% in all the subjects, respectively. Moreover, anxiety symptoms, depressed mood and insomnia interfered with the daily lives of 20, 18.4, and 14.2% of all the subjects, respectively. Dealing with anxiety (18.8%) was found to need the most help, followed by dealing with insomnia (8.9%) and depressed mood (8.7%). Conclusions A significant level of distress was found in about 40% of the total subjects. Nearly 30% of newly diagnosed breast cancer patients reported significant anxiety symptoms and interferences with daily living caused by anxiety, which most commonly needed special care. Early assessment and management of psychological distress, especially anxiety, in breast and thyroid cancer treatment are very important to establish integrated cancer care.

Spontaneous Resolution of Chronic Subdural Hematoma : Close Observation as a Treatment Strategy

  • Objective : Chronic subdural hematoma (cSDH) is common condition in neurosurgical field. It is difficult to select the treatment modality between the surgical method and the conservative method when patients have no or mild symptoms. The purpose of this study is to provide a suggestion that the patients could be cured with conservative treatment modality. Methods : We enrolled 16 patients who had received conservative treatment for cSDH without special medications which could affect hematoma resolution such as mannitol, steroids, tranexamic acid and angiotensin converting enzyme inhibitors. The patients were classified according to the Markwalder's Grading Scale. Results : Among these 16 patients, 13 (81.3%) patients showed spontaneously resolved cSDH and 3 (18.7%) patients received surgery due to symptom aggravation and growing hematoma. They were categorized into two groups based on whether they were cured with conservative treatment or not. The first group was the spontaneous resolution group. The second group was the progression-surgery group. The mean hematoma volume in the spontaneous resolution group was 43.1 mL. The mean degree of midline shift in the spontaneous resolution group was 5.3 mm. The mean hematoma volume in the progression-surgery group was 62.0 mL. The mean degree of midline shift in the second group was 6 mm. Conclusion : We suggest that the treatment modality should be determined according to the patient's symptoms and clinical condition and close observation could be performed in patients who do not have any symptoms or in patients who have mild to moderate headache without neurological deterioration.

The Clinical Report about 33 Patients who Administrated Gami-Eumcheonyijintang with the Symptom of Coughing (가미음천이진탕(加味陰喘二陳湯) 투여환자(投與患者) 33례(例) 분석보고(分析報告))

  • Objectives: We intented to find out the effect of Gami-Eumcheonyijintang to patients who complain with coughing and to make it useful for treating the patients with the symptom. Materials and Methods: We examined the medical of 33 patients who visited with the symptoms of coughing, sputum in Kyungsan University, Hospital of Oriental Medicine from April, 1996 to February, 1999. and we collected the result of the administration with Gami-Eumcheonyijintong. We aimed to find any change in the symptoms, reappearance after recovery, and any special facts with the medicine surveying by telephoning to the patients. Results : the results are as follows, 1. It showed that patients who were under thirties showed higher rate of recovery(75.00%) than who were over thirties(52.94% ). 2, The acute-stage patients who were recovered in less than four weeks showed higher rate of recovery than those chronic-stage patients who were recovered in more than eight weeks. 3. The patients who didn't have the symptoms of sputum or had less serious problems of it with severe coughing showed relatively the highest rate of recovery(73.91% ). 4. The patients who showed the coughing symptom right after URI without any past constitutional history showed the highest rate of recovery(73.68% ). Conclusion : From the above result, we found that Gami-Eumcheonyijintang is more effective in early coughing symptom after URI than chronic stage coughing symptom. It is thought that more studies about medicine composition are needed in the further.

The Effect of Auricular Acupressure Therapy on Dysmenorrhea for Women Working in the Hospital (이압요법이 병원여성근로자의 월경곤란증에 미치는 효과)

  • Purpose: This study was done to provide support material to traditional nursing interventions through investigation of the effect of Auricular acupressure therapy for female staff in the hospital who have dysmenorrhea but cannot be absent from work even with severe symptoms affecting their work and therefore cannot get proper pain management. Method: The participants in this study were 62 women were working in the G-university hospital in Jin-ju, assigned to experimental (30) and 32 control groups (32). The study method was a quasi-experimental, nonequivalent control group non-synchronized design. The experimental group received acupressure treatment (a pin was attached to tape) on the special stimulation points in the inside of the ear, 5 times for 5 seconds, morning, afternoon, and night. Tape without the pressure pin was applied to the same points for women in the control group to evaluate placebo effect. Data were collected from May to November 2007 and were analyzed using SPSS PC+ 12 version. Descriptive statistics, t-test, $x^2-test$, Fisher's exact test, paired t-test, were used to analyze the data. Results: The experimental group showed a significant decrease in dysmenorrhea compared to the control group (t=-2.743, p<.01). In the sub-part, the experimental group showed menstruation related pain (t=-3.726, p<.01), menstruation related to behavioral change (t=-2.421, p<.05) and symptoms however, there was no significant difference related to attention deficit, water retention, and negative image. Conclusion: Application of auricular acupressure therapy was approved for this study and although auricular acupressure therapy was not effective for all of the symptoms, it was effective for dysmenorrhea.

