Acute Toxicity Study on Gumiganghwal-tang(Jiuweiqianghuo-tang) in Sprague-Dawley Rats (Spargue-Dawley 랫드를 이용한 구미강활탕의 급성독성 연구)

  • Objectives : This study was conducted to evaluate the acute toxicity and safety of Gumiganghwal-tang (Jiuweiqianghou-tang) in Sprague-Dawley rats though the current regulatory guideline. Methods : The preliminary study showed that the single oral administration of Gumiganghwal-tang(Jiuweiqianghou-tang) did not induce any toxic effect at a dose level of 2000 mg/kg. Based on the results, 2000 mg/kg was selected as the limited dose. In this study, 10 rats of each sex were randomly assigned to two groups of 5 rats each and were administrated singly by gavage at dose levels of 0 and 2000 mg/kg. Mortalities, clinical signs, and body weight changes were monitored for the 15-day period following administration. At the end of observation period, all animals were sacrificed and complete gross postmortem examinations were performed. Results : Throughout the study period, no treatment-related deaths were observed. There were no adverse effects on clinical signs, body weight, and gross findings at all treatment groups. Conclusions : These results showed that the single oral adminstration of Gumiganghwal-tang(Jiuweiqianghou-tang) did not cause any toxic effect at the dose levels of 2000 mg/kg in rats. In conclusion, the $LD_{50}$ of Gumiganghwal-tang (Jiuweiqianghou-tang) was considered to be over 2000 mg/kg body for both sexes.

  • Purpose: To investigate the caustic ingestion in children among different continents according to demographic characteristics (core purpose), main symptoms, common caustic agents, signs and symptoms, management, treatment and complications. Methods: This systematic review was performed by searching the databases Science Direct, ProQuest, Google Scholar, and PubMed, electronically and manually. We included studies that were published from 1980 to 2013, at University of Medical Sciences of Tabriz, Iran. A strategic search was performed with keywords including caustic, corrosive, ingestion and children, and was limited to articles in English and Persian. Statistical analysis was performed by SPSS ver. 18. Results: Of 63 selected articles of caustic ingestion with 9,888 samples, the proportion of Africa was 3 articles (4.8%) and 95 samples (1%), America 9 articles (14.3%) and 305 sample (3%), Asia 29 articles (46%) and 2,780 samples (28.1%), Europe 17 articles (27%) and 3,002 samples (30.4%), and Oceania 5 articles (7.9%) and 3,706 samples (37.5%). The average age was in the Africa $3.07{\pm}2.02years$, America $3.17{\pm}1.83years$, Asia $3.34{\pm}1.58years$, Europe $3.58{\pm}2.09years$ and Oceania $3.52{\pm}2.02years$. Sex distribution was in Africa 76 males (0.91%) and 19 females (0.23%), America 49 males (0.58%) and 41 females (0.49%), Asia 1,575 males (18.76%) and 1,087 females (12.95%), Europe 1,018 males (12.13%) and 823 females (9.8%), and Oceania 1,918 males (22.85%) and 1,788 females (21.3%). Statistical analysis of the data indicated higher consumption in Europe and Oceania in the boys with higher average age of years. Conclusion: The comparison of caustic ingestion indicated that the cause substances of caustic ingestion in children are different among continents, therefore prevention strategy and different treatment guidelines among continents will be needed.

Epidemiological Patterns of Cancer Incidence in Southern China: Based on 6 Population-based Cancer Registries

  • Background: The epidemiological patterns of cancer incidence have been investigated widely in western countries. Nevertheless, information is quite limited in Jiangxi province, southern China. Materials and Methods: All data were reported by 6 population-based cancer registries in Jiangxi Province. The results were presented as incidence rates of cases by site (ICD-10), sex, crude rate (CR), age-standardized rates (ASRs) and truncated incidence rate (TR) per 100,000 person-years, using the direct method of standardization to the world population. Results: 8,765 new cancer cases were registered in our study during the period 2009-2011. Diagnosis of cancer was based on histopathology in 61.0%, clinical or radiology findings in 4.87% and death certificate only (DCO) in 3.0% of the cases. The median age at diagnosis was 62.0 years (mean, 61; standard deviation, 15). The ASRs were 170.8 per 100,000 for men and 111.2 for women. The ASRs for all invasive cancers from the urban areas (145.7 per 100,000) was higher than that of rural areas (137.1). Incidence rates for lung cancer were higher in rural (35.8) than in urban areas (27.0). Similarly, relatively high rates were observed for stomach cancer in rural (20.1) relative to urban areas (15.5). Conclusions: Our results reveal that the most common cancers were breast and lung in women and lung and liver in men. Interestingly, this study suggested a higher incidence rates for lung and stomach cancer in rural males than in urban population, which may suggest other potential causes, such as over-consumption of smoked meats and high prevalence of Helicobacter pylori infection, respectively. Public education and the promotion of healthy lifestyles should be actively carried out.

