The Clinical Analysis of Short Hospitalization for Patients with Lumbar Disc Herniation (요추 추간판 탈출증 환자의 단기 입원에 대한 임상적 분석)

  • Objective : The short hospitalization or outpatient care for patients undergoing laminectomy is not popular practice in Korea. We evaluated the clinical and economical significance of short hospitalization for patients undergoing microdiscectomy for their lumbar disc herniation. Material and Methods : From March 1999 to December 2000, patients were hospitalized for intended short period hospitalization for lumbar disc surgery. Their radiological and clinical data were analyzed for their clinical and economical results. We compared these data with 20 patients who hospitalized conventionally. Results : Total of 64 patients were evaluated. Three were lost to follow-up. Mean postoperative hospitalization period is 2.0 days(excluding one patient). Excellent to good outcome was achieved in 90.6 % of the patients. They payed about half expenses for hospitalization compared with conventional group. Their mean number of outpatient visit was 3.0. Conclusion : Short hospitalization for lumbar disc surgery is an acceptable option in Korea. It is also economically beneficial while achieving acceptable clinical results.

  • Background : Recently the number of atopic dermatitis patients has increased, but the treatment of atopic dermatitis is not effective, and the recurrence rate of atopic dermatitis is high. Many patients are suffering from pruritus. A new standard treatment system is needed. Objective : This study investigated the effect of Oriental medicine program for atopic dermatitis patients during short term hospitalization. Method : 6 patients were admitted for short term hospitalization program. The program includes Acupuncture, herbal medicine, examination, education, cupping therapy, herbal dressing, exercise and etc. Everyday we evaluated the patients by Severity Scoring Atopic Dermatitis(SCORAD) index and took the photos of lesions, and the patients evaluated themselves by atopic dermatitis diary which consists of emotion, pruritus, sleep loss. Results : Admission duration was 7 to 14 days. The SCORAD scores of them were decreased. Most symptoms of 6 patients were improved. Especially herbal dressing was effective for severe oozing. Subjective scores of atopic dermatitis diary were reduced. Conclusion : We expect that the short term hospitalization program could be helpful for uncontrollable atopic dermatitis patients.

Analysis of Long Term Hospitalization in Korean Medical Hospital and Its Affecting Factors - Based on Usage and consumption of Korean medicine Report In 2014 - (전국 한방병원의 장기입원과 이에 영향을 미치는 요인 - 2014년 한방의료이용 및 한약소비실태조사(보건복지부)를 중심으로 -)

  • Objectives : It was to classify and its affecting factors to the patients of Korean medicine hospital with short term and long term hospitalization. Methods : I focused on long-term hospitalized patients. I was conducted on 344 hospitalized patients among the original data of usage and consumption of Korean medical report in 2014. Among those patients, I have classified them into long term inpatients(131 patients) and short term inpatients(213 patients) based on 16 days of hospitalization. Also multiple regression analysis was conducted to investigate the characteristics of the hospitalization, treatment satisfaction and dissatisfaction, the characteristics of long term hospitalization according to the sociodemographic of the subjects, the top 21 diseases and the distribution of human bodies, side effects and kinds of Korean medicine. Results : There was a statistically significant difference between the short term and long term hospitalized patients due to age, occupation, marital status, all 21 diseases and institutional fees, experience of Korean medical treatment due to traffic accidents. There was no significant difference in gender, education level, residence, income level, type of medical insurance, whether private insurance, type of medical treatment for Korean medicine, medical expenses for car accidents, reason for dissatisfaction with treatment. The number of long term patients at Korean medicine hospitals increased by a statistically significant by age in model 1 where confounding factors were differently controlled. In model 2, the number of long term patients at Korean medicine hospitals increased by a statistically significant by age, among those who earned 5,000,000 Korean won or more, and among those with nerve diseases. The number of long term patients at Korean medicine hospitals decreased by a statistically significant amount among the unemployed and others in model 2. In model 3, the number of long term patients at Korean medicine hospitals increased by a statistically significant by age, among those who earned 5,000,000 Korean won or more, and among those with nerve diseases, while the number decreased by a statistically significant amount among the married. Conclusions : These results suggest that the factors affecting the short term and long term hospitalization of patients with Korean medicine hospital are different from each other. Especially it was significant by age, over 5,000,000won Income per month, nerve disease, but decrease significant married.

