The purpose of this study was to find the characteristics of lateral crphalogram of skeletal class III malocclusion patients to whom orthognathic surgery was essential. For this study 37 patients with skeletal class III and going to treat or be treated orthognathic surgery(age 7-17) were selected to experimental group and 56 people with normal occlusion (age 8-13) were selected to normal group and the two groups were evaluated and statistically analyzed and the results were as follows 1. In comparison of experimental group and normal group in prepubertal group, there were significant differences in ANS-U1/Me-L1, Mx. Length/Mn. Length, S-N/Go-Me, Wits, ANB, SN-Pog, IMPA, Facial Convexity, APDI (p<0.05) 2. In comparison of experimental group and normal group in pubertal group, there were significant differences in ANS-U1/Me-L1, S-Go/N-Me, Mx.Length/Mn.Length, S-N/Go-Me, Wits, Saddle Angle, SNB, ANB, SN-Pog, IMPA, Interincisal Angle, Facial Convexity, APDI (p<0.05) 3. Among items showing characteristics of skeletal class III malocclusion, there were no significant differences between prepubertal group and pubertal group in other items except Mx. Length/Mn. Length,APDI (p<0.05) 4. The significant correlationship was the highest between Saddle Angle and SNB, SN-Pog and SNB, ANB and Facial Convexity in experimental group
Purpose: The first objective of this study was to compare the upper midface morpholgy, focusing on the soft tissues, between skeletal Class III maloccusion patients with midfacial depression and the norm. The second objective was to estimate and analyze the change in the upper midface soft tissues following surgical correction with maxillary advancement by Lefort I osteotomy and mandibular setback by bilateral sagittal split osteotomy (BSSRO). Methods: The samples consisted of 34 adult patients (15 males and 12 females) with an average age of 21 years, who had severe anteroposterior discrepancy with midfacial depression. These patients had received presurgical orthodontic treatment and surgical treatment which consisted of simultaneous Lefort I osteotomy and BSSRO. Results: The correlation coefficient between changes in maxillary advancement and changes in Or' (soft tissue orbitale) was 0.599 (p < 0.05). Change in maxillary plane angle and vertical change of the maxilla were not correlated with the change in Or' (p < 0.05). The ratio of soft tissue change in Or' to maxillary advancement was 43.57 %, and 81.54 % in Sn. Regression equations between maxillary movement and Or' were devised. The $r^2$ value was 0.476. Conclusions: The majority of measurements in the upper midface in skeletal Class III maloccusions when compared to the norm, showed significant differences. In Class III malocclusion with midfacial depression, maxillary advancement produces soft tissue change in the upper midface.

The conventional treatment of skeletal class III malocclusion has been focused on the application of orthopedic forrce primarily to the mandible. Thus moderate Class III malocclusions can be corrected by the anterior displacement of the maxilla and maxillary dentition, possibly by restricting the growth of the mandible or by changing its direction. The patients having skeletal Class III malocclusion were treated with removable appliance & fixed appliance for detailed tooth movement and the following results were observed: 1. The anterior crossbite was corrected. 2. The forward and downward growth of the maxillary complex was obtained. 3. The lingual tipping of the mandiblar incisors was performed and the mandible was rotated in the clockwise direction.

In children with Class III malocclusion, it is important to identify whether the etiology is functional, dental, skeletal. FR-III developed by Rolf Frankel in 1970, has been used during deciduous, mixed, and early permanent dentition to correct class III malocclusion characterized by maxillary skeletal retrusion. According Frankel, the vestibular shields and upper labial pads act to counteract the forces of the surrounding musculature that restrict forward maxillary development and cause a retrusion in maxillary tooth position. This can achieve favorable developments with the basal bone, teeth and alveolar bone. We report FR-III that can be applicated in cases of early mixed dentition with mild maxillary deficiency and deepbite before the eruption of permanent maxillary incisor.

