Compromised extraction sockets: a new classification and prevalence involving both soft and hard tissue loss

  • Purpose: Previous studies have solely focused on fresh extraction sockets, whereas in clinical settings, alveolar sockets are commonly associated with chronic inflammation. Because the extent of tissue destruction varies depending on the origin and the severity of inflammation, infected alveolar sockets may display various configurations of their remaining soft and hard tissues following tooth extraction. The aim of this study was to classify infected alveolar sockets and to provide the appropriate treatment approaches. Methods: A proposed classification of extraction sockets with chronic inflammation was developed based upon the morphology of the bone defect and soft tissue at the time of tooth extraction. The prevalence of each type of the suggested classification was determined retrospectively in a cohort of patients who underwent, between 2011 and 2015, immediate bone grafting procedures (ridge preservation/augmentation) after tooth extractions at Seoul National University Dental Hospital. Results: The extraction sockets were classified into 5 types: type I, type II, type III, type IV (A & B), and type V. In this system, the severity of bone and soft tissue breakdown increases from type I to type V, while the reconstruction potential and treatment predictability decrease according to the same sequence of socket types. The retrospective screening of the included extraction sites revealed that most of the sockets assigned to ridge preservation displayed features of type IV (86.87%). Conclusions: The present article classified different types of commonly observed infected sockets based on diverse levels of ridge destruction. Type IV sockets, featuring an advanced breakdown of alveolar bone, appear to be more frequent than the other socket types.

Various autogenous fresh demineralized tooth forms for alveolar socket preservation in anterior tooth extraction sites: a series of 4 cases

  • The aim of this study was to evaluate the clinical relevance of autogenous fresh demineralized tooth (Auto-FDT) prepared at chairside immediately after extraction for socket preservation. Teeth were processed to graft materials in block, chip, or powder types immediately after extraction. Extraction sockets were filled with these materials and dental implants were installed immediately or after a delay. A panoramic radiograph and a conebeam CT were taken. In two cases, tissue samples were taken for histologic examination. Vertical and horizontal maintenance of alveolar sockets showed some variance depending on the Auto-FDT and barrier membrane types used. Radiographs showed good bony healing. Histologic sections showed that it guided good new bone formation and resorption pattern of the Auto-FDT. This case series shows that Auto-FDT prepared at chairside could be a good material for the preservation of extraction sockets. This study will suggest the possibility of recycling autogenous tooth after immediate extraction.

  • Based on recent studies, the side resistance of rock socketed drilled shafts was affected by unconfined compressive strength of rock, socket roughness, rock types and joints, and initial normal stress. Especially, the socket roughness was affected by rock types and joints, drilling methods, and diameters of pile. In this study, a new roughness measurement system (BKS-LRPS, Backyoung-KyungSung Laser Roughness Profiling System) usable in water was developed. Based on the laboratory model tests, an EMD (Effective Measurement Distances) according to various turbidity was proposed as EMD=$1149.2{\times}T^{-0.64}_b$.

Evaluation of Shear Performance for CSB (Confined Socket Bolt) Shear Connector (CSB (Confined Socket Bolt) 전단연결재의 전단 성능 평가 )

  • This study evaluated a shear capacity of confined socket-bolt (CSB) shear connector developed for utilizing cast in placed pile (CIP) as a permanent underground wall. The push-out tests were performed in the specimens with different CIP types, CSB shear connector types, L/d, and concrete compressive strengths of concrete pile, and with or without waterproofing at interfaces between CIP and underground wall. Test results showed that the specimens with a H-shaped pile were fractured in the CSB shear connector, while the fracture concentrated in the concrete part of the specimens with a reinforced concrete pile was alleviated as the compressive strength of the concrete pile increased, resulting in the severe fracture of CSB shear connector. The maximum shear capacities of the specimens with high strength bolts and reinforcing bars used as CSB shear connector were approximately 1.22 and 1.20 times higher than those of the specimens with a H-shaped pile, respectively, and 1.10 and 1.16 times higher than those of the specimens with a reinforced concrete pile, respectively. Meanwhile, the maximum shear capacity was not significantly affected by the embedding length of the CSB shear connector and overlapping length of reinforcing bar. The predicted shear capacities calculated from the KDS standards were lower than the measured values of all specimens tested in this study.

Histologic Study on the Effect of Two Types of Bovine Bone Powder in Extraction Socket of Beagle Dogs (성견 발치와에 매식한 2종의 Bovine Bone Powder가 치유에 미치는 영향에 관한 조직학적 연구)

  • Several extraction cases with advanced bone loss as a result of periodontal disease, root or labial bone fracture, extensive caries, and periapical lesions occur esthetic, functional problems and severe bone loss. Therefore, to treat these cases used several surgical methods and socket preservation among this therapies have been evaluated simple, effective and good prognosis in the implant placement. Socket preservation therapy have been used with barrier membranes or/and graft materials. Deproteinized bovine bone mineral have been evaluated ideal grafting materials. Recently, calcium-phosphate thin film coated bovine bone powders were developed in our country, but the study for these material wasn't reported. When two types of xenograft materials were implanted in extraction sockets of Beagle dogs, the effects of these were analyzed after 4 weeks and 8 weeks histological views. The results of this study were as follows. 1. In control groups, 4 weeks after implantation, the extraction sockets were filled with connective tissue which has dilated vessels and epithelial growth. And after 8 weeks, irregular connective bundles were observed. But new bone formation was not seen. 2. In Bio-Oss groups, epithelial growth was not seen and bone powder was covered with connective tissue fiber. New bone formation was found around the interproximal bone. There was no special change seen after 8 weeks, connective tissue fibers became more regular, and bone growth near bone powder was not made well. 3. In Ca-P BBP groups, epithelial cells didn't grow in the extraction sockets, there was a lot of new bone made around the bone powder after 8 weeks, new bone around bone powder was replaced with mature bone. It is thought that bone powder grafting into the extraction sockets is very useful for conservation of ridge, and Ca-P BBP is more effective in bone formation than Bio-Oss.

