국내학술지에 발표된 금연중재논문(1980-2007) 분석 (An Analysis of the Research on Effect of Smoking Cessation Intervention)

  • Purpose: We reviewed all the articles on effect of smoking cessation intervention to find out the way of improvement of it. Method: 48 researches were published in Korean journals from 1980 to 2007. The research were analyzed according to: major of first author, subjects, application of theoretical framework, contents of intervention, a term of intervention, the time of evaluation on effect of interventions, research design, dependant variables, and effect of interventions. Results: Research on effect of smoking cessation intervention increased rapidly in the 2000's. At this time. 72.9% of research was published. Most of research subject were students. Theoretical framework were applied in 20.8% of articles. The most common intervention was education(39.4%). The effect of combined program on outcome variable was found to be stronger than only education program. Experimental design was used in 29.2% and quasi-experimental design was adapted in 43.8% of articles. 1- 6months intervention was in 35.4%. The long term intervention more than 1month was found to be effective. Health behavior related index was measured as a dependant variable in 62.3%. conclusion: Accordingly, long term program to apply to various subject is needed.

Quitline Activity in the Republic of Korea

  • To reduce tobacco use and related harm in Korea, telephone based cessation services (Quitlines) began full operation to provide regular behavioral counseling for smoking cessation in 2006. After registration in the cessation program, at least 21 calls per year are given to each client to help quit and encourage maintenance. Tailored programs for males, females, and adolescent smokers have been offered taking into account smokers' characteristics and smoking behavior. Mailing self-help quit packs and e-mail and SMS services are allowable as additional services.A total of 23,201 smokers were registered on the Quitline program from 2006 to 2014. In 2014, an average of 13,343 calls per month have been received by 28 coaches, the 1 year abstinence rate of clients is 26%, and clients' satisfaction rate is 81.6%. After introduction of the call system in 2007, client convenience and effective operations have been achieved with high technology support of a computer-based telephone system. Systematic education and evaluation programs for quit coaches have contributed to quality assurance of the services. Currently, research into development of new programs and evaluation of Quitline performance is being undertaken. A Comprehensive Multi-channel Cessation Center (CMCC) has been suggested and is now planned as a next step in the national program for smoking cessation.

금연프로그램 효과분석 논문의 현황 및 방향 (A Study on Papers of Smoking Cessation Program)

  • The purpose of this study is to analyze papers of smoking cessation program into method and effect 13 papers that purblished from January, 1980 to Oct., 2001 in the country and 30 papers that purblished from Jan., 1980 to Jun., 1999 in other countries were analized. The result of this study can be summarized as follows 1) The most of subjects were students(46.5%) and residents(30.2%). 2) There were a few of papers based on the Theory(9.3%). 3) Programs included education(58.3%), reward (13.9%) and support(12.5%). 4) There were experimental design(46.5%), non-experimental design(29.5%) and experimental design(25.0%). The most of papers of non-experimental desigs were significant. 5) Behavior(48.7%), knowledge(15.4%) and attitude(12.8%) were evaluated as a effect of program. Knowledge had higher significant rate than other evaluation items. 6) Programs that was from a year to two years had the highest significant rate.

Attitude and Practices Among Dentists and Senior Dental Students in Iran Toward Tobacco Cessation as an Effort to Prevent Oral Cancer

  • Background: Oral health professionals are responsible in Iran for providing a brief tobacco cessation program to smoker patients. The aim of this study was to assess Iranian dental student and dentist practice, knowledge and attitudes toward smoking cessation programs. Materials and Methods: A valid and reliable self-administered questionnaire was designed and distributed to 150 dentists working in Isfahan-Iran and 60 dental students. Some questions were developed based on the expected 5A tobacco cessation protocol. Statements on attitudes focused on professional responsibility towards smoking cessation and its effectiveness. Chi-square, ANOVA, and t test were used for statistical analysis. Results: The cessation program in dental settings covers a small group of patients (18%). Some 69.1% (n=96) of dentists reported asking their patients about tobacco use, 64% (n=83) advising their patients to quit, 33.8% (n=47) assessing their patients willingness to quit and 20% (n=28) reported helping their patients in changing their behavior. A far lower percentage reported active involvement in arranging assistance for smokers to quit (4.3%, n=5). Some 22% of students and 26% of dentists disagreed that the tobacco cessation programs should be as part of dentists' professional responsibility and 70% of them were willing to follow the protocol of tobacco cessation for patients. Conclusions: Iranian dentist performance regarding tobacco cessation is weak. Dentists and students indicated their lack of knowledge as the major reason for non-adherence to the protocol. Therefore, planning to encourage dentist to follow the protocol needs continuous educational programs.

