Prevalence of restless legs syndrome and sleep problems in Korean children and adolescents with attention deficit hyperactivity disorder: a single institution study

  • Purpose: Attention deficit hyperactivity disorder (ADHD) is a common disorder in school-aged children. Patients with restless legs syndrome (RLS) often present with ADHD symptoms and vice versa. This study was the first to attempt to identify the prevalence of RLS and sleep problems in children with ADHD in Korea. Methods: Patients diagnosed with ADHD were asked to complete a sleep questionnaire. The sleep questionnaire included items to help identify the presence of four typical symptoms that are used as diagnostic criteria for RLS. Results: A total of 56 patients, including 51 boys and 5 girls (mean age, 10.7 years old) participated. Of these, 24 complained of pain, discomfort, or an unpleasant sensation in the legs. Based on the RLS diagnostic criteria, 2 patients were diagnosed with definite RLS and 4 with probable RLS. There were no significant differences in age, medication dosage, or neuropsychological test scores between the patients with and without RLS symptoms. Conclusion: Approximately 42.9% of patients with ADHD presented with RLS symptoms and 7.1% of these were diagnosed with RLS. Patients with ADHD also experienced various other sleep disorders. Thus, appropriate assessment and treatment for sleep disorders in patients with ADHD is essential.

멀티센서 기반 수면장애 개선 시스템 설계 및 구현 (Design and Implementation of Sleep Disorders Improvement System Based on Multi-Sensor)

  • 대표적인 수면장애로 수면 무호흡증과 코골이가 있다. 하지만 기존에는 수면장애 측정과 진단만 이루어지고 그 원인을 파악하는 연구는 미비하였다. 따라서 본 논문에서는 이를 보완하기 위해 멀티센서를 기반으로 한 수면장애 개선 시스템을 제안한다. 본 논문에서는 제안한 시스템을 설계하기 위해 전체 시스템의 구성을 파악하고 데이터의 흐름을 설계하였다. 설계한 시스템을 구현하기 위해 사용자 인터페이스 및 모바일 애플리케이션은 안드로이드 기반으로 구현하였다. 본 논문에서 설계 및 구현한 시스템의 결과의 정확도를 테스트하기 위해 테스트 시나리오를 작성하였다. 테스트 시나리오에 따른 수면 장애 요인 값을 변화시키며 추론 결과의 정확도를 테스트 한 결과 제안한 수면장애 개선 시스템이 정확하게 동작함을 확인하였다.

학령전기 발달장애 아동의 라이프 케어를 위한 감각처리가 수면장애에 미치는 영향에 관한 연구 (A study of the Effect of Sensory Processing on Sleep Disturbance for Life care of Preschool Children with Developmental Disabilities)

  • 이 연구는 4-7세의 발달장애 아동을 대상으로 하여 감각처리와 수면장애의 관계를 알아보고 감각처리가 수면장애에 미치는 영향을 파악하고자 하였다. 2017년 6월부터 8월까지 발달 클리닉 및 재활병원에 내원하는 전반적 발달장애 아동 110명을 대상으로 하여 최종 101명의 자료를 분석하였다. 감각처리와 수면장애를 평가하기 위해 단축형 감각 프로파일(SSP)과 한국형 아동 수면장애 척도(K-CSHQ)로 자료 조사하였다. 통계분석은 기술분석과 피어슨 상과분석, 다중회귀분석을 사용하였다. 연구결과, 발달장애 아동은 감각처리와 수면장애에 문제를 보이는 경우가 많았다. 감각처리는 수면장애와 관련이 있었으며 수면장애에 대한 감각처리의 최종 영향 요인은 맛/냄새 민감성, 청각필터링과 전반적인 감각처리인 것으로 나타났다. 발달장애 아동의 질 높은 수면을 돕기 위해서는 아동의 감각처리 능력을 중재에 고려할 필요가 있으며 특히 맛/냄새 민감성, 청각 필터링 영역을 중재하는 것이 도움이 될 것이다.

어싱매트가 수면의 질에 미치는 영향 (Effect of Earthing Mat on the Quality of Sleep)

