Deep circumflex iliac artery free flap in the mandibular reconstruction (DCIA를 이용한 하악골 재건술)

  • Vascularized iliac crest flap include bone tissue of good quality and quantity for mandible segmental defect. Even if fibular flap can contain longer bone tissue, iliac crest has esthetic shape for mandible body reconstruction and large height for implant. Conventional vascularized iliac crest osteomyocutaneous flap is too bulky for reconstruction of intraoral soft tissue defect. But modified flap can reduce soft tissue volume, so is good for functional reconstruction of oral mucosa. It takes only one month for completely replace oral mucosa. The final mucosal texture is much better than other skin paddle flap, especially for implant prosthesis. Donor site morbidity of this method looks same level or less with other modalities functionally and socially. In case of oral mucosa-mandible combined defect, vascularized iliac crest with internal oblique muscle flap shows good outcomes for hard and soft tissue.

Traumatic Displacement of the Globe into the Maxillary Sinus: Case Report (외상성 상악동 안구탈출의 치험례: 증례보고)

  • Purpose: Globe displacement due to a blowout fracture is a rare clinical phenomenon. The authors present reduction of a globe displacement to the maxillary sinus due to trauma suffered in a fall and the reconstruction of a large defect left in the medial and inferior orbit. Methods: A 39-year-old male patient was unable to open his left eye after being struck on the periorbital area by a metal edge. Laceration was not noted in that area but we were unable to observe the intraorbital globe. A facial computed tomography (CT) scan showed that the globe was displaced through the maxillary sinus. A transconjunctival approach was used to access the infraorbital margin and the globe entrapped in the inferior margin of the orbit was successfully reduced. A large defect in the medial and inferior orbit was reconstructed using a graft from the iliac bone. Results: In 5 months after the operation, no atrophy of the globe was seen. Both sides retained a similar shape. A satisfactory functionality outcome in terms of improved extraocular muscle movement, and a satisfactory aesthetical outcome were achieved. Conclusion: The authors report the reduction of a globe displaced to the maxillary sinus following a fall and the reconstruction of the large defect left in the medial and inferior orbit.

Friction Stir Welding Tool Geometries Affecting Tensile Strength of AA6063-T1 Aluminum Alloy Butt Joint

  • Friction Stir Welding (FSW) is a solid state welding that could successfully weld the difficult-to-weldmaterials such as an aluminum alloy. In this welding process, the stirrer of the welding tool is one of the important factors for producing the perfect sound joint that indicates the higher joint strength. So, this report aims to apply the friction stir welding using various stirrer geometries to weld the AA6063-T1 aluminum alloy butt joint, investigates the mechanical properties of the joint and then compares the mechanical properties with the microstructure of the joint. An experiment was started by applying the friction stir welding process to weld a 6.3 mm thickness of AA6063-T1 aluminum alloy butt joint. A study of the stirrer geometries effect such as a cylindrical geometry, a cone geometry, a left screw geometry and a right screw geometry at a rotational speed of 2000 rpm and a welding speed of 50-200 mm/min was performed. The mechanical properties such as a tensile strength and a hardness of the joint were also investigated and compared with the microstructure of the joint. The results are as follows. A variation of FSW Stirrer shape directly affected the quality AA6063-T1 aluminum alloy butt joint. A cylindrical stirrer shape and a cone stirrer shape produced the void defect at the bottom part of the weld metal and initiated the failure of the joint when the joint was subjected to the load during the tensile test. Left and right screw stirrer shapes gave the sound joint with no void defect in the weld metal and affected to increase the joint strength that was higher than that of the aluminum base metal.

Forehead Island Flap For Nasal Reconstruction (이마 섬피판을 이용한 코재건술)

  • The nose is the most prominent area of the face, therefore susceptible to trauma and skin cancer. When small sized defect is in nasal tip, it results in disturbance of the facial harmony even if replantation, composite graft, skin graft or median forehead flap has been used for the reconstruction. So it is needed that the best method reconstruction is performed according to the degree of defect or deformity. And at the same time the physiology and anatomy of nose were clarified and its aesthetic subunits were employed. How can we cover the about 3 cm sized nasal defect in nasal tip with cartilage exposure? At first, we can think forehead island flap is most appropriate. We performed 7 cases of the forehead island flap for reconstruction of the defect in nasal tip(4 cases: cancer, 3 cases: trauma) from March, 2001 to August, 2004. This result was satisfactory in the point of texture, color, donor scar, and there were no complication such as wound disruption, infection, flap atrophy, and hematoma. The advantages of forehead island flap are: 1) No injury of deep vessel and nerve, 2) control of shape and volume, 3) Short operation time, 4) primary closure of donor site, 5) one stage operation. Also, forehead island flap can cover the defect in nose where skin graft and local flap can not cover. But, operator always must take care for flap congestion and donor site scar. We thought forehead island flap is one of the best option of reconstruction of nasal tip defect.

