Purpose: Heterotopic calcification is the abnormal deposition of calcium salts in tissues other than bone and enamel, and it occurs in the form of dystrophic calcification or metastatic calcification. This deposition can occur under many conditions, but in some rare cases, it may develop in burns and nonhealed scars. It is difficult to treat the combination of heterotopic calcification and ulceration in scar tissues by using conservative therapy and to determine the margin of excision in such cases. Our study proposes the use of intraoperative C-arm-guided mapping of lesions with heterotopic calcification, and adequate excision of ulcers in chronic scars where heterotopic calcification is also observed. Methods: This study included 2 patients and was conducted from January 2010 to July 2010. The first patient was a 63-year-old woman who presented with atypical calcium deposits and chronic ulceration in the lower one-third region of the right leg. The second patient was a 38-year-old man who presented with a nonhealing ulcer that had developed on the right leg 3 months earlier he had a history of 40% scalding burns on the entire body. Surgery is the most reliable method for treating heterotopic calcification therefore, both patients were treated using intraoperative C-arm-guided marginal mapping of heterotopic calcification, followed by release of contracture, and eventually split-thickness skin grafting. Results: Plain radiographs of the leg showed spotty radiopaque areas in the hard part of the scar well superficial to the underlying bones. Histopathological analysis revealed multiple foci of calcified deposits, increased fibrosis, and inflammation in the scar tissue. Surgery-related complications were not observed. Conclusion: C-arm guided excision of calcified scars and the release of contracture can cure nonhealing ulcers and may therefore prevent recalcification.

Objectives : This study investigated the anti-oxidant and anti-inflammatory activities of Changchulgeumryeontang (CCGRT) extract. Methods : The macrophage cell line RAW 264.7 cells were used and MTT assay was performed to measure the cell viabilities at the various concentrations of CCGRT ($25-200{\mu}g/m{\ell}$). Nitric oxide (NO) and prostaglandin $E_2$ ($PGE_2$) were measured in LPS-induced RAW 264.7 cells. Expressions of iNOS, $NF-{\kappa}B$, $IL-1{\alpha}$, $IL-1{\beta}$ and IL-6 were also performed by real-time PCR. The anti-oxidant activities of CCGRT was measured by DPPH radical scavenging activity. Results : 1. there was no cytotoxicity in RAW264.7 cells treated with CCGRT compared to the control. 2. CCGRT treated group significantly inhibited NO and $PGE_2$ production compared to the LPS treated group. 3. CCGRT treated group significantly decreased mRNA expressions of iNOS, $NF-{\kappa}B$, $IL-1{\alpha}$, $IL-1{\beta}$ and IL-6 compared to the LPS treated group. 4. CCGTR was found to have high DPPH free radical scavenging ability. Conclusions : According to the above results, CCGRT may be a potentional choice for the treatment of inflammatory skin disease. Bamboo leaves, as natural dye provide unartificial fresh green, are known for expert property at anti-bacterial, anti-aging, anti-oxidation, skin whitening, moisturizing, peeling of horny, deodorizing and some skin disease like as eczema and inflammation without any allergic response. Also, It is quite common to obtain bamboo leaves from our surrounding but most of them were not in use. On purpose of development the way of recycle with common material to dye, bamboo leaves were used to dye on ramie fiber which is valuable for both of apparel and industrial field and investigated their dyeing property. Using extract from bamboo leaves, dyeing property was examined with dyeability, colorimetric property, light fastness and wash fastness under different conditions by extract density, dyeing time, dyeing method and mordants. As results, dyeing was generally colored in yellowish, K/S Value was shown more higher on hemp fiber than ramie. The most optimum dyeing condition was investigated on pre-mordant treatment, dyeing temperature $60^{\circ}C$, dyeing time 30min and extract density 400%. On the treatment with mordant(Al, Fe, Cu), dyebility on both of hemp and ramie fiber were increased by mordant. Especially with Copper(Cu) as mordant, dyebility was highly increased than any other mordants and dyeings showed in deep color. Also on the fastness test which is the most important to natural dyeing, wash-fastness showed good performance with 4-5 grade but light-fastness was above 4 grade on both of hemp and ramie fiber, lower than wash-fastness overall.
Catechin (CTEC) is well-known as a very powerful antioxidant, containing the effects of anti-inflammation and skin wound healing. In this study, CTEC/${\beta}$-cyclodextrin (${\beta}$-CD) nanoparticles were incorporated into poly(vinyl alcohol) (PVA)/pectin (PT) hydrogel. The composite was designed for the induction of re-epithelializaton in skin wound. CTEC/${\beta}$-CD nanoparticles were prepared by a molecular complex method. The size of the CTEC nanoparticles formed in the hydrogel was in the range of $250{\pm}17.5$ nm. The incorporation efficiency of CTEC in the nanoparticles was 74%. The cumulative amounts of CTEC released from the hydrogel containing CTEC nanoparticles in the buffers of pH7.4 and 5.5 were $86.51{\pm}3.14%$ and $35.95{\pm}2.14%$ of total CTEC loaded in the hydrogel within 72 h, respectively. Also, in the wound healing test, the CTEC nanoparticles-loaded PVA/PT hydrogel showed faster healing of the wound made in rat dorsum than the CTEC gel.

