A five-year-old spayed female Labrador retriever presented with right forelimb lameness one day earlier. Radiographs and computed tomography of the right shoulder joint revealed conspicuous enthesophytes, bone cyst, and bone fragment at the intertubercular groove. Magnetic resonance imaging showed a loss of continuity of the biceps tendon and inhomogeneous hyperintense signal of the surrounding soft tissue and moderate synovial fluid on T2- and proton density-weighted images with contrast enhancement on the postcontrast T1-weighted images. The dog was diagnosed with tenosynovitis and biceps tendon rupture with osteoarthritis based on a comprehensive evaluation of the computed tomography and magnetic resonance imaging features.

Background: In some patients with rotator cuff tear (RCT), the axial view of magnetic resonance imaging (MRI) shows subtle posterior decentering (PD) of the humeral head from the glenoid fossa. This is considered to result from a loss of centralization that is typically produced by rotator cuff function. There are few reports on PD in RCT despite the common occurrence of posterior subluxation in degenerative joint disease. In this study, we investigated the effect of PD in arthroscopic rotator cuff repair (ARCR). Methods: We conducted a retrospective study of consecutive patients who underwent ARCR at our institute and were followed-up for at least 1 year. PD was identified as a 2-mm posterior shift of the humeral head relative to the glenoid fossa in the axial MRI view preoperatively. The tear size and fatty degeneration (FD, Goutallier classification) were also evaluated using preoperative MRI. Retears were evaluated through MRI at 1 year postoperatively. Results: We included 135 shoulders in this study. Ten instances of PD (including seven retears) were observed preoperatively. Fifteen retears (three and 12 retears in the small/medium and large/massive tear groups, respectively) were observed postoperatively. PD was significantly correlated with tear size, FD, and retear occurrence (p<0.01 each). The odds ratio for PD in retears was 34.1, which was greater than that for tear size ≥3 cm and FD grade ≥3. Conclusions: We concluded that large tear size and FD contribute to the occurrence of PD. Furthermore, PD could be a predictor of retear after ARCR.

In fencing, athletes must hit opponents by the sword of fencing, so they need long reach between their foot and their tip of the sword. We thought the motion of shoulder joint, mainly scapula, is one of the important roles in this action and noticed the difference of right and left side in muscles around scapula caused by this action. We evaluated these muscles with MRI imaging in 5 high-level athletes of fencing who aims at the next Olympics (3 men and 2 women, their average age is 21 years). Besides we observed their physical measurement related with these muscles. We reported the characteristics of these results.

An anterior dislocation of the elbow without a fracture of the olecranon is an extremely rare injury. This paper reports a 36-year-old male who stumbled and fell on his outstretched hand during a soccer game. The anteroposterior and lateral radiographs indicated a simple anterior dislocation of the elbow, which was reduced using a closed method. The elbow joint was stable in the range of motion, but the sensation of the two ulnar digits was still reduced. MRI was useful for the identification of the pathoanatomy. At the follow-up examination three months after the initial trauma, the hypesthesia has fully recovered and the patient regained the full range of the elbow and forearm motion without pain and instability. After 18 months, the patient had a normal elbow function, and could play various sports. If an anterior elbow dislocation is detected early, a closed reduction with careful pathoanatomical considerations would be successful.

기관지기원낭종은 primitive foregut의 발생학적 이상에 의해 형성되는 드문 질환이며, 주로 폐실질과 종격동에 발생한다. 피부나 피하조직에서 발생한 기관지기원낭종은 흔하지 않으며, 이 중 견갑부에 발생한 것은 매우 드문 것으로 알려져 있다. 저자들은 20개월 된 남아의 좌측 견갑부에 발생한 기관지지원 낭종 1예를 보고하고자 한다. 환아는 피부과에서 절개 생검 및 소파술을 시행 받았고, 표피 낭종으로 진단되었다. 하지만 상흔 치유 도중 추가적인 낭성 종괴가 촉진되었다. MRI상 피하조직에 경계가 분명한 낭성 종괴가 관찰되었다. 절제된 낭종은 점액세포를 동반하는 pseudostratified ciliated columnar epithelium로 피복되어 있었으며, 기관지기원낭종으로 진단되었다.

A 29-year-old man visited our clinic owing to a persistent swelling in the anterior part of the left elbow joint that began one year ago. Through magnetic resonance imaging (MRI), we observed tenosynovitis with multiple rice bodies, and so we performed an excisional biopsy and tenosynovectomy. Through pathology and culture tests, we identified tuberculosis in the tissue biopsy that we harvested intraoperatively. Following the anti-tuberculosis medication relieved the patient's symptoms without recurrence. Since tuberculosis of the elbow occurs only rarely, and the symptoms mimic those of rheumatoid synovitis or of non-specific chronic synovitis, early diagnosis and appropriate treatment are often delayed. The authors report this rare case of tuberculous tenosynovitis of the elbow with a review of the relevant literature.

