의료기관 평가제도에 대한 인식조사와 개선방안 고찰 (Opinions and Strategies on the National Hospital Evaluation Program)

  • Purpose: The purpose of this paper was to investigate opinions and experiences of hospital CEOs' and QI managers toward the National Hospital Evaluation Program which was implemented in 2004 and to recommend various strategies to improve the program. Methods: We conducted a mail survey with CEOs and QI managers' of 78 hospitals with 500 beds or more that participated the 2004 National Hospital Evaluation Program. Results: About 70.8% of the participating CEOs and 64.0% of the QI managers felt that the objectives of the evaluation program weren't fully achieved. Most respondents said that the current program required a partial or an overall change. Evaluation Criteria was the most often cited area for a change. Many respondents pointed out the importance of including clinical quality indicators in the evaluation tool. Conclusions: To upgrade hospital services with better quality, it is most important to first reach consensus on objectives and approaches of the evaluation program among various players. For a consistent planning and implementation, it is urgent to set up a more systematic organization and financing mechanism. Also, evaluation approaches, including evaluation criteria, methods, patient satisfaction assessment, as well as ways to summarize and publicize each hospital's performance should be improved.

중소 규모의 치과의원에 균형성과표를 적용하기 위한 핵심성과지표 개발 (Development of Key Performance Indicators to Implement Balanced Scorecard to Small and Medium Size Dental Clinic)

  • The purpose of this study is to develop the KPIs(Key Performance Indices) needed to improve management and strategy in the dental clinic based on the four perspectives of BSC(Balanced Scorecard). The questionnaire was conducted on 52 dentists approved by Dental Managment Research Committee in Seoul National University as a panel. Using the Delphi technique, the top five KPIs for each point of perspective in BSC were extracted from KPI pools. In the third survey, the top five KPIs of all points were compared with each other through AHP(Analytic Hierarchy Process) method, and priority and overall importance rankings were calculated. The biggest difference in the three level AHP results was the customer perspective took priority to others. In the second survey, the financial perspective, which was number one, was pushed back. The overall significance of KPIs was in the order of customer, internal process, finance, learning and growth perspective, with the exception of medical profits (5th of 20) and new patient growth (10th of 20). We were able to overcome the limitations of the Delphi Technique with the AHP method. In general, the financial perspective in BSC is known to be the most important, but we conclude that the customer perspective is more important through the pairwise comparison survey. In the current dental service market, which is a long-term recession, excessive competition, customer satisfaction and customer relationship management seem to be the first goal to pursue in dental clinic.

Effects of Korean Medicine Health Promotion program for Children -Focus on Education and Prevention Programs-

  • Objectives: This study was designed to confirm the effects of the Korean Medicine health promotion program targeting young children. One important aspect of the Korean Medicine health promotion program for children is to bring awareness of health benefits of introducing Korean Medicine methods to young children and to promote incorporating such methods in children's daily activities to develop healthy habits. Methods: The following experiment was conducted for 6 weeks, and the participants were in the age between 2 to 6 years old from 6 child care centers in Wonju and Chungju. 268 participants were in the intervention group, and 277 in the control group. To measure the outcomes of the Children Korean Medicine health program, the intervention group received Korean Medicine prevention programs (children education, parent education, growth exercises, common cold prevention scarf, and pear herb tea), control group did not. This research collected and compared the following data as the children's health indicators to measure the outcomes before and after the program: medical services usage, the number of absences, early leaves, and tardiness to the child care centers due to the participant's health condition. Results: After applying the children's health promotion programs on children between the age of 2-6, there was a decrease in the overall medical services usage, absences, early leaves, and tardiness to the child care center. This health promotion program was purely based on Korean medical theories and Korean Medicine resources without any conventional medical mediation. Conclusions: It can be concluded that a positive change was noted in raising awareness of Korean Medicine through educating both the parents and children. By learning to incorporate Korean Medicine prevention practices to daily habits, not only was there a reduction in the Medical services usage, but the children also became familiar and comfortable with Korean Medicine and its treatment procedures; the overall satisfaction level of the program was also high.

