The Effects of Gum-Chewing on the Recovery of Bowel Movement and Oral Cavity Discomfort after Abdominal Surgery (껌씹기가 개복수술 환자의 장운동 회복과 구강불편감에 미치는 효과)

  • Purpose: This study was aimed to investigate the effects of gum-chewing on the recovery of bowel movement and oral cavity discomfort after abdominal surgery. Method: The nonequivalent control group, non-syncronized repeated treatment design was used. A total of 99 patients were participated in the study. The 44 patients were in the experimental group and the rest in the control group. The experimental group chewed gum three times a day until they passed gas. As the patient reported gas-passing, bowel movement time, subjective symptoms of oral cavity, and oral status were recorded precisely. The frequency, percentage, $x^2$-test and t-test were analyzed by SPSS PC 12.0. Results: There were significant differences in bowel movement, gas passing, oral cavity symptoms, and oral status scores between the experimental and the control group. Conclusion: The findings of the study demonstrated the fact that gum-chewing helps early recovery from post-operative ileus and thirst. It is because gum chewing stimulates bowel mobility and secretion of saliva. Thus gum-chewing seems to be an effective nursing intervention in reducing post-operative side effects for patients with abdominal surgery.

  • In this article, the concept of body fluid is explained in three aspects: the word meaning of body fluid, the origins of the definitions of the body fluid concept and the connotation and extension of body fluid. Investigating data about the time Hwangjenaegyeong(黃帝內經) was written, the author discovers that the meaning of "Aek(液)" is clear, but there are still questions about the meaning of "Jin(津)". The concept of body fluid derived from observation of life phenomenon and ancient philosophy on the "water". The concept of body fluid should be expressed as that body fluid is a general term for all normal liquids in the body. Within the meridians, as the composition of blood components; outside the meridians, constituting the intrinsic body fluids of various organs and tissues. This is the main part of body fluid, coming from diet, constituting the human body and maintaining human life activities, playing the roles of moistening and nourishing various of organs and tissues of the body. In addition, Interstitial fluid, all kinds of normal liquid secretion and metabolic products, such as sweat, tears, nasal discharge, saliva, slobber, gastric juice, intestinal fluid, urine, joint fluid, latex and so on, both belong to body fluid.

A Case Report of Spinal Pharmacopuncture and Herb Medicine for Dry Mouth (구강건조증 환자에 대한 척추 약침 및 한약 치료 1례)

  • Xerostomia is a symptom of subjective complaints due to decreased salivary secretion or various other causes, which is common in about 30% of elderly people. Parasympathetic stimulants or artificial saliva are used as remedies, but this has side effects, and there are limitations in completely resolving symptoms. In this study, an 88-year-old woman with subjective dry mouth syndrome for the previous two to three years was treated with spinal pharmacopuncture and herbal medicine (Yookmigihwang-tang). The patient received pharmacopuncture at the thoracic 3rd, 6th, and 10th level twice a day and took herbal medicine three times daily for seven days. Both the degree of oral dryness and behavior were improved after treatment of Korean medicine, especially in the case of pharmacopuncture for inner core muscles.

Effects of Chronic and Acute Stress on Clusterin Secretion of the Rat Submandibular Gland (급만성 스트레스가 백서 악하선의 Clusterin 분비에 미치는 영향)

  • The aim of this study is to know how the rat submandibular gland changes under various emotional stress condition, using molecular biological methods. Restraint and chronic unpredictable mild stress (CUMS) experiment is conducted on fifty one 7-week old Sprague-Dawley rats (restraint stress experiment: 21, CUMS: 30). The rats were sacrificed, the submandibular glands were excised immediately at certain time, and examined by the use of immunohistochemistry and western blotting. In CUMS experiment, sucrose preference test, water intake change, weight change were implemented at 1 week interval for the experimental period The results are as follows: 1. The number of clusterin-secreting cells of restraint stress group compared to control group showed significantly decreasing tendency in all experimental groups except for the 1st hour group (p<0.001 in the 9th, 24th, 72nd, 120th, and 168th hour group). 2. The number of clusterin-secreting cells of CUMS group compared to control group showed significantly increasing tendency in the 2nd week group (p<0.01), and significantly decreasing tendency in the 4th and 5th week group (p<0.001). 3. Sucrose preference test in CUMS experiment showed significant difference between the 5th week experimental group and control group (p<0.01). 4. Weight change in CUMS experiment showed significant difference between the 5th week experimental group and control group (p<0.01), but water intake change didn't show significant difference compared to control group. 5. In western blot analysis, clusterin expression was decreased on a gradual basis in due time compared to the control group in the restraint stress group. As for CUMS group (chronic unpredictable mild stress group), it was increased till the 2nd week and decreased till the 5th week after that, which is similar to immunohistochemical analysis result and the decreasing tendency of sucrose preference and weigh changes. Through the test, it was proved that expression of clusterin in saliva glands decreases after receiving either acute or chronic stress, indicating relation with depression caused by chronic stress. Unlike other data, however, apoptotic tendency was hardly found in tissues. Diverse possibilities could be suggested on that: first, the stress was not enough to expedite apoptosis; second, apoptosis-related protein was already being secreted though not detected with microscope; third, clusterin, a major secretion molecule of saliva, decreased with saliva's malfunction due to stress. In the respect, it will be necessary to examine proteins expressed in case of cell death or other heat-shock proteins at the same time, in order to see whether any cellular change or death is caused by decreasing clusterin under high stress, and whether the original state is restored as time goes by under mild stress, through longer-term tests using even higher acute stress.

