The Effects of Self-Efficacy Promoting Cardiac Rehabilitation Program on Self- Efficacy, Health Behavior, and Quality of Life

  • Background. Ischemic heart disease results from athesclerotic changes of the coronary artery. These changes are aggravated by hypercholesterolemia, smoking, obesity, lack of exercise, coronary-prone personality, and stress. Because these risk factors affect not only the prevalence of the ischemic heart disease but also recurrence of the disease, cardiac rehabilitation programs were introduced to help patients with ischemic heart disease reduce risk factors. Diverse cardiac rehabilitation programs are needed to motivate participation in cardiac rehabilitation and to enhance patients' quality of life. Objectives. To examine the effect of a self-efficacy promoting cardiac rehabiltation program on self-efficacy, health behavior and quality of life of patients with ischemic heart disease. Methods. Data were collected from 45 hospitalized ischemic heart disease patients. Medical records were reviewed to obtain demographic and clinical characteristics. Data regarding self-efficacy, health behavior, and quality of life were obtained from interviews using structured questionnaires. The nonequivalent control group non-synchronized design was used to conduct this study. One session of conventional group education was given to patients in the control group while they were in the hospital. Patients in the experimental group participated in a newly developed cardiac rehabilitation program. It focused on strengthening self-efficacy with four self-efficacy sources-performance accomplishment, vicarious experiences, verbal persuasion and physical status using two individualized in-hospital education sessions and four weekly telephone counseling follow-up calls after discharge. Results. Four weeks after discharge, the increment of total self-efficacy score was significantly higher in the experimental group than in the control group (p<.0l). There was also a significant difference in the total quality of life scores increments between the two groups (p<.0l). However, no significant changes were noted in the increments of total health behavior scores between the two groups. Conclusion. A cardiac rehabilitation program focusing on promoting self-efficacy was effective in improving self-efficacy, and quality of life of patients with ischemic heart disease.

Relationship of tooth mortality and implant treatment in Type 2 diabetes mellitus patients in Korean adults

  • PURPOSE. The purpose of this study was to to analyze the effect of Type 2 diabetes on tooth mortality, implant treatment and prosthetic status. MATERIALS AND METHODS. 275 Type 2 diabetics and 300 non-diabetics, aged 40-80 years were selected for analysis. The assessment of number of teeth, missing teeth, fixed prostheses (bridge pontics), implants using panoramic radiographs and dental records were carried out. RESULTS. Diabetes mellitus (DM) patients had a higher number of missing teeth (P<.05) and placed implants (P=.074), age (P<.05), male gender percentage (P=.042), smoker percentage (P<.05) than non-DM patients. In univariate analysis, the patients in older group showed significantly higher number of tooth loss rate at the first dental examination than the patients in younger group. Tooth loss rate of smokers did not show higher value than that of non-smokers. When multiple variables including DM, age, smoking, gender were considered together, diabetics and older group patients showed significantly higher tooth loss rate at the first dental examination than non-diabetics and younger group patients, respectively. Smokers and male group did not show a significant difference than nonsmokers and female group, respectively. CONCLUSION. Tooth mortality and implant treatment rate were significantly higher in the DM group as indicated by univariate and multivariate logistic regression analysis. Old age groups showed significantly higher odds ratios and tooth loss rate. As diabetics showed the higher tooth loss rate than non-diabetics, diabetics also had more implant restorations than non-diabetics.

Comparisons of obesity indicators for obesity and chronic diseases management among older adults (중·고령인구에서 비만과 만성질환 관리를 위한 비만 측정 지표 비교)

  • Objectives: This study was to examine the associations between obesity level and major chronic diseases in older population in Korea, using different obesity indicators. Methods: Data was from the 2008 KloSA Biomarker pilot, a nationwide sample of 514 non-institutionalized subjects (age $63.6{\pm}9.8$ years; women 57.2%). Anthropometric information was collected by home visiting nurses. Portable bioimpedence devices(Omran HBF359) were used for measuring fat mass. Obesity cut-offs used the Asian criteria of $BMI{\geq}25$, $WC{\geq}90/85cm$, and $fat%{\geq}25/35$ for men and women. Chronic disease prevalence was defined by physician-diagnosed history of 8 specified diseases. Results: Prevalence of chronic diseases significantly increased with increment of obesity level by BMI, WC and fat %. Odds ratios of acquiring chronic diseases remained significant in the obese group (BMI OR 2.76, 95%CI 1.82-4.19; WC OR 2.73, 95%CI 1.81-4.11; Fat OR 1.87 95%CI 1.26-2.78), after adjusting for age, sex, marital status, education, work participation, household income, smoking, drinking, and exercise. Conclusions: Obesity measured by all three indicators, BMI, WC, and fat mass cannot be disregarded, accentuating the significant effect on increases in disease risks among older population. BMI and WC measure appeared better to assess the risks of chronic diseases.

