한방건강증진HUB보건소사업 실태분석 (An Analysis on Actual Condition of Health Promotion Program through Oriental Medicine in Health Center)

  • This study was carried out to examine the actual condition of health promotion program through oriental medicine in the health center and to provide basic data to develop proper policy of oriental medical health promotion program for the community people. The data were collected from 26 health centers which have been implementing the oriental medical health promotion program, using selfadministered questionnaire for two weeks from 1 October to 15 October 2006. The results are as follows : Generally, the respondents have the positive views on the level of budget and facilities/equipments of the oriental medical health promotion program in health center. However, they have the negative views on the level of manpower and education/training of the program. And also more than 70% of the respondents have the negative opinion on capabilities of formulating and evaluating the oriental medical health promotion program. The respondents indicated that there was the lack of coordination between the oriental medical health promotion program and existing health promotion in health center, and that low rate of utilizing community resources. With regard to the method of selecting the target group for the program, there are differences according to the each program. Many programs tended to select the target group not through the criteria of life-course and illness group but through the efficiency of selecting group. And many programs such as stroke prevention program, constitutional medicine program, oriental medical prenatal program, oriental medical prenatal and postnatal program, oriental medical child care program are mainly composed of the development of educational program and lecture. Regarding the number of the present oriental medical health promotion programs, around 65% of respondents answered that the number of the programs was many and thus they needed to decrease to the proper level. And with regard to the priority of the need, effectiveness and the satisfaction for each programs, on the whole, Qui gong program, stroke prevention program, area-specialty program and oriental medical home visiting program have high score. In particular, oriental medical quit-smoking program has lowest score. From these results, it requires to develop and improve the oriental medical health promotion program in health center considering the need and characteristics of community.

일부 시멘트 산업근로자의 요통 관련 요인 및 요통 지식에 관한 연구 (A study on Related Factor and Knowledge of Back Pain in Workers Employed at Cement factory)

  • 본 연구는 시멘트 사업장 근로자들의 요통 관련 요인과 지식수준을 파악하기 위해 2013년 11월 1일부터 2014년 1월 31까지 충북 C 지역에 소재한 시멘트업에 종사하는 근로자 82명을 대상으로 설문 조사 한 결과 다음과 같다. 근로자의 요통 경험율은 56.1%였으며, 관련 인자는 대도시와 읍면지역에서 성장한 응답자와 업무 스트레스가 많고, 업무환경에 만족하지 않을수록 요통에 대한 정보가 없을수록 요통발생이 높았고, 요통을 경험한 응답자는 흡연을 하다 담배를 끊고, 음주도 줄이는 것으로 나타났고 향후 요통관련 교육을 받을 의향이 있는 것으로 나타났다. 또한 근로자의 요통관련 지식수준은 서로 양의 상관관계를 갖고 100점 환산 점수로 56.29로 낮은 수준을 보였고, 특히 자세와 관련된 요통지식이 50점으로 가장 낮았다. 요통경험과 교육에 따른 지식수준은 향후 요통 교육을 받을 의향이 있는 자에서 운동 관련 요통 지식 점수가 유의하게 높게 나타났으며, 요통 지식정보를 얻지 못한 응답자는 모든 지식수준이 낮은 점수를 보였고 유의한 차이가 있었다. 따라서 요통관련 산업 현장의 적극적인 관심과 더불어 요통관련 보건 교육 프로그램을 통해 요통관련 지식수준을 높임으로 효율적인 치유와 예방이 이루어지도록 해야 할 것이다.

일부(一部) 생산직(生産職) 근로자(勤勞者)들의 건강관련(健康關聯) 삶의 질(質)과 양생수준(養生水準)과의 관계(關係) (The Relationship between Health-related Quality of Life and Yangseng Level among Blue-Collar Workers)

