The Clinical Study on the Body Weight Loss of Taeumin Inpatients in Stroke by Sasang Constitutional Medicinal Treatment (태음인 중풍환자에 있어 체질치료가 체중감소에 미치는 영향에 대한 임상적 연구)

  • The Clinical Study on the Body Weight Loss of Taeumin Inpatients in Stroke by Sasang Constitutional Medicinal Treatment. Purpose: It has been frequently noted that overweight and obesity have a stronger relationship to hypertension and diabetes mellitus than to the risk of stroke. The purpose of this study is to find out whether Sasang Constitutional Medicinal treatment in the stroke patients of Tae-um-in has an effect on local or general weight loss. Methods: Case-control study was applied. The experiment group is the 24 Taeumin stroke inpatients who were in hospital in Kyung-Hee Oriental Hospital from July 1, 2000 to October 1 and received Sasang Constitutional Medicinal treatment. The control group is the 13 Taeumin stroke inpatients who were in the hospital during the same period but didn't receive Sasang Constitutional Medicinal treatment. In the two groups, body weight, BMI, the skin fold(cm) of triceps, suprailiac, abdominal portion, and thigh and the circumference(cm) of wrist, biceps, triceps, and W/H ratio were compared and studied for three weeks. Results: 1. In the experiment group, there were 8 women and 15 men, 23 in total and in the control group, there were 5 women and 9 men, 14 in total. 2. From two weeks later, there was a significant decrease in the experiment group in weight loss, the ratio of loss, and BMI(p<0.05). 3. Concerned with the change in the skin fold of suprailiac portion, there was a significant decrease in each group(p<0.05). 4. Concerned with the change in the skin fold of Chunchu-Hyel portion(天樞穴), thigh and in the circumference of hip, W/H ratio, there was a significant decrease in the experiment group(p<0.05). 5. Concerned with the change in the value of Lipid Profile, GOT, and GPT, only Glucose(GLU) showed a significant decrease(p<0.05) in the experiment group(p<0.05). As Conclusion, Sasang Constitutional Medicinal treatment in the stroke patients of Tae-um-in has an effect on local or general weight loss.

  • Hak-Jil(瘧疾) has a broad sense of affection due to exgenous pathogenic factors including repeating creeping chill, fever and aversion to heat during fever, in Oriental Medicine. Hak-jil(瘧疾) has a very fast progress and seriousness, so patients and protectors want to visiting western medicinal hospital, not oriental medicinal hospital. But, We accidentally met Soeumin man having Hak-jil(瘧疾) and acquired good results by treatment him with Sasang Constitutional Medicinal method. This paper describe the process and contents about the way the patient was cured.

  • We managed a Taeumin(太陰人) patient with pleural effusion due to heart failure. We diagnosed as Wiwan-suhan-pyohan-byung(胃完受寒表寒病), and treated with Jowiseungchungtang(調胃升淸湯), going with western medicinal management. His chief complaint was orthopnea, chest discomfort, cough. The state of patient was measured with patient's complaints and chest X-ray. We gained outstanding improvement on patient's chief complaints, but there was no particular change of chest x-ray until thoracentesis. In this case, it seems that cooperations of east-west medicine would be beneficial to treatment of patients.

  • 1. Objectives The purpose of this case is to report a Taeumin, who was examined as Hwa-Byeong, received a medical treatment with Cheongsimyeonja-tang and consult of Sasang Constitutional Medicine. 2. Methods We examined this patient as Hwa-Byeong by using Hwa-Byeong Diagnostic Interview Schedule(HBDIS) and we also diagnosed a patient's mental state by using MMPI to understand a human instinct of the patient. Additionally, we used QSCC II and PSCC to distinguish a patient's Sasang Constitution. 3. Results We received a positive result by using prescription as Cheongsimyeonja-tang and consult according to a patient's Sasang Constitutional Medicine. 4. Conclusions We suggested that the patient would receive steady treatments as consult of Sasang Constitutional Medicine and medicinal therapy afterwards.

  • 1. Objectives This case study describes a successful treatment process of a Soyangin patient with somatoform autonomic dysfunction symptoms using Soyangin therapeutic measures, including administration of Dokhwaljihwang-tang (獨活地黃湯) and Sibyimijihwang-tang (十二味地黃湯). 2. Methods The patient was treated with several constitutionally discriminated medicinal agents (獨活地黃湯, 十二味地黃湯) and acupunctural therapy accompanied by simultaneous western medical management. The visual analogue scale (VAS) was applied to assess the severity of flushing. 3. Results and Conclusions As flushing was one of the chief complaints, the patient was treated with Soyangin therapeutic measures including Dokhwaljihwang-tang and Sibyimijihwang-tang. The patient responded positively to the therapy, but further studies are anticipated for more definitive conclusions.