  • Objectives: Using as a sleep evaluation tool the Pittsburgh Sleep Quality Index (PSQI), this paper studied the effects of earthing on the amount and quality of sleep by examining a patient population of people who used and did not use earthing mats and a group of general healthy people. Methods: From September to November 2019, 30 patients who voluntarily used an earthing mat, 40 patients who did not use an earthing mat, and 40 healthy people who did not use an earthing mat were selected. A questionnaire applying the PSQI (Pittsburgh Sleep Quality Index) after being translated into Korean was distributed starting November 29, 2019 to 110 subjects. Out of them, 101 subjects who completed the questionnaire were examined. All statistics used SPSS 25.0. Results: The first overall score showed that all three groups had problems sleeping, with five points or more, but the general population had better sleep quality than the patient group. It also showed that the using matgroup had better quality of sleep than the ones that did not use the earthing mat In addition, the analysis of the third group through the variance analysis showed significance at >0.05 for the general population and the patient group. Based on this, a multi-comparison analysis of the third group showed significantly less than 0.05 in patients who do not use an earthing mat compared to the public. First of all, there are no statistically significant differences between the three groups in subjective sleep quality, sleep latency, and sleep time, but an analysis of multiple comparison analysis with the general population group showed that the quality of sleep was worse compared to those that used the mat. In addition, sleep efficiency, sleep disorder, sleeping pills, and daytime dysfunction were found to be less than >0.05 in the three groups. Conclusions: These results suggest that if you ground your body to Earth during sleep, the secretion of cortisol will decrease night levels, sync more with the natural 24-hour circulation rhythm profile, and that when you sleep, sleep is better and pain and stress are associated with the study. While the preceding study cannot confirm that contact with the ground affects the quality of sleep, it suggests that it is relevant as shown in this paper.

지역사회 거주 성인의 수면문제 발생률과 위험요인 (Prevalence and Risk Factors of Sleep Disturbance in Community Dwelling Adults in Korea)

  • Purpose: The purpose of this study was to identify the prevalence and risk factors of sleep disturbance among community dwelling adults in Korea. Methods: Data were collected from nine hundred fifty nine adults dwelling in a metropolitan city in Korea. Subjects were older than 45 years, did not take medication for sleep problems on a regular basis, and did not work at night. The Pittsburgh Sleep Quality Index was used to collect data about sleep disturbance. Sociodemographic, life style, and health related factors were included as risk factors. Chi square test and logistic stepwise regression were used to identify the risk factors. Results: The prevalence of sleep disturbance was 13.1%. The significant risk factors were being female, age, number of toilet use at night, perceived health, and pain. Risk factors were somewhat different by gender. Emotional and job status were the risk factors only for men and menopausal status were for women. Conclusion: Prevalence of sleep disturbance in community dwelling adults in Korea was not greater than those dwelling in other countries. Females, older adults, the number of toilet use at night, perceived health, and pain were the most important risk factors for sleep disturbances. Gender was a factor in reported sleep disturbances. It might be useful to investigate the factors that contribute to the number of toilet use at night as this might have some underlying factors that could be corrected.

간호대학생의 불면증, 수면의 질, 우울 및 일주기 리듬의 관계 (Correlation among Insomnia, Sleep Quality, Depression, and Circadian Rhythm in Nursing Baccalaureate Students)

  • Purpose: This study examined the correlation of insomnia, sleep quality, depression, and circadian rhythm in nursing students. Methods: A total of 213 subjects completed a questionnaire consisting of their general characteristics, Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), Center for Epidemiologic Studies Depression (CES-D), and Composite Scale of Morningness (CSM). The collected data were analyzed by descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficient using the SPSS 23.0 program. Results: The subjects' mean scores were ISI 7.18; PSQI 11.18; CES-D 16.00; and CSM 30.18. In insomnia, there were significant differences according to caffeine, perceived health status and major satisfaction. In sleep quality, there were significant differences according to perceived health status and major satisfaction. Significant differences in depression were observed according to gender, caffeine, subjective health status, major satisfaction, and circadian rhythm by drinking and exercise. A significant positive correlation was observed among ISI, PSQI, and CES-D. ISI and CES-D were negatively correlated. Conclusion: Tailored health care programs should be developed and applied to prevent and manage sleep-related and emotion-related problems in nursing students by considering the health status, major satisfaction, and gender.

Mandibular Advancement Devices for Treating Snoring and Obstructive Sleep Apnea

  • Many therapeutic modalities including continuous positive airway pressure, surgery, and oral appliances are used to treat patients with sleep-disordered breathing. However, there are no definitive treatment modalities for individual patients due to various causes of sleep-disordered breathing. Clinicians should have select best options for individual patients and it is quite challenging process. Oral appliances attracted clinical attention for its convenience and safety. Several designs of oral appliances are introduces such as soft palate lifter, tongue retaining device, and various appliances which aimed to mandibular advancement. Among these oral appliances, mandibular advancement devices (MADs) are considered the most excellent based on their effectiveness and patient tolerance. Although MADs are not guarantee dramatic outcome and less consistent than continuous positive airway pressure, they offer several advantages over continuous positive airway pressure and surgical methods, including non-invasiveness, silence, portability, and tolerability, simplicity. Therefore, general dental practitioner who had passed sleep dental curriculum or coursework can treat the patients with sleep problems. This article reviews the history, clinical indications, suggested mechanism of actions, various positive effects and several side effects, factors predicting a favorable outcome, determining amounts of mandibular advancement, compliance and long-term efficacy of MADs use.

