일 보건소 방문건강관리사업 고혈압대상자의 사례관리 효과 (The Effect of Case Management for Clients with Hypertension in Home Visiting Health Services)

  • Purpose: This study was conducted in order to present the effects of case management of a home visiting health service for clients with hypertension. Method: One-group pretest-posttest design was used. The subjects were 280 clients who received case management among the first and second registered group. The data were eight-week-case management results from January to December, 2010. In order to evaluate the effect of the visiting nursing service, biological indexes(blood pressure, total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein, BMI) and self-care abilities(knowledge/attitude of hypertension, management of medication, nutritional care, confidence of self care) were measured. Result: Biological indexes and self-care abilities were improved, except total cholesterol, low density lipoprotein. Conclusion: The case management of home visiting health service by public health centers is considered to be an effective nursing service. Therefore, greater effort is needed for better maintenance of case management, and more research is needed in order to examine a variety of biological indexes.

뇌졸중 환자의 불확실성, 생리적 위험지표, 자기효능감이 자기관리에 미치는 영향 (Influence of Uncertainty, Physiologic Risk Factors, Self-efficacy on Self-management in Stroke Patients)

  • Purpose: The purpose of this study was to investigate the level of uncertainty, physiological risk factors, self-efficacy, and self-management among stroke patients and to identify factors influencing their self-management. Methods: A descriptive correlational design was used for this study. A convenience sample of 149 patients with stroke were enrolled at C national university hospital from February to April in 2016. Data were collected using a structured questionnaire and electronic medical record. Collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlations, and multiple regression analysis with the SPSS/WIN 21.0 program. Results: There were significant negative correlations between uncertainty and self-efficacy (r=-.56, p<.001); between uncertainty and self-management (r=-.56, p<.001); and between total cholesterol and self-management (r=-.23, p=.005). There were significant positive correlations between self-efficacy and self-management (r=.78, p<.001); between uncertainty and total cholesterol (r=.24, p=.003). The significant factors influencing self-management were uncertainty and self-efficacy. Theses variables explained 62.7% of the variance in self-management. Conclusion: The results suggest that intervention programs to reduce the level of uncertainty and to increase the level of self-efficacy among patients would improve the self-management of stroke patients.

학령전기 아동 어머니를 위한 협동학습 기반 아동건강관리 프로그램 개발 및 효과 (Development and Effects of a Child Health Management Program Based on Cooperative Learning for Mothers of Preschool Children)

  • Purpose: This study was conducted to develop and test effects of a child health management program based on cooperative learning for mothers of preschool children. Methods: For this quasi-experimental study, a nonequivalent control group pretest-posttest design was used. Outcome measures were level of child health management self-efficacy, child health management practices, child's health behavior. Participants were 55 mothers (experimental group 28, control group 27). The program was composed of 6 categories of home health management for preschool children, and provided for three 120-minute sessions. Data were collected from September to October, 2015, at 3 times: baseline, 3 and 6 weeks after beginning the intervention. Data were analyzed using SPSS/WIN 21.0 program. Results: After 3 and 6 weeks, there were significant differences between the two groups in child health management self-efficacy (F=18.33, p<.001), child health management practices (F=8.91, p<.001), and child's health behavior (F=9.91, p<.001). Conclusion: Study findings indicate that this child health management program based on cooperative learning is effective and can be recommended as an intervention for mothers of preschool children. It will contribute to enhanced child health management self-efficacy and health management practices and improved child's health behavior.

간호대학생의 스트레스, 자아존중감, 정신건강에 관한 연구 (A Study on the Stress, Self-Esteem, and Mental-Health in Nursing Students)

  • This study is intended to identify relationship between stress, self-esteem and mental health in nursing students. The data were collected from 448 nursing students using self-report questionnaire. The data were analyzed by frequencies, t-test, ANOVA, Pearson's correlation coefficients and Stepwise multiple regression using the SPSS 19.0 program. The result are as followed; First, the average of stress, self-esteem, and mental health was $1.25{\pm}0.55$, $3.33{\pm}0.42$, and $2.79{\pm}1.20$ point. Second, mental health was correlated with stress positively and self-esteem negatively. Third, the influencing factor in mental health of the participant were interpersonal relationship and clinical based stress, $R^2=.561$. Thus, development of guidance program to improve mental health of nursing students is recommended.

국내 인터넷건강정보 자율규제방안 (Self-regulation of a Health Information On the Internet)

  • While making a vigorous discussion about self-regulation for exponential growth of harmful health information on the Internet, many countries lave made various efforts to select and circulate high quality health information on the Internet. The purpose of this study Is to review the serf-regulation methods of health information on the Internet and to suggest quality control methods of health information on the Internet suitable for Korea. Self-regulation methods of the health information on the Internet include ‘content rating system(or content selection system)’, ‘codes of conduct or guideline’, ‘internet hot-line’, ‘education for information providers and consumers’. Any self-regulation method should be used with other methods. We can regulate health information on the Internet effectively by using both self-regulation methods and compulsive methods such as law. Also information providers, information consumers, specialists, consumer representatives, scholars, governments officers should take part in doing these efforts and make concern.