Purpose: Rigid external distraction(RED) is a highly effective technique for correction of maxillary hypoplasia in patients with cleft or syndromic craniosynostosis. Despite many advantages of RED, it also has the problem of relapse as the conventional advancement surgery. Bicoronal approach, that is the common approach to gain access to the craniofacial skeleton, had some morbidity, such as hair loss, sensory loss, wide scar and temporal hollowing. We present our clinical experience of RED distraction with minimal invasive approach and early rigid fixation to overcome these disadvantages. Methods: A 27-year-old female patient with Crouzon's disease underwent Le Fort III osteotomy and RED device application through the minimal invasive direct skin incisions. After the latent period of 5 days, distraction was undertaken until proper convexity and advancement were obtained. During the rigid retention period, inflammation occurred on the right cheek, and proper conservative managements were done including continuous irrigation. To maintain the stability of distraction, early rigid fixation was undertaken on the osteotomy sites through another skin incisions. Preoperative and postoperative orthodontic treatments were performed. Serial photographs and cephalometric radiographs were obtained preoperatively, after distraction and 6 months after distraction. Results: The cephalometric analysis demonstrated postoperatively significant advancement of the maxilla and improvement of facial convexity. After 6-month follow-up period, the maxilla was stable in the sagittal plane and no relapse was found. Facial scars were not noticeable and other deformity and morbidity did not occur. Conclusion: This effective and stable technique will be a good alternative for the patients who need large amount of distraction and for adult patients with severe maxillary hypoplasia or syndromic craniosynostosis.

Background: Adiponectin, an adipokine secreted from adipocytes, affects energy metabolism and also shows anti-diabetic and anti-inflammatory properties. Recent studies have reported that adiponectin plays a role in regulating skin inflammation. Objective: This study aimed to investigate the effect of adiponectin on the expression of filaggrin (FLG) in normal human epidermal keratinocytes (NHEKs). Methods: NHEKs were serum-starved for 6h before being treated with adiponectin. Afterward, cell viability was assessed by MTT assay. We also treated with calcium, interleukin (IL)-4, and IL-13 to provide positive and negative comparative controls, respectively. Gene mRNA expression was quantified using real time reverse transcription polymerase chain reaction, and protein expression was evaluated using Western blot. To evaluate the relationship among mitogen-activated protein kinases (MAPKs), activator protein 1 (AP-1), and FLG, we also treated cells with inhibitors for MAPKs JNK, p38, and ERK1/2. Results: FLG and FLG-2 mRNA expression in NHEKs significantly increased after treatment with $10{\mu}g/ml$ adiponectin. Adiponectin also restored FLG and FLG-2 mRNA expression that was otherwise inhibited by treatment with IL-4 and IL-13. Adiponectin induced FLG expression via AP-1 and MAPK signaling. Conclusion: Adiponectin positively regulated the expression of FLG and could be useful as a therapeutic agent to control diseases related to disrupted skin barrier function.

Tests of stability and toxicity, and clinical evaluation of anti-acne activity suggest that cryptotanshinone, a constituent of the roots of Salvia miltiorrhiza Bunge, is an effective active ingredient for acne vulgaris treatments. Acne vulgaris, called acne or pimples, is the most common disease of the pilosebaceous follicle unit of the skin. It affects nearly 80% of people between the ages of 11 and 30. Approximately 30% of teenagers have acne of sufficient severity to require medical treatment. Acne is a follicular disorder of the skin. It occurs in specialized pilosebaceous units on the face and body. Acne develops when these specialized follicles undergo pathologic alterations that result in the formation of non-inflammatory lesions (comedones) and inflammatory lesions (papules, pustules and nodules). An abnormality of keratinizing epithelium of these follicles, thought to be due to the action of sebum synthesized and secreted by the androgen-sensitive sebaceous glands, leads to inflammation induced by the follicular bacterium Propionibacterium acnes. Therapy involves treatments that modify these pathogenic factors and includes drugs with antikeratinizing, antibacterial and antiseborrheic actions. Acne vulgaris is a very frequent disease, seen primarily in adolescents, involving the sebaceous follicles. Acne vulgaris is characterized by a great variety of clinical inflammatory and non-inflammatory lesions: comedones, papules, pustules, nodules, cysts and scars. Acne vulgaris is a multi-factorial disease. Although its pathogenicity is unclear, extensive studies have shown that hyperseborrhea, superinfection by P. acnes and endocrinologic androgenic changes play a role in the development of acne vulgaris.

