코로나19 두려움과 예방실천 간의 관계에서 정신건강과 수면 문제의 매개효과 (Mediating Effects of Mental Health and Sleep Problems in the Relationship between Fears of COVID-19 and Preventive Practices)

  • 본 연구의 목적은 코로나19 두려움이 예방실천에 영향을 미치는지 살펴보고, 정신건강 문제와 수면 문제의 이중 매개효과를 확인하는 것이다. 연구대상자는 수도권에 거주하는 310명의 성인이었다. 자료수집은 코로나19 두려움, 정신건강 문제, 수면 문제 및 예방실천을 평가하는 설문지를 사용하여 온라인 설문조사를 수행하였다. 수집된 데이터는 이중 매개효과를 검증하기 위해 PROCESS Macro (Model 6)를 사용하여 분석하였다. 연구결과, 코로나19 두려움이 정신건강 문제, 수면 문제 및 예방실천에 직접적인 영향을 미쳤다. 또한 코로나19 두려움이 예방실천에 영향을 미치는 과정에서 정신건강 문제와 수면 문제는 이중 매개효과가 있었다. 코로나19 예방실천은 두려움이 증가할 때 정신건강 문제를 통한 매개효과가 가장 강하게 작용하였다. 따라서 코로나19 확산으로 인해 두려움이 증가할 때 정신건강과 수면 문제가 발생하지 않도록 적극적인 관리를 수행하는 것이 코로나19 예방실천을 향상시킬 수 있다.

  • Sleep disorders such as insomnia, obstructive sleep apnea (OSA), and restless legs syndrome (RLS) are very common disorders and may cause significant burden in terms of individual as well as societal aspects. Sleep insufficiency from such sleep disorders may cause deleterious effects on daily work life and may be associated with other major medical or psychiatric disorders including cardiovascular disease, diabetes mellitus, depression, and anxiety disorder. Various motor or occupational accident may result from the sleep problems. In addition, recent researches provide the method to evaluate the lost productivity time in terms of absenteeism and presenteeism. Moreover, several studies on cost-effectiveness of treatment of sleep disorders show that it is cost-effective.

  • Sleep disturbance is a one of common complaints among patients with panic disorder. However, clinicians and researchers did not give much attention to the sleep symptoms of panic disorder yet. Several previous studies suggested that the sleep disturbance in panic disorder is mediated by nocturnal panic attack. In terms of the pathophysiology of panic disorder, nocturnal panic attack seems to be closely associated with the sleep problems in panic disorder. In this article, the authors reviewed various previous studies about sleep of panic disorder and intended to give importance of evaluating sleep disturbances and nocturnal panic attack in panic disorder for both clinical and research purpose.

  • Coronavirus disease 2019 (COVID-19), which was a global pandemic caused by severe acute respiratory syndrome corona virus 2 (SARS-CoV-2), is still a serious public health problem. COVID-19 causes various symptoms not only in the respiratory system but also in various parts of the body and has a significant effect on sleep. Insomnia and poor sleep quality were observed at high rates in patients with COVID-19 as well as in the uninfected general population. Obstructive sleep apnea is also considered a risk factor in patients with severe COVID-19. Virus-induced central nervous system damage is likely to be the cause of many sleep disorders in COVID-19, but psychosocial influences also seem to have played a significant role. Sleep problems persisted at high rates for a considerable period after the infection phase was over. More attention and research on the effect of COVID-19 on sleep is needed in the future.

등마사지가 위암 수술 환자의 통증 정도, 상태불안 및 수면의 질에 미치는 효과 (The Effect of Back Massage on Degree of Pain, State Anxiety and Quality of Sleep of Postoperative Patients with Gastrectomy)

  • Purpose: Most of postoperative patients experience pain, state anxiety and sleep disturbance. These problems negatively influence the recovery of postoperative patients. So alleviating these problems has been one of the nurses' central roles. The purpose of this study was to examine the effects of back massage on pain, state anxiety and quality of sleep of postoperative gastrectomy patients. Methods: A non-synchronized non-equivalent control group pre and post-test design was used. The research instruments used in this study were the Numerical Rating Scale (NRS) for pain, the State-Anxiety Inventory (STAI) for anxiety and the Verran and Synder-Halpern scale for quality of sleep. The subjects were patients admitted to a university hospital located in D city. Twenty-nine patients in the experimental group had a 10 minute manual back massage stimulation for 5 days from the 1st day to the 5th day after their operation, and 25 patients in the control group did not. Results: The degree of pain was significantly reduced according to post operation day and quality of sleep was significantly increased. However state anxiety was not significantly reduced. Conclusion: Back massage is a partially effective nursing intervention for postoperative patients with gastrectomy who experience pain and sleep disturbance.