Objective: To classify facial asymmetry (FA) phenotypes in adult patients with skeletal Class III (C-III) malocclusion. Methods: A total of 120 C-III patients who underwent orthognathic surgery (OGS) and whose three-dimensional computed tomography images were taken one month prior to OGS were evaluated. Thirty hard tissue landmarks were identified. After measurement of 22 variables, including cant (°, mm), shift (mm), and yaw (°) of the maxilla, maxillary dentition (Max-dent), mandibular dentition, mandible, and mandibular border (Man-border) and differences in the frontal ramus angle (FRA, °) and ramus height (RH, mm), K-means cluster analysis was conducted using three variables (cant in the Max-dent [mm] and shift [mm] and yaw [°] in the Manborder). Statistical analyses were conducted to characterize the differences in the FA variables among the clusters. Results: The FA phenotypes were classified into five types: 1) non-asymmetry type (35.8%); 2) maxillary-cant type (14.2%; severe cant of the Max-dent, mild shift of the Man-border); 3) mandibular-shift and yaw type (16.7%; moderate shift and yaw of the Man-border, mild RH-difference); 4) complex type (9.2%; severe cant of the Max-dent, moderate cant, severe shift, and severe yaw of the Man-border, moderate differences in FRA and RH); and 5) maxillary reverse-cant type (24.2%; reverse-cant of the Max-dent). Strategic decompensation by pre-surgical orthodontic treatment and considerations for OGS planning were proposed according to the FA phenotypes. Conclusions: This FA phenotype classification may be an effective tool for differential diagnosis and surgical planning for Class III patients with FA.
Objective: The purpose of this retrospective study was to investigate short-term and long-term skeletodental outcomes of Class III activator treatment. Methods: A Class III activator treatment group (AG) comprised of 22 patients (9 boys, 13 girls) was compared with a Class III control group (CG) comprised of 17 patients (6 boys, 11 girls). The total treatment period was divided into three stages; the initial stage (T1), the post-activator treatment or post-mandibular growth peak stage (T2), and the long-term follow-up stage (T3). Cephalometric changes were evaluated statistically via the Mann-Whitney U-test and the Friedman test. Results: The AG exhibited significant increases in the SNA angle, ANB angle, Wits appraisal, A point-N perpendicular, Convexity of A point, and proclination of the maxillary incisors, from T1 to T2. In the long-term follow-up (T1-T3), the AG exhibited significantly greater increases in the ANB angle, Wits appraisal, and Convexity of A point than the CG. Conclusions: Favorable skeletal outcomes induced during the Class III activator treatment period were generally maintained until the long-term follow-up period of the post-mandibular growth peak stage.
This study has been performed to determine whether significant differences in the maxillary basal bone pattern exist between skeletal class II malocclusion and normal occlusion. Materials for the skeletal Class III sample consisted of lateral cephalometric roentgenograms and maxillary cast models of 29 adult individuals, 15 males and 14 females. The average age was 19.75 years with a range from 16.4 to 29.1 years. A normal control sample consisted of lateral cephalometric roentgenograms and maxillary cast models of 24 adult individuals, 13 males and 11 females. The average age was 24.25 years with a range from 20.8 to 29.4 years. The results of this study can be summarized and concluded as like follows. 1. In comparing sexual difference of maxillary basal bone morphology in skeletal Class III malocclusion, the following Parameters of males were found to be significally larger than those of females : inter first premolar width, inter molar width, oblique canine height, oblique molar height and maxillary basal bone perimeter. 2. In comparing sexual difference of maxillary basal bone morphology in normal occlusion, the following parameters of males were found to be significally larger than those of females : inter canine width, inter first and second premolar width, inter molar width, oblique canine height and oblique molar height. 3. In comparing maxillary basal bone morphologic difference between skeletal Class III malocclusion md normal occlusion in males, the following parameters were found to be significally larger in normal occlusion : inter canine width, inter canine height, inter molar height, oblique canine height and oblique molar height. 4. In comparing maxillary basal bone morphologic difference between skeletal Class III malocclusion and normal occlusion in females, the following parameters were found to be significally larger in normal occlusion : inter canine height, inter molar height, oblique canine height, oblique molar height and maxillary basal bone perimeter.