The Effects of Visiting Nursing Intervention on Self Care Behavior and Blood Pressure in Persons with High Risk of Cerebrovascular Diseases (방문간호 중재가 뇌혈관질환 고위험자의 자가간호행위 및 혈압에 미치는 효과)

  • Purpose: This study was to evaluate the effects of visiting nursing intervention on self care behavior and blood pressure with high risk of cerebrovascular diseases. Method: A one-group pretest-posttest was designed. Data collection was done in 150 participants from March. 2003 to November, 2003 at a public health center. Self care behavior was assessed by the scores of smoking, alcohol, diet, exercise, stress and medication compliance. The scores of knowledge related cerebrovascular diseases, blood pressure, blood glucose were estimated. The Participants were intervened in with basic assessment, emotional support, education. This program took a period ranging from 3 months to 7 months depending on the cerebrovascular risk level. The mean number of visiting times was 1.67 per month. Result: 1. Knowledge level was improved(t=-2.542, p= .012). 2. Systolic and diastolic pressure were lower(t=5.439, p<.001; t=4.966, p<.001) 3. Self care level was higher (t=-12.981, p=.001) after the intervention. Conclusion: The visiting nursing intervention was found to have an effect on the scores of knowledge, self care behavior and blood pressure with high risk of cerebrovascular diseases. The results provided evidences for the importance of visiting nursing intervention in the high risk factor of cerebrovascular disease for self care.

The effect of dental health education on dental health knowledge, dental health behavior of adult group (구강보건교육 프로그램이 성인집단의 구강보건지식 및 구강건강행위에 미치는 영향)

  • This study was conducted to evaluated the effects of improvement on dental health behavior and dental health knowledge of adult group through dental health education program, the study subjects were 75, education group being 37(male 19, female 18) and non-education group being 38(male 19, female 19). who being in 2 office. The results of this study were as follow: 1. In the primary survey, the education group and non-education group showed no differences in sex, age, job year, income, smoking, self-rated dental health statues, DMFT index, a number of cervical abrasion tooth, dental health behavior, dental health knowledge. 2. before education program the use of tooth brushing method of horizontal+vertical was 40.6%, after education program the use of tooth brushing method of rolling was 89.2% in education group. 3. after education program the tooth brushing times was increased after-meal brushing, especially increased from 40.5% to 93.8% at after midday meal brushing. 4. the difference of mean change of dental health knowledge score after oral health education program had been studied. dental health knowledge score increased in 9.8 in the education group and non-education group in 1.6(pE0.001). 5. the difference of mean change of oral health promotion behavior after oral health education program had been studied, the frequence of tooth brushing(pE0.05), flossing(pE0.001), tongue brushing(pE0.001) was significantly increased in education group compare to non-education group. Above findings suggest that dental health education program was effective in improving the dental health behavior, dental health knowledge of the adult group.

Effect of perceived stress on general health and oral health status in elderly: results from the Korea national health and nutrition examination survey 2014 (노인의 스트레스 인지가 전반적인 건강상태 및 구강건강상태에 미치는 영향)

  • Objectives: This study was conducted to identify the rates of perceived stress of elderly people over 65 years old and to confirm the influence of stress on general health and oral health status. Methods: Using data from the National Health and Nutrition Survey for 2014, 1,472 people over 65 years of age were selected as final subjects. Stress was used as an independent variable and dependent variables were included physical health (perceived health status), mental health (depression), and oral health (perceived oral health status). The following confounding variables were adjusted for the current study: demographic characteristics (gender, age, education level, house income) and health - related characteristics (drinking, smoking, exercise, frequency of tooth brushing, using oral care product, dental exam, comorbidity, restrict activity). Complex sampling analysis was applied and logistic regression was performed to determine the effects of stress on physical health, mental health and oral health status. Odds ratio (OR) and 95% confidence interval (95% Confidence Interval, 95% CI) were calculated. Results: Logistic regression indicated that stress was significantly associated with low physical health (OR=2.18, 95%CI: 1.49-3.20), low mental health (OR=8.68, 95%CI: 4.98-15.11), low oral health (OR=1.53, 95%CI: 1.06-2.21) after adjusting for confounding variables. Conclusions: The perceived stress of the elderly was found to be related to the general health and oral health status. Therefore, it is necessary to evaluate stress as a predictor of health risk for the health promotion of the elderly on multidisciplinary assessment and continuous evaluation. In addition, health support policies should be provided to achieve good health status for elderly.

