The efficacy of computerized tomographic scan for chest trauma (흉부외상 환자에 대한 전산화 단층촬영의 효용성)

  • Computerized tomography(CT) is an effective technique in the initial evaluation of the abdomen and head following blunt trauma. To evaluate the efficacy of CT of the thorax, a retrospective study comparing early thoracic CT scanning with initial chest roentgenogram (CXR) was carried out on 134 patients with blunt trauma on the chest. Among 134 patients, 45 patients had normal initial chest roentgenogram and 24 patients showed normal CT findings. Sensitivities of diagnosing pneumothorax and pleural effusion by CXR were low (46.2 and 62.9% respectively), whereas 71.4%(45/63) of patients had thoracostomy only by CXR. Although sometimes abused, CT of the thorax is effective in the initial diagnosis.

The Utility and Benefits of External Lumbar CSF Drainage after Endovascular Coiling on Aneurysmal Subarachnoid Hemorrhage

  • Objective : Cerebral vasospasm still remains a major cause of the morbidity and mortality, despite the developments in treatment of aneurysmal subarachnoid hemorrhage. The authors measured the utility and benefits of external lumbar cerebrospinal fluid (CSF) drainage to prevent the clinical vasospasm and its sequelae after endovascular coiling on aneurysmal subarachnoid hemorrhage in this randomized study. Methods : Between January 2004 and March 2006, 280 patients with aneurysmal subarachnoid hemorrhage were treated at our institution. Among them, 107 patients met our study criteria. The treatment group consisted of 47 patients who underwent lumbar CSF drainage during vasospasm risk period (about for 14 days after SAH), whereas the control group consisted of 60 patients who received the management according to conventional protocol without lumbar CSF drainage. We created our new modified Fisher grade on the basis of initial brain computed tomography (CT) scan at admission. The authors established five outcome criteria as follows : 1) clinical vasospasm; 2) GOS score at 1-month to 6-month follow-up; 3) shunt procedures for hydrocephalus; 4) the duration of stay in the ICU and total hospital stay; 5) mortality rate. Results : The incidence of clinical vasospasm in the lumbar drain group showed 23.4% compared with 63.3% of individuals in the control group. Moreover, the risk of death in the lumbar drain group showed 2.1 % compared with 15% of individuals in the control group. Within individual modified Fisher grade, there were similar favorable results. Also, lumbar drain group had twice more patients than the control group in good GOS score of 5. However, there were no statistical significances in mean hospital stay and shunt procedures between the two groups. IVH was an important factor for delayed hydrocephalus regardless of lumbar drain. Conclusion : Lumbar CSF drainage remains to playa prominent role to prevent clinical vasospasm and its sequelae after endovascular coiling on aneurysmal subarachnoid hemorrhage. Also, this technique shows favorable effects on numerous neurological outcomes and prognosis. The results of this study warrant clinical trials after endovascular treatment in patients with aneurysmal SAH.

Multiple Transmit Focusing Method With Modified Orthogonal Golay Codes for Ultrasound Imaging (초음파 영상에서 변형된 직교 골레이 코드를 이용한 동시 다중 송신 집속 기법)

  • Coded excitation with complementary Golay sequences is an effective means to increase the SNR and penetration of ultrasound imaging. in which the two complementary binary codes are transmitted successively along each scan-line, reducing the imaging frame rate by half. This method suffers from low frame rate particularly when multiple transmit focusing is employed, since the frame rate will be further reduced in proportion to the number of focal zones. In this paper. a new ultrasound imaging technique based on simultaneous multiple transmit focusing using modified orthogonal Golay codes is proposed to improve lateral resolution with no accompanying decrease in the imaging frame rate, in which a pair of orthogonal Golay codes focused at two different focal depths are transmitted simultaneously. On receive, these modified orthogonal Golay codes are separately compressed into two short pulses and individually focused. These two focused beams are combined to form a frame of image with improved lateral resolution. The Golay codes were modified to improve the transmit power efficiency (TPE) for practical imaging. Computer simulations and experimental results show that the proposed method improves significantly the lateral resolution and penetration of ultrasound imaging compared with the conventional method.

