Antitumor and Immunomodulatory Activities of the P. grandiflorum Cultivated for More Than 20 Years (다년생 도라지의 항암 및 면역활성)

  • Platycodon radix is a dried root of Platycodon grandiflorum (P. grandiflorum) A. DC, a perennial grown on the hills and fields in Korea and Japan, or cultivated in various districts. Recently, P. grandiflorum (Changkil) has been successfully cultivated for more than 20 years and generally has been employed as folk remedy for adult diseases such as hyperlipidemia, hypertension and diabetes. We investigated various biological activities of the extracts from Changkil. When treated in vitro with B16-F1 mouse melanoma cell lines, it showed 100% laminin-binding inhibitory activities at the concentration over 0.125mg/ml. In in vivo test it showed 61.5% reduction of the solid tumor weight transplanted in mice and exhibited anticancer activity of 128% ILS against Sarcoma-180 ascites. It also increased the ratio of positive cells of natural killer cells in lymphocytic composition against Sarcoma-18O ascites and solid tumor transplanted in ICR mice when tests were carried out by FACScan method.

Nonfunctioning Neuroendocrine Tumor of the Pancreas in a 15-year-old Girl - a Case Report - (15세 여아에서 발생한 췌장의 비기능성 신경내분비종양 - 1예 보고 -)

  • We report a case of nonfunctioning neuroendocrine tumor of the pancreas in a 15-year-old girl who presented with back pain. On physical examination, there was mild tenderness in the left upper quadrant of the abdomen. The patient had no pancreatic hormone-associated symptoms. An abdominal ultrasonography showed a well-demarcated hypervascular solid mass with calcification in the tail of the pancreas. An abdominal computed tomography scan showed a $6{\times}5cm$ sized well-encapsulated enhancing solid mass with cystic component in the tail of the pancreas. Distal pancreatectomy was performed. Pathology revealed awelldifferentiated nonfunctioning low grade malignant neuroendocrine tumor of the pancreas. The postoperative course was uneventful.

Effects of Gunbibosinhangam-tang and Gunbibosinhangam-tang-gamibang on Solid Tumor and Immune Cells in Mice (건비보현항암탕(健脾補腎抗癌湯) 및 건비보현항암탕가미방(健脾補腎抗癌湯加味方)이 고형암 및 면역세포에 미치는 영향)

  • This study was carried out to investigate the anti-cancer effects of Gunbibosinhangam-tang (GBHT I) and Gunbibosinhangam-tang-gamibang (GBHT II, GBHT III) on solid tumor and immune cells. The animals were divided into 4 groups ; Control, no treatment. GBHT I, treatment with GBHT itself. GBHT II, treatment with GBHT increased the quantity of Hedyotis Diffusae twice. GBHT III, treatment with GBHT increased the quantity of Hedyotis Diffusae four times. We investigated the effects of GBHT on proliferation of solid tumor cells (S-180), thymocytes, splenocytes in vitro in order to examine cytotoxicity for S-180 and immuno-stimulating activities. The experiments that is about solid tumor weight and survival rate in tumor bearing mice were performed also. As compared with the control group, treatments with GBHT II and GBHT III suppressed the proliferation of S-180 effectively. Treatments with all experimental groups accelerated the proliferation of thymocytes and splenocytes significantly. In addition, GBHT III was significantly decreased on solid tumor weight and increased on survival rate in tumor bearing mice. Based upon these results, we suggest that GBHT and GBHT-gamibang have both anti-cancer effects for S-180 and immuno-stimulating activities for thymocytes and splenocytes. Therefore, we conclude that GBHT and Hedyotis Diffusae is useful to treat the patients with cancer.