A Study on the Diseases and Pharmacy of the Soumin's Sinsooyul-Pyoyul-Byung theory (소음인(少陰人) 신수열표열병론(腎受熱表熱病論)의 병증(病證) 및 약리(藥理)에 대한 연구(硏究))

  • The main purpose of this study is to arrange the diseases, principal medicinal substances and the prescriptions for the Soumin's Sinsooyul-Pyoyul-Byung(少陽人 腎受熱表熱病) by historical examination in the viewpoint of the Constitutional Medicine(四象醫學). In this study, first I collected the original texts of the diseases and priscriptions in Dongyi-Soose-Bowon(東醫壽世保元), and described the development process of the Soumin(少陽人)'s diseases and priscriptions by comparing with the contents in Dongyi-Soose-Bowon(東醫壽世保元). Next, I collected the efficacy of the medicinal substances and priscriptions by the examination of successive literatures referred it, and tried to explain the efficacy of each medicinal substances in the viewpoint of the Constitutional Medicine(四象醫學) by the analysis of successive literatures. As a result, the conclusions could be summarized as follows. 1. The Differentiation of Syndromes(辦證) in the Existing Medicine(旣存醫學) was formed according to the Eight Principal Syndromes(八綱), the Five Elements(五行), the Merdian system(經絡), therefore there were many confusions with the changes of the times. The differentiation of Syndromes(辦證) in the Constitutional Medicine(四象醫學) was formed according to the pathological change of Ascending and Descending Yin Yang(陰陽升降) in the Four Internal Organs(四臟), therefore, there was the consistency in the recognition of diseases. 2. The classification of the Soumin's Pyoyul-Byung(少陽人 表熱病) was divided into Yukuang Disease(鬱狂證) and Mangyang Disease(亡陽證) according to sweating or not. The conception of these diseases was mostly achived in Shanhanlun(優寒論). The treatment of Shanhanlun(優寒論) was alike to that of Dongyi-Soose-Bowon(東醫壽世保元) in a part. 3. Je ma, Lee thinked that the cause of the Soumin's Pyoyul-Byung(少陽人 表熱病) is the breakdown of the balanced equilibrium in ascending and descending Yin Yang(陰陽升降) for a Large Sin and a Small Bi(腎大脾小), and it is treated by warming and ascending Yang(溫補升陽). 4. The medicinal substances composing prescriptions for the Soumin's Pyoyul-Byung(少陰人 表熱病) is characterized with the efficacy of Ascending Yang and supplying Qi(升陽益氣) and regulating the digestive organs with the efficacy of warming it. 5. The medicinal substances composing the precriptions for the Soumin's Pyoyul-Byung(少陰人 表熱病) is divided into four parts with the state of Yang Qi(陽氣) in the disease it used. 6. The purpose of prescriptions for the Soumin's Pyoyul-Byung(少陰人 表熱病) is to strengthen the Soumin(少陰人)'s Yangnan Qi(陽暖之氣). Je ma, Lee kept the principle not to mix with another constitution's medicinal substances in the creation of the priscription.

A Review on Acne Treatment in Korean Medicine by Analyzing Case Studies with Herbal Medicine Treatment (국내 한약 치험례 분석을 통한 한의학적 여드름 치료법에 대한 고찰)

  • Objectives : The purpose of this study is 1) to explore the general tendency of the herbs or herbal formula for acne treatment in clinical practice 2) and to provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism by analyzing the herbs or herbal formula using for acne treatment in clinical practice. Methods : In the domestic databases (Oriental medicine Advanced Searching Integrated System-OASIS, Korean Traditional Knowledge Portal-KTKP, National Discovery for Science Leader-NDSL, Research Information Sharing Service-RISS), we selected among the papers published using search terms related to "acne". Reports related to sasang constitutional medicine were excluded and a total of 23 papers were finally selected. Results : 29 prescriptions were retrieved from 23 papers. Chungan-tang (淸顔湯) notified formulas ranked first by Cheongsangbangpung- tang (淸上防風湯), Hwangnyeonhaedok-tang (黃連解毒湯), Dangguijakyaksan (當歸芍藥散) and Bipachungpe-eum (枇杷淸肺飮). Commonly used herbs are Forsythiae Fructus (連翹), Scutellariae Radix (黃芩), Gardeniae Fructus (梔子), Ligustici Rhizoma (川芎), and Angelicae Gigantis Radix (當歸). In most patients with inflammatory lesions or excessive flushing, exterior-releasing medicinal (解表藥) and heat-clearing medicinal (淸熱藥) were mainly prescribed for acne treatment, while in patients with acne accompanied hyperkeratinization, Excess sebum secretion, Dysmenorrhea or indigestion, qi-tonifying medicinal (補氣藥) or blood-tonifying medicinal (補血藥) were mainly used. KAGS (Korean Acne Grading System) was the most commonly used method for acne severity assessment. Conclusions : These findings suggest that the herbs or herbal formula for acne treatment in Korean Medicine is prescribed differently according to the cause and symptoms of acne in a unique way for control the whole body balance. Also, our findings could provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism in clinical practice.