Deprivation of Esophageal Boluses and Dry Forage Intake in Large-type Goats

  • In goats fed on dry forage twice a day, an esophageal fistula was used to investigate the physiological factors present in the marked suppression of dry forage intake that occurs after 40 min of feeding. The animals used in this study were five large-type male esophageal- and ruminal-fistulated goats. Roughly crushed alfalfa hay cubes with any large remaining chunks removed were used as feed for this research. The study was conducted under both normal feeding conditions (NFC) and sham feeding conditions (SFC). In the NFC control, the esophageal fistulae were closed by plugs and the animals ate dry forage in the normal manner. In the SFC treatment, before starting the experiment the plugs for closing the esophageal fistula were removed and the cannulae for collecting boluses were fitted into the fistulae. Therefore, the esophageal boluses were removed via an esophageal fistula before they entered the rumen. In the NFC control, eating rates sharply decreased in the first 40 min of feeding and were subsequently maintained at low levels. However, eating rates in the SFC treatment remained high after 40 min of the feeding period had elapsed and the goats ate continuously during the 2 h feeding period. In comparison with the NFC control ($1,794{\pm}203.80\;g$/2 h), cumulative dry forage intake in the SFC treatment ($3,182{\pm}381.69\;g$/2 h) was 77.4% greater (p<0.05) upon conclusion of the 2 h feeding period. In the SFC treatment, cumulative bolus output ($6,804{\pm}469.92\;g$/2 h) was about twofold the cumulative dry forage intake due to cumulative salivary secretion volume ($3,622{\pm}104.13\;g$/2 h) upon conclusion of the 2 h feeding period. The result indicates that large amounts of secreted saliva during dry forage feeding act in conjunction with consumed feed to form the ruminal load responsible for ruminal distension. The increased plasma total protein concentrations were higher in the SFC treatment than in the NFC control. However, plasma and ruminal fluid osmolalities increased in the NFC control during and after feeding but were mostly unchanged in the SFC treatment. In comparison with the NFC control ($3,440{\pm}548.04\;g$/30 min), thirst level in the SFC treatment ($1,360{\pm}467.02\;g$/30 min) was 60.5% significantly less (p<0.05) upon conclusion of the 30 min drinking period. The results of the present study indicate that In the second hour of the 2 h feeding period, dry forage intake is regulated by factors produced when boluses enter the rumen.

MARSUPIALIZATION FOR TREATMENT OF ORAL RANULA (조대술에 의한 하마종의 치료)

  • Ranula is a mucocele which occurs at the mouth floor and is mostly related to sublingual gland. In other words ranula is definded as a pseudocyst which occurs as the secretion duct of sublingual gland is destructed there as the saliva from the secretion duct flows out and retention in the soft tissue. The cause of ranula is destruction or obstruction of the duct. The clinical findings of ranula is a painless, unilateral bluish transparent swelling around the frenum and shows fluctuation when palpated. Histological finding represent a formation of cavity inside the connective tissue, but a pseudo-cyst can be seen which the wall of the cyst is composed of granulation tissue rather than epithelial cells. The first treatment of ranula can be considered as marsupialization. which induces the inner wall of the Ranula to be a part of oral mucosa. This case report shows a treatment of marsupialization with gauze packing in a young patient representing a clinical finding of characteristic ranula.

The Effects of Orofacial Myofunctional Training on the Changes of Lip and Tongue Strength in Elderly People

  • Background: Increasing age and loss of teeth weaken oral muscle strength. This study aimed to investigate the positive effects of orofacial myofunctional training on elderly people. Methods: Thirty six elderly individuals in a welfare center and a senior citizen center were included in this study. A survey regarding lip and tongue strength suvsequent to orofacial myofunctional training and oral health-related quality of life was conducted from April to June, 2019. Data were analyzed using PASW statistics ver. 18.0. Results: The experimental group, showed an increase in lip strength (from 9.79 to 10.31) and tongue strength (from 41.26 to 43.97) after orofacial myofunctional training. The perception of oral health-relatedquality of life was enhanced (from 2.26 to 2.07). In particular, the frequency of dry cough for food removal while swallowing decresed due to an incresed in the secretion of saliva (p<0.05). Correlation analysis showed that the number of functional teeth was positively related to oral health-related quality of life. Conclusion: Orofacial myofunctional training for improving lip and tongue strength is effective in the elderly. In older societies, oral strength should be maintained to improve oral health-related quality of life. It also suggests the interest of oral health workers and applying orofacial myofunctional training programs.