Gamma Knife Radiosurgery for Remnant or Recurred Craniopharyngiomas

  • Objective : The authors assess the long term effectiveness of gamma knife radiosurgery[GKS] for remnant or recurred craniopharyngiomas on tumor control and possibly set proper radiation dose for tumor control with utmost preservation of the adjacent structures. Methods : Sixteen GKS were done in 14 patients with recurred or remnant craniopharyngiomas after surgery. Mean follow up duration was 44.2 months [range $11.3{\sim}123.6\;months$]. Follow up MR imagings were analyzed. Results : Mean tumor volume was $3.6cm^3$ [range $0.6{\sim}18cm^3$] and mean margin dose was 12.2Gy [range $8{\sim}22.4Gy$]. Tumor control was achieved in 87.5% [14 of 16 tumors] which were either solid or cystic in nature. Dose to optic apparatus was mean 7.9Gy and no radiation related complications were observed. Conclusion : GKS seems to be effective treatment modality for craniopharyngiomas regardless of nature of tumor whether it is cystic or solid. Dose of 8 to 8.5Gy may be sufficient to achieve long term tumor control for remnant or recurred craniopharyngiomas.

  • We report the case of a 16-year-old boy with a solid pancreatic mass which proved to be a nonfunctioning, malignant pancreatic neuroendocrine tumor (PNET). In pediatric patients, malignant pancreatic tumors are rare, especially malignant PNET. When dynamic contrast enhanced MRI showed a well enhancing solid pancreatic tumor on arterial and delayed phases and combined with malignant features, such as vascular invasion, invasion of adjascent organs, and lymphadenopathy, we should include malignant pancreatic neuroendocrine tumor in the differential diagnosis of childhood pancreatic tumors.

Acacia ferruginea Inhibits Tumor Progression by Regulating Inflammatory Mediators-(TNF-α, iNOS, COX-2, IL-1β, IL-6, IFN-γ, IL-2, GM-CSF) and Pro-Angiogenic Growth Factor-VEGF

  • The aim of the present investigation was to evaluate the effect of A ferruginea extract on Dalton's lymphoma ascites (DLA) induced tumours in BALB/c mice. Experimental animals received A ferruginea extract (10 mg/kg.b.wt) intraperitoneally for 14 consecutive days after DLA tumor challenge. Treatment with extract significantly increased the life span, total white blood cell (WBC) count and haemoglobin (Hb) content and decreased the level of serum aspartate transaminase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP), gamma glutamyl transferase (${\gamma}$-GT) and nitric oxide (NO) in DLA bearing ascites tumor models. In addition, administration of extract significantly decreased the tumour volume and body weight in a DLA bearing solid tumor model. The levels of pro-inflammatory cytokines such as tumor necrosis factor-alpha (TNF-${\alpha}$), interleukin-1 beta (IL-$1{\beta}$), interleukin-6 (IL-6) and granulocyte monocyte-colony stimulating factor (GM-CSF), as well as pro-angiogenic growth factors such as vascular endothelial growth factor (VEGF) and inducible nitric oxide synthase (iNOS) were elevated in solid tumour controls, but significantly reduced by A ferruginea administration. On the other hand, the extract stimulated the production of interleukin-2 (IL-2) and interferon-gamma (IFN-${\gamma}$) in animals with DLA induced solid tumours. Increase in $CD4^+$ T-cell population suggested strong immunostimulant activity for this extract. GC/MS and LC/MS analysis showed quinone, quinoline, imidazolidine, pyrrolidine, cyclopentenone, thiazole, pyrazole, catechin and coumarin derivatives as major compounds present in the A ferruginea methanolic extract. Thus, the outcome of the present study suggests that A ferruginea extract has immunomodulatory and tumor inhibitory activities and has the potential to be developed as a natural anticancer agent.

  • Tumor lysis syndrome is a life-threatening complication of anti-cancer therapy that typically occurs in patients with large, rapidly growing and treatment-sensitive tumors such as high-grade lymphomas and acute leukemias. However, its incidence in solid tumors has been known to be very low. Tumor lysis syndrome in solid tumors has a high mortality rate owing to the lack of prophylactic therapy to prevent this complication. We report a case of fatal tumor lysis syndrome developed during chemotherapy in extensive-stage small cell lung cancer, along with a brief review of the relevant literature considering the rarity of this manifestation in solid tumor.