  • Purpose: Mumps meningitis is a common complication of mumps infection; however, information on mumps meningitis in the postvaccine era is limited. The purpose of the present study was to determine factors associated with mumps meningitis and to discuss the effect of vaccination on this disease. Methods: We retrospectively reviewed patients younger than 19 years with mumps, diagnosed at a university hospital in Korea between 2003 and 2013. Patients were divided into groups with and without meningitis, and the clinical features of the 2 groups were compared. Results: The study enrolled 119 patients: 19 patients with meningitis and 100 patients without. Univariate analysis showed that older age (median: 15 years vs. 9.5 years, respectively), a longer interval from last vaccination (median: 10.2 years vs. 4.8 years, respectively), and febrile presentation (94.7% vs. 31.0%, respectively) were significantly associated with mumps meningitis. Sex, number of vaccination doses, bilateral parotitis, and the presence of complications other than meningitis did not differ between the 2 groups. In multivariate logistic regression analysis, age (odds ratio, 1.38; 95% confidence interval, 1.01-1.89; P=0.04) and fever (odds ratio, 30.46; 95% confidence interval, 3.27-283.61; P<0.01) remained independent factors for mumps meningitis. Conclusion: Clinicians in the postvaccine era should be aware of the possibility of mumps meningitis in febrile cases of mumps in adolescents, regardless of the number of vaccination doses. To establish the role of vaccination in mumps meningitis, further studies will be necessary.

  • A new silkworm variety "Buhungjam" for autumn rearing season is $F_1$ hybrid between Jam149, a japanese race bred from introduction breeding and Jam150, a chinese race from introduction breeding. Jam149, japanese parent of the "Buhungjam" showed high GCA in pupation percentage, cocoon shell percentage and filament length and Jam15, chinese parent showed high GCA in cocoon shell percentage and cocoon filament length. In the local adaptability test performed at 8 local areas in spring of 2003, Buhungjam records 7% higher in cocoon yield and 8% in raw silk yield than the check variety, Daesungjam.

A Study of the Nursing Needs of Hospitalized medical Patients by Means of Nursing History Form (간호력(Nursing History)을 통해 본 내과 입원환자의 간호요구)

  • Personalized Patient centered nursing care is increasingly difficult to achieve despite the fact that it remains one of our consistent goals. So, we must find away to individualized nursing care. One means, to achieve this is by use of the nursing history form, which has been developed to help the nurse make maximum use of her limited time with the patient, by obtaining systematically the information needed to plan his nursing care. The nursing history form can be used to collect data about individual nursing needs but also it lends itself to the collection of epidemiological data relevant to the needs of patient population. So this study was undertaken in an attempt to describe the general characteristics of the population studied, to find out their perceptions and expectations related to their illness and hospitalization, to find out specific basic needs and to examine the relationship between the patients nursing needs and demographic characteristics through the responses to the nursing history questions. The study population defined and selected was all the patients (70) who were admitted to Yonsei University Hospital from October 1 - 15, 1975. The direct interview method was used and the data were categorized by the investigator, according to the nature of responses to each question and were subjected to the percentile and the chi- square tests. The findings can be summarized as follows 11. General characteristics of the study population ; The population was made up mainly of urban patients who were highly educated. The age was equally distributed. 2. Patients, perceptions and expectations related to illness and hospitalization ; 88.6% of the patients knew about the reason for hospitalization but 64. 5% could state symptoms only. 37.5% recognized the cause of illness. Approximately three fourth of the patients expressed on expectation for early recovery. 60.0% of the patient responses indicated, that they expected less than 10 days of hospitalization. Of the total responses regarding hospitalization, 45.7% were negative about the post -hospitalization expectation, 45.7% expected to return to work. As a result of these findings, we can see that there is a great educational need, a psychological need and environmental need for adaptation to the hospital and a socio- economic need for the post - hospitalization period. 3. Specific basic needs : The physical needs include the problem of getting sufficient sleep (50.0%), difficulty in food intake(47.1%), problems with hospital diet (47.0%), abnormal condition of the skin(44.3%), problems with bowel elimination(35.7%), assistance with bathing(35.7%), pain(30.0%), difficulty in walking(30.0%) , difficulty in seeing(30.0%) problems in urinary elimination(20.0%) , and difficulty in hearing(10.0%), 4. Nursing needs and epidemiological characteristics ; Age distribution was related to the rendition of the mouth but no significant differences were observed statistically with the patients responses to the other nursing history questions according to the epidemiological variables of age, sex, education and residence.

The Effect of Knowledge and Perception of Environmental Problems on Pro-environmental Behavior of Youth (청소년의 환경문제에 대한 지식 및 인식이 환경친화적 행동에 미치는 영향)

  • The purpose of this study was to analyze the effect of knowledge and perception of environmental problems on pro-environmental behavior of youth. The data used for this study were collected using questionnaire. The sample for this study consisted of 654 junior and high school students living in Seoul and Gyeong-gi Province. Descriptive statistics and multiple regression analysis were used to analyze the data. The following is a summary of the major findings. (1) The significant variables affecting pro-environmental behavior of youth were need of environmental education, source of environmental knowledge, level of perception on air pollution and scarcity of resources and energy, sex and total amount of allowance of students. These variables explained 20.5% of variance of pro-environmental behavior. (2) Environmental education related variables were the best predictor of pro-environmental behavior; knowledge of environmental problem variables had limited predictive power. (3) Recommendations for teaching-learning strategies have been suggested to enhance youth's pro-environmental behavior in school.