Preliminary study on the efficacy of xerostomia treatment with sialocentesis targeting thyroid disease patients given radioiodine therapy

  • Background: Radioiodine therapy has been widely used for thyroid disease patients, but hyposalivation and xerostomia may occur in 10~30% of patients. Sialocentesis is a procedure that removes inflammatory substances in the salivary duct and expands the duct for the secretion and delivery of saliva. In this study, thyroid disease patients treated with radioactive iodine were selected among the patients with xerostomia who visited the hospital, and the effect of sialocentesis was compared and analyzed. And then, comparison between the radioiodine therapy-experienced group and the non-radioiodine therapy-experienced group was conducted. Results: In this study, we studied xerostomia patients who underwent radioiodine therapy due to thyroid diseases and who underwent sialocentesis at the Korea University Anam Hospital. Sialocentesis is conducted by one surgeon. The study also compares the clinical symptoms before and after the surgery. After the procedure, the discomfort due to xerostomia was reduced, and the symptom was improved effectively. Conclusions: The results of this study showed that sialocentesis has a clinical effect in the treatment of xerostomia, which is a side effect of radioiodine therapy. In addition, the possibility of further clinical application of sialocentesis in the future is found.

Quantitative Analysis of Normal Salivary Gland Function using Salivary Scintigraphy (타액선 신티그라피를 이용한 정상 타액선기능의 정량분석)

  • This study was to investigate normal salivary flow rates and normal indices of Quantitative analysis of salivary scintigraphy. 96 adult volunteers were studied by Questionnaire evaluating salivary conditions and clinical examinations. 35(male 23, female 12, age range 23-31years) that absented subjective and objective symptoms related saliva were classified as normal group. The normal group underwent measurement unstimulated and stimulated salivary flow rates and salivary scintigraphy. The obtained results were as follows: 1. There were not significant in sex differences of unstimulated and stimulated salivary flow rates. The unstimulated salivary flow rate was $0.66{\pm}0.41g/min$, stimulated salivary flow rates was $1.61{\pm}0.69g/min$. 2. As comparing of parameters of salivary scintigraphy, the Uptake ratio(UR), $T_{max}$, $T_{min}$, Maximum accumulation (MA), Maximum secretion(MS) of parotid and submandibular glands were not significant in sex and side-ralated differences. 3. The UR, $T_{max}$, MA, MS of parotid gland were significantly higher than those of submandibular gland; in the parotid gland, UR, $3.67{\pm}0.88$, $T_{max}$, $18.77{\pm}0.43min$, MA, $41.35{\pm}9.22%$, MS, $43.13{\pm}9.13%$; in the submandibular gland, UR, $3.04{\pm}0.10$, $T_{max}$, $18.48{\pm}0.52min$, MA, $36.47{\pm}14.18%$, MS, $36.88{\pm}12.20%$. 4. As classifying of time-activity curve, the most of parotid gland was N-type(97.1%), submandibular gland was observed in order of M-type(67.1%), N-type(21.4%), F-type(11.4%), however, was not observed S-type. 5. As the type of time-activity curve of submandibular gland was more flattened, the UR, $T_{max}$, MA, MS were significantly decresed.

Analysis of Correlation among Oral Environment, Oral Myofunction, and Oral Microorganisms

  • Background: Factors affecting oral function include tooth number, oral muscle strength, and oral diseases. This study aimed to investigate the relationship among oral environment, muscle, and microbiology. Methods: Fifty-six elderly individuals in a day care center were included in the study. The survey regarding tongue and lip muscle strength and oral microorganisms was conducted from November to December 2018. Results: Tongue and lip muscle strength were greater in men than women (p>0.05). Tongue muscle strength was greater in the ${\leq}80-year-old$ group ($34.94{\pm}9.85$) than the ${\geq}90-year-old$ group ($25.57{\pm}7.54$) (p<0.05). Tongue muscle strength and lip muscle strength were greater in the ${\geq}15$ functional teeth group ($34.08{\pm}9.31$ and $9.25{\pm}1.63$, respectively) than in the <15 functional teeth group ($28.08{\pm}7.53$ and $7.76{\pm}1.51$, respectively) (p<0.05). Age was significantly correlated with functional tooth number, denture use, and tongue muscle strength. The number of functional teeth was positively correlated with tongue muscle strength, lip muscle strength, and oral microorganisms. Denture use was negatively correlated with tongue and lip muscle strength. Tongue muscle strength was significantly correlated with lip muscle strength. The number of Eubacterium nodatum was higher in men than women. The number of Parvimonas micra and Enterococcus faecalis was higher in the groups with ${\geq}15$ functional teeth, denture use, and greater tongue and lip muscle strength. The number of Lactobacillus casei was higher in the group that uses dentures and with greater tongue strength. Conclusion: Oral microbiology is more important in oral environment and management than oral muscle function. The correlation between oral muscle and oral microorganism requires further study. Therefore, oral care training should be conducted to improve the oral care practice of elderly individuals, maintain oral health through oral care, and prevent the decrease in saliva secretion by aging.
