성인여성의 피로와 수면장애에 관한 연구 (A Study on Women's Fatigue and Sleeping Disturbance)

  • Purpose: This study was to identify the relationship between fatigue and sleep disturbance in adult women so as to provide basic data for developing a women's health promotion program and nursing intervention. Method: A convenience sample of 184 women who visited a Well-baby clinic at P city Maternal child center during October in 2004 was obtained. Data was collected by self-administered questionnaires. Results: The Subjects' mean age was 36.87 years and 70% were employed and had more education than high school. The mean level of fatigue was 56.65, mean sleeping hours were 6.54, and time until falling asleep was 17.58 minutes. Scores of fatigue differed by subject's age, level of education, and menstruation status. In addition, sleeping hours differed by age, occupation status, and income level. There were significant low levels of correlation between fatigue and sleeping hours (r=-.16) and getting up too early (r=-.15). Conclusion: This study concludes that fatigue and sleeping patterns have a significant relationship.

입원환자의 수면 방해 요인이 수면의 질에 미치는 영향 (Factors Influencing Quality of Sleep of Disturbance Factors Sleep by Inpatients)

  • Purpose. The purpose of this study was to identify the factors affecting quality of sleep and sleep disturbance among inpatient. Methods. A descriptive correlational study was conducted Participants were 200 inpatient in a hospital located in B city. A structured questionnaire was used for data collection. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation, and stepwise multiple regression. Result. Hospitalized patient's score of sleeping quality was 38.26 on an overall scale of 60, indicating it to be higher than 'Medium (30 points). Almost hospitalized patient in this study suffer from sleep disturbances. As results of univariate analyses, Educational level, Departments, Why hospitalized, Insomnia / depression, Chronic Disease, Scale of pain, physical symptoms, environmental factors, emotional factors, and sleep promoting behavior, disturbed sleep among hospitalized. However, the result of stepwise multiple regression analysis identified that physical symptoms, environmental factors, emotional factors, sleep promoting behavior, and Why hospitalized disturbed sleep and were significant score of sleeping quality for hospitalized and these sleep disturbance factors accounted 46.8% of variance of sleeping quality among hospitalized. Conclusion. These findings suggest that hospitalized patients with poor sleep quality should have their health carefully screened for physical symptoms, environmental factors, emotional factors, sleep promoting behavior, and why hospitalized. In addition, we recommend the development of a nursing program for improving sleep quality.

남성 사무직 근로자들의 스트레스와 관련된 사회인구학적 특성과 생활습관 (Stress-related Socio-demographic Factors and Life Style on Male White Collar Workers)

  • This study was conducted to evaluate the degree of stress state and stress related factors in 280 male white collar workers by using Psychosocial Well-being Index. The results were as follows; 1. According to Psychosocial Well-being Index, mild stress state was 78.6 %, healthy state was 12.9 %, and high risk stress state was 8.6 %. Single marital status, low education level, low income and low frequency of exercise group had high score of stress. 2. The total stress score was highly associated with the factors of social performance and self confidence, depression, general well-being and vitality, and sleeping disturbance and anxiety in order. 3. In reliability test of stress factors, Cronbach's a coefficients of social performance and self confidence, sleeping disturbance and anxiety, depression, general well-being and vitality were 0.91, 0.91, 0.90, and 0.89 respectively. In conclusion, it suggested that marital status, income, education, and exercise were associated with stress score. All of the above factors should be considered to white collar workers health.

사회심리적 건강측정도구를 이용한 사무직 및 생산직 근로자들에서의 스트레스 평가 (The Assessment of Stress between White and Blue Collar Workers by using Psychosocial Well-being Index)

  • This study was conducted to evaluate the degree of stress state and the risk factors related to in 967 white collar workers and 275 blue collar workers by using Psycosocial Well-being Index. The results obtained were as follows : 1. In the white collar workers and blue collar workers, young age, low education and low income groups had high scores of stress, while in white collar workers, female had high scores of stress but in blue collar workers, male had high scores of stress. 2. According to psychosocial well-being index, mild stress sto were 73.9% and 53.1%, high risk stress state were 8.9% and 44.4%, and healthy state were 17.2% and 2.6% in white collar and blue collar workers respectively. 3. The total stress score was highly associated with the factors of depression, social performance and self-confidence, general well-being and vitality, and sleeping disturbance and anxiety in order. 4. In reliability test of stress factors, Cronbach's $\alpha$ coefficients of depression, social performance and self-confidence, general well-being and vitality, and sleeping disturbance and anxiety were 0.89, 0.81, 0.79, and 0.74 respectively. In conclusion, it suggested that age, sex, marital status, income, education, sleeping time, smoking and exercise habit were associated with stress score, all of the above factors should considered to occupational health.