The purpose of this study was to investigate the variations of the soft tissue profile contour in relation to the skeletal patterns and to confirm the correlation of soft tissue angles to the sagittal or vertical skeletal elements. Lateral cephalograms of 79 cases which were older than 17 years in age, were traced and statistically analyzed. The obtained results were as follows : 1. Nasolabial, interlabial, lower lip, mentolabial, and symphyseal angles showed the significant differences between skeletal Class II and skeletal. Class III group, but nasofrontal, upper lip, mentolabial, and symphyseal angles showed the significant differences between high angle and low angle group. 2. ANB value showed the significant positive correlation to nasolabial, symphyseal, and interlabial angles and the significant negative relation to lower lip angle. 3. SN-GoMe value showed the significant positive correlation to mentolabial, symphyseal, nasofrontal, and upper lip angles. 4. Soft tissue profile contour in SK. Class II group showed greater accordance to SN-GoMe value than those in SK. Class III group.
The purpose of this study was to evaluate growth changes and skeletal characteristics of Korean children with Class III malocclusions from 10 to 14 years of skeletal age. Radiographs of 60 children with Class III malocclusion and 60 normal controls were assessed. Both groups were subdivided into 6 samples according to sex and skeletal age. Skeletal age was assessed using handwrist X-rays using the Greulich and Pyle norms. The Krogman-Walker plane (occipitale-maxillon) through Sella was used as a reference plane in this study with x-axis perpendicular to the x-axis. Sir Student t-tests were conducted to compare the control group with the Class III group according to each gender a:nd age. The characteristics of Class III malocclusion group compared to the control group included shorter anterior and posterior cranial base, shorter and retrusive maxilla, forger mandible, increased molar-incisor distance, retroclined lower incisors, labially proclined upper incisors, and anteriorly located mandibular molar, smaller upper and middle facial depth, and larger lower facial depth. Landmarks representing facial depth, size of maxilla and mandible, and their AP relationship including anterior facial height indicate that growth characteristic was determined early in life. But growth Pattern of cranial base and some of the dental landmarks showed progressive divergence between Control and Class III groups with age. The position of the posterior border of the mandible was found to be significantly forward in both females and males by the age of 14 and at the anterior border in males and females at all ages. Hyperdivergent mandibular plane, changes in anterior segment of mandible, small anterior cranial base, and decrease in cranial base flexure was also noted.
Numbers of postulations lie on the difference of integumental changes with two major surgical remedies of one jaw vs. two jaw surgery in skeletal Class III malocclusion. Accordingly it was the aim of the study to elucidate the skeletal profile changes with an accompanying disposition of soft tissues, consequently to yield the correlation and ratio of soft tissue changes with two types of surgical procedures, which in turn make it possible to predict the soft tissue outcomes by means of assembled regression equations. Cephalometric headfilms of fifty two adult skeletal Class III comprised of 26 maxillary advancement by Le Fort I osteotomy and mandibular setback by sagittal split ramus osteotomy simultaneously (double jaw surgery, group A), 26 mandibular setback alone (one jaw surgery, group B) were statistically analyzed. Group A manifested 72.4% soft tissue advancement to skeletal changes in the upper lip area, while group B appeared to have no statistically significant changes. The nasolabial angle showed more increment in group A than in group B, whereas the mentolabial angle illustrated more reduction in group B. The backward movement of soft tissue pogonion to skeletal change revealed 98% in group A, and 109% in group B. The double jaw surgery group characteristically revealed remarkable integ umental change in the upper lip area, while the one jaw surgery had major effects in the lower lip and soft tissue pogonion areas.