Background The result of illicit polymer injection is chronic inflammation with foreign-body granuloma (FBG) formation. Treatment can be divided into medical and surgical. Some patients develop severe complications with need surgical treatment. This study aims to describe patients who underwent surgical removal of the FBGs and autoimmune/inflammatory syndrome induced by adjuvants (ASIA); additionally, we evaluated the quality of life after surgery. Methods In this retrospective single-center study, the authors examined data of patients who underwent surgical removal of FBG caused by illicit polymer injection for cosmetic purposes and confirmed ASIA from 2015 to 2020 by three different surgical approaches. Descriptive summary statistics were reported on patient demographics, presenting symptoms and clinical examination features, treatment strategies, histopathology reports and quality of life. Results The cohort included 11 female patients with FBGs and ASIA. The most affected anatomical zones were the combination of gluteal region, thighs and legs (40%); and thighs with legs (20%). Main presentation was: skin hyperpigmentation (90.9%), skin induration (63.6%), chronic fatigue (63.6%), and ulcers (36.4%). Surgical modalities consisted of: ultrasonic-assisted liposuction in four patients (36.4%); open en bloc excision and primary closure in four patients (36.4%); and open en bloc excision and microsurgical reconstruction in three patients (27.2%). The postoperative quality of life visual analog scale score was 83.9. Conclusions ASIA treatment represents a challenge for the plastic surgeon. Adequate surgical treatment emphasizing, when possible, the total or near-total resection of the FBG must be performed to improve ASIA evolution.
Hair loss is one of the most common chronic diseases, with a detrimental effect on a patient's psychosocial life. Hair loss results from damage to the hair follicle (HF) and/or hair regeneration cycle. Various damaging factors, such as hereditary, inflammation, and aging, impair hair regeneration by inhibiting the activation of hair follicle stem cells (HFSCs) and dermal papilla cells (DPCs). Formyl peptide receptor 2 (FPR2) regulates the inflammatory response and the activity of various types of stem cells, and has recently been reported to have a protective effect on hair loss. Given that stem cell activity is the driving force for hair regeneration, we hypothesized that FPR2 influences hair regeneration by mediating HFSC activity. To prove this hypothesis, we investigated the role of FPR2 in hair regeneration using Fpr2 knockout (KO) mice. Fpr2 KO mice were found to have excessive hair loss and abnormal HF structures and skin layer construction compared to wild-type (WT) mice. The levels of Sonic hedgehog (Shh) and β-catenin, which promote HF regeneration, were significantly decreased, and the expression of bone morphogenetic protein (Bmp)2/4, an inhibitor of the anagen phase, was significantly increased in Fpr2 KO mice compared to WT mice. The proliferation of HFSCs and DPCs was significantly lower in Fpr2 KO mice than in WT mice. These findings demonstrate that FPR2 impacts signaling molecules that regulate HF regeneration, and is involved in the proliferation of HFSCs and DPCs, exerting a protective effect on hair loss.

Background In implant-based breast reconstruction, acellular dermal matrix (ADM) is essential for supporting the inferolateral pole. Recent studies have compared non-sterilized freeze-dried ADM and sterilized pre-hydrated ADM, but have not assessed whether differences were attributable to factors related to sterile processing or packaging. This study was conducted to compare the clinical outcomes of breast reconstruction using two types of sterile-processed ADMs. Methods Through a retrospective chart review, we analyzed 77 consecutive patients (85 breasts) who underwent tissue expander/implant breast reconstruction with either freeze-dried ADM (35 breasts) or pre-hydrated ADM (50 breasts) from March 2016 to February 2018. Demographic variables, postoperative outcomes, and operative parameters were compared between freeze-dried and pre-hydrated ADM. Biopsy specimens were obtained for histologic analysis. Results We obtained results after adjusting for variables found to be significant in univariate analyses. The total complication rate for freeze-dried and pre-hydrated ADMs was 25.7% and 22.0%, respectively. Skin necrosis was significantly more frequent in the freeze-dried group than in the pre-hydrated group (8.6% vs. 4.0%, P=0.038). All other complications and operative parameters showed no significant differences. In the histologic analysis, collagen density, inflammation, and vascularity were higher in the pre-hydrated ADM group (P=0.042, P=0.006, P=0.005, respectively). Conclusions There are limited data comparing the outcomes of tissue expander/implant breast reconstruction using two types of sterile-processed ADMs. In this study, we found that using pre-hydrated ADM resulted in less skin necrosis and better integration into host tissue. Pre-hydrated ADM may therefore be preferable to freeze-dried ADM in terms of convenience and safety.
