The evaluation of implementing smart patient controlled analgesic pump with a different infusion rate for different time duration on postoperative pain management

  • Background: Control of postoperative pain is an important aspect of postoperative patient management. Among the methods of postoperative pain control, patient-controlled analgesia (PCA) has been the most commonly used. This study tested the convenience and safety of a PCA method in which the dose adjusted according to time. Methods: This study included 100 patients who had previously undergone orthognathic surgery, discectomy, or total hip arthroplasty, and wished to control their postoperative pain through PCA. In the test group (n = 50), the rate of infusion was changed over time, while in the control group (n = 50), drugs were administered at a fixed rate. Patients' pain scores on the visual analogue scale, number of rescue analgesic infusions, side effects, and patients' satisfaction with analgesia were compared between the two groups. Results: The patients and controls were matched for age, gender, height, weight, and body mass index. No significant difference in the mount of drug administered was found between the test and control groups at 0-24 h after the operation; however, a significant difference was observed at 24-48 h after the operation between the two groups. No difference was found in the postoperative pain score, number of side effects, and patient satisfaction between the two groups. Conclusions: Patient-controlled anesthesia administered at changing rates of infusion has similar numbers of side effects as infusion performed at a fixed rate; however, the former allows for efficient and safe management of postoperative pain even in small doses.

  • This paper is for the a study on home healthcare system for a patient using SAW Tag. This system is useful when there's a patient in home. Many patients are wounded in restroom. But restroom is very private place and nobody want to be disturbed here. The place where the patient is most injured in the house is the toilet, and it is the point where the privacy of the individual should be kept and the patient's condition should be checked and cope with in case of emergency. And using saw tag, we can calculate position and ID through reflected rf signal. Therefore we propose a general home healthcare system. The management system utilized a seat bed system to monitor at-home patients. The system mounts a bio-signal measuring unit on a toilet seat, measures a bio-signal from the bio-signal measuring unit, and the measured bio-signal is transmitted to the main server through the user's smart phone. With proposed system, we can expand operation area to silver town where many patients are cared. In this paper we developed sensor system and saw tag positioning system and showed the result.

The Usefulness of a Wearable Smart Insole for Gait and Balance Analyses After Surgery for Adult Degenerative Scoliosis: Immediate and Delayed Effects (척추측만증 환자의 수술 효과 평가 수단으로서 웨어러블 스마트 깔창을 이용한 보행분석의 유용성)

  • This study presents a gait analysis method (including time series analysis) using a smart insole as an objective and quantitative evaluating method after lumbar scoliosis surgery. The participant is a degenerative lumbar scoliosis patient. She took 3-min-gait-test four times(before and 8, 16, and 204-days after surgery) and 6-min-gait-test once(204-days after surgery) with smart-insoles in her shoes. Each insole has 8-pressure sensors, an accelerometer, and a gyroscope. The measured values were used to compare the characteristics of gait before and after surgery. The analysis showed that all of the patient's gait parameters improved after surgery. And after 6 months, the gait was more stable. However, after long walk, the swing duration of one leg was slightly shorter than that of the other again. It was a preclinical problem that could not be found in the visual examination by the practitioner. With this analysis method we could evaluate the improvement of patient quantitatively and objectively. And we could find a preclinical problem. This analysis method will lead to the studies that define and distinguish gait patterns of certain diseases, helping to determine appropriate treatments.

  • Recently medical facilities that can help to heal the interpretation have evolved as a healing space. Accordingly, the patient's emotional, psychological and social ward plan to meet the needs of the patient to recover is important. The medical team is also related to the nature of human life, so they are experiencing high stress levels compared to other professionals. However, most places don't consider the user's psychological behavior. The purpose of this study is to build user-friendly lighting environment using LED lighting. LED lighting is easy to adjust illuminance and color temperature, and it is can be implemented in the medical area.

  • If a human is taken with a disease, the electric resistance of the diseased part is higher than the surrounding area. The inherent current of the human body does not flow well in the diseased part due to high electric resistance. In this paper, we simulated the process to calculate the exact time of electronic acupuncture suitable for patient's physical condition using fuzzy logic and inference. Moreover, In this paper, It utilizes fuzzy logic and fuzzy inference rule to estimate the proper treatment duration for each patient. Physical condition, related disease, and age effects are studied for electronic acupuncture.

  • In this study, an end-effector robot which is a two-axis delta robot type for upper-limb rehabilitation is designed. It is not only rehabilitation functions that has designed robot but also mechanical and electrical safety devices were constructed to ensure patient safety. By constructing the two-axis delta robot is combined with an LM guide, the operating range and rigidity required for rehabilitation were secured. The electrical safety system which is required for the medical robot was designed, and a safety strategy was established to ensure patient safety and it is applied in the integrated safety circuit. The safety is considered in whole design process from the robot's mechanical design to the electric control unit.