3D Face Modeling based on 3D Morphable Shape Model (3D 변형가능 형상 모델 기반 3D 얼굴 모델링)

  • Since 3D face can be rotated freely in 3D space and illumination effects can be modeled properly, 3D face modeling Is more precise and realistic in face pose, illumination, and expression than 2D face modeling. Thus, 3D modeling is necessitated much in face recognition, game, avatar, and etc. In this paper, we propose a 3D face modeling method based on 3D morphable shape modeling. The proposed 3D modeling method first constructs a 3D morphable shape model out of 3D face scan data obtained using a 3D scanner Next, the proposed method extracts and matches feature points of the face from 2D image sequence containing a face to be modeled, and then estimates 3D vertex coordinates of the feature points using a factorization based SfM technique. Then, the proposed method obtains a 3D shape model of the face to be modeled by fitting the 3D vertices to the constructed 3D morphable shape model. Also, the proposed method makes a cylindrical texture map using 2D face image sequence. Finally, the proposed method builds a 3D face model by rendering the 3D face shape model with the cylindrical texture map. Through building processes of 3D face model by the proposed method, it is shown that the proposed method is relatively easy, fast and precise than the previous 3D face model methods.

Particle Spacing Analysis of Frozen Sand Specimens with Various Fine Contents by Micro X-ray Computed Tomography Scanning (Micro X-ray CT 촬영을 통한 동결 사질토 시료의 세립분 함유량에 따른 입자간 거리 분석)

  • The mechanical characteristics of frozen sand greatly depend on the frozen temperature and the fine contents according to the previous study by Chae et al. (2015). There are two hypotheses to explain this experimental results; one is the unfrozen water contents greatly affected by the fine contents and frozen temperature and the other is the sand particle spacing greatly affected by the pore-ice. To evaluate the latter hypothesis, the micro X-ray CT scan was performed. The micro X-ray CT scanning, one of the actively performed interdisciplinary research area, has a high resolution with micrometer unit allows to investigate internal structure of soils. In this study, X-ray CT technique was applied to investigate the effect of the frozen temperature and fine contents on the sand particle minimum and average spacing with the developed image processing techniques. Based on the spacing analysis, the frozen temperature and fine contents have little influence on the sand particle spacing in the frozen sand specimens.

Evaluation of bone quality in alveolar crest obscured by dental implants ; A pilot study by densitometric digital analysis in mandibular bone specimen (치과 임플란트 주변 협설측 치조골의 변화분석 - 하악골 시편에서의 디지털 농도분석법을 이용한 실험적 고찰 -)

  • Despite of technical difficulties, the combination of occlusal projection and densitometric digital analysis may ultimately provide a means of detection of subtle bone loss at the facial and lingual side of dental implant (Oblique occlusal view is more useful for $ITI^{(R)}$ dental implant due to its contour of shoulder as like tulip flower). In this study, conventional periapical projections of x-ray beam had shown more high sensitivity to detect the bony defects than oblique occlusal projections in alveolar crest obscured by dental implants or not, even if the difference was not statistically significant. Unlike conventional periapical projections. occusal projections combined with densitometric digital analysis technique may provide a means for detection of subtle bone change at the all around of implants without obscuring effect by implant itself. Although the results from this in vitro study were performed under limited circumstances, these results might afford more possibility and versatile modality of diagnosis options to clinician in the implant practice.

Estimation of Medical Ultrasound Attenuation using Adaptive Bandpass Filters (적응 대역필터를 이용한 의료 초음파 감쇠 예측)

  • Attenuation coefficients of medical ultrasound not only reflect the pathological information of tissues scanned but also provide the quantitative information to compensate the decay of backscattered signals for other medical ultrasound parameters. Based on the frequency-selective attenuation property of human tissues, attenuation estimation methods in spectral domain have difficulties for real-time implementation due to the complexicity while estimation methods in time domain do not achieve the compensation for the diffraction effect effectively. In this paper, we propose the modified VSA method, which compensates the diffraction with reference phantom in time domain, using adaptive bandpass filters with decreasing center frequencies along depths. The adaptive bandpass filtering technique minimizes the distortion of relative echogenicity of wideband transmit pulses and maximizes the signal-to-noise ratio due to the random scattering, especially at deeper depths. Since the filtering center frequencies change according to the accumulated attenuation, the proposed algorithm improves estimation accuracy and precision comparing to the fixed filtering method. Computer simulation and experimental results using tissue-mimicking phantoms demonstrate that the distortion of relative echogenicity is decreased at deeper depths, and the accuracy of attenuation estimation is improved by 5.1% and the standard deviation is decreased by 46.9% for the entire scan depth.

