A Case of Ceftriaxone Associated Biliary Sludge (Ceftriaxone과 연관된 소아의 담도 오니(biliary sludge) 1례)

  • Ceftriaxone, an effective third generation cephalosporin with a wide range of antimicrobial activity, has become widely used by pediatricians for a variety of bacterial infections including meningitis. It has been associated with the development of sludge or stone in the gallbladder of some patients treated with this drug. Ceftriaxone associated biliary sludge has unusual acoustic characteristics and resembles gallstone. The sludge can cause symptoms such as cramping abdominal pain, and disappears after stopping ceftriaxone administration. Because of these seemingly confusing observations, it is important for the clinicians to recognize these findings that ceftriaxone treatment can cause. We report a case of ceftriaxone associated biliary sludge in Korean children. A 6-year-old girl who was treated for meningitis with ceftriaxone (100mg/kg/day) developed cramping upper abdominal pain from 5th hospital day. Physical examination, liver function tests and X-ray revealed no specific abnormal findings. But abdominal ultrasound revealed high amplitude echogenic sludge with prominent post-acoustic shadow in gallbladder and its diameter was 1.5cm. We stopped ceftriaxone administration and tried conservative care. Abdominal cramping pain subsided after 3 days of ceftriaxone removal. Second abdominal ultrasound confirmed the disappearance of sludge at 3 weeks later.

Feasibility Studies on Anaerobic Sequencing Batch Retractor for Sludge Treatment

  • Digestion of a municipal wastewater sludge by the anaerobic sequencing batch reactor (ASBR) was investigated to evaluate the performance of the ASBR process at a critical condition of high-solids-content fined. The reactors were operated at an HRT of 10 days with an equivalent loading rate of 0.8-1.5 gVS/L/d at 35$^{\circ}C$ The main conclusions drawn from this study were as follows: 1. Digestion of a municipal wastewater sludge was possible using the ASBR in spite of high concentration of settleable solids in the sludge. The ASBRS with 3- and 4-day cycle period showed almost identical high digestion performances. 2. No adverse effect on digestion stability was observed In the ASBRS in spite of withdrawal and replenishment of 30% or 40% of liquid contents. A conventional anaerobic digester could be easily converted to the ASBR without any stability problem. 3. Flotation thickening occurred in thicken step of the ASBRS throughout steady state, and floating bed volume at the end of thicken period occupied about 70% of the working volume of the reactor Efficiency of flotation thickening in the ASBRS could be comparable to that of additional gravity thickening of a completely mixed digester. 4. Solids were accumulated rapidly in the ASBR during start-up period. Solids concentrations in the ASBRS were 2.6 times higher than that in the completely mixed control reactor at steady state. Dehydrogenase activity had a strong correlation with the solids concentration. Dehydrogenase activity of the digested flu형e in the ASBR was 2.9 times higher than that of the flu형e in the control reactor, and about 25 times higher than that of the subnatant in the ASBR. 5. Remarkable increase in equivalent gas production of 52% was observed at the ASBRS compared with the control reactor in spite of similar quality of clarified effluent from the ASBRS and control reactor. The increase in gas production from the ASBRS was believed to be combined results of accumulation of microorganisms, higher driving force applied, and additional long-term degradation of organics continuously accumulated.

Operation of UASB Reactor for Treatment of Dairy Wastewaters (유가공폐수 처리를 위한 UASB 반응조 운전)

  • The performance and the operational problems of UASB(Upflow Anaerobic Sludge Blanket) reactor for treatment of dairy wastewaters were investigated. Synthetic milk wastewater was successfully treated up to the loading rate of 3.9kg $COD/m^3.day$, with a specific gas production rate of 1. 23 I/I. day and a COD removal efficiency of over 90%. However, the sludge rising was observed at the loading rate of 2.1kg $COD/m^3.day$, due probably to the formation of scum layer at the surface of settling compartment. The BMP(biochemical methane potential) of raw milk wastewater and ice cream wastewater, measured by using SBT(serum bottle test), were 0.135 and 0.66ml $CH_4/mg\;COD_{added}$, respectively. The sludge activity increased more than 8 times from 0.159g $COD-CH_4/g$ VSS. d during 90 days of operation.

