A maxillary canine with two separated root canals: a case report (두 개의 근관을 가진 상악 견치: 증례보고)

  • Maxillary canines have less anatomical diversities than other teeth. They usually have a single root and root canal. This report describes an endodontic treatment of a maxillary canine with two separated root canals which have not been reported through the demonstration of radiography and computerized tomography (CT). Even though appropriated endodontic treatment has been performed, the severe pain could happen due to lack of consideration of anatomical variations of the teeth. Therefore, the clinicians should be well aware of the possibility of anatomical variations in the root canal system during endodontic treatment even if the number of root canals is obvious such as in this case.

  • Objectives: To determine the optimal timing for post space preparation of root canals sealed with epoxy resin-based AH Plus sealer in terms of its polymerization and influence on apical leakage. Materials and Methods: The epoxy polymerization of AH Plus (Dentsply DeTrey) as a function of time after mixing (8, 24, and 72 hours, and 1 week) was evaluated using Fourier transform infrared (FTIR) spectroscopy and microhardness measurements. The change in the glass transition temperature ($T_g$) of the material with time was also investigated using differential scanning calorimetry (DSC). Fifty extracted human single-rooted premolars were filled with gutta-percha and AH Plus, and randomly separated into five groups (n = 10) based on post space preparation timing (immediately after root canal obturation and 8, 24, and 72 hours, and 1 week after root canal obturation). The extent of apical leakage (mm) of the five groups was compared using a dye leakage test. Each dataset was statistically analyzed by one-way analysis of variance and Tukey's post hoc test (${\alpha}=0.05$). Results: Continuous epoxy polymerization of the material with time was observed. Although the $T_g$ values of the material gradually increased with time, the specimens presented no clear $T_g$ value at 1 week after mixing. When the post space was prepared 1 week after root canal obturation, the leakage was significantly higher than in the other groups (p < 0.05), among which there was no significant difference in leakage. Conclusions: Poor apical seal was detected when post space preparation was delayed until 1 week after root canal obturation.

Fracture incidence of Reciproc instruments during root canal retreatment performed by postgraduate students: a cross-sectional retrospective clinical study

  • Objectives: To evaluate the fracture incidence of Reciproc R25 instruments (VDW) used during non-surgical root canal retreatments performed by students in a postgraduate endodontic program. Materials and Methods: From the analysis of clinical record cards and periapical radiographs of root canal retreatments performed by postgraduate students using the Reciproc R25, a total of 1,016 teeth (2,544 root canals) were selected. The instruments were discarded after a single use. The general incidence of instrument fractures and its frequency was analyzed considering the group of teeth and the root thirds where the fractures occurred. Statistical analysis was performed using the χ2 test (p < 0.01). Results: Seven instruments were separated during the procedures. The percentage of fracture in relation to the number of instrumented canals was 0.27% and 0.68% in relation to the number of instrumented teeth. Four fractures occurred in maxillary molars, 1 in a mandibular molar, 1 in a mandibular premolar and 1 in a maxillary incisor. A greater number of fractures was observed in molars when compared with the number of fractures observed in the other dental groups (p < 0.01). Considering all of the instrument fractures, 71.43% were located in the apical third and 28.57% in the middle third (p < 0.01). One instrument fragment was removed, one bypassed, while in 5 cases, the instrument fragment remained inside the root canal. Conclusions: The use of Reciproc R25 instruments in root canal retreatments carried out by postgraduate students was associated with a low incidence of fractures.

Comparative Study on the Ability of Instruments to Maintain Original Canal Curvature of Continuous rotary System and Single File System (Continuous rotary system과 single file system의 만곡 근관 형태 유지능에 대한 비교 연구)

