Objective : The purpose of this study was to examine the therapeutic compliance and its related factors in the rural hypertensives. Method : A questionnaire survey and blood pressure measurement were performed to 3,876 residents of a rural area, and 660 hypertensives were selected as subjects of study. The study employed a hypothetical model which was composed of constructs from the health belief model and KAP model. The analysis techniques employed included contingency table analysis and structural equation modeling. Result : The proportion of those who were compliant to the treatment of hypertension was 44.2% of subjects. As the result of structural equation modeling, when patients had more favorable attitude toward treatment, higher perceived benefit, or lower perceived barriers to treatment, the therapeutic compliance was significantly higher(T>2.0). When patients had more knowledge about hypertension, or higher perceived severity of hypertension, the attitude toward the treatment of hypertension was more favorable significantly(T>2.0). And when patients had the support for treatment from family or neighbor, the attitude toward treatment was more favorable(T>2.0). When patients had experience of health education, they had more knowledge, higher perceived susceptibility of complication, perceived severity for hypertension, and perceived benefit of treatment, compare to patients without health education(T>2.0). Conclusion : In consideration of above findings, in order to improve the therapeutic compliance in the rural hypertensives, it would be necessary to change attitude, perception, knowledge about hypertension and its treatment, by various methods such as effective health education and programs for maintaining the supportive environment for hypertension treatment.
The acute and chronic toxicity of ambient and effluent water discharged to Lake Shihwa were investigated by using Vibrio fischeri, Daphnia magna and Oryzias latipes. Physicochemical characteristics including biochemical oxygen demand (BOD) and nine heavy metals in a total of 15 water samples were evaluated and were satisfied with relevant Korean Water Quality Standards (KWQS) except for Hg in one sample. Acute toxicity was observed in five samples collected from three sampling locations. When impacts on reproduction and growth after chronic exposure were evaluated with D. magna, all the samples showed significant chronic effects. Reproduction appeared relatively more sensitive endpoint. In 21 days chronic tests on O. latipes, survival, mean egg number per female per day, hatching success rate and time to hatch were affected by increasing sample concentration. The organ-level changes such as gonadosomatic index (GSI), and hepatosomatic index (HSI), and molecular biomarker of vitellogenin (Vtg) induction that evaluated with O. latipes increased as exposure concentrations increased. It is noteworthy that the samples that did not exceed the KWQS resulted in acute and chronic toxicities. The results suggested that numeric criteria based on physicochemical parameters may not be protective of aquatic ecosystem. Acute and chronic toxicity tests with organisms representing different trophic groups should be supplemented in order to provide adequate level of environmental protection.
The purpose of this study is to clarify the background of the controversial attempt to establish a new public medical school linked to compulsory service as a means of strengthening public healthcare in Korea, and to raise anticipated problems with possible solutions. In Korea, healthcare is predominantly provided by the private sector focused on medical care, rather than public healthcare, even under the national health insurance system. The government has been mainly in charge of public health and unmet medical services from a residual perspective, but health inequalities still exist. To resolve this issue, the government created the concept of public health and medical service (PHMS) from a universal perspective and tried to strengthen the infrastructure of public healthcare and to foster core PHMS doctors by establishing a new public medical school linked to compulsory service in medically vulnerable areas. This study investigated the reality and concept of the new public medical school planned by the government, and identified problems such as the possibility of obtaining accreditation and evaluation before its establishment, the side effects of dividing doctors' roles, the waste of huge amounts of resources, and insensitive policies. In conclusion, in order to resolve health inequalities in Korea, we need to train doctors through medical school education that strengthens the social responsibility of doctors along with strengthening public healthcare infrastructure, and to provide a better environment for doctors working in medically vulnerable areas through sophisticated policies.

Green tea is a popular beverage and its health benefits are well known. However, inconsistent results have been reported in observational studies concerning the association between green tea consumption and the lung cancer risk. In this commentary, several methodological issues underlying the measurement of tea exposure are highlighted. The recommendations should be useful for designing and planning prospective cohort studies to ascertain the protective effect of green tea against lung cancer.
Objectives: This study was performed to determine the relationships among health concern, health practice and health status of the disabled. Methods: A self-administered questionnaire survey was carried out for 1,662 disabled persons in Taegu city from April to July, 1997. Results: Health concern had a significant and positive relationship with health practice(p<0.05) and health practice also had a significant and positive relationship with self-rated health status(p<0.05) in men and women. To determine the structural model of the Health concern, health practice, health status and sociodemographic variables, the covariance structural analysis was used. In men, age, economic status, medical security type, educational level and type of disability had significant direct effects on Health concern(T>2.0). Educational level and type of disability had significant direct effects on health practice(T>2.0). And Economic status, medical security type and job status had significant direct effects on health status(T>2.0). In women, economic status and educational level had significant direct effects on Health concern(T>2.0). However there was no variable which had a significant direct effect on health practice. Job status had a significant direct effect or health status(T>2.0). In men and women, health practice was significantly increased with increasing health concern and the more health practice, the higher health status(T>2.0). Conclusions: It is recommended that the institutional approach which improve the economic status of the disabled with understanding their behavior and attitude should be established to increase health status, in addition, the health policy for encouraging the disabled, such as health education, consulting and health promotion program, should be done.
Objectives: This paper aims to identify the dimensions of community capacity in a public residence lease apartment complex in Seoul as a case study, and discusses their application to community health promotion. Methods: The research team performed a focus group interview and a brief survey with resident committee representatives to explore community context and to identify the dimensions of community capacity and their order of priority. Results: We identified many groups with different senses of community and various dynamics in the community. Seven dimensions of community capacity were identified. However, there are different developmental stages among them. Also there are some influences with the different directions in a dimension, e.g. leadership and citizen participation. Conclusions: This study illustrated the identification of the dimensions of community capacity focused on the perspective and recognition of the community and community member, as an effort to understand community capacity in domestic community health promotion context.