Preparation and Characterisation of Titanium Dioxide Produced from Ti-salt Flocculated Sludge in Water Treatment (수처리 티탄염 응집 슬러지에서 생산한 산화티탄의 제조와 특성 조사)

  • During the past few years, titanium salts were investigated as alternative coagulants for the removal of organic matter of different molecular sizes in contaminated water. The flocculation efficiency of Ti-salt was comparable to those of $FeCl_3$ and $Al_2(SO_4)_3$ salts, commonly used coagulants. Incinerated sludge-$TiO_2$ showed higher surface area and photocatalytic activity than commercially available $TiO_2$. Metal-doped forms were produced by adding coagulant aids such as iron (Fe-), aluminium (Al-) and (Ca-) calcium salts during Ti-salt flocculation to increase pH. Ca- and Al- doped $TiO_2$ showed very high photocatalytic activity compared to Fe-doped $TiO_2$. When tested in a pilot scale plant for treatment of dye wastewater to check practical feasibility of the novel process, the removal ratio of the chemical oxygen demand was comparable to those of commonly used coagulants but the settling of sludge was faster. The $TiO_2$ generated after sludge incineration showed a high photocatalytic activity for degradation of volatile organic compounds and increased the rate of hydrogen production by water photosplitting. $TiCl_4$ coagulant and $TiO_2$ produced from different water sources with different concentrations had low acute toxicity compared to heavy metals and commercial $TiO_2$ when examined based on D. Magna mortality. This paper presents the production, characterisation and the photoactivity of $TiO_2$ produced from Ti-salt flocculated sludge. Different case studies are discussed to highlighted recent advances in this field.

Selective Inhibition of Ammonia Oxidation and Nitrite Oxidation Linked to $N_2O$ Emission with Activated Sludge and Enriched Nitrifiers

  • Nitrification in wastewater treatment emits a significant amount of nitrous oxide ($N_2O$), which is one of the major greenhouse gases. However, the actual mechanism or metabolic pathway is still largely unknown. Selective nitrification inhibitors were used to determine the nitrification steps responsible for $N_2O$ emission with activated sludge and enriched nitrifiers. Allylthiourea (86 ${\mu}M$) completely inhibited ammonia oxidation and $N_2O$ emission both in activated sludge and enriched nitrifiers. Sodium azide (24 ${\mu}M$) selectively inhibited nitrite oxidation and it led to more $N_2O$ emission than the control experiment both in activated sludge and enriched nitrifiers. The inhibition tests showed that $N_2O$ emission was mainly related to the activity of ammonia oxidizers in aerobic condition, and the inhibition of ammonia monooxygenase completely blocked $N_2O$ emission. On the other hand, $N_2O$ emission increased significantly as the nitrogen flux from nitrite to nitrate was blocked by the selective inhibition of nitrite oxidation.

Inhibition Mechanism of Ammonia Nitrogen on the Granules in an Upflow Anaerobic Sludge Blanket Reactor (암모니아성 질소 첨가에 따른 상향류 혐기성 블랭킷 반응조내 입상슬러지의 저해 기작)

  • The upflow anaerobic sludge blanket (UASB) reactor can be effective for treating simple organic compounds containing high concentration of ammonia nitrogen. The chemical oxygen demand (COD) removal efficiency was about 80% at ammonia nitrogen concentration up to 6,000 mg-N/L. This result also showed that it would be possible to treat propionate effectively at free ammonia nitrogen concentration up to 724 mg-N/L if sufficient time was allowed for adaptation. However the specific methanogenic activity (SMA) of granule was lower than that of granule in the reactor with lower ammonia nitrogen concentration. At 8,000 mg-N/L, the inhibition of high ammonia concentration was observed with evidence of increase of the volatile suspended solids (VSS) concentration in the effluent. It might be ascribed to the decrease in the content of extracellular polymer (ECP), which resulted to the sloughing off of obligated proton-reducing acetogens and heterogenotrophic methanogens from the exterior of granular sludge. This caused a great portion of the finely sludge to be easily washed out. Therefore, failure to maintain the balance between these two groups of microorganism cause accumulation of the hydrogen partial pressure in the reactor, which could have inhibited the growth of acetate utilizing methanogens.