The effect for exercise intensity on hypertension using propensity score (성향점수를 이용한 운동강도가 고혈압에 미치는 영향)

  • This study aims to identify the effect for exercise intensity on hypertension using propensity score based on the sixth Korea National Health and Nutrition Examination Survey data and to provide an evidence for the most effective exercise intensity for prevention or treatment of hypertension. Specifically, we select 3,486 subjects who aged between 18 and 65 years after excluding some subjects who are expected to have limited athletic ability. We estimate propensity scores for exercise intensity based on the confounders such as sex, age, smoking, drinking, and natrium intake. Considering the complex survey design, we conduct a descriptive analysis and multiple logistic regression for hypertension with propensity score as a covariate. Although the results of the study did not show statistically significant relationship between exercise intensity and hypertension, we expect that it can be used as a basis evidence that the appropriate exercise of moderate intensity may be more effective for the prevention and treatment of hypertension rather than strong intensity exercise and non-exercise.

Clinical Course of Transferred Patients for Operation Under the Impression of Biliary Atresia (담도폐쇄증 의심하에 수술을 위해 전원된 환자의 임상 경과 분석)

  • Biliary atresia (BA) is very difficult to distinguish from neonatal hepatitis (NH) and its prognosis depends on the age at the time of Kasai operation. Therefore early differentiation between these two conditions is very important. Although various clinical and laboratory tests have been reported to differentiate between them, they are still of limited value. From 1980 to 1999, forty-five infants were referred to our pediatric surgical unit for operation for suspected BA. Eight patients underwent Kasai operation immediately because of late diagnosis. These were excluded from the study. The clinical history, physical findings. radiologic and laboratory examinations of 37 cases were analyzed retrospectively. The average age of BA (n=20) was $55.1{\pm}16.7$ days, and that of NH (n=17) was $55.8{\pm}15.6$ days. The sex ratio of BA was 13:7, and that of NH was 14:3. All the patients had obstructive jaundice and acholic stool except 4 BA and 6 NH patients. Acholic stool with yellow component was more frequent in NH. Onset of jaundice was within 2 weeks after birth in 85 % of BA, and in 65% of NH. The onset of acholic stool was within 2 weeks after birth in 60 % of BA, and in 23.5 % of NH. The duration of jaundice and acholic stool of BA were $50.9{\pm}16.6$ days and $41.3{\pm}18.4$ days and those of NH were $40.1{\pm}23.1$ days and $26.6{\pm}25.4$ days respectively. The ultrasonogram and hepatobiliary scan were useful, but not a definitively reliable method for the differentiation of these two diseases. There was no difference in laboratory data. Seventeen cases had NH among 45 referred cases for Kasai operation with the clinical impression of BA, and 4 cases of 17 NH cases needed to be explored to rule out BA. In conclusion. false positive rate of clinical impression of BA was 37.8 %. and negative exploration rate was 8.9 %, Therefore. careful clinical observation for 1-2 weeks by an experienced pediatric surgeon was very important to avoid unnecessary operation to rule out NH up to the age of 8 - 10 weeks. so long as the stool had yellow component.

Impact of Complementary Private Health Insurance on Public Health Spending in Korea (실손형 민간의료보험의 도입이 국민건강보험 재정에 미치는 영향)

  • Limited coverage for health care services of National Health Insurance(NHI) in Korea has been ongoing policy issue but additional NHI financing through raising contribution or taxes in order to improve coverage faces substantial obstacles. Private health insurance(PHI) is often considered as an alternative financing source to improve coverage. Recent reform that attempted to stretch the role of PHI allowed life insurance companies to provide complementary PHI, indemnity plan which will pay for uncovered services by NHI and out-of-pocket spending for covered services. Although complementary PHI may relieve financial burden of patients, it may significantly raise NHI spending as well as total health expenditure since little out-of-pocket spending may increase utilization of health care. So far, there has not been enough discussion about concerns of potential adverse effect resulting from extended role of PHI. This study investigated potential increase of NHI spending followed by extension of complementary PHI through sensitivity analysis. The amount of NHI spending for services that would be covered by complementary PHI was calculated using 2005 NHI statistics and expected complementary PHI enrollment rate by age and sex. Expected utilization increases were obtained based on price elasticities$(-0.2{\sim}-0.5)$ from previous studies and expected coverage rate$(50{\sim}80%)$ of complementary PHI and then converted to monetary figures. Because coverage rate of complementary PHI has not been determined yet, we employed the sensitivity analysis using coverage rate of $50{\sim}80%$. Findings demonstrate that additional spending for health care services is expected to be $426{\sim}1,702$ billion won, corresponding amount payed by NHI $298{\sim}1,192$ billion won. In conclusion, since complementary PHI may raise NHI spending significantly, there should be an agreement whether this additional cost would be accountable and acceptable in our society. Potential inefficiency resulting from extended role of complementary PHI should be considered since public and private financing do not operate in isolation and there should be more discussion on proper role of PHI in Korea.