The Effect of Thoracoscopic Pleurodesis in Primary Spontaneous Pneumothorax: Apical Parietal Pleurectomy versus Pleural Abrasion

  • Background: The standard operative treatment of primary spontaneous pneumothorax (PSP) is thoracoscopic wedge resection, but necessity of pleurodesis still remains controversial. Nevertheless, pleural procedure after wedge resection such as pleurodesis has been performed in some patients who need an extremely low recurrence rate. Materials and Methods: From January 2000 to July 2010, 207 patients who had undergone thoracoscopic wedge resection and pleurodesis were enrolled in this study. All patients were divided into two groups according to the methods of pleurodesis; apical parietal pleurectomy (group A) and pleural abrasion (group B). The recurrence after surgery had been checked by reviewing medical record through follow-up in ambulatory care clinic or calling to the patients, directly until January 2011. Results: Of the 207 patients, the recurrence rate of group A and B was 9.1% and 12.8%, respectively and there was a significant difference (p=0.01, Cox's proportional hazard model). There was no significant difference in age, gender, smoking status, and body mass index between two groups. Conclusion: This study suggests that the risk of recurrence after surgery in PSP is significantly low in patients who underwent thoracoscopic wedge resection with parietal pleurectomy than pleural abrasion.

Supplement Dose and Health-Related Life Style of Vitamin-Mineral Supplement User among Korean Middle-Aged (중년기의 비타민ㆍ무기질 보충제 사용량과 건강관련 생활습관 조사)

  • We studied daily micronutrient intake from vitamin-mineral supplements, health-related life style, clinical case of diseases and food frequency of the Korean middle-aged (40-59 yr, n = 404) to compare the characteristics of non-user (n = 270) and user (n = 134) of vitamin-mineral supplements. Rate of supplement use of the middle-aged was 33.2% and there was significant difference in education level (p = 0.0084) and family income (p = 0.0476) of user and nonuser. Smoking habit (p = 0.0844) and drinking frequency (p = 0.0606) tended to be lower in a supplement user than a non-user. The medical history of a case was significantly higher in users (67.9%) than in non-users (44.4%) (p = 0.001), which suggests that medical history is one of the important motivations of supplement use. Supplement users had the medical history of digestive disease (34.1%), anemia (11.0%) and hypertension (9.9%) in order. Vitamin C was the most frequently supplemented nutrient (81.3%) among vitamin-mineral supplement, and the next orders were vitamins E (73.1%), B$_2$(68.7%) and B$_{6}$ (60.4%). Mean intakes of vitamin B$_1$, iron, selenium, vitamin E, and vitamin C from supplement was 4,260%, 4,030%, 1,660% and 1,330% of RDA, respectively. The supplement users tended to consume most food items including milk & milk products (p < 0.01), rice (p < 0.01), grains (p < 0.05) and cookies (p < 0.01) less frequently than non-users. Conclusively, nutrient intake of vitamin B$_1$, iron, selenium, vitamin E, and vitamin C from supplement was excessively high compared to RDA. We suggest that the toxic effect of excessive supplementation should he informed to supplement user and nutritional education should be focused on the optimal supplement dose.e.

A Study of Dietary Life Related Factor according to the Acculturation Degree on Chinese Students in Korea (재한 중국인 유학생의 문화적응 정도에 따른 식생활 관련 요인 분석)

  • This study was performed to provide fundamental data on the dietary life according to the acculturation degree. The subject was 305 Chinese students in South Korea region. The questionnaire respondents are consisted of 148 male students (48.5%) and 157 female students (51.5%). There was a statistically significant difference in age, education level, residence time, and Korean language ability according to acculturation degree (p<0.05), but there was no statistically significant difference in gender, residence pattern, purpose of coming to Korea. There was a statistically significant difference between meals frequency, outside frequency, and the intake of Chinese food according to acculturation degree (p<0.05), but there was no significant difference in snake taking frequency (p>0.05). There was a statistically significant difference in drinks and computer time according to acculturation degree (p<0.05), but there was no statistically significant difference between smoking and exercise (p>0.05). The food intake style of Chinese students was 2.47 in noodles, 2.34 in lunches and 2.15 in breads. According to the acculturation degree, the food intake patterns showed statistically significant differences in dumping kind, congee, hamburger and pizza, while meat products, smoked meat, noodle, lunch, cereal, kimbap, sandwich. And there was no significant difference. The correlation between the factors influencing the acculturation degree of Chinese students showed a statistically significant effect on dietary habits, food intake, education level, residence period, and Korean language ability.

The Effect Common Health Disorders of Foreign Workers - Focused on Daegu and Kyungpook Region - (외국인 근로자들의 일상적인 건강장애에 미치는 영향 - 대구경북지역을 중심으로 -)

  • This study seeks to promote the mental and physical efficiencies of the foreign workers working in our country through the promotion of their health, and provide help in their effective use of medical treatments and granting appropriate medical benefits, and provide the basic information helpful in managing the health of the foreign workers who are playing a role in the advancement of the industry in Korea for the companies and employers that are employing them by researching the health condition and management status centering on the foreign workers from a part of Asia and South America who are working in our country. I made a regression analysis toward 231 foreign workers who have employed in industrial workplace from Mar. 1 to Jun. 30, 2008. The result said the factors that affect significant impact on common health disorder of foreign workers in their daily circumstances were staying period in Korea($\beta$=.270),difficulty to access hospital($\beta$=.268), inferior sanitation of work place($\beta$=.218), smoking or not($\beta$=.170), actual condition of health maintenance($\beta$=-.251).($R^2$=0.501)