Scar Status, Depression, Sleep, and Health Related Quality of Life Following Severe Burn Injury: A Cross-sectional Descriptive Study

  • Purpose: Severe burn injuries require long periods of hospitalization and treatment, which results in various physical and psychological issues. The main purpose of this study was to identify burn characteristics and psychological problems that influence Health Related Quality of Life (HRQoL) after discharge. Methods: A cross-sectional descriptive study using mobile/web or paper-based survey methods was conducted from a major burn center. A total of 145 patients completed the scar assessment, quality of life, sleep disorders, and depression scales. Results: The overall mean HRQoL and scar status scores were 2.28 out of 5 and 34.45 out of 60 points, respectively. Participants with higher burn degree, joint involvement, and emotional distress reported significantly lower HRQoL and scar status. Participants with depression and sleep problems also had lower HRQoL. Significant predictors of HRQoL included burn range, scar status, depression, and sleep issues. Conclusion: The results show that patients with severe burn injury experience high levels of physical and psychological problems. Patients with severe burn injury and psychological problems such as depression and sleep are likely to experience a reduced HRQoL. Psychological management and intervention in home care setting may improve HRQoL of burn patients.

Sleep Quality of Patients with Temporomandibular Disorders: Relationship to Clinical and Psychological Characteristics

  • Purpose: The association between pain and sleep is described as a vicious cycle and psychological distress is well known as comorbid condition in the patients with pain and sleep problems. The aim of the study was to evaluate the prevalence of self-reported sleep disturbance and its relationship to clinical and psychological profiles in temporomandibular disorder (TMD) patients. Methods: The sample consisted of 123 TMD patients (90 women and 33 men), with a mean age of $39.9{\pm}15.4years$. Self-report measures of sleep quality, pain and psychological profile were conducted via the Pittsburgh Sleep Quality Index (PSQI), the Brief Pain Inventory and the Symptom Checklist-90-Revision at the initial visit. The primary diagnosis of TMD were categorized as TMD with internal derangement without pain, TMD with joint pain, TMD with muscle pain and TMD with joint-muscle combined pain. The chi-square test, independent t-test, oneway ANOVA and multiple linear regression analysis were used for statistics. Results: The patients was grouped as good sleepers (n=32, scores of 5 and lower) and poor sleepers (n=91, scores of 6 and higher) according to the recommended cutoff point of the global PSQI score (>5). TMD patients with pain showed poor sleep quality than TMD patients with internal derangement without pain. Poor sleepers had high pain interference and elevated psychological distress. Among them, pain interference and depression were significant predictors to sleep quality. Conclusions: The results suggest that sleep disturbance is a prevalent complaint in TMD patients, and sleep problems in TMD patients are associated with pain and psychological distress.

기혼여성 간호사의 수면문제와 직무열의 간의 관계에서 일-가정 갈등의 매개효과 (Mediating Effect of Work-Family Conflict in the Relationship between Sleep Problems and Job Engagement in Married Female Nurses)

  • 본 연구는 기혼여성 간호사의 수면문제와 일-가정 갈등이 직무열의에 미치는 영향을 파악하고 일-가정 갈등의 매개효과를 규명하여, 직무열의를 증진하기 위한 전략을 제공하기 위해 수행되었다. 본 연구는 제5차 근로환경조사를 이용하여 이차 분석을 실시하였고, 총 259명의 기혼여성 간호사를 연구대상으로 하였다. 자료는 SPSS 24와 AMOS 24 프로그램을 이용하여 분석하였다. 연구 결과, 수면문제(β=-.33)와 일-가정 갈등(β=-.30)은 직무열의에 유의한 영향 요인이었고, 수면문제와 직무열의 간의 관계에서 일-가정 갈등의 매개효과가 있는 것으로 나타났다(p<.001). 직무열의는 이 변수들에 의해 28.0% 설명되었다. 본 연구는 기혼여성 간호사의 직무관련 웰빙을 증진하기 위하여 기혼여성 간호사가 지각하는 수면문제와 일-가정 갈등이 직무열의 예측인자임을 규명하였고, 이를 고려하여 기혼여성 간호사의 직무열의를 증진시킬 수 있는 융합 중재프로그램 개발을 위한 기초 자료로 활용할 수 있을 것으로 기대한다.

수면과 2형 당뇨병 (Sleep and Type 2 Diabetes)

  • Adequate amount and quality of sleep are important for metabolic control in patients with type 2 diabetes. Too short or too long sleep time disrupts glycemic control in both prediabetes and type 2 diabetic patients. Circadian misalignment such as shift work is also associated with an increased risk of developing type 2 diabetes. Clinicians should pay attention to the sleep problems and circadian patterns of patients. However, the pathophysiologic mechanism of the association between sleep and diabetes is likely to be complex and bidirectional. The underlying mechanism remains poorly understood, and further research is warranted.

Research on Development Strategy Based on Domestic Sleep Industry Survey

  • The domestic sleep industry is rapidly increasing in market size as it is linked to sleep apnea in the longterm care insurance for the elderly and domestic IT technology. In addition, due to the aging society, the government's support policy is also increasing, and many domestic companies are focusing on the sleep industry. Therefore, this study intends to examine the development strategies of the domestic sleep industry due to the increased social interest and corporate activities. To this end, a survey of 108 domestic companies was conducted by means of a Frequency Analysis, revealing some problems, such as poor sleep companies, insufficient resources for technology development, and limitations in distribution channels. As a result, it was found that for the development of the domestic sleep industry, strategies to strengthen the ecosystem, such as softening strategies through the sleep technology culture, strengthening ecosystems such as revitalizing industrial connections, and reforming laws and regulations, are urgent.
