The Study on Vascular Dementia recorded in Chinese Medical Journal (중의잡지(中醫雜誌)에 보고(報告)된 뇌혈관성치매에 대(對)한 고찰(考察))

  • This study analyzed the contents of the research papers concerning the vascular dementia(VD) recorded in Chinese medical journal published over the period between 1998 and 2000. As a result, the following conclusion was drawn. The Chinese medical category of VD includes Sun-Mang(善忘), Chi-Mae, Jung-Pung-Chi-Mae and so on, and the Major mechanism of the VD is Bon-Heo- Pyo-Sil(state of deficient vital essence and excessive pestilential vapor;本虛標實), Bon(本) includes Sin-Jeong-Heo-Son(腎精虧損), Ki-Hyeol-Hyu-Heo(氣血虧虛) and Pyo(標) includes Dam-Chae-Eo-Jo(痰滯瘀阻), Ki-Chae-Hyeol-Eo(氣滯血瘀), Gan-Yang-Sang-Hang(肝陽上亢). It is mainly used DSM and evidence of brain-vascular disease in brain CT or MRI as the criterion for diagnosis in Western medicine and MMSE, HDS, ADL is used as assistant diagnostic indicator. "Principle of clinical study guidance for treatment of dementia by Chinese medicine, Western medicine"("中藥新藥治療痴呆 病的臨床硏究指導原則") is mainly used as the criterion for diagnosis in Chinese medicine. It is mainly used "Principle of clinical study guidance for treatment of senile disease by Chinese medicine, Western medicine"("中藥新藥治療老年病臨床硏究指導原則"), "Principle of clinical study guidance for treatment of dementia by Chinese medicine, Western medicine" as the criterion for treatment effect evaluation, and score change of MMSE and HDS is used as assistant evaluation indicator. The research papers reported that the use of prescriptions such as Hwan-Chong-Dan(還聰丹), Noi-Hyeol-So-Tong-Gu-Bok-Yaek(腦血疏通口服液), Yik-Gi-Gyo-Nang(益智膠囊), Cheon-Ma-Chok-Gi-Chung-Je(天麻促智沖劑), Sam-Gap-San-Ga-Mi-Bang(三甲散加味方), Seon-Ryong-Gyo-Nang(仙龍膠囊), Seong-Noi-Tang II(醒腦湯II號), Bo-Sin-Geon-Noi-Tang(補腎健腦湯), Ga-Mi-Tong-Gyu-Hyal-Hyeol-Tang(加味通竅活血湯), So-Jo-Yik-Ji-Bang(疏調益智方), Tong-Gyu-Hwal-Hyeol-Tang-Ga-Gam(通竅活血湯加減), Yik-Sin-Seong-Noi-Tang(益腎醒腦湯) led to the high efficacy.

  • "Sarcopenia", sarcopenia is an old age syndrome, and used to describe the reduction of skeletal muscle. Initially, it was thought that sarcopenia was only a senile disease characterized by degeneration of muscle tissue. However, its cause is widely regarded as multifactorial, with neurological decline, hormonal changes, inflammatory pathway activation, declines in activity, chronic illness, fatty infiltration, and poor nutrition, all shown to be contributing factors. Skeletal muscle mass can be measured by a variety of methods, currently, the commonly used methods are dual-energy X-ray scanning (DXA), computer tomography (CT), magnetic resonance imaging (MRI), etc. Muscular skeletal disorders can also be assessed by measuring appendicular skeletal muscle (ASM), particularly muscle tissue content. At the same time, sarcopenia refers to skeletal muscle cell denervation, mitochondrial dysfunction, inflammation, hormone synthesis and secretion changes and a series of consequences caused by the above process and is a progressive loss of skeletal muscle syndrome, which can lead to the decrease of muscle strength, physical and functional disorders, and increase the risk of death. Sarcopenia is mainly associated with the aging process, but also related to other causes such as severe malnutrition, neurodegenerative diseases, and disuse and endocrine diseases associated with muscular dystrophy, and it is the comprehensive results of multi-factors, so it is difficult to define that sarcopenia is caused by a specific disease. With the aging problem of the population, the incidence of this disease is increasingly common, and seriously affects the quality of the life of the elderly. This paper reviews the etiology and pathogenesis of myopathy, screening methods and diagnosis, the influence of eating habits, etc, and hopes to provide reference for the diagnosis and treatment of this disease. At present, adequate nutrition and targeted exercise remain the gold standard for the therapy of sarcopenia.