지속적 양압술과 수면중 주기적 사지운동 장애의 관계에 대한 예비적 연구 : 앙와위가 주기적 사지운동 장애와 관련되는가? (Preliminary Study of The Periodic Limb Movement Disorder Following Nasal CPAP : Is It Associated With Supine-Sleeping Position?)

  • Introduction : Periodic limb movement disorder (PLMD) is shown to common in patients with OSA and may become evident or worsened when treated with nasal continuous positive airway pressure (CPAP). Whether this is due to im proved sleep continuity. adverse nocturnal body positioning, uncovered by CPAP, or due to the CPAP stimulus is still debat-ed. We hypothesized that the increase in PLM activity following CPAP is associated with more supine-sleeping tendencies when being treated with CPAP. In the present work, we compared differences in the PLMD index (PLMI) and sleeping position of patients with sleep disordered breathing before and after CPAP treatment. Method : We studied 16 patients (mean age 46 yr, 9M, 7F) with OSA (11 patients) or UARS (5 patients) who either had PLMD on initial polysomnogram (baseline PSG) or on nasal CPAP trial (CPAP PSG). All periodic leg movements were scored on anterior tibialis EMG during sleep according to standard criteria (net duration; 0.5-5.0 seconds, intervals; 4-90 seconds. 4 consecutive movements). Paired t-tests compared PLMD index (PLMI), PLMD-related arousal index (PLMD-ArI), respiratory disturbance index (RDI), and supine sleeping position spent with baseline PSG and CPAP PSG. Results : Ten patients (63%) on baseline PSG and fifteen patients (94%) on CPAP PSG had documented PLMD ($PLMI{\ge}5$) respectively with significant increase on CPAP PSG(p<0.05). Ten patients showed the emergence (6/10 patients) or substantial worsening (4/10 patients) of PLMD during CPAP trial. Mean CPAP pressure was $7.6{\pm}1.8\;cmH_2O$. PLMI tended to increase from baseline PSG to CPAP PSG, and significantly increase when excluding 2 outlier (baseline PSG, $19.0{\pm}25.8/hr$ vs CPAP PSG, $29.9{\pm}12.5/hr$, p<0.1). PLMD-ArI showed no significant change, but a significant decrease was detected when excluding 2 outlier (p<0.1). There was no significant sleeping positional difference (supine vs non-supine) on baseline PSG, but significantly more supine position (supine vs non-supine, p<0.05) on CPAP PSG. There was no significant difference in PLMI during supine-sleeping and nonsupine-sleeping position on both of baseline PSG and CPAP PSG. There was also no significant difference in PLMI during supine-sleeping position between baseline PSG and CPAP PSG. With nasal CPAP, there was a highly significant reduction in the RDI (baseline PSG, $14.1{\pm}21.3/hr$ vs CPAP PSG, $2.7{\pm}3.9/hr$, p<0.05). Conclusion : This preliminary data confirms previous findings that CPAP is a very effective treatment for OSA, and that PLMD is developed or worsened with treatment by CPAP. This data also indicates that supine-sleeping position is more common when being treated with CPAP. However, there was no clear evidence that supine position is the causal factor of increased PLMD with CPAP. It is, however, suggested that the relative movement limitation induced by CPAP treatment could be a contributory factor of PLMD.