Synovial chondromatosis is a rare, benign, monoarticular arthropathy that is characterized by the development of highly cellular, metaplastic cartilaginous nodules in the synovial membrane. It commonly affects larger joints such as the knee, elbow, wrist, shoulder, and hip. Synovial chondromatosis of the temporomandibular joint(TMJ) is rare. Moreover, the temporal involvement of synovial chondromatosis without connection with joint is greatly rare. A 44-year-old women had experienced pain of the right TMJ area and limitation of mouth opening. MRI and CT revealed multiple calcified loose bodies and widening in right upper joint space of TMJ and osteolytic lesion in right temporal bone. Treatment consisted of removal of multiple loose bodies, resection of the osteolytic lesion through the preauricular approach. She was diagnosed with primary transitional synovial chondromatosis of TMJ with involvement of temporal bone. In spite of remaining of the loose bodies, pain and mouth opening improved and there have been no recurrence of signs and symptoms for 5 years follow up.

목적: 흉쇄관절에는 드물게 발생하는 화농성 관절염과 결핵성 관절염에서 조기에 흉쇄 관절의 완전한 절제와 철저한 변연 절제술을 시행하여 그 치료 결과를 보고하고자 한다. 대상 및 방법: 2003년 9월에서 2008년 9월까지 본원에서 흉쇄관절에서 발생한 2예의 화농성 관절염과 1예의 결핵성 관절염에 대하여 수술적 치료를 시행한 환자들 세 예를 대상으로 진단 및 그 임상적 결과를 평가하였다. 결과: 1년 이상 추시한 단순 방사선 소견 및 혈액 검사상 염증의 소견은 호전되었으며, MRI소견상 원위 쇄골단과 흉골에 골부종과 함께 흉쇄 관절에 경도의 활액막염이나 액체 저류 소견 등이 관찰되었으나, 임상 증상은 호전되었으며 1년 이상 추시하여 더 이상의 재발 없이 좋은 결과를 얻을 수 있었다. 결론: 흉쇄관절에서 화농성 관절염과 결핵성 관절염은 드물게 발생하는 질환으로, 진단 시 이미 흉쇄관절 주위의 골로 파급되어 골수염의 소견이 관찰되므로 조기에 흉쇄 관절의 골수염에 대한 완전한 변연 절제술과 철저한 소파술을 조기에 시행하여 만족스러운 결과를 얻을 수 있을 것으로 보인다.

건초 섬유종은 수부나 상지에서 발생하는 양성 연부조직 종양으로 관절내에서 발생하는 경우는 아주 드물다. 대부분 천천히 자라는 섬유성 결절 조직으로 조직학적으로는 산재한 섬유모세포및 치밀한 기질과 미세 혈관세포들로 구성된다. 저자들은 54세 남자환자의 관절와 상완 관절 후상방 관절막에서 기시한 건초 섬유종을 경험하였다. 자기 공명 영상에서 저 강도 신호의 결절성 종물로 관찰되었으며 관절경하에서 절제하여 조직검사로 확진하였다. 관절내 건초 섬유종은 견관절 관절경 시술시 관절 내에서 관찰될 수 있는 종양의 하나로 감별해야 할 것으로 사료된다. 있는 술식으로 전방십자인대 수술시 좋은 치료 선택의 하나로 사료된다.

팔꿉관절은 3개의 뼈대가 조합하여 관절을 이루고 있으며 신체적 활동 및 외력에 의한 이상 시 X-선 등, 방사선 검사로 영상진단을 한다. 팔꿉 관절 선행연구에 의하면 전완부(forearm)의 Z축 거상에 의한 기울기 자세에서 영상평가 기준에 부합하는 새로운 검사법이 발표되어 이에 최적화된 기구를 설계하고 다른 상지 검사에서도 활용 가능한 보조기구를 개발하고자 하였다. 2D 도면 및 3D 모델링 설계로 디자인 후 4부분의 파트로 구분 된 디자인을 ABS 소재의 3D 프린팅 방법으로 출력 후 조립 단계를 거쳐 제품을 완성하였다. 개발된 보조기구는 Z축 거상 기울기 4단계 각도(0, 5, 10, 15도) 기능과 1도 단위로 360도 회전되어 고정하는 기능으로 구성하였다. 개발된 보조기구는 사용 빈도의 누적 및 신체적 활용에 의한 하중 문제에 대해 구조해석을 통하여 최대 등가응력(equivalent stress) 56.107 Pa, 변위(displacement) 1.6548e-5 mm 의 안전한 물성치 범위로 개발되었다. 또한 개발된 보조기구는 팔꿉관절의 X-선 검사뿐만 아니라 위팔뼈 회전에 의한 어깨 기능검사에서도 활용할 수 있으며 비금속 소재로 MRI, CT 검사에서도 적용이 가능하도록 개발되었다. 개발된 보조기구는 향후 다양한 장치 및 의료영상 검사의 임상 적용을 통하여 검사의 정확성 및 효율화에 기여하리라 판단한다.