  • Objective : This research was done to discover whether or not workers' health related quality of life(HRQOL) depends on their own Yangseng(養生) level, which is also known as one's care of one's health, and also if their HRQOL is affected by the level, to what extent. The subjects of the study were blue-collar workers of a workplace. Method : Blue-collar workers of a workplace were asked to fill out their pre-organized questionaires given to them by their company as a process of health examination. The questionaires carried questions regarding their levels of Yangseng and their HRQOL. For the purpose of the research, a total of 961 data were selected from the questionaires filled out by the blue-collar workers and then analyzed. Result : Each workers's level of Yangseng becomes high or low by one or all of general characteristics. With regard to Yangseng level according to health-related lifestyle were found to have a relatively higher level of Yangseng. With reference to HRQOL according to general characteristics, those in the 20s were found to have a physical summary scale(PCS), but no significant difference was found in the other scales. As regards the quality of life(QOL) according to health-related lifestyle, those who exercise regularly were found to have a relatively higher PCS, while no smokers, those who do not drink alcoholic, and those who sleep at least seven hours a day were found to have a relatively higher mental summary scale(MCS). As to the relation between the level of Yangseng and HRQOL, the level of Yangseng was found to have an interrelation with PCS and MCS in terms of its degree. Statistics also show that each area of Yangseng level has a significant impact on the two qualities-PCS and MCS. Conclusion : One who has a higher level of Yangseng was found to be the one who enjoys a higher HRQOL. Specially, the three kinds of habits that are good for good health -non-smoking, no drinking and seeping seven hours a day- were found to be able to improve the QOL. In this respect, those who want to stay healthy are recommended to cultivate a healthier habit of living.

모바일 개인건강기록(Personal Health Records: PHR) 어플리케이션의 이용이 소비자 건강행태에 미치는 영향 (The Effects of a Mobile Personal Health Records (PHR) Application on Consumer Health Behavior)

  • 본 연구는 모바일 개인건강기록(Personal health record: PHR) 어플리케이션의 장단점을 분석하고 그 어플리케이션의 사용이 소비자 건강정보행태에 미치는 영향을 파악하고자 하였다. 본 연구를 위해 총 27명의 대학생들이 3개월 동안 PHR 어플리케이션을 사용하였으며, 그 경험을 바탕으로 서면인터뷰를 실시하였다. 내용분석 결과, 시각적 인터페이스를 통한 예방 차원의 건강관리 및 건강 향상을 위한 동기부여와 구체적 가이드라인의 제공, 가족과의 데이터 공유 및 간병인(caregiver)의 환자 건강관리지원, 무엇보다도 환자와 의료전문가와의 상호작용 강화 등이 PHR의 혜택으로 부각되었다. 반면 근력운동과 같은 콘텐츠의 미비 내지는 타 기기와의 호환성 미흡 등이 문제점으로 지적되었다. PHR의 도움으로 건강을 향상시키고자 하는 소비자들은 감정적 지원을 받는가 하면, 수면장애극복, 금주, 금연, 그리고 체중감량 및 식습관의 변화 등 건강행태를 변화시키고자 하는 동기부여가 생겼으며, 실제로 건강행태의 변화를 성취함으로써 자기효능이 증가하기도 하였다. 본 연구는 국내 연구에서는 도입단계인 PHR에 대한 연구를 시도했다는 것에 학문적 의의가 있으며, 헬스케어분야의 패러다임 변화에 대응하기 위한 건강 및 의료정보서비스를 모색하는데 통찰력 있는 시사점을 제공한다.

Association of urinary 3-phenoxybenzoic acid levels with self-reported depression symptoms in a rural elderly population in Asan, South Korea

  • Objectives This study aimed to evaluate the association between presence of depression symptoms and the exposure level to insecticides among aged population in rural area, determined via measured levels of urinary 3-phenoxybenzoic acid (3-PBA), after controlling for socioeconomic confounding factors. Methods Using a cross-sectional study design, we randomly recruited participants for our study (161 male and 239 female) from rural areas of Asan, Chungnam, Korea. Environmental risk factor exposure was assessed using a questionnaire, and gas chromatography-mass spectrometry was used to analyze urinary 3-PBA levels. We used a logistic regression analysis to assess the association of urinary 3-PBA levels with the presence of self-reported depression symptoms. Results After controlling for creatinine levels, the median (interquartile range) concentration of 3-PBA was approximately 1.5 times (p<0.05) higher among female (1.54 [0.90 to 2.35]) ${\mu}g/g$) than among male (1.06 [0.64 to 1.81] ${\mu}g/g$). Our study found that among female participants, the unit increase in 3-PBA levels exhibited a likely positive association (odds ratio, 1.12; 95% confidence interval, 1.00 to 1.25) with an increased risk of presence of self-reported depression symptoms, after adjusting for socioeconomic insurance type, daily physical condition, marital status, smoking status, and age. Conclusions Given our finding of a potential association between the presence of self-reported depression symptoms and 3-PBA levels, precautions should be considered to minimize exposure to insecticides and thus protect the health of aged residents in rural areas.