Behaviors of Providers of Traditional Korean Medicine Therapy and Complementary and Alternative Medicine Therapy for the Treatment of Cancer Patients

  • Objectives: In Korea, cancer is one of the most important causes of death. Cancer patients have sought alternative methods, like complementary and alternative medicine (CAM) together with Western medicine, to treat cancer. Also, there are many kinds of providers of CAM therapy, including providers of Korean oriental medicine therapy. The purpose of this study is to identify the behaviors of Korean oriental medicine therapy and CAM therapy providers who treat cancer patients and to provide background knowledge for establishing a new policy with the management and quality control of CAM. Methods: Structured and well organized questionnaires were made, and 350 persons were surveyed concerning the providers of CAM or Korean oriental medicine. The questionnaires were collected and analyzed. Results: The questionnaires (182) were collected. The questionnaires identified a total of 73 known providers, such as medicinal professionals or other providers of CAM suppliers, 35.6% of whom had had experience with treating cancer patients (52.6% vs. 29.6%). The treatment methods were a little different: alternative therapy and nutritional therapy being preferred by medicinal professionals and mind body modulation therapy and alternative therapy being preferred by other CAM providers. Four patients (7.4%) experienced side effects, and 6 patients (12.5%) experienced legal problems. As the method for managing the therapy, CAM providers, medicinal professionals, and other CAM providers had different viewpoints. For example, some CAM providers stated that both legislation and an official education on CAM or a national examination were needed as a first step to establish the provider's qualifications and that as a second step, a license test was needed for quality control. To the contrary, medicinal professionals stated that a license test was needed before legislation. Conclusion: Adequate management and quality control of CAM providers is thought to involve both education and legislation.

Analysis of Korean Herbal Medicine Used in Case Studies of Treating Urticaria (두드러기 증례 연구에서 사용된 한약 처방 분석)

  • Objectives : The purpose of this study is to analyze the use of herbal medicine for the treatment of urticaria in case studies and to find the general tendency of herbal medicine treatment and to establish the primary treatment direction of urticaria. Methods : In the domestic databases, Oriental medicine Advanced Searching Integrated System (OASIS), Korean Traditional Knowledge Portal (KTKP), National Discovery for Science Leader (NDSL), Research Information Sharing Service (RISS), we selected among the case study papers published in the last 20 years using search terms related to "urticaria". Reports related to Sasang Constitutional medicine were excluded and total of 15 papers were finally selected. Results : 23 prescriptions were retrieved from 15 papers. The most commonly used prescription was Seungmagalgeun-tang (升麻葛根湯), and Bangpoongtongsung-san (防風通聖散) and Pyeongwi-san (平胃散) were presented twice. Commonly used herbs are Glycyrrhizae Radix et Rhizoma (甘草), Zingiberis Rhizoma Recens (生薑), and Scutellariae Radix (黃芩). 18 out of 78 herbs are belonging to heat-clearing medicinal (淸熱藥) and 16 out of 78 herbs are belonging to exterior-releasing medicinal (解表藥). Concurrent therapies such as acupuncture, pharmacoacupuncture, moxibustion, cupping therapy, ice pack applying therapy were performed and several evaluation methods were used, patient's subjective remission rate was the most used. Conclusions : Through this study, we were able to discover the tendency of herbal medicine prescription for the treatment of urticaria.

Relieving effect for respiratory inflammation of Sojadodamgangki-tang (소자도담강기탕(蘇子導痰降氣湯)의 호흡기 염증 완화효과)

  • Objectives : Sojadodamgangki-tang and its main components are traditional korean medicinal methods for treatment of cough, sputum and dyspnea. Using a respiratory inflammatory model, we intend to reveal the anti-inflammatory effect and its immune mechanism of Sojadodamgangki-tang. Methods : We used a papain-induced respiratory inflammatory mouse model. 8-week-old female BALB/C mice were divided into 3 groups as follows: the following groups: saline control group, papain treated group (vehicle), papain and Sojadodamgangki-tang(200 mg/kg) treated group (n=4). To evaluate the anti-inflammatory effect of Sojadodamgangki-tang extracts, inflammatory cell infiltration was measured in bronchoalveolar lavage fluid (BALF) and nasal lavage fluid (NALF). In addition, the effects of Sojadodamgangki-tang extracts on Th2 cell population in lung were determined by using flow cytometry. Results : Sojadodamgangki-tang extracts administration reduced inflammatory cell infiltration in BALF and NALF, especially of eosinophils. Furthermore, total immunogloblin (Ig)-E levels was reduced in BALF and serum by drug administration. Interestingly, Sojadodamgangki-tang extracts treatment also decreased the Th2 cell (CD4+GATA3+) population in lung. Conclusions : Our findings indicate Sojadodamgangki-tang extracts have anti-inflammatory effects by mediating Th2 cell and B cell activation.