Design of Cooling System of Over-molding Mold for Socket Component of Automobile Wiper (자동차 와이퍼 소켓 부품 제작용 오버몰딩 금형의 냉각 시스템 설계)

  • The objective of this study is to design of a cooling system of the over-molding mold for a socket component of an automobile wiper by performing numerical analyses. Hot spots in which the temperature distributions are higher than those of other region, were estimated by an initial over-molding analysis for the initial design of the mold. On the basis of the initial over-molding analysis, two types of cooling system designs with a linear cooling channel and a volumetric heat sink, were considered to improve the cooling characteristics of hot spots. To obtain an appropriate cooling system design, the effects of the diameter and the position of the linear cooling channels on the cooling characteristics and the product qualities were quantitatively examined. In addition, the effects of the design of the volumetric heat sink on the cooling characteristics and deformation distributions in the molded product were investigated. The results of the over-molding analysis of the two types of cooling systems showed that the multi-sliced over-molding mold with a volumetric heat sink can improve both the product quality and the cooling characteristics of the mold.

Calibration of Borehole Roughness Measurement System for Large Diameter Drilled Shafts in Water (수중에서 적용가능한 대구경 현장타설말뚝의 굴착공 벽면거칠기 측정장치의 보정에 관한 연구)

  • Based on recent studies on rock socketed drilled shafts, it was found that the side resistance of rock socketed drilled shafts is affected by unconfined compressive strength of rock, socket roughness, rock types and joints, and initial normal stress. Especially, the socket roughness is affected by rock types and joints, drilling methods, and diameters. Since existing roughness measurement systems could be conducted only in the air, a new roughness measurement system, which can measure rock socket roughness in the air and also in the water, is needed. However, the development of new roughness measurement system fur civil engineers has been faced with difficulties of electrical applications. In this study, the laboratory verification system far BKS-LRPS (Backyoung-KyungSung Laser Roughness Profiling System) was developed, which can be applied both in the water and air. Based on the laboratory verification, it was found that the improved BKS-LRPS could define effective measurement distances for the conditions reflecting the apparatus and in-situ situations.

Comparison of clinical and histological characteristics of orthodontic tooth movement into recent and healed extraction sites combined with corticotomy in rats

  • Objective: This study was performed to investigate the rate of tooth movement and histological characteristics of extraction sockets those were subjected to corticotomy. Methods: A split-mouth randomized controlled trial experiment was designed. Thirty-two adult, male Wistar rats were divided into 2 groups: healing extraction socket (H) and recent extraction socket (R); these groups were randomly classified into 4 subgroups (0/7/21/60 days). The first maxillary molar was extracted on 1 side and 2 months were allowed for complete bone healing; then, the corresponding molar was extracted on the other side and surgical intervention was performed at the mid-alveolar point of the first maxillary molar. Ten grams of continuous force was applied. The outcomes measured were rate of tooth movement, percentage of periodontal space and histological evaluation. The rate of tooth movement was calculated as the measured distance divided by the duration of molar movement. Histomorphometric evaluations were performed on the second and third maxillary molars. The Wilcoxon signed rank test was used to compare differences between the two groups. Results: There were no significant differences in the rates of tooth movement between H and R groups at any of the 4 time points. The histological appearance and percentage of periodontal space between the R and H groups also demonstrated no significant differences. Conclusions: The rates of orthodontic tooth movement into recent and healed socket sites did not differ between the groups. Histological analysis of tooth movement revealed regional acceleration during every time period.

Side resistance of rock socketed drilled shafts considering in situ rock mass condition (현장조건을 고려한 현장타설말뚝의 단위주면마찰력)

  • Rock socketed drilled shafts transfer significant portion of structural loads at the socketed part. Therefore, a proper design of side and base resistances of a shaft at the socket is a major concern for the geotechnical engineers. In this study, we modified the Hoek-Brown criterion to estimate side resistance of rock socketed drilled shafts. Earlier method to compute side resistance of a shaft is linear or power functions of intact rock masses. However, side resistance is mobilized like shearing which influenced by the mechanical properties of concrete and rock masses, adhesion of rock/concrete interface, roughness of rock socket. Therefore, a single coefficient or power of uniaxial compressive strength of intact rock cannot provide accurate values of side resistance in a wide range of the uniaxial compressive strength. A new approach proposed in this study can consider in situ rock mass condition (frequency or discontinuities, weathering condition), and rock types thus, it has a wider applicability than the earlier models.

Study on Web Services Middleware for Real-Time Monitoring in the IoT Environment

  • Recently, the need for real-time systems which are providing various types of information that occur in large quantities in IoT environment is increasing. In this paper, we propose a middleware system which can monitor in real time on a web environment. The proposed system is designed to be integrated by using communication functions provided by a network operating system and external sensors. The proposed system is compared with an existing system and analysed by the server performance testing tool.