Influences of Tobacco-Related Knowledge on Awareness and Behavior towards Smoking

  • Background: A considerable amount of research has shown that knowledge and appropriate awareness are essential for encouraging positive behaviors and promoting health. In Korea, the roles that behavioral changes play in the prevention of cancer have been an important issue since the introduction of the 10 codes for cancer prevention in 2006. Thus, the present study investigated the associations of tobacco-related knowledge with awareness and attitudes towards positive smoking-cessation behaviors. Methods: The present study analyzed data from the 2010 national questionnaire survey (n = 1,006). This study evaluated sociodemographic characteristics, smoking status, self-rated health status, health-related interests, and the accuracy of 12 tobacco-related statements to determine knowledge level and to investigate its impact on awareness and behaviors related to smoking. These parameters were examined and staged using the Precaution Adoption Process Model. Results: A higher level of tobacco-related knowledge was significantly associated with a positive attitude towards smoking cessation (5-8 correct answers: odds ratio [OR], 2.53; 95% confidence interval [CI], 1.57-4.08; ${\geq}9$ correct answers: OR, 3.90; 95% CI, 2.22-6.82; reference: ${\leq}4$ correct answers). Interestingly, among current smokers, only those who correctly responded to ${\geq}9$ of 12 tobacco-related statements were significantly associated with a positive attitude towards smoking cessation. Conclusion: This study found that having a higher level of tobacco-related knowledge had a significant impact on positive attitudes towards smoking cessation. This suggests that there is a need to disseminate appropriate knowledge to the general population to encourage positive attitudes and promote healthful behaviors in terms of smoking.

HIV 감염인을 위한 융합금연프로그램 앱 개발 : 사용성 평가 (Development of the Smoking Cessation Convergence Program App for HIV Infected Persons : Usability Evaluation)

  • 본 연구는 인간면역결핍바이러스(HIV) 감염인의 금연을 위한 융합프로그램 앱을 개발하고 그 사용성을 평가한 방법론적 연구이다. 본 프로그램은 계획된 행위이론, HIV 감염 및 금연분야의 전문가팀에 의해 개발된 후 휴리스틱 평가와 사용자 평가로 구성된 사용성 평가를 통해 최종 완성되었다. 휴리스틱 평가는 구조화된 설문지를 사용하여 세 명의 모바일 전문가에 의해 시행되었고, 확인된 문제는 개선되었다. 사용자 평가는 HIV 감염인 10명을 대상으로 설문조사와 인터뷰를 통해 시행되었고, 수집된 자료는 기술통계 및 내용분석을 통해 분석하였다. 모든 참가자는 남성이었고 연령은 40대에서 60대였으며 앱 유용성에 대한 평가는 4.2로 나타났다. 대부분의 참가자는 프로그램을 사용하는 데 어려움을 겪지 않았으며 만족스럽다고 응답하였다. 본 연구결과는 HIV 감염인의 금연을 위해 개발된 이론기반 융합프로그램 앱이 HIV 감염인에게 사용가능하고 유용하며 HIV 감염인의 금연을 위한 타당한 전략으로 사용될 수 있음을 보여주었다.

금연과 관련된 개인의 특성, 경험, 그리고 인지·감정요인들이 금연행동 계획수립 및 실천에 미치는 영향 분석 -Pender의 개정된 건강증진 모형 적용- (Analysis for Influences of Individual Characteristics, Experience, Cognition, and Affect Relating to Smoking Quitting Behavior on Commitment to a Plan of and Practice for Smoking Quitting Behavior)

  • This study was conducted to examine if the revised HPM was appropriate to explain smoking quitting behavior. A convenience sample of 400 college students enrolled in the universities located in Seoul and Kyunggi-Inchon province was selected. According to the study results, smoking-specific cognitions and affect included in the study could significantly explain commitment to a plan to quit smoking which was one of the behavioral outcomes in the revised HPM. The study result showed that among predictors, smoking-related affect, perceived benefit of quitting smoking, and perceived self-efficacy significantly explained commitment to a plan to quit smoking, but perceived barriers and interpersonal influences did not. The model for smoking quitting behavior formulated with smoking-specific cognitions and affect was statistically significant and the model explained 48 percent of variance in smoking quitting behavior. More specifically, it was shown that among smoking-specific cognitions and affect, only smoking-related affect, interpersonal influences, and perceived self-efficacy were the significant predictors to explain smoking quitting behavior. Smoking-related affect was the most important variable to explain smoking quitting behavior, followed by perceived self-efficacy. However, the influence of commitment to a plan to quit smoking on smoking quitting behavior was not statistically significant. Lastly, the model for smoking quitting behavior formulated with individual characteristics and experiences could explain 13 percent of the variance in behavior to quit smoking. Although the model is statistically significant, only the number of quitting attempts had significant and direct influence on behavior to quit smoking, while the remaining variables did not.

