A Study on Burden of Middle Aged Spouses of Rheumatoid Arthritic Patients (류마티스 관절염 환자 배우자의 부담감)

  • Rheumatoid arthritis as one of the chronic illness requiring management in long period of time puts great burden to patients, their family and society. For patients with chronic illnesses, providing a social support is important and the most important source comes from spouses. Therefore we assessed burden of husbands of female rheumatoid arthritic patients and also found out the factors affecting burden. The sample of study was 107 female rheumatoid arthritic patients and their spouses. The tool of assessing spouses' burden was the revised version of subjective and objective parameters developed by Montgomery et al.(1985). The results are as follows: 1. General characteristics of patients and spouses: The mean age of the patients was 48 years. Educational level of patients was high school 41.1%. The mean age of the spouses was 51years. Educational level of spouses was mostly high school(40.2%) and college(29.9%) graduate. The mean marital period was 23.4years. Average income per month was 1,609,000 won. The average duration since diagnosis was 9.4years. As a therapy, 67.3% chose standard drug therapy. Average rating of discomfort by patient was 3.05(range 1-5) and that of severity was 3.48 and that of dependency was 2.58. The husband's rating of their spouses disease severity was 3.68. 2. Husbands' burden: The average burden in subjective items was 21.61(range 6-36) and objective items was 35.24(range 10-60). The average of total burden was 56.59(range 16-96). 3. Husband's total burden correlated with patient's age, educational level of patients, therapy method, patient's level of discomfort, patient's severity, patient's level of dependence, husband's recognition of level of severity in statistical level. Husband's objective burden correlated with patient's age, educational level of patient, patient's level of discomfort, husband's recognition of level of severity. Husband's subjective burden correlated with patient's age, educational level of patients, therapy method, patient's severity, patient's level of dependence, husband's recognition of level of severity. 4. Linear correlation analysis on burden: The husbands' total burden is explained in 37 7% by husband's recognition of level of severity and husband's age. The husbands' objective burden is explained in 31.2% by patient's level of dependence, husband's age, husband's recognition of level of severity. The husbands' subjective burden is explained in 26.7% by husband's recognition of level of severity and patient's age. In conclusion, husbands' level of burden is affected by many factors and therefore nursing strategy for relieving burden of middle aged husbands should be individualized taking these factors into consideration.

  • The study aims to examine the current status and differences in the burden of disease in Korea during 2008-2018. We calculated the burden of disease for Koreans from 2008 to 2018 using an incidence-based approach. Disability adjusted life years (DALYs) were expressed in units per 100 000 population by adding years of life lost (YLLs) and years lived with disability (YLDs). DALY calculation results were presented by gender, age group, disease, region, and income level. To explore differences in DALYs by region and income level, we used administrative district and insurance premium information from the National Health Insurance Service claims data. The burden of disease among Koreans showed an increasing trend from 2008 to 2018. By 2017, the burden of disease among men was higher than that among women. Diabetes mellitus, low back pain, and chronic lower respiratory disease were ranked high in the burden of disease; the sum of DALY rates for these diseases accounted for 18.4% of the total burden of disease among Koreans in 2018. The top leading causes associated with a high burden of disease differed slightly according to gender, age group, and income level. In this study, we measured the health status of Koreans and differences in the population health level according to gender, age group, region, and income level. This data can be used as an indicator of health equity, and the results derived from this study can be used to guide community-centered (or customized) health promotion policies and projects, and for setting national health policy goals.

A Study on the Development of Self-help Program with Care Burden for Families with Demented Elderly Persons (치매노인 가족의 부양부담 경감을 위한 자조집단프로그램 개발)

  • Purpose: This study were to understand the caregiving situation of families with the demented elderly and to develop a self-help program for them. Method: A self-help program was designed to increase caregivers' confidence and skills in problem solving. In addition, it was designed to provide information about the demented elderly. The self-help program of this study was composed of nine sessions. Result: Family caregivers of elderly patients with dementia obviously experience high level of stress. which often leads the caregivers to negative outcomes such as depression, interference with social activities and health difficulties. That is, caring is stressful for caregivers because the demands of the caregiving situation tend to overwhelm the caregivers' coping resources. Therefore, families with demented elderly persons need assistances in order to be relieved from their care burden. Conclusion: The self-help program includes the understanding of dementia, the family caregiving experiences of the demented elderly persons, the time for self-care, the search for helping, the improving of interpersonal relations and communications.