Comparison of the Effects of Hand Massage Provided with Different Intervals and Periods on Pain and Sleep Disturbance after Orthopedic Surgery

  • Purpose: The purpose of this study was to compare the effects of hand massage provided with different intervals and periods on pain and sleep disturbance after orthopedic surgery. Methods: A non-equivalent control group pretest- posttest design was used. The subjects were admitted in an orthopedic hospital to get a surgery. They were divided into three groups. Group I (n=30) had hand massage every day. Group II (n=30) had hand massage every other day. Control group (n=31) had usual care. Data of all three groups were collected on the day before operation, POD (postoperative day) 6 and POD12. Hand massage was given for 2 and half minutes per hand. Results: Pain on POD6 of experimental group II was reduced more than those of control group. Pains on POD12 of both experimental groups were reduced more than those of control group. On POD6, only perceived sleep disturbance (PSD) was significantly different among groups. On POD12, PSD, total sleeping time, and sleep efficiency were more improved in the experimental groups. Conclusion: Hand massage was effective on the reduction of pain and sleep disturbance after orthopedic surgery. Applying hand massage on alternate day was effective enough. Also the effects were more obvious after 12 days.

모발석 2 예 보고 (Trichobezoar - Two Cases of Unusual Presentation -)

  • Two cases of trichobezoar with unusual presentation in female children are described. The first case is a 7-year-old female with a 3-day history of abdominal pain, obstipation, and emesis. She developed intestinal obstruction and showed double bezoars in the stomach and intestine respectively. She had been in a habit of biting or sucking hairs before sleeping from infancy until 5 years of age. The other patient is a 6-year-old girl referred for an epigastric mass, emotional disturbance and trichotillomania. In spite of the psychological treatment, 4 laparotomies were necessary due to repeated recurrences.

  • Objectives : The purpose of this study was to observe the clinical effects of acupuncture therapy and Ecklonia cava extract on sleep disturbance in ALS patients. Methods : In this study one patient received acupuncture therapy and took Ecklonia cava extract. The effects of the combined administration of these treatments were evaluated using Numeric Rating Scale and Pittsburgh Sleep Quality Index. Results : The NRS score for sleep disturbance decreased from 10 to 1 and the PSQI score decreased from between 12~13 to 8. After treatment, the patient stopped taking sleeping pills and antidepressant. Conclusions : Combined administration of acupuncture therapy and Ecklonia cava extract could be effective in improving sleep disturbances in ALS patient.

구안와사 환자 50례의 스트레스 정도에 관한 임상적 연구 (The Clinical Study of Stress Perceived by 50 patients with Bell's palsy)

  • Objective : This study is to explore the various of the stress by perceived by paitents with Bell's palsy Methods : 50 patients who were diagnosed as Bell's palsy were sequentially interviewed and examined Results : The results were as follows 1. The mean average PWI(Psychosocial well-being index, total : score)was $41.58{\pm}17.53$ :Factor I(Social performance and self-confidence) with a mean of $11.18{\pm}8.38$, Fact II( Depression) with a mean of $8.94{\pm}6.26$, Factor III(Sleeping disturbance and anxiety) with a mean $of7.56{\pm}5.20$, Factor IV(General well-being and vatality)with a mean $of9.90{\pm}4.44$ 2. With the respect to the general characteristics of subjects, there were statistically significant differences in the patients's stress perception by sex, having an occupation and the amount of sleeping time. Conclusions : We needed the further study about the relation between stress and Bell's palsy

입원환자의 수면형태 및 수면에 영향을 미치는 요인 (Sleep pattern changes and related factors in general hospital inpatients)

  • Sleep is an inevitable part of human existence and a part of the dynamic quality of life. The purpose of this study is to explore sleep pattern changes and related factors of patients hospitalized. The participants are 158 patients hospitalized in one general hospital. The questionnaire was developed by modification of VSH sleep scale (1987). Open question was used for research of related factors. $SPSS/PC^+$ are utilized for data analysis. The results are as follows ; There is significant difference before and after admission in the sleep disturbance. There is retroverse significant difference before and after admission in the sleep effectiveness. There is no significant difference between two groups in the nap supplementation. The patients reported pain by reason of sleep latency and mid-sleep awakening and noise due to early awakening. The reasons for lack of sleep were pain and noise. The method for overcoming their sleep disturbance, most of the subjects waited with closed eyes, imaged by themselved and took medicate sleeping pills. Discomfort manifested in the case of insufficient steepness was fatigue, headache and aggressiveness.