Effects on osteoporosis of Senescence Accelerated Mice (SAM) P6 by Injection of Placenta extracts ($Beuronel^{(R)}$) (SAM p6 mouse에서 자하거 주사제의 골다공증 개선에 대한 연구)

  • Objectives : The purpose of this study was to examine the effects of the Hominis placenta extracts on improvement of senile osteoporosis (Type 2) in SAM P6. In traditional Oriental medicine, Placenta (Jahageo) is indicated for insufficiency of blood and tidal fever due to steaming bone disorder. Methods : At 10 weeks after birth, Placenta ex. was given 100 mL, 200 ml/kg/day, i.p. 3 times a week for 4 weeks in SAM P6 mice. And then we measured complete blood cells, blood urea nitrogen, creatinine, inorganic phosphate and total iron. In addition, we tested bone mineral density (BMD) using the soft X-ray. Results : As a result, in SAM P6, injection of Placenta ex. (200 mL) increased in RBC, HB and PLT, in comparison with control group. It was also found that the inorganic phosphate levels increased significantly in the injection groups of the Placenta ex. from that of the control group, but blood urea nitrogen was no significant. Moreover, Placenta ex. were showed a trend of increase in bone mineral density (BMD) of the vertebrae lumbales and tibia in P6 mice. Conclusions : These findings suggest that Placenta ex. are effective in preventing bone loss in SAMP6.

  • The enlightingment of the aging because society's role to reduce the elderly dependency Increase and old class income security or medical security is becoming a serious social problem, all sorts of senile disease increased and the elderly raising difficulties caused by the increased the elderly of the seriousness of this problem. To complete the social security against the social risks it is on the one hand necessary to provide an elaborate system of coordination in the field of social security. It should be pursued on the dimension if law as well as policy. On the other hand national commitment if social security benefits should be comparatively analysed to have implications for the national legislation. It is one of the most important fact that prepare for the advanced elderly society which are the law of elderly man welfare and policy is divided into an employment guarantee policy, an income guarantee policy, a health welfare policy etc.

A Clinical Trial of Orally Administered Alkaline Reduced Water

  • Effects of alkaline reduced water (ARW) on the scavenging of reactive oxygen species (ROS) and diabetes were recently reported. However, no studies have yet been investigated on general drinking effects. In this study, we examined 100 patients hospitalized for senile disease treatment and recuperation. All examines received the ARW for 2 months. WBC level was decreased from $6.34\pm1.9(\times10^9/L)$ to $5.66\pm1.7(\times10^9/L)$. SGPT level was decreased from 27.47$\pm$14 Ut to 23.95$\pm$8.8 U/L. Cholesterol level was increased from 167.9$\pm$32 mg/dl to 176.21$\pm$42 mg/dl within normal range. However, the levels of potassium, adiponectin, SGOT and blood pressure showed no significant difference. The levels of growth hormone and homocysteine and mini-mental state examination also showed no significant difference. We suppose that the slight blood parameter trends indicate an improved physical condition induced by the ARW. Furthermore, we could found no harmful effect of the ARW in this study.