Nurses' Perception of the Performance and Necessity of Nursing Services for Patients Engagement (간호사의 환자참여 간호서비스 수행정도와 필요성 인식)

  • Purpose: This study aimed to investigate the performance of patient engagement nursing services perceived by nurses and necessity in Korea. Methods: This study was a descriptive research. A total of 205 nurses participated in the study. The Smart Patient Engagement Assessment Checklist was developed by the investigators to assess patient engagement nursing services performance and necessity. The data were collected using online survey. Descriptive analysis and $x^2$ analysis were performed using SPSS 25.0 program. Results: The mean age of participants was $36.6{\pm}8.5years$ and the mean working experience was $12.92{\pm}9.23years$. Seventy eight percent of participants reported that patients and family participated in care as advisors through customer's suggestion or patient satisfaction assessment. The rate of patients' and family's engagement in care as advisors was significantly higher in tertiary hospitals ($x^2=28.54$, p<.001). About 89% of participants communicated with patients and family to make clinical decisions with a multidisciplinary approach. The rate of communication for multidisciplinary decision making was significantly higher in tertiary hospitals ($x^2=6.30$, p=.012). With regards to nurses' bedside patient handoff, 22.0% of participants reported that they were performing bedside patient handoff, and there was no significant difference between type of hospitals. About discharge planning, 72.2% of participants reported utilizing discharge checklist. Conclusion: Currently, patient engagement nursing services are applied partially in Korea. It seems that care protocols to be applied for patient engagement nursing services are insufficient. Therefore, patient engagement care protocols need to be developed to improve patient's health outcome and safety.

A Study on Medical Information Platform Based on Big Data Processing and Edge Computing for Supporting Automatic Authentication in Emergency Situations (응급상황에서 자동인증지원을 위한 빅데이터 처리 및 에지컴퓨팅 기반의 의료정보플랫폼 연구)

  • Recently, with the development of smart technology, in medical information platform, patient's biometric data is measured in real time and accumulated into database, and it is possible to determine the patient's emergency situations. Medical staff can easily access patient information after simple authentication using a mobile terminal. However, in accessing medical information using the mobile terminal, it is necessary to study authentication in consideration of the patient situations and mobile terminal. In this paper, we studied on medical information platforms based on big data processing and edge computing for supporting automatic authentication in emergency situations. The automatic authentication system that we had studied is an authentication system that simultaneously performs user authentication and mobile terminal authentication in emergency situations, and grants upper-level access rights to certified medical staff and mobile terminal. Big data processing and analysis techniques were applied to the proposed platform in order to determine emergency situations in consideration of patient conditions such as high blood pressure and diabetes. To quickly determine the patient's emergency situations, edge computing was placed in front of the medical information server so that the edge computing determine patient's situations instead of the medical information server. The medical information server derived emergency situation decision values using the input patient's information and accumulated biometric data, and transmit them to the edge computing to determine patient-customized emergency situation. In conclusion, the proposed medical information platform considers the patient's conditions and determine quick emergency situations through big data processing and edge computing, and enables rapid authentication in emergency situations through automatic authentication, and protects patient's information by granting access rights according to the patient situations and the role of the medical staff.

  • These days it is becoming more and more common to find electronic medical screening systems installed in Oriental hospitals and clinics. This is a relatively new development for the practice of traditional Oriental medicine. Specifically, Pulse detection machines are being utilized in order to help determine a patient's disease scientifically. However, identifying and diagnosing the specific disease correctly for each patient is still very difficult in Oriental medicine. The intention of this paper is to propose a solution which uses two separate Electronic systems working together to produce a better likelihood of finding the correct diagnosis for each patient. It is proposed that an EMR intelligent electronic chart system be developed and employed, which would utilize both Pulse wave system and a tongue detection system at the same time, in order to solve the problem. Computer simulation results have proven to show that EMR systems used in hospitals and clinics are more efficient and yield a more accurate diagnosis than traditional methods.

  • It is used the important method that Oriental doctor determines patient's disease status observing patient's state of tongue in Oriental medicine clinic. In this paper, it developed the how to use the pulse diagnosis and tongue diagnosis based on s mart based electronic acupuncture. It will do objective judgment without wrong diagnosis. In this paper, we developed the algorithm that it automatically determines patient health condition and smart electronic acupuncture kit using fuzzy logic and fuzzy reasoning system were completed. In this paper, Simulation results proved that acupuncture is effective than the traditional method of using electronic intelligence.