Both buccal mucosa transposition flap for reconstruction of lower lip near-total mucosal defect (아래 입술 점막 전체 결손의 재건을 위한 양측 볼점막 자리 옮김 피판술)

  • Purpose: Squamous cell carcinoma(SCC) of the lower lip is the most common malignant tumor comprising 90% of all lip SCC. The typical picture of SCC of the lower lip is of an ulcerated lesion with raised margins. Surgery is the treatment of choice for SCC of lower lip. Depending on the location and size of the tumor, different types of flaps are used. We used new method - 'both buccal mucosa transposition flap' for the reconstruction of the near total mucosal defect of the lower lip. Methods: This 67 - year - old men presented with the crusted $1cm{\times}1cm$ sized ulceration of the lower lip that was arised 30 years ago. There were no size and color change, except the bleeding and ulceration. At first, We diagnosed the SCC through the incisinal biopsy. Then We performed the wide excision of the tumor and reconstruction of the lower lip. After the excision of the whole tumor, the defect was measured at $8cm{\times}3.5cm$. We designed the buccal mucosa transposition flap taking care to avoid the parotid duct. The flap was made in a triangular shape for the reconstruction of defected lower lip. The donor site defect can be sutured primarily. Results: A patient in this study had no postoperative complications such as necrosis, dehiscence, infection of the flap or donor site. Reconstructed lower lip is relatively close to that of the natural lip; More satisfactory aesthetic and functional results can be obtained by using this technique rather than other techniques. Conclusion: 'Both buccal mucosa transposition flap' is reliable method for the reconstruction of the large lower lip mucosal defect. The operation is simple and performed in one stage, with no postoperative complications. This technique can offer consistently good functional and esthetic outcomes after reconstruction of lower lip mucosal defect.

  • We simulated insulation defects of stator winding wire on high voltage generator by 5 types. 4 types have one discharge source and other one has multi discharge source by simulation. For accurate decision, measurements used to PRPD pattern to occurred partial discharge source of various types. In this research, when PRPD pattern carried out or analyzed pattern recognition of discharge source, it used to powerful tools. In this result, PRPD Pattern defined to have single discharge source of 4 types by insulation defect. When insulation defect simulated, all the defected winding have not the same result. Errors for a little different can make mistakes from a subtle distinction. The difference between internal and void discharge have magnitude of pulse amplitude of inner discharge bigger than void discharge and have a shape of bisymmetry. But void discharge has a shape of bisymmetry against maximum value on polarity respectively. In cases of slot and surface discharge, we confirmed to show similar results those other researchers. In case of multi-discharge, as a result of we could classify not perfect match with occurred patterns in single discharge eachother. In the future, we will have to recognize and classify with results of multi-discharge.

A study on mathematical modeling and heat transfer analysis to predict weld bead geometry in horizontal fillet welding (수평필릿용접의 용접부 형상을 예측하기 위한 수학적 모델링 및 열전달 해석에 관한 연구)

  • The horizontal filet welding is prevalently used in heavy and ship building industries to join the parts. The phenomena occurring in the horizonal fillet welding process are very complex and highly non-linear, so that its analysis is relatively difficult. Furthermore, various kinds of weld defect such as undercut, overlap, porosity. excess weld metal and incomplete penetration can be induced due to improper welding conditions. Among these defects, undercut, overlap and excess weld metal appear frequently in horizontal filet welding. To achieve a satisfactory weld bead geometry without weld defects, it is necessary to study the effect of welding conditions in the weld bead geometry. For analyzing the weld bead geometry with and without weld defects in horizontal fillet welding, a mathematical model was proposed in conjunction with a two-dimensional heat flow analysis adopted for computing the melting tone in . base metal. The reliability of the proposed model was evaluated through experiments. which showed that the proposed model was very effective for predicting the weld bead shape with or without weld defects in horizontal fillet welding.

Analysis of Weld Beads for Wall Thinning Defects in the Weld Zone of the Boost Pump Recirculation Pipe for Power Plants and Evaluation of Their Integrity (플랜트 승압펌프 재순환 배관 용접부의 용접비드에 의해 발생한 감육결함 분석 및 건전성 평가)

  • The wall thinning phenomenon of pipes was simulated as metal loss due to erosion and corrosion. Such wall thinning defects in the pipes of power plants are a very important safety consideration. In this study, we analyzed wall thinning defects that occurred by weld bead of weld zone of boost pump recirculation pipe. From the results of the analysis of pipe failures, numerical analysis was performed by Fluent v6.3.26 using the standard k-$\varepsilon$ model of the weld bead shape, such as an elliptical or a spherical shape, on the inner wall of the pipe. Using the results obtained, we showed the overlap effect by cavitations corrosion and erosion-corrosion at the bottom of the wall-thinning defect.

  • The Hub is an auto mobile component used as aircon clutch. The important aspects in cold forging of the Hub with complex geometry are the design of an initial shape of the workpiece, the possibility of the forming by one-stage operation and the determination of number of performs, etc. Based on the systematic procedure of process sequence design, in this paper, the forming operation of cold forged part of the Hub is designed by the rigid-plastic finite element method. The two design criterion of geometrical filling without defect and an even distribution of effective strain in final product are investigated in controlling the initial shape of the workpiece and preform configuration. It is noted that one preforming operation is required in order to obtain final product of the Hub.

  • According to the variation of hydrostatic pressure on the central axis of deformable material, the V-shaped central bursting defect may be created in extrusion or drawing processes. The process factors which affect the generation of defects are semi-angle of die, reduction ratio of cross-sectional area, friction factor, material properties and so on. The combination of these factors can determine the possibility of defect creation and the shape of various round holes which have been created inside already. By the rigid plastic finite element method, this paper describes the observations of change in shape of round holes with process conditions such as semi-angle of die, reduction ratio of cross-sectional area and friction factor at the non-steady state of axisymmetrical extrusion process when the round hole is already existed inside the original billet. Also, the effects of process factors are investigated to prevent the possible defects.
