  • Objective : The purpose of this study was to report the improvement of a patient with tinnitus treated by herb-formulae medication based on Shanghanlun provisions. Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, a patient was diagnosed with Taeyang-byung, 40th provision and was administered Socheongryong-tang herbal medication for 45 days. The numeric rating scale (NRS) was used to estimate the response. Results : The NRS score changed from 10 to 0 and the tinnitus disappeared by 40th provision of Socheongryong-tang selected according to Shanghanlun provisions. Conclusions : This case report suggests that the two words in the 40th provision of Shanghanlun, '表不解, 水氣'(exterior do not release, water qi) indicate a close relationship between economic difficulty and stuffiness of being in water, which affect tinnitus in this case.

  • BACKGROUND : Socheongryong-Tang(小靑龍湯, SCRT), a herbal medicine, has been widely used for the control of respiratory disease in Eastern countries. Recent years SCRT was known as anti-allergic agent. However, its therapeutic mechanisms, especially an inhibitory effect on inflammatory cell infiltration and airway remodeling in allergic asthma are unclear. METHODS : We investigated the effects of SCRT on levels of IL-4, and IFN-g using flow cytometric analysis and infiltration of inflammatory cells in bronchoalveolar lavage fluid(BALF) using cytospin methods in allergen-induced asthma. RESULTS : SCRT decreased levels of IL-4, a Th2-driven cytokine. But INF-g, a Th1-driven cytokine, was not changed. With SCRT treatment, infiltration of inflammatory cells showed reduced tendency. CONCLUSION : In conclusion, we demonstrated that regulation of Th1/Th2 imbalance may be one of mechanism contributed to reduction of inflammatory cell infiltration.

  • Objective : The purpose of this study was to investigate the changes in the contents of constituents in Socheongryong-tang (CY) and its fermentations (FCY) with 10 species of lactic acid bacteria. Methods : Ten strains of lactic acid bacteria, Lactobacillus casei 127, L. acidophilus 128, L. casei 129, L. plantarum 144, L. amylophilus 161, L. curvatus 166, L. delbruekil subsp. lactis 442, L. casei 693, B. breve 744, and B. thermophilum 748, were used for the fermentation of Socheongryong-tang. The increased and decreased constituents were identified using HPLC/DAD and various liquid chromatographic techniques, and the structure was elucidated using NMR and MS. These compounds were quantitatively analyzed using an HPLC/DAD system. Results : The increased constituents were identified to be liquiritigenin (1) and cinnamyl alcohol (2), and the decreased constituent was determined to be liquiritin (3). Liquiritigenin (1) and cinnamyl alcohol (2) were increased in all of the FCYs, while liquiritin (3) was decreased. The fermentation of the ten lactic acid bacteria demonstrated that the decomposable rate of these three compounds in FCYs were different. Socheongryong-tang fermented by L. plantarum 144 and L. amylophilus 161 showed the most remarkable changes. Conclusions : CY could be increased antibacterial, neuroprotective, or antiinflammatory effect by fermentation with lactic acid bacteria, especially with L. plantarum and L. amylophilus, considering their known biological activities. In addition, it is expected that this study will help to establish quality control parameters for FCY.

  • In the present study, the author intended to investigate whether two oriental medical prescriptions named socheongryong-tang(SCRT) and Kamichihyo-san(KCHS) significantly affect mucin release from cultured hamster tracheal surface epithelial(HTSE) cells. Confluent HTSE cells were metabolically radiolabeled with ³H-glucosamine for 24 hrs and chased for 30 min in the presence of SCRT or KCHS to assess the effect of each agent on ³H-mucin release. Possible cytotoxicities of each agent were assessed by measuring lactate dehydrogenase(LDH) release. Also, the effects of SCRT and KCHS on contractility of isolated tracheal smooth muscle were investigated. The results were as follows: (1) SCRT significantly inhibited mucin release from cultured HTSE cells, without cytotoxicity; (2) KCHS significantly increased mucin release without cytotoxicity; (3) SCRT and KCHS did not affect contractility of isolated tracheal smooth muscle. We suggest that the effects of SCRT and its components should be further investigated and it is of great value to find, from oriental medical prescriptions, novel agents which have the possible inhibitory effects on mucin release from the viewpoint of management of hypersecretion of airway mucus.