언론인의 직무 스트레스와 대처방안 척도에 관한 연구 -서울지역 일간지 취재기자를 중심으로- (Research on the job stress of journalists and indicators of countermeasures -Focusing on daily newspaper reporters and journalists in area of Seoul-)

  • 본 연구는 서울지역에 본사를 둔 일간지 취재 기자들을 대상으로 업무를 수행하면서 겪고 있는 직무 스트레스와 대처방식 척도에 관한 요인을 파악하고자 한다. 연구대상자는 일간지 취재기자 170명에 대해 2013년 3월 1일부터 7월 30일까지 대인면접 설문조사를 실시하였다. 자료 분석은 SPSS Win 21.0 프로그램을 사용하여 빈도분석, T-test와 일원 변량분석, 상관관계분석을 하였고, 실증분석은 유의수준 5%에서 검증하였다. 연구 분석결과 첫째, 직업에 대한 만족도가 높으면 직무 스트레스는 낮아 유의한 것으로 나타났다. 둘째, 흡연을 많이 하는 경우와 스트레스 해소방법이 '예술 활동'인 경우 스트레스 대처방법이 높아 유의하게 나타났다. 셋째, 직무 스트레스가 높으면 대처방식도 높아 유의한 차이로 나타났다. 본 연구의 시사점으로 언론인들의 원활한 업무수행이 가능할 수 있는 근무환경을 제공하기 위한 기초적인 정책 자료가 될 것으로 사료 된다.

전자유통경로의 성과평가 척도에 관한 연구 (A Study on the Determinants for Measuring Performance of the Electronic Goods Distribution Channel)

  • 유통경로의 성과를 측정할 수 있는 요인들을 분류하는 접근방법은 연구자의 관점에 따라 다양하게 이루어져 왔다. 그러나 이같이 다양한 성과측정변수들이 실제로 국내의 경로 구성원에 의해 유통성과를 평가할 때 사용되고 있는지의 여부와 국내 유통경로의 경영자들이 판단하는 총체적인 경영성과에 이들 변수들이 차지하는 비중이 차별화 되고 있는지에 대한 연구는 미미하여 왔다. 이에 본 연구는 국내의 전자 유통대리점들을 대상으로 경로 구성원들이 사용하는 성과평가척도의 요소를 살펴보고 이들 요소들이 총 유통성과에서 차지하는 비중을 도출하며 경로구성원의 특성에 따른 측정요소의 차이점이 발생하는가를 검증하는데 그 목적을 두었다. 이를 위해 요인분석을 통해 성과 측정 요소들이 어떠한 공통의 집단으로 나뉘어 지는가를 살펴보며 상관분석을 통해 이들이 총 유통성과를 평가하는데 기여하는 비중을 도출한다. 이에 따른 연구의 결과를 살펴보면 경로구성원의 성과 요인은 재무적 성과 마케팅 지원 성과, 판매원 서비스 및 소비자 만족도 성과 그리고 소비자 소비스 성과의 4 요인으로 분류 되었고 재무적 성과와 마케팅 지원성과 요인이 총유통성과와의 상관도가 가장 높았다. 또한 경로구성원의 특성에 따라 성과척도의 중요 요인에 차이가 발생한다는 사실을 확인 할 수 있었다.

통합여수시 지역주민의 삶의 질 변화와 도시발전의 과제 (The Direction of Urban Development and the Change of Quality of Life in Integrated Yosu City)

  • 본 연구는 통합여수시를 사례로 도시생활환경의 측정 및 삶의 질 변화에 대한 도시민의 인식.평가의 변화를 설문조사를 통해 살펴보았고, 이를 바탕으로 지역주민의 삶의 질을 향상시킬 수 있는 정책적 대안을 제시하였다. 통합여수시의 삶의 질은 5년 전에 비해서 전체적으로 삶의 질은 향상된 것으로 분석되었다. 그러나 삶의 질에 관한 9개 지표 중에서 주거생활, 도시행정 및 편익서비스 등은 5년전보다 상대적으로 향상된 반면, 자연환경, 문화 및 여가생활, 교통환경 등은 나빠진 것으로 평가하였다. 그리고 여수시민들은 통합여수시의 지속적인 발전을 도모하기 위해서는 교통문제, 문화 및 여가시설의 확충 등이 주요한 과제라고 인식하였고, 21세기의 통합여수시가 해양관광도시, 교육문화도시로 발전하기를 선호하였다. 따라서 여수대도시권을 구축율 위한 주요 과제로는 경제기반시설의 확충, 워터프런트의 개발 및 보전, 관광산업의 활성화, 시민중심의 도시경관 조성, 지역문화운동의 전개 등이 필요한 것으로 밝혀졌다.