  • Alzheimer's disease(AD) is associated with a characteristic neuropathology. The major hallmarks of AD are senile plaques (SPs) and neurofibrillary tangles(NFTs). ${\beta}$-amyloid protein($A{\beta}$) is derived from the proteolysis of amyloid precursor protein(APP) and then converted to SPs. Mature SPs produce cytotoxicity through direct toxic effects and activation of microglia and complement. NFTs are composed of paired helical filaments(PHFs) including abnormally phosphorylated form of the microtubule-associated protein(MAP) tau and increased tau level in cerebrospinal fluid may be observed in most AD. The aggregation of $A{\beta}$ and tau formation are thought to be a final common pathway of AD. Acetylcholine, dopamine, serotonin, GABA and their receptors are associated with AD. Especially, decreased nicotinic acetylcholine receptors(nAChRs) in AD are reported. Genetic lesions associated with AD are mutations in the structural genes for the APP located on chromosome 21, presenilin(PSN)1 located on chromosome 14 and PSN2 located on chromosome 1. Also, trisomy 21, Apo-E gene located on chromosome 19, PMF locus, low density lipoprotein receptor-related protein and ${\alpha}$-macroglobulin increase risk of AD. In this article, we will review about the neurobiology of AD and some newly developed research areas.

  • Pruritus is [CHECK DEFN] itchy feeling of one's skin. We often face stroke patients who complain about Pruritus. They occasionally fail to fall asleep or have a secondary infection as a result of scratching. For these reasons, severe Pruritus brings down general condition and interferes with recovery. The causes of Pruritus are distributed to from skin diseases and from internal diseases. Especially among the skin diseases, Xerotic Eczema, which is called Senile Eczema causes the dry skin in the elderly, especially lower limb's extensor part. According to the epidemiology, 20% of the old have Xerotic Eczema. In oriental medicine, Pruritus is called Pungsoyang(風瘙痒), Pungyang(風痒), Yangpung(痒風), Sinyang(身痒). The cause of Pruritus is divided into two. One is endogenous factors and the other is exogenous factors. The former are deficiency of blood(血虛), blood fever(血熱), wind-heat due to internal damage(內傷風熱), damp-heat in the liver and gallbladder(肝膽濕熱), endogenous wind stirring in the liver(肝風內動), deticiency syndrome of the spleen(脾虛), deficiency of Yin of the liver and kidneys(肝腎陰虛) and deficiency of the Penetration and Conception Vessels(衝任不足). The latter are wind-cold due to exogenous affection(外感風寒) and wind-heat due to exogenous affection(外感風寒). We report two stroke patients who complained of severe Pruritus They were diagnosed as having Xerotic Eczema by a dermatologist. We regarded their Pruritus as blood fever(血熱) and wind-heat(風熱) and prescribed Sopung-San to these patients. These patients showed significant improvement.

Effects of Chilbokyeumgamibang(七福飮加味方) on the Cerebral Cortex Neuron injured by Glucose Oxidase (칠복음가미방(七福飮加味方)이 Glucose Oxidase에 의해 손상(損傷)된 대뇌피질(大腦皮質) 신경세포(神經細胞)에 미치는 영향(影響))

  • As the average life span have been lengthened and the rate of senile population have been raised, chronic degenerative diseases incident to aging has been increased rapidly and become a social problem. With this social background, recently, the facts that oxygen radicals(OR) have toxic effects on Central Nervous System and Peripheral Nervous System and cause neuropathy such as Parkinson's Disease, Alzheimer Disease have been turned out, and accordingly lots of studies on the mechanism of the toxic effects of OR on nerves, the diseases caused by OR and the approaches to curing the diseases have been made. The purpose of this study is to examine the toxic effects caused by Glucose Oxidase(GO) and the effects of herbal extracts such as Chilbokyeum(CBY), Chilbokyeumga Acori Rhizoma(CAR), Acori Rhizoma(AR) on the treatment of the toxic effects. For this purpose, experiments with the cultured cell from the cerebrums of new born mice were done. The results of these experiments were as follows. 1. GO, a oxygen radical, decreased the survival rate of the cultured cells on NR assay, MTT assay and amount of neurofilaments and increased the amount of total protein, lipid peroxidation and the amount of LDH. 2. CBY have efficacy of increasing the amount of neurofilaments and total protein and decreasing lipid peroxidation and the amount of LDH. 3. CAR have efficacy of increasing the amount of neurofilaments and total protein and decreasing lipid peroxidation and the amount of LDH. 4. AR have efficacy of increasing the amount of neurofilaments and total protein. From the above results, It is concluded that Chilbokyeumgamibang has marked efficacy as a treatment for the damages caused in the GO-mediated oxidative process. And Chilbokyeumgamibang is thought to have certain pharmacological effects on controlling over aging and treating Dementia. Further clinical study of this pharmacological effects of Chilbokyeumgamibang should be complemented.