Todai Health Index를 이용한 항공기 승무원의 건강상태에 관한 연구 (A Study of the Health Status of Flight Attendants by Todai Health Index)

  • In order to evaluate the physical and mental health status of airline flight attendants, 136 airline flight attendants were given a general health questionnaire (Todai Health Index), and the prevalence of their subjective complaints was measured. Collected data were classified according to age distribution, length of employment, service area, smoking status, job satisfaction, gender, and marital status. The results obtained were summarized as follows: 1 The scores of health complaints of physical items were higher in the oldest group $(age 50\~60)$ than in younger groups. The THI eye and skin symptom scores were significantly higher for the oldest groups than for the younger groups. 2. The health complaints scores of physical items were higher in the long-length employment group (more than 5 years), whereas most scores of mental items were higher in the short- length employment group (less than 2 years). The THI mouth and anus scores for the long-length employment group were significantly higher than for groups of workers who had been employed for a shorter time. 3. THI scores were higher for domestic airline crews, dissatisfied workers and females. 4. The irregular life THI score was significantly higher among domestic crews than international crews. 5. The impulsiveness and depression scores were significantly higher in the dissatisfaction group. 6. The female group showed higher scores in the multiple subjective symptoms, mental irritability, depression, and irregular life categories. 7, The THI scores of the mouth, anus and nervousness were significantly higher for the married group than for the unmarried group. In summary, this study shows that the health complaints scores of physical and mental symptoms tended to be higher among the aged group, domestic airline crews, the dissatisfaction group, and females. These results can be used for improving the psychosomatic health status and working environments of flight attendants.

Association between Low-level Mercury Exposure and Neurobehavioral Functions in Korean Adults Living in a Coastal City

  • Objectives The aim of this study was to determine the association between low-level mercury exposure and neurobehavioral functions in adults living in coastal regions of Korea. Methods We selected 172 adults aged 20-65 years living in a city in the coastal region of Korea. A sociodemographic survey was conducted, mercury levels in the blood, urine, and hair were measured, and the associations according to computerized neurobehavioral tests were determined using univariate analysis. After adjustment for associated variables, a multivariate linear regression analysis was performed. Results The geometric mean mercury levels in the blood, urine, and hair were $5.41{\mu}g/L$ (range, $0.00-15.84{\mu}g/L$), $1.17{\mu}g/g$-creatinine (range, $0.00-32.86{\mu}g/g$-creatinine), and 1.37 mg/kg (range, 0.42-6.56 mg/kg), respectively. Variables that were associated with simple reaction time according to the neurobehavioral test results were age and urine mercury level. Variables associated with choice reaction time were the recent use of Korean traditional medicine and urine mercury level. Variables associated with the right-hand finger tapping speed test were age, gender, smoking behavior, education level, monthly household income, and urine mercury level. Variables associated with the left-hand finger tapping speed test were age, gender, education level, and urine mercury level. After adjustment for associated variables, there was no significant association between urine mercury level and simple reaction time (${\beta}=25.96$; p =0.47), choice reaction time (${\beta}=50.37$; p =0.32), or the number of left-hand finger taps (${\beta}=-1.54$; p =0.21). However, urine mercury level was significantly associated with the number of right-hand finger taps (${\beta}=-3.86$; p =0.01). Conclusions We found no evidence that low-level mercury exposure in adults is associated with deficits in neurobehavioral functions. A longer follow-up study is required to confirm this conclusion.

제주지역 성인의 Apolipoprotein E Phenotype 분포와 식생활 및 혈청지질 농도의 관련인자 연구 (The Effects of Dietary Patterns and Apolipoprotein E Phenotype on the Blood Lipid Profiles of Individuals from Cheju Area)