  • Objectives : Socheongryong-Tang(小靑龍湯, SCRT), a herbal remedy, has been widely used to treat respiratory disease such as cough and asthma in Oriental countries. Recent years SCRT was known as anti-allergic agent. However, its therapeutic mechanisms including immunoglobuline such as IgE, IgG1, IgG2a productions are unclear. Methods : We investigated the effects of SCRT on levels of antigen specific total antibody, IgE, IgG1, IgG2a using ELISA method in serum from allergen-induced asthma mice. Results : SCRT decreased level of antigen specific IgE significantly. And SCRT treated mice showed downward tendency of IgG1, a Th1 relative antibody, level. But, SCRT did not affect levels of antigen specific total antibody and IgG2a, a Th1 relative antibody. Conclusions : we demonstrated the strong possibility of SCRT as a complementary or alternative drug to western drug also demonstrated that regulation of Th1/Th2 imbalance may be one of mechanism contributed to treatment for respiratory disease by SCRT.

  • Socheongryong-tang(SCRT) is widely used to treat the common cold. The purpose of this study was to evaluate the efficacy of SCRT on Punghan type(風寒型) and Pungyeol type(風熱型) common cold. 98 registered students with commom colds of recent onset were randomized to the double blind, placebo-controlled study. SCRT extract in capsule, $5.4g(1.8g{\times}3cap)$, orally dissolved 3 times a day. The severity of illness was assessed by the physician, using a 5-point scale on start and finish. In Pungyeol type common cold, no statistically significant differences were detected between the SCRT and placebo groups for any of the measured outcomes. In Punghan type common cold, SCRT significantly reduced rhinorrhea (p=0.034) and nasal stuffiness (p=0.048) compared with placebo. This study shows that SCRT is effective in treating cold symptoms in Punghan type common cold. If patients with cold are able to be administered SCRT according to common cold type, the benefit would be expected to increase.

  • Objectives : In this study, the author tried to examine whether Gamijinhae-tang and Socheongryong-tang-ga-seokgo significantly affect both PMA-induced mucin production and MUC5AC gene expression from airway epithelial cells. Materials and Methods : Confluent NCI-H292 cells were pretreated for 30 min in the presence of JHT and STS and treated with PMA (10ng/ml), to assess the effects of the agents on PMA-induced mucin production by enzyme-linked immunosorbent assay (ELISA). Also, the effects of the agents on PMA-induced MUC5AC gene expression from the same cells were investigated. Possible cytotoxicities of the agent were assessed by examining the rate of survival and proliferation of NCI-H292 cells after treatment of agents during 48 hrs. Results : (1) JHT and STS did not show significant cytotoxicity to NCI-H292 cells. (2) JHT significantly decreased PMA-induced mucin production from NCI-H292 cells. However. STS did not affect mucin production. (3) JHT significantly inhibit the expression levels of PMA-induced MUC5AC gene in NCI-H292 cells. STS slightly decreased the expression levels of PMA-induced MUC5AC gene. Conclusion : These results suggest that JHT can not only affect the production of mucin but also affect the expression of the mucin gene, and this explains the traditional use of JHT in oriental medicine. The effects of JHT and STS with their components should be further investigated using animal experimental models that simulate pathophysiology of airway diseases through future studies.

Of the 51 herbal formulae, 13 herbal formulae with antimicrobial activity were selected and their dose-dependency were confirmed. Results: Nine herbal formulae, including Gyeji-tang, Dangguisu-san, Saengmaek-san, Samul-tang, Ssanghwa-tang, Socheongryong-tang, Yukmijihwang-tang, Jakyakgamcho-tang, and Paljung-san, presented antibacterial activity against B. cereus. The effects of Saengmaek-san and Paljung-san was sustained for 48 hr. On L. monocytogenes, Dangguisu-san and Hyangsapyeongwi-san showed antimicrobial activity, but only Hyangsapyeongwi-san maintained the activity for 48 hr. Thirteen herbal formulae such as Galgeun-tang, Gyeji-tang, Dangguisu-san, Mahwang-tang, Banhasasim-tang, Saengmaek-san, Socheongryong-tang, Yukmijihwang-tang, Jakyakgamcho-tang, Cheonwangbosim-dan, Palmijihwang-tang, Paljung-san, and Hwanglyeonhaedok-tang showed antimicrobial activity against V. parahaemolyticus, and the activity was maintained for 48 hr. The 51 herbal formulae did not show any antimicrobial activity against seven strains such as E. coli DH5α, E. coli O157, S. aureus, S. enteritidis, Y. enterocolitica, S. flexneri, and H. pylori. Conclusions: Nine, two, and thirteen herbal formulae showed antimicrobial activities against B. cereus, L. monocytogenes, and V. parahaemolyticus in a dose-dependent manner, respectively. The results of antimicrobial activity of 51 herbal formulae against 10 microorganisms might be used as the basis for new application of herbal formulae.