Study on Family Caregiving Burden Scale of Dementia-Korea(FCBSD-K) (치매환자 가족부담감의 한국형 도구개발)

  • The purpose of this study was to develop and validate the scale to measure dementia patient's caregiver burden of Korea. In the first phase of the study, 15 caregivers of dementia patients were interviewed to provide narrative data from which items were developed. Initially 65 items were generated from the interview data of 15 caregivers. Content validity was judged by two separate panels of experts with 27 professionals and 30 family caregivers. These items were analyzed through the Index of Content Validity and 33 items were selected which met .80 or more of the CVI. This preliminary FCBSD-K was tested with 207 adult caregivers for reliability and construct validity including item analysis and orthogonal(Varimax) factor analysis. Eight items were deleted because of high or low item-item correlation. The result of the second factor analysis produced six factors that coincided with the conceptual framework posed for the scale developed. The six factors were labeled as 'physio social factor' 'emotional factor' 'family cultural factor' 'role obligation' 'guilt feeling' and 'financial & supportive system factor'. The alpha coefficient relating to internal consistency was .9264 for reliability. In conclusion, cultural factor is related to dementia patient's caregiver burden and FCBSD-K was useful in assessing the dementia patient's caregiver burden in Korea.

The Effect of Family Caregiver Burden on Psychological Well-Being among Middle-Aged Working Mothers: Focused on the Moderating Effect of Family Support Resources (중년기 취업모의 가족부양부담이 심리적 복지감에 미치는 영향 - 가족지지 자원의 조절효과를 중심으로 -)

  • This study aimed to explore the moderating effect of family support resources on family caregiver burden as it in turn affects psychological well-being among middle-aged working mothers. Through purposive sampling, the study recruited 325 married employed mothers age 40-50 years who live in Seoul and who have more than 1 child and living parents or parents-in-law. The investigation was performed April 1-30, 2018, with the participants sampled from companies, schools and religious organizations in Seoul. The collected data were statistically analyzed using the SPSS 21.0 package. The results of this study were as follows: First, of all categories of caregiver burden, the score for burden of caring for elderly parents was 3.01 (SD = .81), slightly higher than the median of 3 points; childcare burden scored 3.16 (SD = .73), also higher than the median value; and family support resources scored 3.40 (.89), higher than the median of 3. Overall psychological well-being rated 3.25 (SD = .56). Second, health and educational level, the burdens of caring for elderly parents and childcare and family support resources were found to significantly affect psychological well-being. Third, family support resources, a moderating variable, were found to significantly moderate and ease the effect of childcare burden on middle-aged working mothers' psychological well-being. Furthermore, in the analysis of their moderating effect, family support resources were confirmed to positively affect psychological well-being by moderating childcare burden experienced by middle-aged working mothers.

Social Welfare Policy Expansion and Generational Equity: Generational Accounting Approach (복지지출 확대가 세대 간 형평성에 미치는 효과 분석: 세대 간 회계를 이용한 접근)

  • We study the sustainability of the current fiscal policy of Korea, and the effects of the social welfare policy expansion, which has been recently discussed among the political circles, on the government budget and the generational equity, using generational accounting. We follow the generational accounting approach, considering the fact that most of the social welfare policies are the entitlement programs, which imposes the limitation of the policy maker's discretion to control the cost of their provision. The social welfare expenditure will change due to the change in the policy environments of the future, such as population aging. Therefore, we need to take into account the government cash flow of the future as well as of the present to investigate its effects on the fiscal sustainability, which implies that the national debt or the budget balance is not a proper index for the investigation. Our findings are as follows. The current fiscal policies are not sustainable, and the long-term budgetary imbalance is shown very serious. The required tax adjustment, which is defined as the percentage change of tax burden required to attain the long-term budgetary balance, is very large. Unless the level of the government expenditure is properly controlled, the tax burden and the social contribution level will rise to the untolerable level. Moreover, the expansion of the social welfare policies, which has been discussed among the political circles, will substantially increase the fiscal burden of the future generations. Even though the provision of the free lunch to the primary and the secondary school students, the free child care, and the discounted college tuition do not increase the fiscal burden much, because their magnitude at present is not large and will decrease due to the decrease in the number of the newborns and the students resulting from the fall in the fertility rate, that of the free health care service will increase tax burden of the future generations very much, because the magnitude of the government expenditure needed at present is very large and the population aging will further increase the magnitude of the health care expenditure. The findings indicate that the structural reforms, to prevent the explosive increase in the social welfare expenditure in the future, are necessary before the implementation of the welfare policy expansion. In particular, the cost control of the social transfers to the elderly needs to be made, because the speed of the population aging of Korea is among the highest in the world. The findings also indicate that the budget balance or the national debt can cause the fiscal illusion, which makes the Korean government budget look sound, even though the fiscal policy will rapidly increase the social welfare expenditure in the future, as the population ages. The generational accounting, which takes into account the cash flow of the future as well as of the present, unlike the budgetary balance and the national debt, which shows the results of the government financial activities of the past and the present, is a useful method to overcome the fiscal illusion.

