An odontogenic cutaneous fistula is a pathological communication between the outer skin surface of the face and the oral cavity. Facial cutaneous fistula is a complication of odontogenic infection that is often misdiagnosed with skin infection. We report a rare case, which was diagnosed as basal cell carcinoma based on the biopsy of skin lesions in the patient who had been diagnosed with odontogenic cutaneous fistula. A 64-year-old male patient presented with a cutaneous odontogenic fistula. The patient had undergone surgical extraction of fistula tract and loose tooth before dermatology or plastic surgery consultation. With the biopsy and computed tomography, it was confirmed that fistula and basal cell carcinoma. However, the connection between the fistula and skin cancer was not clear. Positron emission tomography-computed tomography scan was performed and was not detected as other local or distant metastasis. After that, wide excision of the skin lesion was performed. Although skin cancer is not commonly observed, it is necessary to rule out this disease entity by performing biopsy of skin lesions.

Objectives: This study was carried out to estimate the magnitude of skin disease related to occupation and to find out the characteristics of it. Methods: We collected and analyzed the cases of occupational skin disease reported by surveillance system composed of doctors and nurses in 150 enterprises with dispensary or attacked hospital and physicians in 92 specific health examination institutes and 150 dermatologists from May to November, 1998. Results: Among members of surveillance system, 66 enterprises and 47 specific health examination institutes and 55 dermatologists reported 571 cases of occupational skin disease in 512 workers. Excepting 81 cases reported by dermatologists, We analyzed 490 cases reported by enterprises and specific health examination institutes. Among 490 cases, contact dermatitis was most common(368 cases, 75.1%) and the second was hyper or hypopigmentation(36 cases, 7.3%). When we analyzed the characteristics of workers with occupational contact dermatitis, male workers were 281 (79.2%) and female were 74(20.8%). 165 workers(64.5%) had chronic skin disease with repeated cure and relapse. 245 workers(72.5%) answered positively that their coworkers had similar skin disease. 27 workers(8.7%) experienced absence due to contact dermatitis related to occupation. To analyze the type of industries of workers with occupational contact dermatitis, automobile and trailer manufacturing industry was most common(105 cases, 29.6%) and the second was manufacturing industry for image, sound and communication equipment(55 cases, 15.5%). Organic solvent(183 cases, 46.7%) was the most common treating material of workers with contact dermatitis and the second was various kinds of chemicals(59cases, 15.1%). Conclusions: This is the first study using nationwide surveillance system to collect data of occupational skin disease. We found that many workers had skin disease related to occupation and characteristics of occupational skin disease were chronic and clustering. Therefore, we had to establish counterplan to manage occupational skin disease and to operate surveillance system to identify trends of occupational skin disease, continuously.

Verrucous carcinoma (VC) is a rare subtype of squamous cell carcinoma that commonly occurs in the oral cavity. However, VC of the facial skin is relatively rare. We report a case of a 91-year-old woman with VC of the facial skin in the left zygoma area. She was diagnosed with actinic keratosis (4×3 cm) of the same site approximately 12 years previously, but declined further treatment. The mass was excised with a minimum of 0.4 cm from gross margins with the result of free from tumor of all margins by frozen section, allowing for primary closure after skin undermining. Basal resection was performed in the preplatysmal plane. The diagnosis of VC was confirmed by histopathological examination. Postoperatively, the wound healed without incident and with no signs of facial nerve injury. To our knowledge, this is the first reported case of VC of facial skin arising from actinic keratosis.