A Case of Solid and Papillary Epithelial Neoplasm of the Pancreas (고형 및 유두상 췌장 종양 1례)

  • Solid and papillary epithelial neoplasm of the pancreas is an uncommon low-grade malignant tumor found predominantly in young females. It is rare in childhood. The origin of the tumor is probably from a multipotential stem cell of the pancreas. Neoplasm usually behave like a very low grade malignancy, so that complete removal is the treatment of choice for the tumor arising anywhere in the pancreas. We report a case of solid and papillary epithelial neoplasm of the pancreas in a 14 year -old girl who presented with intermittent abdominal pain and abdominal mass. CT scan shows a large, about $7{\times}6\;cm$ sized, well-marginated hypodense round mass in the head of the pancreas with some ill defined enhancing solid internal portion. She had taken complete excision of the pancereatic mass and Roux-en-Y pancreaticojejunostomy and histologically comfirmed solid and papillary epithelial neoplasm of the pancreas. A brief review of literature was made.

Tumor Growth Inhibitory and Immunomodulatory Activities of Cordyceps Militaris Water Extracts in ICR Mice Bearing Sarcoma-180 Solid Tumor (누에번데기 및 누에애벌레 밀리타리스동충하초(Cordyceps militaris) 열수추출물의 투여가 고형암이 유발된 마우스의 종양성장 억제 및 면역기능에 미치는 영향)

  • Hot water-extracts prepared from Cordyceps militaris of silkworm pupa (CMP) or Cordyceps militaris of silkworm larva (CML) were tested for tumor growth inhibitory and immunomodulatory activities in ICR mice bearing sarcoma-180 cells solid tumor, and compared with those of the known compound, cordycepin, found in Cordyceps militaris. Mice were subcutaneously injected with sarcoma-180 cells, and i.p. injected with either saline (Control), 50 mg/kg or 100 mg/kg of CMP (CMP50 or CMP100, respectively), or CML (CML50 or CML100, respectively), or 1 or 2 mg/kg of cordycepin (C1 or C2, respectively) for 10 days. Mice injected with CMP50 or CMP100 showed a 47.3% or 57.6% inhibition in the solid tumor growth (P<0.05), while those injected with CML50 or CML100 exhibited a 35.5% or 37.1% reduction (p<0.05) in solid tumor size compared to the value for control mice treated with saline. Animals injected with corcycepin showed a 26∼30% inhibition in the solid tumor growth (P<0.05). Mice bearing solid tumor and injected with CMP or CML showed a significantly increased thymus weight (38∼44% increase), lymphocyte percentages of CD4+ T-cell, CD8+ T-cell, and NK-cell (63∼110% increase) in the spleen, and interleukin-2 excretion (33∼51% increase) by the isolated splenocytes compared to those in control mice (p<0.05). These results indicate that the anti-tumor activity of hot water extracts of Cordyceps militaris, raised on both silkworm pupa and silkworm larva, appears to be associated with their immunomodulatory activity, and these activities found in Cordyceps militaris are superior to those for the single compound, cordycepin.

A Case of Combined Germ Cell Tumor in Testis (고환의 원발성 결합성 생식세포 종양 -A case report with literature review-)

  • Primary germ cell tumor of the testis is, rare, which occupies 1 to 2% of all reported malignant male neoplasms. Combined primary germ ceil tumor of the testis composed of embryonal carcinoma and seminoma is more rare tumor. The authors experienced a case of 50-year old male who presented with painless enlargement of right testicle. He has had radical orchiectomy and diagnosed by pathologically as combined germ cell tumor of testis. The testicle is measuring 180 gm in weight and $9{\times}6{\times}5$ cm in dimension, and almostly replaced by tumor mass. Grossly the tumor is rubbery solid smooth tumor mass, with variegated cut surface with geographically outlined diffuse necrotic area. Histologically the tumor is composed of two components of tumor, which are solid growth pattern of large round to polyhedral cells with clear or granular cytoplasm and distinct cell border, and anastomosing glandular and papillary arrangement of anaplastic epithelial cells. The former corresponds to seminoma, and the latter to embryonal carcinoma. Each tumor lobule is separated by abundant fibrous stroma.