간호대학생의 수면의 질, 피로와 우울; 이론수업과 임상실습 시 비교 (Comparison of Sleep Quality, Fatigue and Depression among Nursing Students between School Lessons and Clinical Practicum Term)

  • Objectives: The purpose of this study was to compare the differences of sleep quality, fatigue and depression between school lessons and clinical practicum term in nursing students. Methods: This study was conducted through a survey on 84 $4^{th}$ grade nursing students. Data collection was conducted from April to June, 2015. Participants completed a questionnaire assessing sleep quality, fatigue and depression. Data were analyzed by the SPSS/WIN 21.0 using descriptive statistics, paired t-tests, ANOVAs, Pearson's correlations and multiple regression. Results: The mean scores of fatigue and depression during the school lesson were significantly high compared to those of the clinical practicum. On the contrary, the score of sleep quality did not showed difference. Sleep quality showed negative correlations with fatigue and depression respectively, and fatigue showed positive correlation with depression. Subjective health status was the influential factor predicting fatigue and depression after school lessons, and subjective health status and major satisfaction were the influential factors predicting fatigue and depression after clinical practicum term. Conclusions: Nursing students showed different characteristics between school lessons and clinical practicum term. Thus, it is essential to identify physical and psychological problems and related factors respectively, then to devise appropriate programs to improve nursing curriculums.

Associations between Sleep and Work-Related Cognitive and Emotional Functioning in Police Employees

  • Aim: We aimed to examine the cross-sectional and longitudinal associations between sleep and work-related impaired cognitive and emotional functioning in police employees. Methods: This study included 410 participants (52% men) employed in a police district in Norway at baseline, of which 50% also participated in the study at 6 months later follow-up. The questionnaires included items measuring work schedule, sleep length, insomnia, as well as impaired cognitive and emotional functioning at work. Results: The results showed that insomnia was related to impaired work-related emotional functioning measured at baseline, and to impaired cognitive functioning measured at both baseline and follow-up. Sleep length and rotating shift work were not associated with future decline in cognitive or emotional functioning. Conclusion: Our study indicates that the relationship between insomnia and emotional functioning at work may be transient, whereas insomnia can be related to both immediate and future impaired cognitive functioning. Replication of the findings in larger samples is advised. The findings call for an emphasis on the prevention and treatment of sleep problems among police employees as a mean of maintaining and improving cognitive and emotional functioning at work, and thereby reducing the risk for impaired performance and negative health and safety outcomes.

고려 홍삼이 수면의 질에 미치는 영향 : 무작위 위약-대조군 연구 (Effect of Korean Red Ginseng on Sleep : A Randomized, Placebo-Controlled Trial)

  • 목 적: 인삼은 한국을 비롯한 아시아 국가에서 전통적으로 불면증의 치료에 사용되어 왔으며 쥐를 이용한 동물실험에서 수면을 촉진시키는 효과가 있다는 연구 결과가 있어왔다. 하지만 이러한 효과를 입증할 만한 임상 실험은 거의 이루어지지 않았다. 이에 본 연구에서는 고려 홍삼이 수면에 미치는 영향을 알아보고자 한다. 방 법: 건강한 성인 남성 20명을 대상으로 하였으며 이들은 무작위로 홍삼군과 위약군에 배정되어 2주간의 실험이 진행되었다. 홍삼군의 경우 하루 4,500 mg의 홍삼을 섭취하였다. 실험 시작시점과 2주 후인 종료시점에 수면다원검사를 시행하여 두군간의 변수의 변화를 비교하였다. 결 과: 총 15명의 대상자, 즉 홍삼군 8명과 위약군 7명이 수면다원검사를 시행 받았으며 홍삼군이 위약군 보다 실험 종료 시점에서 3단계 수면이 증가한(p=0.087) 반면에 2단계 수면이 감소하는(p=0.071) 경향을 나타냈다. 결 론: 본 연구 결과는 고려 홍삼이 깊은 잠은 증가시키고 얕은 잠은 감소시키는 경향이 있음을 보여준다. 이는 통계적으로 유의한 결과는 아니지만 홍삼이 수면의 질을 향상시키는 데 효과적임을 짐작하게 한다. 그러나, 보다 많은 대상군과 충분한 홍삼 용량으로 보다 긴 기간 동안 진행되는 연구를 통해 고려 홍삼의 수면에 대한 효과를 확인할 필요가 있다.