Usefulness of a FDG-PET Scan in Assessing Recurrent Gastric Cancer (재발한 위암의 진단에 사용된 FDG-PET의 유용성)

  • Purpose: For curative resection of recurrent gastric cancer, it is imperative that there be no unrecognized foci of tumoral disease outside the operation field. PET (positron emission tomography) with FDG (18 fluoro-2 deoxy-D-glucose) is a whole-body imaging technique that exploits the increased rate of glycolysis in tumor cells to detect disease. The authors evaluated the usefulness of FDG-PET in assessing resectability of recurrent gastric cancer. Materials and Methods: Seven patients with recurrent gastric cancer were studied with FDG-PET from December 1998 to October 2000. All FDG-PET images were interpreted in conjunction with conventional diagnostic methods. All imaging results were correlated with the pathological diagnosis and clinical outcome. Results: A final diagnosis of recurrence was obtained at 14 sites in all 7 patients by histology or clinical follow up. Locoregional recurrence, including distant metastasis, developed in 6 of 7 patients and distant recurrence in only one. FDG-PET detected all recurrent sites (5 locoregional and 5 distant) in 5 patients without peritoneal recurrence, but did not detect peritoneal seeding in 2 patients with peritoneal recurrence. The accuracy of FDG-PET in estimating resectability was $71.4\%$ (5/7), and that of CT and PET together was $85.7\%$ (6/7). A curative resection could be performed in three of the recurrent patients (2 locoregional and 1 distant recurrence). Conclusion: Our results suggest that FDG-PET may be useful for detecting locoregional and distant recurrence of gastric cancer and for selecting appropriate treatment. However, considering that FDG-PET was limited in detecting peritoneal seeding and determining the exact anatomical extension of tumor, it should be used in conjunction with other anatomical images.

3-D Radiosurgery Planning Using Personal Computer (Personal Computer를 이용한 3차원적 뇌정위적 방사선 치료계획)

  • Recently, stereotactic radiosurgery plan is required with the information of 3-D image and dose distribution. The purpose of this research is to develop 3-D radiosurgery planning system using personal computer. The procedure of this research is based on three steps. The first step is to input the image information of the patient obtained from CT or MR scan into personal computer through on-line or digitizer. The position and shape of target are also transferred into computer using Angio or CT localization. The second step is to compute dose distribution on image plane, which is transformed into stereotactic frame coordinate. and to optimize dose distribution through the selection of optimal treatment parameters. The third step is to display both isodose distribution and patient image simultaneously using superimpose technique. This prototype of radiosurgery planning system was applied recently for several clinical cases. It was shown that our planning system is fast, accurate and efficient while making it possible to handle various kinds of image modelities such as angio, CT and MRI. It is also possible to develop 3-D planning system in radiation therapy using beam's eye view or CT simulation in future.

Perfusion RRI of the Brain Using Oxygen Inhalation (산소 호흡을 이용한 뇌의 관류 자기공명영상)

  • Purpose : To know the possibility of clinical application of MRI using oxygen inhalation as a perfusion MRI Materials and methods : Two healthy volunteers and three patients of one moyamoya disease, one acute infarction and one meningioma were studied using a 1.5 Tesla MRI unit. Oxygen (15 liters/min) mixed with room air was given using face mask from 8 second to 35 second during the study. Images were acquired 25 times (scan time per study were 1.6 seconds) using susceptibility contrast EPI (echo planar image) sequence. Difference maps were acquired by early (study 12-18), and late (study 19-25) O2 inhalation image groups minus pre-O2 inhalation image group (study 3-9) with a Z-score of 0.7-1.0 using VB31C program of Magneton Vision. The resulting perfusion images were created by superimposition of difference maps on corresponding T1 weighted anatomic images. On moyamoya patient, similar perfusion images were acquired after Gd-DTPA injection, and compared with O2 inhalation perfusion images. Results ; The author can get the perfusion images of the brain by oxygen inhalation with susceptibility contrast EPI sequence at the volunteers, and the patient of moyomoya disease, acute infarction and meningioma. On moyamoya patient, perfusion images with O2 inhalation are similar with perfusion images by Gd-DTPA injection. Conclusion 1 This study has demonstrated that the susceptibility contrast EPI by oxygen inhalation can be used as the clinically useful perfusion MRI technique