A Study on Permanency Planning for Adult with the Mental Disability (성인 정신장애인의 평생계획에 관한 연구)

  • When parents as primary care takers to the mentally disabled adult are no longer taking care of their care-needed offsprings because of their own death or illness, instead themselves. who take care of their offsprings with the mental disability? Therefore, 'permanency planning' is very important for reduction of parents' care burden and social integration of mentally disabled adults. Accordingly, this study aims to find out factors which are related to permanency planning for adults with the mentally disability For the purpose of the study, 192 parents of the adult with the mental illness and mental retardation were conducted a survey regarding type of permanency planning, and its related factors including social functioning level of the mentally disabled, care burden, parents' self-perception of being aged, help from offspring without mental disability, social support, and financial ability. Furthermore, this study examined correlation between these factors and residential planning. Results obtained by the study were as follows: 1) 51% of the parents are having a plan for institution and most parents want other family member to take care for financial planning for their mentally disabled offsprings. 2) As a result of multiple regression for finding out factors which affect parents' permanency planning, social functioning level of the mentally disabled, parents' self-perception of being aged, help from offspring without mental disability, social support, and financial ability were statistically significant influenced factors, which has 23.3% of explanatory power. 3) As a result of step-wise multiple regression, financial ability, parents' self-perception of being aged, and help from offspring without mental disability were the most powerful influenced factors for permanency planning. 4) In case of having a plan for residential types-which are institution and community living-, parents who have a plan for the mentally disabled offsprings' future residence as community living than institution have the offsprings with more social functioning and also have more help from offspring without mental disability. Therefore, this study concluded that welfare policy for mental health and the handicapped which secure various types of community living facilities and income security is strongly needed. At the same time, mental health profession is needed to have more active interest and intervention for permanency planning for their adult clients and parents.

  • The report of the Korean National Statistics Office shows that Korea has been emerging as an elderly society rapidly, and it will burden the Korean society with excessive social welfare cost for the aged in the near future. If we can help the aged to live healthy in some ways, the social burden for the health care of the aged will be lessened. In order to help physical and mental health of the elderly person, we have developed an exercise apparatus called intelligent arm wrestling machine system. This paper presents the mechanism and scenario of the proposed intelligent arm wrestling machine system. The proposed mechanism and scenario are peculiar. In particular, the proposed scenario determines randomly who will win between the man and the robot and generates a game process that the arm-wrestler cannot predict in advance.

Horticultural Therapy Programs Enhancing Quality of Life and Reducing Depression and Burden for Caregivers of Elderly with Dementia

  • Background and objective: The problem that follows the increase of dementia patients is the burden of caregivers caring for dementia patients. The purpose of this study was to examine the effects of horticultural therapy programs improving the quality of life and reducing the depression and burden of caregivers of the elderly with dementia. Methods: In this study, 19 caregivers of the elderly with dementia were selected, and the experiment was conducted by dividing the control group (n=9) and the experimental group (n=10) by random distribution. The experimental group was given eight horticultural therapy programs twice a week for a total of 4 weeks. Subjects were assessed using the depression(CES-D), quality of Life (WHOQOL-BREF), and care burden scales. The evaluation results were verified at a 95% significance level using descriptive statistics, the Mann-Whitney U test, and Wilcoxon signed-rank test. Results: In the case of depression, the control group's score tended to increase, and the experimental group's score appeared to decrease, but it was not a statistically significant change. In the quality of life, the control group was not statistically significant, but scores decreased overall. On the other hand, in the experimental group, the general quality of life increased significantly from 11.60 to 14.20 points (p = .02), and the total quality of life increased to a marginally significant level from 61.59 points to 68.85 points (p = .059). In the post-test of the total care burden score, a marginally significant difference was found between the control group (94.44 points) and the experimental group (82.50 points; p = .079). Conclusion: This study confirmed the applicability to reduce the burden of caregiving and improve the deterioration of quality of life of the caregivers. In particular, the results will serve as an opportunity to confirm accessibility in a new way to support the caregiver of dementia patients by demonstrating the applicability of horticultural therapy at a time when problems such as the burden of supporting the caregiver are emerging as social problems.

Who concedes seat to the old and the weak in subway? And Why?: Focused on inductive factors and inhibitory factors (지하철에서 누가, 왜 자리를 양보하는가?: 양보유발요인과 억제요인을 중심으로)

  • This study was to investigate two main purposes; The first was to explore the factors which have an effect on conceding seats to the old or the weak in subway(concessive action). The second was to examine the influence that the psychological burden and the responsibility had on concessive action and it's gender differences. The results of study 1 showed that affective empathy, morality, and negative-state relief were important inductive factors, while 3 factors were revealed as important inhibitory factors for the concessive action; psychological burden, repulsion for coercive demand for conceding seats and vagueness of concession situation. In study 2, We could find the fact that the psychological burden and the responsibility had a main effect on the concessive action. The more participants felt the psychological burden, they were likely to evaluate that the tendency to concessive action would be decreased. And the more participants felt the responsibility, they were inclined to evaluate that concessive action would be increased. Secondly, the interaction effect was also revealed: On the situation that the psychological burden was high and responsibility was low, participants judged that people tended not to concede seats. Finally, We were able to identify gender differences. Specifically, Judging the tendency to concessive action, women were more responsive to the psychological burden than men.