Effects of Reactor Configuration on Upflow Anaerobic Sludge Digestion (반응조 형상이 상향류 혐기성 슬러지 소화에 미치는 영향)

  • Digestion of primary sludge was conducted to evaluate the effects of reactor configuration using UAD, CUAD, TPAD, and semi-continuos CSTR. Highest VSS reduction and gas production were obtained in CUAD at all HRT. More efficient digestion was accomplished in upflow digesters compared to TPAD and CSTR. Higher thickening of solids in reactor and longer solids retention were main reasons for the enhanced digestion in CUAD and UAD. Performance based on the SRT of CUAD was nearly identical to that of UAD. However, those of TPAD and CSTR were lower than that of CUAD. Particulate and soluble organics in upflow reactors were well adsorbed due to secreted extracellular polymeric substances from the sludge granules. These might result in close proximity of microorganisms and substrates and enhanced hydrolysis. Additionally, diverse anaerobic microorganisms and neutral pH in upflow reactor could induce more activity of hydrolytic enzymes and sludge granules might offer lower thermodynamic energy state. While, excessive mixing in CSTR could break conglomerates of enzymes and substrates into fine particles, which resulted in lowered hydrolysis. Low pH level in acid fermenter of TPAD lowered hydrolysis of the particulate substrates.

Relationship between Toxicity of Heavy Metals and Sludge Retention Time in Sequencing Batch Reactor Process (연속회분식반응조 공정에서 슬러지 체류시간과 중금속 독성의 관계)

  • In order to elucidate the relationship between the sludge retention time(SRT) and the toxicity of heavy metals, such as copper (Cu), cadmium(Cd), and zinc(Zn), in sequencing batch reactor(SBR) process, IC50 was estimated with measuring of INT-dehydrogenase activity in variable SRTs. When the concentrations of heavy metals were increased, the activity of INT-dehydrogenase was gradually decreased indicating the heavy metals inhibit bacterial activity. Cu showed higher toxicity than Zn and Cd. $IC_{50}$ of Cu, Cd, and Zn ranged from $0.37\sim1.96$ mg/L, $15.4\sim16.9$ mg/L, and $9.70\sim23.4$ mg/L, respectively. The toxicity of Cu and Zn was reversely proportional to the length of SRT. It is probably caused by the increased concentration of extracellular polymeric substances in longer SRT which absorb heavy metals. Therefore, the operation of SBR with increased SRT is desirable in treatment of industrial wastewater containing heavy metals.

A study on an intermittent aeration membrane bioreactor system using ammonia sensor to decrease energy consumption and sludge concentration by tubular membrane (암모니아 센서를 이용한 간헐폭기 Membrane bioreactor공정에서의 전력비 저감과 관형막을 이용한 슬러지 농축에 관한 연구)

  • It is essential to decrease energy consumption and excess sludge to economically operate sewage treatment plant. This becomes more important along with a ban on sea dumping and exhaustion of resource. Therefore, many researchers have been study on energy consumption reduction and strategies for minimization of excess sludge production from the activated sludge process. The aeration cost account for a high proportion of maintenance cost because sufficient air is necessary to keep nitrifying bacteria activity of which the oxygen affinity is inferior to that of heterotrophic bacteria. Also, additional costs are incurred to stabilize excess sludge and decrease the volume of sludge. There were anoxic, aerobic, membrane, deairation and concentration zone in this MBR process. Continuous aeration was provided to prevent membrane fouling in membrane zone and intermittent aeration was provided in aerobic zone through ammonia sensor. So, there was the minimum oxygen to remove $NH_4-N$ below limited quantity that could be eliminated in membrane zone. As the result of this control, energy consumption of aeration system declined by between 10.4 % and 19.1 %. Besides, we could maintain high MLSS concentration in concentration zone and this induced the microorganisms to be in starved condition. Consequentially, the amount of excess sludge decrease by about 15 %.

Anaerobic Ammonium Oxidation Process in an Upflow Anaerobic Sludge Blanket Reactor with Granular Sludge Selected from an Anaerobic Digestor

  • The purpose of this work was to evaluate the development of the anammox process by the use of granular sludge selected from a digestion reactor as a potential seed source in a lab-scale UASB (upflow anaerobic sludge blanket) reactor system. The reactor was operated for approximately 11 months and was fed by synthetic wastewater. After 200 days of feeding with $NH_4^+\;and\;NO_2^-$ as the main substrates, the biomass showed steady signs of ammonium consumption, resulting in over 60% of ammonium nitrogen removal. This report aims to present the results and to more closely examine what occurs after the onset of anammox activity, while the previous work described the start-up experiment and the presence of anammox bacteria in the enriched community using the fluorescence in situ hybridization (FISH) technique. By the last month of operation, the consumed $NO_2^--N/NH_4^+-N$ ratio in the UASB reactor was close to 1.32, the stoichiometric ratio of the anammox reaction. The obtained results from the influent-shutdown test suggested that nitrite concentration would be one key parameter that promotes the anammox reaction during the start-up enrichment of anammox bacteria from granular sludge. During the study period, the sludge color gradually changed from black to red-brownish.