A Study on the Mixed Organic Solvent Dose and Subjective Symptoms of Direct and Indirect Bonding workers in Shoes Manufacturing Industrial (신발제조업체의 접착제 사용에 따른 직접·간접폭로 근로자들의 복합유기용제 폭로량과 자각증상 비교)

  • This study was performed to find out the exposure level of mixed organic solvents, excretion of urinary hippuricacid and subjective symptoms according to the exposure of organic solvents of female workers who works on 5 shoes manufacturing industries in Taejon City from 24, september to 20, october 1993. The studied groups were divided into 3 groups that were consist of direct exposure group(48 workers), indirect exposure group(49 workers) and non-exposure group(68 workers) to the organic solvents. The exposure levels of toluene of direct exposure group which $89.86{\pm}56.20ppm$ had higher than that of indirect exposure group which had $40.23{\pm}47.21ppm$. In the exposure level of mixed organic solvent(R-value), direct exposure group was $2.84{\pm}1.53$ and exceeded approximatly 3 times the R-value. Whereas, indirect exposure group was not exceeded the R-value as $0.80{\pm}0.61$. In the excretion level of urinary hippuric acid, direct exposure group was $1.78{\pm}1.25g/l$, indirect exposure group was $1.22{\pm}0.93g/l$ and non-exposure group was $0.51{\pm}0.18g/l$ respectively. Therefore both direct exposure group and indirect exposure group were significantly higher than non-exposure group(P<0.01). In the correlation between toluene levels and urinary hippuric acid level, the direct exposure group had positive correlation(R=0.8309, P<0.01), also indirect exposure group had positive correlation(R=0.5859, P<0.05) and also in the correlation between the R value of mixed organic solvents and the urinary hippuric acid levels, the direct exposure group had positive correlation(R=0.4492, P<0.05), and indirect exposure group had ositive correlation(R=0.7911, P<0.01). In the complain rates of the worker's subjective symptoms at work, both direct exposure group and indirect exposure group were higher than non-exposure group(P<0.05, P<0.01). But the sujective symptoms of "floating sensation" of direct exposure group had significantly higher than indirect exposure group. In the percent of subjective symptoms complaints during the worker's daily life, both direct exposure group and indirect exposure group had generally more statistical significance than nonexposure group(P<0.01), direct exposure group had not statistical significant difference from indirect exposure group. As the results mentioned above, it has been analysed that the indirect at the adjacent manufaturing process are exposed to the considerable amount of solvent. Therefore, I think that there should be the betterment of surrounding through the complete working environment management to the occurrence source of the organic solvent, the changes of health management system to the indirect-exposed workers, and the systematic management of the special medical examination and the like.

Seasonal Occurrence of Yellow Tea Thrips, Scirtothrips dorsalis Hood (Thysanoptera: Thripidae) in Citrus Orchards and Its Damage Symptoms on Citrus Fruits (노지재배 감귤에서의 볼록총채벌레 발생 양상과 피해 증상)

  • Silver-grey or dark brown scaring, or dark brown discolored damages caused by the yellow tea thrip, $Scirtothrips$ $dorsalis$ Hood (Thysanoptera: Thripidae) have occurred on citrus fruits cultivated in the open field in east-south area orchards of Jeju Island since 2007. The weekly population of yellow tea thrips adults caught with a yellow sticky trap increased gradually from early May and peaked in late August, 2009. In 2010 and 2011, the most population was in early October. The yellow tea thrips of some species usually dominated from July in open citrus fields, and seven generations, at least, had occurred from May to late October 2009 by a yellow sticky trap survey. In 2009, $S.$ $dorsalis$ larva were observed from July and with peaks from late August to mid September and early October on citrus fruits. The damage symptoms appeared in the mid September. In 2011, the population peaks were in early July and late September, and the symptoms increased continually from mid July to late October. The symptoms were silver-scars or initial dark discolorations and then gradually changed to dark-brown discolorations or dark-brown scars. The percent of damaged fruits were 0.9%, 0.4%, 0.8% and 2.7% in 2008, 2009, 2010 and 2011, respectively, by survey in 64 typical orchards in Jeju Island. We are the first to report symptoms of the yellow tea thrip ($Scirtothrips$ $dotsalis$) on citrus fruits.

Deep Brain Stimulation of the Subthalamic and Pedunculopontine Nucleus in a Patient with Parkinson's Disease

  • Deep brain stimulation (DBS) of the pedunculopontine nucleus (PPN) is a novel therapy developed to treat Parkinson's disease. We report a patient who underwent bilateral DBS of the PPN and subthalamic nucleus (STN). He suffered from freezing of gait (FOG), bradykinesia, rigidity and mild tremors. The patient underwent bilateral DBS of the PPN and STN. We compared the benefits of PPN-DBS and STN-DBS using motor and gait subscores. The PPN-DBS provided modest improvements in the gait disorder and freezing episodes, while the STN-DBS failed to improve the dominant problems. This special case suggests that PPN-DBS may have a unique role in ameliorating the locomotor symptoms and has the potential to provide improvement in FOG.

Conservative Management of Esophageal Perforation; Clinical analysis of 14 cases (식도 천공의 보존적 치료 - 임상적 고찰 14례)

  • Twenty-five patients with perforation of esophagus were managed at Pusan National University Hospital, from 1981 to 1993. The hospital course of 14 of these patients was evaluated with a special emphasis on the cause and location of perforation, clinical presentation, time elapsed from perforation to treatment, method of treatment, and outcome. Patients with anastomotic leak and cancer were excluded.The perforation was iatrogenic in 7 patients, spontaneous in 5, ingestion of foreign body in 1, and traumatic in 1. There were 7 cervical perforations, 2 upper thoracic perforations, and 5 lower thoracic perforations. Chest pain, fever, and dyspnea were frequent symptoms. Esophagography was most diagnostic [11 patients] but thoracentesis was of little diagnostic aid.Antibiotics were administered intravenously to all patients:hyperalimentation was accomplished intravenously in 11, and nasogastric suction was used in all cases. No patient required any surgical procedure, minor or major.