청년의 세대관계 유형화: 세대 간 결속의 하위차원을 중심으로 (Typology of Young Korean Adults' Relationships with their Parents from an Intergenerational Solidarity Lens)

  • The literature on parent-child relationships in young adulthood lacks a systemic approach that considers the multifaceted nature of intergenerational relationships. In this paper, we identify the latent profiles of young Korean adults' relationships with their parents based on the six dimensions of intergenerational solidarity (structural, associational, affectual, consensual, functional, and normative solidarity) as indicators. We considered solidarity for the mother and the father separately for structural, associational, affectual, and consensual dimensions. In terms of functional and normative solidarity, we measured both upstream and downstream intergenerational support. The sample included 1,015 young adults who were 19-34 years old, never married, not in secondary school, and had both parents living. The latent profile analysis revealed four profiles: (a) independent but intimate (22.7%), (b) coresident, outwardly intimate (32.4%), (c) detached from father (6.7%), and (d) coresident, intimate (38.2%). The factors that predicted each profile included the young adults' education, income, subjective socioeconomic status, and experiencing the features of emerging adulthood along with the parents' marital status, father's employment, and overparenting. The "coresident, intimate" group reported significantly higher levels of life satisfaction and fewer depressive symptoms compared to the other three groups. The lowest levels of psychological adjustment were found in the "coresident, outwardly intimate" group and "detached from father" group.

의료인문학 교육과정 개편에 대한 Kern의 교육과정개발 모델에 근거한 비판적 성찰 (A Critical Review of Medical Humanities Education Curriculum Development Based on Kern's Curriculum Development Model)

  • Medical humanities education (MHE) is as essential as basic medical sciences and clinical medicine education. Despite the importance of MHE, MHE curriculum development (CD) has proven to be challenging. This critical review examines the MHE CD at one medical school. The critical review methodology was developed based on Kern's six step CD model to systematically examine the CD of "Doctoring and Medical Humanities (DMH)" at the Yonsei University College of Medicine. Five review questions were developed related to (1) necessity, (2) direction and purpose, (3) design, (4) operation, and (5) evaluation of CD based on Kern's model. The review showed that the process of DMH CD mapped to components of Kern's model. The DMH curriculum content selected was closely related to medical practice and aimed to combine the acquisition of understanding and skills by designing a student-participatory curriculum based on clinical cases. Assessment methods that emphasized students' reflections were actively introduced in the evaluation section. Since the regular committee for DMH continued the work of the special ad hoc committees for DMH CD, the CD was effectively completed. However, the planning and evaluation functions and responsibilities of the DMH committee need to be strengthened. Despite the apparent limitations, the fact that students showed a high satisfaction rate and preferred small group discussions based on clinical cases has significant implications in the instructional design of MHE, where changes in self-awareness and attitude are more important than the acquisition of information. It is necessary to systematically review and study students' reflection results produced by the changed assessment methods and to develop assessment indicators for MHE that reflect the achievements of the MHE competencies of students.

과학기술 정보유통을 위한 업무 프로세스 표준화 (Business Process Standardization of Information Flow for Science & Technology)

  • 최근에 많은 정부출연 연구기관과 정보유통서비스 기관들이 ISO 9001 품질경영시스템을 도입하여 그들 조직의 업무 수행도를 향상시키는데 대한 관심이 두드러지게 증가하고 있는 실정이다. 하지만 도입시에 조직의 특성 및 현황을 제대로 반영하지 않아서 시스템의 요구사항과 프로세스가 지향하는 목표화의 현실적 차이 때문에 ISO 9001 품질경영시스템 운영에 따른 효과를 제대로 누리지 못하고 있다. 이에 한국과학기술정보연구원(KISTI)은 프로세스 표준화를 통한 생산성 증대 및 조직의 경쟁력 증가를 위해 ISO 9001 시스템의 요구사항을 반영하기 위한 프로세스 분석을 통하여 표준시스템 지향적 프로세스를 도출하였다. 그 결과, KISTI는 2003년 11월 11일에 ISO 9001:2000 인증을 받았고, 품질경영 업무를 수행함으로서, 고객 만족도 증가, 업무의 효율성 증대 및 업무처리 시간 단축 등의 효과를 얻을 수 있었다.

공공보건의료인력 임상교육효과 평가: 지역거점공공병원 간호사 대상 (The Effects of Clinical Education Program for Nurses in Regional Public Hospital)

  • Background: The purpose of this study is to evaluate the outcomes of clinical education program for nurses in regional public hospital, utilizing the Kirkpatrick's model. Methods: Kirkpatrik's 4-level model was applied to this study. Trainees were asked to fill out questionnaires in the middle and at the end of the program. Also administrators of excellent trainees were asked to fill out the questionnaires regarding nursing management performance after 1-2 months from the end of the training course. Results: All trainees had positive reactions to the clinical education program. Not only the results of individual level (satisfaction and achievement scores, academic achievement scores, practical application rate, and educational transition factors) but also the scores of organization level (nursing management performance scores) are improved. Conclusion: By showing a correlation between the effectiveness factors we need to verify the relationship between these factors in a future study. In addition, development of quantitative and qualitative performance indicators are needed. To establish a long-term education system, it is required to applying the excellent trainee's successful experiences.