시각장애 학생들의 원인질환 및 사회적 지위, 자아존중감, 건강관리 행위에 관한 연구 (A Study on Disease, Self-Esteem, Social Support and Health Management Behavior in Blind & Visually Handicapped Students)

  • Blindness or visually handicaps are found to be an important factor destroying lifestyles as well as a physical problem. According to the data of handicapped person's(v.h.p) registered in Korea, the number of v.h.p. has not decreased. Especially, visual handicapped adolescents (v.h.a.) suffer from an insufficiency of social support under the current education system. There is a special education system for v.h.a. in Korea, and, as a result v.h.a.'s are educated separately from general students and communites. Compared with the self-conception of general students, v.h.a.'s have a more negative self-conception. Among former studies 51.7% of v.h.p's are not in good health. This, of course, means that their handicap impacts on other health problems. The first purpose of this study is to find out the causes of disease and the degree of social support, self-esteem, health management behavior provided and then to analyze the effect of self-esteem and social support on their behavior. To accomplish these purposes, visually handicapped the middle and high school students in Seoul city and Kyung-gi province were selected and 300 students sampled. The main results are as follows : 1) The highest specific handicap is impaired lenses (19.7%). 2. There were high correlations between health management behavior, self-esteem, and social support. 3. Social support is a more important variable than self-esteem on health management behavior. 4. Age of entry and length of stay in their special dormitories, and teacher's support account for 16% of psychological health management behavior 5. Age of entry in their special dormitory and total degree of social support account for 12% of preventive health behavior.

자기조절프로그램이 고혈압환자의 혈압 및 자가간호행위에 미치는 효과 (Effects of a Self-regulation Program on Blood Pressure Control and Self-Care in Patient with Essential Hypertension)

  • Purpose: This study was done to verify the effects of a self-regulation program for management of hypertension. Method: Thirty patients with hypertension registered in a community health center were selected as the experiment group, and control group were patients in another community health center, matched for age and gender. The self-regulation program included daily blood pressure checks, periodic counseling, and health education. A self-check digital device with instructions was provided for self-monitoring of blood pressure, and the participants were interviewed before they took part in the program. Results: The first hypothesis was supported: There will be a greater reduction in both systolic and diastolic blood pressure for patients with hypertension who participate in the self-regulation program compared to patients in the control group. The second hypothesis was also supported: Patients with hypertension who participate in the self-regulation program will perform self-care activities better than those in the control group. Conclusion: The findings indicate that a self-regulation program reduces systolic and diastolic blood pressure and improves self-care in patients with hypertension. It is recommended that this self-regulation program be used in community health clinics for management of hypertension and prevention of complications.

제 2형 당뇨병 환자를 위한 포괄적 자기효능증진 자가관리 프로그램 개발 (Development of a Comprehensive Self-Management Program Promoting Self Efficacy for Type 2 Diabetic Patients)

  • Purpose: The purpose of this study was to develop a comprehensive self-management program promoting self efficacy for Type 2 diabetic patients. Methods: The study was a methodological research design in which previous related research was reviewed to develop the comprehensive self-management program promoting self efficacy using self efficacy theory. Results: Comprehensive self-management programs promoting self efficacy included the whole range of eight self-management domains: diet, exercise, medication, self-testing of blood glucose, complication or foot, stress, time, and general health, and consisted of four sources of self efficacy: enactive mastery experience, vicarious experience, verbal persuasion, and physiological and affective states, as strategies to promote self efficacy. Developmental methods included, in addition to large and small group education, individual education or counseling, and telephone counseling. Conclusion: Further studies are needed in community health centers or hospitals to establish the effects on self-management compliance and glycemic control of the comprehensive self-management program promoting self efficacy.

성인 천식환자의 자가관리교육 중재의 효과 메타분석 (A Meta-analysis of Intervention Studies on the Effects of Self Management and Education in Adult Asthmatic Patients)

  • Objectives : This study was did a meta-analysis of emergency room visits decreased effectiveness of self management education interventions in asthma among adult patients with asthma. Methods : A search of the database PubMed resulted in identifying 8,619 studies done between January 1990 and November 2012. STATA version 10.0 was used to analyze the effect size, explore possible causes of heterogeneity, and determine publication bias with a funnel plot. Results : We included 7 cases on emergency room visits decreased effectiveness of self management and education. Asthmatic patients experienced fewer attacks (summary effect size, 0.75; 95% confidence interval [CI], 0.65-0.95). We found that emergency room visits decreased effectiveness of self management and education were associated with statistically significant effects. Conclusions : Based on these finding, self management and education on interventions are necessary to prevent asthmatic attacks in patients and to reduce the severity of the attacks, and self management and education programs are needed in Korea.

유방암 생존자의 자가관리 지지 경험 (Experiences of Self-management Support among Breast Cancer Survivors)

  • Purpose: The purpose of this study was to explore self-management support experiences among breast cancer survivors. Methods: Individual in-depth interviews were performed for survivors who had treated breast cancer after completion of chemotherapy and radiation therapy. Data were collected from April 2014 to July 2014 and analyzed using phenomenological method by Colaizzi. Results: Five essential themes were found as follows: 1) Need for continuous help and support to maintain self-management, 2) Need for professional resources to carry out regular exercise, 3) Overcoming psychological difficulties through others rather than health care professionals, 4) Disappointment with time with and circumstances of health care consultation, 5) Dependence on means of media for health related information. Conclusion: The results might contribute to an understanding of self-management support experienced by breast cancer survivors in Korea. It is suggested that evidence-based program for self-management support should be developed and applied to nursing intervention for breast cancer survivors.