궐련 흡연 청소년과 전자담배를 중복 사용하는 흡연 청소년의 금연시도 영향요인 비교 (Comparison of the Factors for Attempts to Quit Smoking by Adolescent using Tobacco Cigarettes only and those Adolescents using Tobacco Cigarettes Together with Electronic Cigarettes)

  • Purpose: This study was conducted to compare and analyze the smoking-related social-ecological factors affecting attempts to quit smoking by adolescents using tobacco cigarettes only and also those adolescents using tobacco cigarettes along with electronic cigarettes. Methods: This study, as secondary analysis research, used the raw data from the 14th Korea youth risk behavior survey 2018. The data was analyzed by frequency analysis, the Rao-scott χ2-test and logistic regression analysis when considering the complex sample's analysis. Results: On logistic regression analysis, during their first smoking period, intense physical activities and having friends who smoked were associated with significantly more attempts to quit smoking by cigarettes smokers, and their first smoking period and experiences of undergoing smoking cessation education were associated with significantly more attempts to quit smoking for dual smokers of both tobacco cigarettes and electronic ones. Conclusion: As a result of this study, we identified the need to differentiate different types of smoking cessation counseling and education according to the types of smoking in adolescents.

  • Health education aims at behavior change rather than just delivering health knowledge to people. In Korea health education activities in public sector began in 1960 and they were included in the primary prevention program in communities. This article reviewed current health education programs in healthy living practice programs provided by local public health centers in Korea and drew implications for the future role of health education in community setting. Health education has been a core function of the National Health Promotion programs in the nation since the enactment of the National Health Promotion Law in 1995. The National Health Promotion programs are funded by the National Health Promotion Fund which are drawn from tobacco tax. The National Health Promotion programs include healthy living practice programs (smoking prevention and cessation programs, moderate alcohol use programs, physical activity promotion programs, and nutrition programs), chronic disease prevention programs, oral health programs and public hygiene programs. Methods of the National Health Promotion programs include health education, health counseling, health class, health information management, survey and research. Smoking prevention and cessation programs include smoking cessation clinic, smoking cessation education, non-smoking environment program, and non-smoking campaign. Moderate alcohol use programs include alcohol use education, moderate alcohol use campaign, alcohol use counseling, and alcohol free environment programs. Physical activity promotion programs include obesity control, targeted exercise program, and exercise civic group programs. Nutrition programs include nutrition management, obesity management, nutrition education, breakfast eating program, and nutrition counseling and treatment programs. The health education programs in community are not efficient today because there are many overlapping contents and short term goals. Community health education programs needs to be more comprehensive. Workforce development is another big issue at the moment because the National credential program will begin in 2009. Variety of community health education programs should be developed and funded by the national health promotion fund.

급성관상동맥증후군 남성 환자의 흡연과 스트레스에 영향을 미치는 요인 (Predicting Factors of Smoking and Emotional Stress among Male Patients with Acute Coronary Syndrome)

  • Purpose: This study was conducted to identify the factors that predict a current smoking behavior and higher emotional stress among male patients with acute coronary syndrome (ACS). Methods: The study was approved by an institutional review board from a university hospital, 2010. A face to face interview using questionnaires was performed with 185 first-time ACS male patients who were undertaken a percutaneous coronary intervention at a cardiovascular care unit. Data were analyzed using SPSS/WIN 15.0. Results: About 54% of the study subjects were currently smoking. The current smokers had dyslipidemia and reported bad eating habits compared to the non-smokers. The current smokers were younger, living alone, and reported lower perceived benefit on smoking cessation than the non-smokers, and 15% of them did not consider quitting (precontemplation stage). Smoking status was not significantly related to emotional stress. Logistic regression analysis revealed that being employed including professional or labor increased the odds of current smoking four or three times compared to the non-employed or retired. Low income or dyslipidemia also increased the likelihood of current smoking 2.8 and 2.1 times, respectively. Blue collar workers or heavy drinkers had 2.9 and 2.8 times more risks of having higher level of stress. Conclusion: An occupational background and health habits should be considered to develop an effective educational strategy for smoking cessation and stress reduction among male patients with ACS.