Comparison of Ambulatory Care with Hospitalization for a Severe Atopic Dermatitis Patient (중증 아토피피부염 환자의 외래치료와 입원치료 비교)

  • Background : Atopic dermatitis is chronic exzemaous dermatitis and the recurrence rate of atopic dermatitis is high. Many patients are suffering from pruritus. Objective : This study compared ambulatory care with hospitalization for atopic dermatitis treatment of one patient, and investigated the effect of Oriental medicine program for atopic dermatitis patients during short term hospitalization. Method : We treated a 5 year-old boy with uncontrollable atopic dermatitis by ambulatory care and hospitalizaion. Herbal medication and herbal dressing was done samely during ambulatory care and hospitalization. Intensive life management, education for compliance was done only during hospitalization. Results : Even though he had been treated for 5 months by ambulatory care, he still suffered from severe itching, erythema, oozing and insomnia. Admission duration was 10 days. During admission, the SCORAD scores and subjective scores were decreased rapidly. Conclusion : We concluded that effects of the treatments can be different due to compliance of the patient and in that view the short term hospitalization program could be more helpful than ambulatory care for uncontrollable atopic dermatitis patients.

The Impact of Health Service Quality Factors on Patient's Satisfaction According to Duration of Hospitalization (입원기간에 따른 의료서비스 품질요인이 고객만족에 미치는 영향에 관한 연구)

  • The purpose of this study is to identify which factors of medical service quality were considered important by patients based on survey on eight hospitals located in Seoul. We analyzed the difference in impact of health service quality factors between the two groups with short and long inpatient days. In addition, we analyzed which service factors affected the customer satisfaction more differently across the inpatient groups. We found that patient satisfaction was not different between the two groups with short and long inpatient days. But the two groups showed different priority orders of medical service factors that affect patient satisfaction. In the group with long inpatient days, the order of medical service factors are cleanness, speciality, process. In the group with short stay, important factors are speciality, cleanness and convenience of facilities. These findings imply that hospitals should set up different marketing strategies across the different inpatients groups.

Survival Rate of Hospitalized Coal Workers' Pneumoconiosis Patients: According to Characteristics at the Time of Hospitalization (요양 중인 탄광부 진폐증자의 생존율 연구 - 요양 입원시 특성을 중심으로 -)

  • The authors investigated mortality rate of hospitalized CWP(coal workers' pneumoconiosis) patients. Date, which was composed of age, date of hospitalization, date of death, and radiological findings(profusion of small opacity, type of large opacity, tuberculosis, emphysema, pneumothorax, and cardiac abnormality), was gathered from medical charts and chest x-ray films at the time of hospitalization of CWP patients. Among 738 CWP patients, that were entered survey differently and have followed different period, 160 CWP patients were died during different observational period. Mean value of observational period was 203 weeks, and mean age at hospitalization was 51 years. Because of short observational period, mean survival time could not found. There was statistically significant difference of mortality rate between group of small opacity and that of large opacity. In group of small opacity 5 year survival rate was 0.80 and in group of large opacity that was 0.73. And 80 percentile survival time was 57 months in group of small opacity and that was 40 months in group of large opacity.

A Study on Newborn Infection during Hospitalization (일 종합병원 신생아실 감염증에 관한 소고)

  • This study was conducted to evaluate the difference of staphylococcal colonization between lactating mothers and nursery stares. Samples were obtained from the hands and noses of the newborn(admitted to S Univ. hospital's nursery from 1991. 5. 23 to 6. 8), their mothers and nursery staffs. They were cultured, and then the presence/absence of pathogenic staphylococci in them were analyzed by Fisher exact probability test which led to these conclusion of significance as follows: 1. No differences were detected in pathogenic rate of nasal swab culture between breast~fed newborns and formula-fed ones. 2. Delivery type and hospitalization the ones from the brest-fed newborns show high rate of Sta. aureus coa(+). In case of either C-sec delivery or long (more than 4 days) hospitalization, formula-fed ones in case of NSVD or short (less than 3 days) hospitalization. 3. The ones from breast feeding mothers show higher sta. aureus coa(+) than those from nursery staffs, which was of no significance statistically. Considering nursery staffs only, however, the comparison of those with the data in April indicates that the pathogen rate is higher for hand than nose, and Sta aureus coa(+) for hand is far lower in June than in April, which was statistically significant. These results lead us to infer that newborn infection in hospitalization could be largely reduced by maintaining the ordinary hygienes. Such as the handwashing of mothers as well as nursery staffs (directly involved in newborn care) Thus the pre-enterance hand washing of a mother who visits the nursery only for breast feeding (without any other medical responsibility) should be done, like other medical agents, with $0.05\%$ chlorhexidine antiseptics rather than simple soap cleansing; the one 'that is worth emphasizing thoroughly.