  • Shaping the root canal system to maintain original canal curvature is essential to clinical success in endodontic treatment. Opposed to most root canals that are curved, endodontic instruments are made from straight metal blanks. They have a tendency of straightening the root canal during preparation and frequently result in procedural errors. A new treatment method to maintain original canal curvature during shaping has been introduced for preventing procedural errors. The aim of this study was to compare the ability of instruments to maintain original canal curvature of continuous rotary system and single file system. Thirty ISO 15, 0.02 taper, Endo Training Blocks(Dentsplay Maillefer) were used. Specimens were assigned to 1 of 3 groups for shaping: specimens in group 1 were shaped with ProFile #20/.06 at the WL. Specimens in group 2 were shaped with Mtwo #35/.04 at the WL. Specimens in group 3 were shaped with WaveOne Primary reciprocating files at the WL after the glide path was achieved with PathFile. Pre- and postinstrumentation digital images were superimposed and processed with Matlab r2010b(The MathWorks Inc, Natick, MA) software to analyze the curvature-radius ratio(CRr), representing canal curvature modification. Data for comparison on the ability of instruments to maintain original canal curvature depending on each Ni-Ti file were analyzed with 1-way ANOVA(P<.05). Data for comparison on the ability of instruments to maintain original canal curvature depending on each Ni-Ti file system were analyzed with independent t-test(P<.05). A statistically significant difference(P<0.05) was noted on each Ni-Ti file. ProFile and WaveOne instrumentations maintained the original canal curvature significantly better(P<0.05) than Mtwo file. There were no significant difference(P>0.05) between continuous rotary system and single file system. Under the conditions of this study, ProFile and WaveOne instruments maintained the original curvature significantly better than Mtwo file and were less modification of the canal curvature compared. There was no significant difference between continuous rotary system and single file system in shaping of simulated canals. As clinical practitioners, it may be advantages to use hybrid approach when root canal shapes depending on the design and usage of Ni-Ti files.

  • This paper presents experimental results of the frequency offset locking of He-Ne lasers and the stability analysis. The master laser is free running, and the slave laser is a single-mode operating laser. The frequency difference of two lasers is stabilized to 200 MHz which can be synchronized using PLL servo. The measured beat frequency between two lasers was 200.004 MHz ${\pm}$ 0.15 MHz. The square root of Allan variance as a measure of stability in time domain is also measured. The long-term stability of the beat was worse than sort-term stability. With a gate time $\tau=1000\;s$, the square root of Allan variance was about 1 GHz. The results of the square root of Allan variance of the stabilized beat signal was a gate time of $\tau=1000\;s$, the square root of Allan variance was about 1.5 kHz. The long-term stability was improved by more than several hundred times compared with that without the stabilization.

EFFECT OF STRENGTH MISMATCH AND DYNAMIC LOADING ON THE DUCTILE CRACK INITIATION FROM NOTCH ROOT

  • It has been well known that ductile fracture of steels is accelerated by triaxial stresses. The characteristics of ductile crack initiation in steels are evaluated quantitatively using two-parameters criterion based on equivalent plastic strain and stress triaxiality. It has been demonstrated by authors using round-bar specimens with circumferential notch in single tension that the critical strain to initiate ductile crack from specimen center depends considerably on stress triaxiality, but surface cracking of notch root is in accordance with constant strain condition. In order to evaluate the stress/strain state in the specimens, especially under dynamic loading, a thermal, elastic-plastic, dynamic finite element (FE) analysis considering the temperature rise due to plastic deformation has been carried out. This study provides the fundamental clarification of the effect of strength mismatching, which can elevate plastic constraint due to heterogeneous plastic straining, loading mode and loading rate on critical condition to initiate ductile crack from notch root using equivalent plastic strain and stress triaxiality based on the two-parameter criterion obtained on homogeneous specimens under static tension. The critical condition to initiate ductile crack from notch root for strength mismatched bend specimens under both static and dynamic loading would be almost the same as that for homogeneous tensile specimens with circumferential sharp notch under static loading.

Antibody-Mediated Resistance to Rhizomania Disease in Sugar Beet Hairy Roots

  • Agrobacterium rhizogenes-mediated transformation of sugar beet hairy roots expressing single-chain variable fragment (scFv) was exploited to evaluate the efficacy of four antibody-based constructs for interfering with the Beet necrotic yellow vein virus infection. The scFv specific to a major coat protein of virus, p21, was targeted to various cellular compartments including the cytosol (pIC and pICC constructs), apoplast (pIA), and mitochondrion (pIM). After mechanical virus inoculation, most of the hairy root clones expressing scFv in the cytosol displayed low virus titers while the majority of transgenic hairy root clones accumulated antibody in outer membrane of mitochondria or apoplast were infected. This hairy root system provided an efficient and rapid approach to initially investigating root disease resistance like rhizomania prior to transform whole recalcitrant plants such as sugar beet.

