A Case of Cholecystotomy in a Dog with Cholelithiasis (개에서 담석제거를 위한 담낭절개술의 일례)

  • A twelve-month old female, shih-tzu, was presented to diagnose a hind limb ataxia to the Animal Teaching Hospital, Gyeongsang National University. Even though a variety of physical and neurological examination was performed, the cause of the ataxia was not understood. In serum analysis, an elevated levels of ALT, $\gamma$-GTP and LDH were found. A gallstone was subsequently observed at the right 9 and 10th intercostal region on ultrasonography and radiography. Cholelithiasis was diagnosed, and then cholecystotomy was undertaken. No complications were occurred after the operation. The gallstone removed by cholecystotomy was composed of calcium, carbonate and oxalate, identified by a black pigment gallstone.

  • A 1-year and 6-month-old, intact male, Shih-tzu dog was referred to the Konkuk University Veterinary Teaching Hospital, due to ascite, melena, severe anemia, and polyuria/polydipsia. Complete blood count showed moderate leukocytosis, lymphocytosis, monocytosis, and microcytic hypochromic nonregenerative anemia. On serum biochemistry profiles, hypoalbuminemia and hypoproteinemia were observed, and proteinuria was detected on urinalysis. Pleural effusion and mild ascites were noted in radiological findings, and bilateral nephromegaly was noted on ultrasonography. Babesia gibsoni was detected by PCR. Babesiosis and glomerular disease was treated at a time with prednisolone and buparvaquone. This therapeutic regime was very efficacious and clinical sings were rapidly improved. Proteinuria and severe anemia were disappeared following therapy.

  • A 7-year-old intact female Shih-Tzu with chylothorax was presented. Percutaneous popliteal computed tomographic lymphangiography was performed to evaluate the thoracic duct and seek any potential cause of chylothorax. Despite two attempts, visualization of the thoracic duct failed and perianal subcutaneous computed tomographic lymphangiography with injection of iodinated, nonionic water-soluble contrast medium (0.6 ml/kg) was performed. A single branch of intact thoracic duct and dilated and tortuous lymphatics were detected. It was diagnosed as idiopathic chylothorax. Perianal subcutaneous lymphangiography is considered a less-invasive, easy and reliable method to visualize lymphatics in patients with chylothorax.

  • A 3-year-old, spayed female Shih-tzu dog was presented due to acute vomiting, diarrhea, and generalized weakness. The dog had generalized weakness, increased respiratory rate, and respiratory muscle effect. Neurologic examination revealed appendicular muscular weakness and decreased in tone of the anal sphincter. Megaesophagus was confirmed by radiographic examinations. Other than type 2 fiber atrophy, no specific abnormalities were identified in histopathologic examinations of muscle biopsies from the left pelvic limb. Serum acetylcholine receptor (AChR) antibody titer was increased (0.78 nmol/L reference range, less than 0.6 nmol/L), confirming a diagnosis of acute fulminating myasthenia gravis. The dog dramatically responded to pyridostigmine bromide and had marked improvement in muscle strength, megaesophagus, and respiratory function. The dog has been successfully managed for 7 months after initial treatment.

  • A 7-year-old Shih Tzu cross dog was presented for severe abdominal pain, persistent vomiting and anorexia. Laboratory tests revealed mild anemia and hypoproteinemia. Abdominal survey radiography revealed marked gastric distension and large circular foreign body in the gastric pylorus. The foreign body was removed using a videoendoscope, fishing line and retrieval forceps. The dog's clinical signs resolved following foreign body removal.

  • Compared to the other chemodectoma, aortic body tumors, the carotid body tumor is rarer and more often malignant. In the present case, a 12-year-old, intact female Shih-tzu dog presented to the hospital with a right ventral cervical mass. The mass was diagnosed by immunohistochemical staining with chromogranin A (CgA) as a carotid body tumor. The cervical mass and metastasized lymph node were removed by surgical resection. Because the dog had valvular heart disease, chemotherapy with carboplatin was initiated over 6 months to prevent metastasis and recurrence as an adjunctive treatment. After over two years of management, tumor metastasis and recurrence were not observed until recently. This report shows that proper management and chemotherapy as an adjunctive therapy can treat carotid body tumors in elderly dogs with concurrent heart disease.

  • A 9-year-old, intact female Shih-tzu dog presented with a three-week history of anorexia, lethargy, and weight loss. Abdominal radiography revealed a mass in the right cranial quadrant of the abdomen that was displacing the right kidney caudally. Ultrasonography showed a large, irregular, heterogeneous hepatic mass and a small amount of peritoneal fluid. On computed tomography (CT), a large hypoattenuating mass with heterogeneous contrast enhancement occupied the right lateral lobe, and parts of the right medial and caudate lobes of the liver. There was no evidence of regional lymph node or pulmonary metastasis. Lobectomy of the right medial, right lateral and caudate liver lobes was performed, however, the patient did not recover from surgery. Hepatoblastoma was confirmed by histological examination. Hepatoblastoma is rare in dogs, and this is the first report describing CT imaging of hepatoblastoma in a dog.

  • A 5-year-old, intact female Shih-Tzu dog was presented with 1 year history of icterus, ascites and anorexia. The serum biochemistry revealed elevated liver enzyme levels. Microhepatica and decreased serosal detail were detected in abdominal radiography. Abdominal ultrasonographic findings included irregular liver margins, multifocal hypoechoic nodules in the liver parenchyma, and ascites. Computed tomography (CT) showed multifocal hypodense nodules with ring-like contrast enhancement. Cytologic and histopathologic examination by liver core biopsy revealed fibrosis. Cirrhosis was diagnosed based on above results. This report focuses on the imaging characteristics of ultrasonography and CT for liver cirrhosis in a dog.

Gastric Adenoma in the Pyloric Outflow Tract of a Shih-tzu Dog (시츄견에서 위유문부에서 발생한 위선종 증례)

  • A 12-year-old castrated male Shih-tzu dog presented with vomiting, anorexia, and weight loss. The patient's medical history and hematological, radiographic, ultrasonographic, and endoscopic findings were used for diagnosis. The patient's medical history showed that he was being treated for chronic kidney disease since a few weeks. The serum biochemical analysis showed moderate azotemia. However, abdominal radiographic and ultrasonographic findings showed the presence of a mass in the pylorus area of the stomach. On gastrointestinal endoscopic examination, the proliferative mass was found in the pyloric antrum. The histopathological results of the endoscopic biopsy specimen indicated that it was strongly suspected a gastric adenoma; therefore, we surgically excised the pyloric outflow region via a Y-U pyloroplasty. The histopathological examination of the surgically excised specimen also indicated that the mass was a gastric adenoma. After surgical treatment, the patient's condition improved gradually, and the clinical symptoms diminished in approximately 3 weeks after surgery. The patient's condition was well controlled until recently, i.e., 18 months after surgery. This case report describes the clinical findings, imaging characteristics, endoscopic findings, and histopathological features of a gastric adenoma in the pyloric outflow tract of a dog.