A Case Series Report on 11 Patients of Primary Palmar/Plantar Hyperhidrosis in Children and Adolescents Treated with Hospitalization Program of a Hyperhidrosis Clinic (다한증 입원 프로그램을 통해 치료한 소아 및 청소년 수족다한증 11례에 대한 증례보고)

  • Objectives : The purpose of this study was to report the clinical effect of short- term admission program on primary palmar/plantar hyperhidrosis in children and adolescents. Methods : Between January 2011 and January 2012, a total of 11 patients with clinical diagnosis of primary palmar/plantar hyperhidrosis were treated with hospitalization which consisted of iontophoresis, electro-acupuncture, herbal medicine, topical therapy and physical therapy. The effects were evaluated as quality of life with dermatology life quality index (DLQI), severity of discomfort using visual analogue scale (VAS), and the patients' subjective satisfaction. We analyzed the patients' improvement before and after admission treatment, as well as six month later. Results : After treatment, the 11 patients' quality of life and severity of discomfort improved significantly. They also were satisfied with oriental treatments for hyper hidrosis. Six months later, 6 of 11 patients had visited the hyperhidrosis clinic. Their symptoms had improved and remained stable. Conclusions : The short-term admission program is effective on primary palmar/plantar hyperhidrosis. Further studies with a larger number of cases will be needed in the future.

Lobectomy with Video-Assisted Thoracoscopy - 4Cases Report - (비디오 흉강경을 이용한 폐엽절제술;4례 보고)

  • Video thoracoscopic surgery is a new modality that gains acceptance rapidly from thoracic surgeons. We have experienced two left lower lobectomies, one left upper lobectomy & one right upper lobectomy with using video thoracoscopy for the four patients with lung parenchymal disease from July 1992 to February 1993. The post-operative courses were uneventful. The final pathologic diagnosis were sclerosing hemangioma, adenocarcinoma, bronchiectasis, leiomyoma & the post-operative courses were short. These patients needed less analgesics because postoperative pain was reduced markedly, and hospitalization was shortened due to rapid recovery. We would like to prefer video thoracoscopic lobectomy to the lobectomy through standard thoracotomy in uncomplicated patients with simple pulmonary parenchymal diseases.

Minimally Invasive Posterior Lumbar Interbody Fusion with Threaded Fusion Cage(TFC) (Threaded Fusion Cage(TFC)를 이용한 최소 침습적 요추체 후방융합술)

  • Objective : In general, to perform posterior lumbar interbody fusion(PLIF), it has been used more invasive procedure than simple discectomy. However we try to perform PLIF with TFC with smaller invasion almost same as in simple discectomy. This study is about its procedure and clinical results. Materials and Methods : The authors retrospectively analyzed 43 cases of minimally invasive PLIF with TFC from July 1998 to May 2000. Operative procedure, operative complication, change of disc height, blood loss, ambulation time, hospitalization period, clinical success rate, and bony fusion rate were analyzed. Results : 40 patients were capable to walk on the 2nd day of the post-operation. The average hospitalization period is 5.6 days. The average blood loss was 0.19L/level with no transfusion or wound drainage. The height of disc changed from 8.84mm to 13.54mm. Clinical success rate is 95% when evaluated by the Prolo's scale. The complication was delayed wound infection(2) and transient paresthesis(1). The bony fusion was shown in 17 patients (94.4%) out of 18 patients who passed one year. Conclusion : As a result of minimally invasive PLIF, pain was decreased and early ambulation and short hospitalization was possible. Complication was similar or lower than other studies, and the bony fusion rate and clinical success rate were also similar during follow-up.