  • The purpose of this study was to determine the relation between serum lipid profiles, apolipoprotein E phenotype, and dietary patterns in a cross-section of healthy individuals from Cheju-Do. Age, gender, anthropometric measurements, blood pressure, dietary consumption, drinking / smoking habits and menopausal status were surveyed. Total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride, fasting blood glucose, and insulin levels were measured from overnight fasting blood. The study involved a total of 286 individuals(147 men and 139 women) between the ages of 20 and 60 years old. All of the subjects were recruited from a population of healthy individuals living in Cheju-Do. The results of the study are as follows : 1) Among the males, those in their 20's had the maximum food intake, while those in their 40's had the minimum food intake. For the females, food intake was the highest for those in their 30's. Energy and nutrient intakes were directly proportional to the amount of food intake. Men in their 30's were heavier than other men and women in their 40's were heavier than other women. The activity index for men in their 20's and 30's appeared to be lower than that of men above 40. The activity index of women in their 20's appeared to be lowest among all aged groups, and the index appeared to increase from the age of 30 onwards. 2) In terms of changes In serum constituents with age, men in their 40's appeared to have the highest levels of serum constituents such as lipids, glucose, and insulin. Men in their 50's showed the highest levels of serum LDL-cholesterol and glucose. Men in their 30's showed peak levels of serum triglycerides. On the other hand, women in their 50's appeared to have peak levels of serum total cholesterol, LDL-cholesterol, and triglycerides. There was no ch:ange with age in HDL-cholesterol and insulin levels for men and women. The percentage of the subjects had the following apo E phenotypes : E3/3, 91.3% ; E3/2, 5.4% ; E4/3, 2.5% ; E4/2, 0.7%. Lee's reserch with Korean female college students showed that the percentage of ApoE3/3, E3/2, E 4/2, E4/3, and E4/4 were 84.8%, 6.7%, 6.7%, 0.9%, 0.9%, respectively. The number of samples with ApoE mutation was so small that there was no statistical significance in the relation between apolipoprotein E phenotype and se겨m lipids. 3) To investigate the relati onship between weight and serum constituents, the subjects of this study were divided into three groups by BMI underweight, normal weight, and overweight. The serum constituents of men and women below the age 40 in the overweight groups belonged to the normal domain. On the other hand, serum cholesterol levels of both men and women above the age 40 in the overweight groups remained in the borderline-high region(above 200mg/dl), and the mean value of LDL-cholesterol(above 130mg/dl) and triglycerides of men were above normal. Fasting blood glucose levels also remained in the borderline-high region. Total cholesterol levels of women above the age 40 in the overweight group was in the borderline-high region. 혈중 납 농도와 역학적 요인 및 체내 철 수준과의 관련성 (The Relation between Blood Lead Concentration, Epidemiologyic Factors and Body Iron Status)

  • Essential metals have been known to interact with non-essential toxic metals in the aspects of absorption, transport and deposition in the body. Iron deficiency has been reported to increase lead and/or cadmium absorption. The relation between iron and lead has been understood well in children but not in adults. Two hundred seventy adults (118 males and 152 females) were recruited from 3 different residental areas (rural, coastal and urban) to investigate the effects of environmental lead exposure on body iron status. The subjects were interviewed for life-style and diet of the last 24 hours, and measured for blood lead and body iron. The lead concentration in the whole blood was determined by a flameless method using an atomic absorption spectrophotometry. The body iron was evaluated with values of hemoglobin, hematocrit, RBCs, serum total iron, unsaturated iron binding capacity, total iron binding capacity and ferritin. The mean concentration of blood lead in adult was $3.31{\mu}g/dL$. The concentration was higher in male ($3.97{\mu}g/dL$) than in female ($2.86{\mu}g/dL$). The blood lead was influenced by residental area, life-style, smoking and drinking, occupation and diet habit of subjects, but not by age. A positive correlation was observed between the blood lead level and the serum iron or ferritin. These results suggest that environmental lead exposure in Korean adult may not be higher than other developed and developing countries. It is further indicated that blood lead in adult could be influenced by life-style, and environmental and genetic factors but no inverse relation with body iron as shown in children.

과거의 직업적 납 노출에 의한 혈중납 및 골중납의 증가가 남성 골밀도 변화에 미치는 영향 (Effect of increased blood and tibia lead on the change of bone mineral density in retired male lead workers)

  • This study was designed to investigate the effect of increased blood and tibia lead on the change of bone mineral density in retired male lead workers. One hundred nine retired male lead workers who worked in 4 different lead industries and 51 nonoccupationally lead exposed male subjects were recruited from March 2004 to October 2004. Bone mineral density(BMD) was measured by broadband ultrasound attenuation(BUA) at left calcaneous bone area with broadband ultrasound attenuation method of QUS-2(Metra Biosystems Inc, USA). Tibia bone lead was measured for skeletal bone lead with K-xray fluorescence(K-XRF) and blood lead was analyzed with flameless atomic spectrophotometer. Hemoglobin, hematocrit, serum calcium and iron were also analyzed. In addition, information for smoking and drinking status and basic personal data such as age and lead exposure were also collected using questionnaire inquiry. Blood lead was correlated with tibia lead (r=0.711) and these two variables were negatively correlated with BUA in bivariate analysis. BUA and tibia lead showed significant main effects on the change of blood lead after adjusting covariates. The effect modification by the level of BMD (low: lower than the median of BUA and high: higher than the median of BUA) was observed between the association of tibia lead and blood lead after adjustment of covariates. The subjects who had higher BMD seemed to have lower blood lead by the increase of tibia lead than those of lower BMD. In the multiple regression analysis of blood lead and tibia lead on BUA after adjustment of covariates, only blood lead showed statistically significant effect on BUA. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.