AN EXPERIMENTAL STUDY ON THE EFFICACY OF DIFFERENT IRRIGATION METHODS AND IRRIGATION SOLUTIONS IN INFECTED ROOT CANALS (근관세척액과 방법에 따른 세균제거 효과에 관한 실험적 연구)

  • The purpose of this study was to evaluate the disinfection efficacy of root canal irrigation method and irrigation solution in infected root canals. Access cavity were prepared in single-rooted extracted human teeth and each canals were prepared with step-back method and sterilized with ethylene oxide gas. Prepared teeth were experimentally inoculated in the canals for 30 minutes with four aerobes(Streptococcus mutans, Stapylococcus aures, Streptococcus sanguis, and E. coli) and two anaerobes(Streptoccus intermedius, and Clostrium perfringens). Normal saline, 3% $H_2O_2$, and 3% NaOCl were used as irrgation solutions. And root canals were irrigated with each irrigation solutions manually with disposible syringe or ultrasonic instument. (Apoza Uitrasonc, Taiwan). The irrigation effect on the eradication of S. aures and S. intermedius with normal saline was not similar between manual method and ultrasonic method. No significant differences were observed in the eradication of E. coli and C. perfringens among the irrigation solution and irrespective of the irrigation methods. In S. aurues and S. mutans, there was somewhat different eradication effect according to irrigation solutions, but there was no significant difference between manual method and ultrasonic methods. These results suggest that ultrasonic irrigation method with saline is more effective in some strains than manual irrigation.

AN in vitro STUDY OF THE CORONAL LEAKAGE OF TWO ROOT CANAL FILLING TECHNIQUE USING ANAEROBIC BACTERIAL LEAKAGE MODEL (근관 충전된 치아에서의 혐기성세균을 이용한 치관부 미세누출측정에 관한 연구)

  • The aim of this in vitro study was to investigate the coronal leakage of obligate anaerobes into root canals obturated with two different techniques. 48 extracted human teeth with straight, single root canals were prepared with crown-down technique with Profile under copious irrigation until the master apical file was size 40. The teeth were divided randomly into experimental groups (40 teeth) and control groups (8 teeth). In the experimental groups, 20 teeth were obturated with lateral condensation and other 20 teeth were obturated with continuous wave technique with System B. Coronal leakage of two root canal filing technique was evaluated using anaerobic bacterial leakage model with Fusobacterium nucleatum(ATCC 25586) for 60 days. The results were as follows 1. The incidence of bacterial leakage of experimental groups was 65% in group 1 (lateral condensation) and 60% in group 2 (continuous wave technique with System B). This difference was not statistically significant (P>0.05). 2. There was no statistically significant difference(P>0.05) in leakage score between group 1 (lateral condensation) and group2 (continuous wave technique with System B).

Periodontal attachment loss of extracted teeth for periodontal reasons (발거치에 나타난 부착상실의 양상에 대한 연구)

  • The factors influencing long-term prognosis of teeth must be carefully considered. Among these, attachment level is strongly associated with tooth loss. The purpose of the present study was to estimate pattern of attachment loss based on attachment area in extracted teeth. 197 satisfied the criteria for assessment after staining. The protocol described by Waerhaug(l975) was performed. An indir ect method, based on digital image abstracted from digital camera and digital imaging software program, was used to calculate the root surface area and the attachment loss area. The data were analysed using SPSS. Except maxillary central incisior and mandibular canine, no statistical significant differences between each root surfaces were observed in anterior teeth. In posterior teeth, statistical significant differences in palatal surface of maxillary molar and mandibular molar compared with others were observed. Statistical significant difference in buccal surface compared with others was lowly observed in single and multi rooted. This study did not reveal progressive loss pattern of attachment area in each root surface but clarified root surface that has relative high loss rate of attachment area at extraction. Thus understanding this pattern of attachment loss is helpful for dentist to treat the periodontitis.