  • This experiment was done to investigate the effects of Ojajiwhangeumja(OJWEJ) to the symptoms of senility. To prove the effects of OJWEJ we injected scopolamine(1mg/kg) to the vein of rats. And we measured the blood cells(WBC, RBC, Platelet), constituents of serum(BUN, creatinin, glucose, uric acid), endurance of films of the red blood cell to erythrocyte hemolises, the activity of cholinesterase in serum, TBA and the activity of catalase, SOD in the purified microsome of brain tissue of rats. The results were as follows; 1. The number of white blood cells, platelet was increased significantly in the group treated by OJWEJ in comparison with control group. 2. The number of BUN, creatinin, glucose of serum decreased significantly in the OJWEJ treated group compared with control group. 3. The erythrocyte hemolises in red blood cells restrained significantly in the group treated by OJWEJ in comparison with control group. 4. The activity of cholinesterase in OJWEJ treated group increased significantly compared with control group. 5. The amounts of malondialdehyde of serum decreased significantly in the OJWEJ treated group in comparison with control group. 6. The catalase in the microsome of rat brain was activated significantly in the group treated by OJWEJ compared with control group. 7. The superoxide dismutase in the group treated by OJWEJ activated significantly in comparison with control group. According to this experiment it is suggested that OJWEJ accelerates the activity of colinesterase and restrains the creation of erythrocyte hemolises and accumulation of senile substance. But I look forward to see the further research to be made.

The Effects of OJaJiHwangEumJa(OJJHEJ) Hot water extract & Ultra-fine Powder on Proinflammatory Cytokine of Microglia and Memory Deficit Model (오자지황음자(五子地黃飮子) 열수추출물과 초미세분말이 싸이토카인과 건망증 생쥐모델 기억력감퇴에 미치는 영향)

  • Background: Microglia produces a barrage of factors (IL-l, TNF-$\alpha$, NO, superoxide) that are toxic to neurons and playa major role in the cellular immune response associated with the pathology of Alzheimer's disease(AD). OJaJiHwangEumJa(OJJHEJ) has been usually used for the treatment of senile disorders. For enhancing efficacy and convenience, the change of the drug delivery device of oriental herbal medicine is required. Objective: This experiment was designed to investigate the effect of the OJJHEJ hot water extract & ultra-fine powder on proinflammatory cytokine of microglia and memory deficit model. Method: The effects of the OJJHEJ hot water extract on production of IL-1$\beta$, IL-6, TNF-$\alpha$, in BV2 microglial cell line treated by lipopolysacchaide(LPS) were investigated. The effects of the OJJHEJ hot water extract & ultra-fine powder on the behavior of the memory deficit mice induced by scopolamine and AChE in serum of the memory deficit mice induced by scopolamine were investigated. Results: 1. The OJJHEJ hot water extract suppressed the production of IL-1$\beta$, IL-6, TNF-$\alpha$ in BV2 microglial cell line and the production of IL-6 was suppressed significantly. 2. The OJJHEJ hot water extract & ultra-fine powder decreased AChE significantly in the serum of the memory deficit mice induced by scopolamine. 3. The OJJHEJ hot water extract & ultra-fine powder groups showed significantly inhibitory effect on the scopolamine-induced impairment of memory in the experiment of Morris water maze. Conclusions: This experiment shows that the OJJHEJ hot water extract & ultra-fine powder might be effective for the prevention and treatment of memory impairment diseases. Investigation into the clinical use of the OJJHEJ hot water extract & ultra-fine powder for Alzheimer's disease is suggested for future research.