1. Objectives Sasang constitutional medicine is unique in Korean traditional medicine. It diagnoses and treats patients based on his/her Sasang constitution (SC). Skin properties have been used as an effective diagnostic component in the classification of SC types in clinics. In this paper, we investigated the SC-based health relevance of skin elasticity and wrinkle properties. 2. Methods The skin elasticity and wrinkle of forearm and dorsal hand were measured in 299 elderly female subjects. To determine the subject's Sasang constitution, we adopted the classification results from a newly developed SC diagnostic tool. The health states of the subjects were scored by two Korean traditional medical doctors, by whom each subject was categorized either into the healthy state or the unhealthy state. 3. Results As a result, the elasticity hysteresis of forearm (E_HYS), the visco-elasticity (VE_MEAN), and the wrinkle frequency energy of backhand (W_HAND) showed significant differences between Taeum-in group and Soeum-in group. In case of the Soeum-in on unhealthy state, VE_MEAN was decreased significantly (p<.05). W_HAND and W_ARM_H of the healthy Taeum-in were less than those of the unhealthy Taem-in. 4. Conclusions In this study we showed that, for an elderly female population, skin elasticity and viscosity were significantly different not only between each SC type but also between healthy group and unhealthy group in each constitution. In particular, Soeum-in subjects were inferred to be superior in retaining skin softness when they were healthy, and Taeum-in subjects were easy to lose their firmness of skin surface when they became unhealthy.
Exposure to ultraviolet B(UVB) radiation causes skin inflammation such as pigmentation and the induction of cyclooxygenase-2(COX-2) gene expression. In this study, we investigated the effect of natural extracts from Tea, EGb 761 and Korean red ginseng(KRG), on the pigmentation and expression of COX-1 and COX-2 mRNA in UVB-irradiated C57BL/6 mice. Before UVB irradiation, the skin color was significantly showed the lightening effect by topical application of natural compounds (p<.05). In the case of UVB irradiated mice, we observed a decrease in pigmentation by compounds (p<.05). In irradiated skin, COX-1 mRNA expression is not changed following UVB irradiation, but COX-2 gene increases. Also, natural compounds lowered mRNA levels of COX-2. Therefore, these results suggest that COX-2 mRNA increases by UVB irradiation. Also, Tea, EGb 761 and KRG as a topical application may inhibit skin pigmentation and modulate COX-2 mRNA level.
This study was to consider the effect of PUVA treatment on patients with skin disease. The result can be summarized as follows : 1) PUVA treatment may effect directly on lymphocytes to be permeable locally in Skin disease. 2) PUVA treatment may effect on abnormal immune reacton in skin disease. The significance on the effect of PUVA treatment is still unknown. So, we will study continuously for PUVA treatment on patients with skin disease.

Objectives: The main purpose of this study is to produce background data which can be compared with several large-scale industrial complex regions. Methods: The framework of this study was designed to evaluate and monitor health effects associated with low-level and long-term exposure to environmental pollutants. In this study, 1,009 local residents were recruited in the city of Chungju using personal questionnaires, medical check-ups, and allergen skin-prick tests for 12 common allergens. The prevalence rate of skin allergens was reviewed together with that from large-scale industrial areas. Results: The diagnosis prevalence and medical treatment prevalence of asthma was 5.6% and 1.7%, respectively. Allergic rhinitis was 25.4% and 16.7%, allergic dermatitis was 16.6% and 9.9% and allergic conjunctivitis was 18.7% and 7.9%, respectively. These results were similar with those of industrial complexes. The prevalence of allergen skin-prick test was 16.4% and the most common allergen was dust mites (27.9-31.6%). This result was lower than those of the industrial complexes (22.5-39.9%) and Gangneung (18.2-24.7%). Conclusion: Further study is required to find the cause of the regional differences in skin prick prevalence.

The purpose of this study was to investigate the skin irritation of bee venom (melittin) in human. Skin irritation test was performed at 0.05% bee venom by 48 h single patch test in 30 healthy volunteers with no skin disease and skin sensitization test was performed by local lymph node assay in animal. In single patch test, 28 human subjects showed no reaction and 2 subjects showed 1+ or 3+ grade skin reaction. In a skin sensitization test of bee venom conducted using mice. no erythema was observed on the dorsal side of mice up to 8 days after application of bee venom on the skin. The results of the average stimulation index by ATP values showed that there was no irritation to the mice skin at 0.005% and 0.01% of bee venom. Therefore, bee venom is not likely to induce a significant skin irritation under 0.05% concentration.

Background: Surgery for reconstruction of defects after surgery should be performed selectively and the many points must be considered. The authors conducted this study to compare the local flap and skin graft by facial location in the reconstruction after resection of facial skin cancer. Methods: The authors performed the study in patients that had received treatment in Department of Plastic Surgery, Gyeongsang National University. The cases were analyzed according to the reconstruction methods for the defects after surgery, sex, age, tumor site, and tumor size. Additionally, the authors compared differences of aesthetic satisfaction (out of 5 points) of patients in the local flap and skin graft by facial location after resection of facial skin cancer by dividing the face into eight areas. Results: A total of 153 cases were confirmed. The most common facial skin cancer was basal cell carcinoma (56.8%, 87 cases), followed by squamous cell carcinoma (37.2%, 57 cases) and bowen's disease (5.8%, 9 cases). The most common reconstruction method was local flap 119 cases (77.7%), followed by skin graft 34 cases (22.3%). 86 patients answered the questionnaire and mean satisfaction of the local flap and skin graft were 4.3 and 3.5 (p=0.04), respectively, indicating that satisfaction of local flap was significantly high. Conclusion: When comparing satisfaction of patients according to results, local flap shows excellent effects in functional and cosmetic aspects would be able to provide excellent results rather than using a skin graft with poor touch and tone compared to the surrounding normal skin.
